With the growth of the geriatric population, it is no coincidence that within a few months the Office of Technology Assessment (OTA) issued Technologies for Managing Urinary Incontinence (1) and this journal published an article by Burgio and colleagues (2) on biofeedback and urinary incontinence. The issue of urinary incontinence is indeed no stranger to Annals; in December 1982, an article by Williams and Pannill (3) detailed current knowledge on urinary incontinence in the elderly, and in February 1980, an article by deGroat and Booth (4) described the physiology of the urinary bladder and urethra. *  Aruku's Pelvic Health Centre: EVALUATION OF FEMALE URINARY INCONTINENCE
In case of urine leakage during straining, a Bonney's test may confirm urethral hypermobility though this test is uniformly acceptable. This is performed by placing the index and middle finger on each side of the urethra at the level of the bladder neck and supporting it with care taken not to cause compression of the urethra against the pubic bone. The patient is asked to cough or strain while the bladder neck is pushed up. If urine leakage is controlled with lifting motion, a positive result, bladder neck descent and urethral hypermobility with laxity in paraurethral support is assumed to be the cause of the incontinence and urethral elevation prevents further urine leakage. We Rarely perform this test this days. ...

*  Urinary Urinary Nerve Growth Factor (NGF) as a Biomarker for Mixed Urinary Incontinence - Full Text View - ClinicalTrials.gov
Female urinary incontinence (UI) can be caused by pure stress UI (SUI), pure urgency UI (UUI) and mixed SUI and UUI (MUI). Clinically it might not be that easy to separate MUI and SUI. Patients might perceive urgency when they have an incompetent urethra and a full bladder.. Urinary tract nerve growth factor (NGF)is produced by bladder urothelium and smooth muscle. Increased levels of NGF have been reported in the bladder tissue and urine of patients with overactive bladder (OAB).. If the urinary levels of NGF differ among women with pure SUI and MUI, then urinary NGF level might be a biomarker in the differential diagnosis of MUI in women.. Also, decreased urinary NGF level was reported in OAB patients of whom the symptoms were improved. So, we might expect that the remnant OAB symptom including urgency incontinence can be ...

*  Urinary Incontinence - Diagnosis, Symptoms, and Treatments
Urinary incontinence is the loss of a person's ability to control his or her bladder. This is a common problem that ranges from occasionally leaking urine when the bladder gets compressed when coughing, sneezing or bearing down to having a urinary urge that is so sudden and tough that one fails to seek relief on time.. If urinary incontinence is affecting the person's activities of daily living, he/she must seek medical consultation. Simple lifestyle modifications or medical interventions could address this problem or totally stop it from occurring.. Urinary Incontinence is stigmatized, under-diagnosed, underreported and an under-treated condition that is erroneously thought to be normal in the aging process. Based on expert analysis, 25 million adults in the US alone experience transient or chronic Urinary Incontinence. The National ...

*  Urinary Incontinence in Women: Evaluation and Management - American Family Physician
Because the prevalence of urinary incontinence increases with age, a working knowledge of the diagnosis and treatment of the various types of urinary incontinence is fundamental to the care of women. As the population of the United States ages, primary care physicians can expect to see an increasing number of patients with urinary incontinence. By obtaining a careful medical history and performing a comprehensive physical examination, the primary care physician can initiate successful treatment for the majority of patients without the need for invasive testing. This article offers a comprehensive approach to the evaluation and management of urinary incontinence in women.

*  Age at surgery, educational level and long-term urinary incontinence after radical prostatectomy
OBJECTIVE To identify predictors for long-term urinary leakage after radical prostatectomy.. PATIENTS AND METHODS A consecutive series of 1411 patients who underwent radical prostatectomy (open surgery or robot-assisted laparoscopic surgery) at Karolinska University Hospital between 2002 and 2006 were invited to complete a study-specific questionnaire. Urinary leakage was defined as use of two or more pads per day. RESULTS Questionnaires were received from 1288 (91%) patients with a median follow-up of 2.2 years. Age at surgery predicts in an exponential manner long-term urinary incontinence at follow-up with an estimated relative increase of 6% per year. Among the oldest patients, 19% had urinary incontinence compared with 6% in the youngest age group, translating to a prevalence ratio of 2.4 (95% confidence interval [CI], 1.5-8.1). Low educational level, ...

Aiming at decreasing side effects, other drug delivery systems have been developed (e.g. intravesical, transdermal, intravaginal and rectal). In patients using intermittent catheterization, intravesical oxybutynin at a dose of 0.2 mg/Kg can be an alternative to reach higher plasma levels with better tolerability. Extended-release transdermal patches are still under investigation, but seem to avoid peak plasma concentrations by maintaining oxybutynin and N-DEO plasma levels more stable with fewer side effects. The use of oxybutynin promotes a reduction in urge-incontinence episodes by up to 88%, but the incidence of dry mouth is 87% with the usual dosage against 68% with oxybutynin extended-release forms. The relative risk of dry mouth is 3.23 compared with placebo. The incidence of constipation, drowsiness and dizziness is 30-31%, 38-40% and 28-38%, respectively15;16. Tolterodine Tolterodine tartrate is also a tertiary amine of potent antimuscarinic action that has a 8-fold ...

*  Female urinary incontinence and wellbeing: results from a multi-national survey | BMC Urology | Full Text
These results showed that reduced wellbeing is observed in UI. This effect is observed in all aspects of wellbeing and most sub-groups of UI sufferers. Factors which reflect greater severity of UI lead to a greater reduction in wellbeing. Lifestyle influences wellbeing and those with UI who exercise less frequently or have sex infrequently are especially likely to report reduced wellbeing. Wellbeing decreases as a function of the severity of UI and reductions in HRQol. Again, these changes reflect all aspects of wellbeing measured by WEMWBS. Previous analyses [13] showed that when both quality of life (ICIQ-LUTSqol) and symptom severity (ICIQ-UI Short Form) were included in the same regression analysis, the only significant effect on mental well-being was ICIQ-LUTSqol. This shows that UI symptoms do not directly affect mental well-being, but that symptoms influence quality of life, which in turn influences well-being.. This is the first multi-national study which has ...
