*  HKU Scholars Hub: Use of second-line therapies for management of massive primary postpartum hemorrhage
Objective To determine rates of use and success of second-line therapies for massive primary postpartum hemorrhage (PPH). Methods A retrospective cohort study was conducted among 91 women who gave birth at Kwong Wah Hospital, Hong Kong, between January 1, 2006, and December 31, 2011. Inclusion criteria were gestational age of at least 24 weeks and massive PPH (defined as blood loss â ¥ 1500 mL within 24 hours after birth). Second-line therapies assessed were uterine compression sutures, uterine artery embolization, and balloon tamponade after failure of uterine massage and uterotonic agents to stop bleeding. Results The rate of massive PPH was 2.65 per 1000 births. Second-line therapies were used among 42 women with PPH, equivalent to a rate of 1.23 per 1000 births. Only 21.4% of the women who received second-line therapies required rescue hysterectomy. A rising trend was observed for the use of second-line therapies, whereas the incidence ...

*  192 Do health gains match investment in post partum haemorrhage prevention and care? Lessons learned from two French programs,...
Post partum haemorrhage (PPH) is the most common obstetric complication at delivery and a major cause of maternal death worldwide. In France, PPH remains the primary cause resulting in a maternal mortality ratio of 2.4 per 100 000 live births vs less than 1 per 100 000 in the UK.. Ten years ago, the National committee of medical experts on maternal mortality draw the attention of the clinicians on the substandard care found in 80% of the maternal deaths due to PPH. Such substandard care was also observed in a survey on severe PPH (2001) where clinical audit was used. Finally the HAS with the CNGOF published in 2004 their Recommendations for clinical practices related to PPH.. In parallel, two teams of clinicians very aware of that poor situation launched actions associated with evaluative research studies. The first team was lead by M Dreyfus in the Basse Normandie region and the other was managed by RC Rudigoz à Lyon. The Sphere and the Pithagore6 programs were similar in their final goal, ...

*  Efficacy of Rectal Misoprostol for Prevention of Postpartum Hemorrhage
Postpartum hemorrhage is an important cause of maternal morbidity and mortality after delivery. Active management of postpartum hemorrhage by an uterotonic drug decreases the rate of postpartum hemorrhage. The aim of this study is to evaluate the efficacy of rectal misoprostol for prevention of postpartum hemorrhage. This double blind randomized clinical trial was performed on full term pregnant women candidate for vaginal delivery, referred to Zahedan Imam Ali Hospital during 2008-2009. They were randomly divided into two groups of rectal misoprostol and oxytocin. The women in misoprostol group received 400 µg rectal misoprostol after delivery and the women in oxytocin group received 3 IU oxytocin in 1 L ringer serum, intravenously. Rate of bleeding, need to any surgery ...

*  RCT of Misoprostol for Postpartum Hemorrhage in India - Full Text View - ClinicalTrials.gov
Despite existing knowledge of ways to effectively treat postpartum hemorrhage (PPH), lack of resources in rural India has impeded improvement in rates of maternal mortality and morbidity. Most births take place at home, and local auxiliary nurse midwives are not trained or certified to administer injectable uterotonics. Reduction in postpartum hemorrhage may decrease other adverse maternal outcomes such as the need for additional uterotonic agents, blood transfusion, surgical intervention or death. The main hypothesis of the study is that misoprostol administered orally during the third stage of labor will significantly reduce the incidence of acute postpartum hemorrhage. The advantages of misoprostol are: that it is relatively inexpensive, is an oral preparation of 600 mcg with a long shelf life, and does not require refrigeration. One ...

*  villagemidwife: Preventing and treating post partum haemorrhage
I recently heard interesting statistics about blood loss and active management where a hospital was audited and it was found that there was a higher incidence of PPH with active management than physiological management. Unfortunately I can't remember where I heard this...I think it was the NZ College of Midwives Conference in Auckland this year. I realise this isn't backed up by the majority of the research. I do wonder if we are doing more harm than good interfering as we do but as you say, there are lots of factors and the mere fact of the excitement around the birth and everyone in the room trying to get their hands on the baby distracts and interferes in the natural process ...

Postpartum hemorrhage is a serious obstetrics emergency occurring independently from the mode of delivery. The traditional definition of PPH is based on quantification of blood loss but has variable criteria and several limitations across countries or obstetrics national societies. In general, for mild PPH, a blood loss ,500 ml and a severe blood loss ,1000 ml are considered after spontaneous delivery [13] or above 1000 ml in cesarean section [3]. Regardless of definitions used, only a visual assessment of blood loss is inaccurate [14], and many postpartum women are subsequently underdiagnosed/overdiagnosed for PPH. Thus, alternative blood loss assessment is a check for a drop in hematocrit of 10% counted before and after delivery [15]. The bleeding after labor can occur within 24 hours (primary form of PPH) or, later, from 24 hours after delivery until 6 weeks postpartum (secondary form) of ...

The postpartum hemorrhage (PPH) is the major complication of the delivery. In clinical practice, if after giving birth, the placenta is not expelled naturally, an active management should be triggered. Escalating therapy after obstetric maneuvers (placenta, uterus, examination of the birth canal), begins with uterotonic treatments for invasive treatments lead to embolization, vessel ligation and hysterectomy. However, the morbidity of these techniques and the desire to preserve fertility required to devise new therapeutic solutions, which have recently led to the development of an innovative medical device intrauterine hemostasis.. The postpartum haemorrhage are mainly the result of weak and bleeding from the surface corresponds to the placental insertion, which is no longer localized. With the innovative medical device, our main hypothesis is that the ...

*  Preventing postpartum hemorrhage in low-resource settings.
OBJECTIVES: To review the literature to determine the most effective methods for preventing postpartum hemorrhage (PPH), the single most important cause of maternal death worldwide. METHODS: Systematic review of published randomized controlled trials and relevant reviews. RESULTS: Review of the literature confirms that active management of the third stage of labor, especially the administration of uterotonic drugs, reduces the risk of PPH due to uterine atony without increasing the incidence of retained placenta or other serious complications. Oxytocin is the preferred uterotonic drug compared with syntometrine, but misoprostol also can be used to prevent hemorrhage in situations where parenteral medications are not available (e.g. at home births in developing countries). CONCLUSIONS: The use of active management of the third stage of labor to prevent PPH ...

May 11, 2010 Abortions Are 'One Small Chunk Of Bigger Puzzle' For Improving Maternal, Child Health National Post columnist Jonathan Kay examines the ongoing debate over whether Canada's G8 child and maternal health initiative should include abortions, which according to a 2006 Oxford University Press-World Bank report, are "one small chunk of a bigger puzzle. A much larger problem, in terms of the number of female lives affected, is the decidedly less headline-grabbing subject of hemorrhages â€" which include antepartun hemorrhages … and primary postpartum hemorrhages. Then there is sepsis …, blood-pressure disorders associated with pregnancy and obstructed labour." Kay continues: "On the infant side of the equation, life-threatening conditions in need of G8 attention include low birthweight, birth asphyxia and infections … statistically speaking, these are the real ...
