*  New in the Literature: Physical Fitness Training for Stroke (Cochrane Database Syst Rev. 2011 Nov 9;11:CD003316)
Although the effects of physical fitness training on death, dependence, and disability after stroke are unclear, there is sufficient evidence to incorporate cardiorespiratory training that involves walking within poststroke rehabilitation programs to improve speed, tolerance, and independence during walking, say authors of a meta-analysis published in November in the Cochrane Database of Systematic Reviews. The authors searched the Cochrane Stroke Group Trials Register, the Cochrane Central Register of Controlled Trials, MEDLINE, EMBASE, CINAHL, SPORTDiscus, and 5 additional databases. They also searched ongoing trials registers, handsearched relevant journals and conference proceedings, screened reference lists, and contacted experts in the field.. Selection criteria included randomized trials comparing either cardiorespiratory training or ...

*  Granulocyte-colony stimulating factor (G-CSF) for stroke: an individual patient data meta-analysis - Nottingham ePrints
Granulocyte colony stimulating factor (G-CSF) may enhance recovery from stroke through neuroprotective mechanisms if administered early, or neurorepair if given later. Several small trials suggest administration is safe but effects on efficacy are unclear. We searched for randomised controlled trials (RCT) assessing G-CSF in patients with hyperacute, acute, subacute or chronic stroke, and asked Investigators to share individual patient data on baseline characteristics, stroke severity and type, end-of trial modified Rankin Scale (mRS), Barthel Index, haematological parameters, serious adverse events and death. Multiple variable analyses were adjusted for age, sex, baseline severity and time-to-treatment. Individual patient data were obtained for 6 of 10 RCTs comprising 196 stroke patients (116 G-CSF, 80 placebo), mean age 67.1 (SD 12.9), 92% ischaemic, median NIHSS 10 (IQR 5-15), randomised ...

OBJECTIVES: To survey the frequency of use of indirect comparisons in systematic reviews and evaluate the methods used in their analysis and interpretation. Also to identify alternative statistical approaches for the analysis of indirect comparisons, to assess the properties of different statistical methods used for performing indirect comparisons and to compare direct and indirect estimates of the same effects within reviews. DATA SOURCES: Electronic databases. REVIEW METHODS: The Database of Abstracts of Reviews of Effects (DARE) was searched for systematic reviews involving meta-analysis of randomised controlled trials (RCTs) that reported both direct and indirect comparisons, or indirect comparisons alone. A systematic review of MEDLINE and other databases was carried out to identify published methods for analysing indirect comparisons. *  Families First Edmonton (FFE) - Full Text View - ClinicalTrials.gov
This study is designed to provide clear evidence for health and social policymakers about the influence of alternate service-delivery models and practices on enhancing and sustaining low-income family linkages to available services. A challenge faced by Canadian health and social service providers is to promote health for low-income families in a proactive and cost-effective manner. Families with low incomes experience an array of health and social barriers that compromise their resilience, lead to negative family outcomes, and act as barriers to available services. Family barriers are compounded by service delivery barriers and result in reduced opportunities for effective, primary-level services and in increased use of secondary-level services (e.g., emergency room visits, emergency intervention, police involvement), with the obvious increase in costs. Randomized-controlled trials are rare in community-based intervention research.. This ...

We pooled treatment effects of LIPUS on similar outcomes across eligible trials, regardless of clinical subgroups, focusing on complete case analysis. We calculated pooled estimates and associated 95% confidence intervals using random effects models for meta-analysis with three or more studies and fixed effects models for meta-analysis with two studies. We examined heterogeneity associated with all pooled analyses using both the χ2 test and I2 statistic. SAS version 9.4, R version 3.1, and Review Manager 5.3 provided software for the statistical analysis.. For time to event outcomes, we pooled hazard ratios. For studies that did not apply methods of survival analysis, we considered time to event reported as a continuous variable (for example, days to return to work) at the longest follow-up time. We used the relative effect measure ratio of means (mean LIPUS/mean control) to account for the baseline difference in fracture healing depending on type of bone and (such as ...

*  Parenteral vs. enteral nutrition in the critically ill patient: a meta-analysis of trials using the intention to treat principle
Eleven trials met the inclusion criteria for the review. Nine (n=559) were included in the primary analysis.. None of the 11 trials reported the maintenance of allocation concealment, one reported the use of blinding and nine allowed the conduct of an ITT analysis. There was no evidence of publication bias in the funnel plot.. Based on the 9 trials presenting ITT results, a statistically significant reduction in mortality rates was found with PN compared with EN (OR 0.51, 95% confidence interval, CI: 0.27, 0.97, P=0.04). No evidence of statistical heterogeneity was detected (P=0.50; I-squared 0%).. Six trials with complete follow-up reported on infectious complications. There was a statistically significant increase in infectious complications with PN compared with EN (OR 1.66, 95% CI: 1.09, 2.51, P=0.02). Some evidence of statistical heterogeneity was shown (I-squared ...

EDITOR-Jaakkimainen et al's meta-analysis concludes that an improvement in dyspeptic symptoms occurred among patients with non-ulcer dyspepsia in whom Helicobacter pylori was eradicated.1 Unfortunately, there is a small but crucial problem at the heart of the analysis. The authors report that the summary estimates are statistically homogenous, but this is incorrect. In the observational studies the P value of ,0.001 indicates massive heterogeneity between the results of the studies included. In the therapeutic trials the P value of 0.046 also indicates heterogeneity.. Meta-analysts faced with such heterogeneity have three choices: they may ignore the heterogeneity and pool the results with a fixed effects model; they may use a random effects model, which takes the heterogeneity into account; or they may decide not to pool the results. In this instance the authors chose to use a fixed effects model despite the heterogeneity. In consequence the confidence intervals of the ...

Whereas the authors concluded that HHV 7 institutes a G2/M arrest, it can be unclear in case the newly synthesized DNA observed in these cells is viral or cellular. In reality, elevated amounts of cyclin B were observed in cells with DNA contents corresponding towards the G1, S, and G2/M phases on the cell cycle. Lev els of Cdk1 were also elevated following HHV 7 infec tion. No other cell cycle markers had been analyzed. The outcomes may very well be constant with the authors conclusion, or could mimic the results viewed with HCMV, in which cells are arrested on the G1/S border but in addition express cyclin B, and in which the DNA articles maximize in contaminated cells is attributed to viral, but not cellular DNA replication. Far more do the job has examined the effects of HHV 6 infection on cell development. *  Summary Meta-analysis - StatsDirect
Menu location: Analysis_Meta-Analysis_Summary.. This function provides a substitute for a proper meta-analysis when only summary statistics (odds ratio, relative, risk difference - with confidence intervals or standard errors) are known from a group of related studies.. Please do not use this function if the raw data from original studies are available, as the pooled estimates will be much more precise when using the original raw data. Better alternatives to this function are odds ratio, relative risk or risk difference meta-analysis based on counts from the included studies.. A generalised meta-analysis is calculated for the fixed effects model and the random effects model. Stratum weights are calculated as the inverse of the variance for the summary statistic (Y) supplied. If Y is specified as a ratio then calculations are performed on a natural log transformation of Y. The pooled estimate of Y is calculated as a weighted mean, i.e. the sum of weighted Y or ln(Y) for each stratum divided by ...

Boing Boing recently covered an article by Dr. Hisham Ziauddeen and colleagues in Nature Reviews Neuroscience that questioned the idea that common obesity represents food addiction-- an idea that I often encounter in my reading. Maggie Koerth-Baker asked me if I wanted to respond. I sent her a response explaining that I agree with the authors' conclusions and I also doubt obesity is food addiction per se, as I have explained in the past, although a subset of obese people can be addicted to food. I explained that the conclusions of the paper are consistent with the idea that food reward influences fat mass. You can find my explanation here ...
