The premature failure of the Charnley Elite-Plus stem: a confirmation of RSA predictions. - Surgical Intervention Trials Unit
We performed a clinical and radiological study to determine the rate of failure of the Charnley Elite-Plus femoral component. Our aim was to confirm or refute the predictions of a previous roentgen stereophotogrammetric analysis study in which 20% of the Charnley Elite-Plus stems had shown rapid posterior head migration. It was predicted that this device would have a high early rate of failure.We examined 118 patients at a mean of nine years after hip replacement, including the 19 patients from the original roentgen stereophotogrammetric study. The number of revision procedures was recorded and clinical and radiological examinations were performed. The rate of survival of the femoral stems at ten years was 83% when revision alone was considered to be a failure. It decreased to 59% when a radiologically loose stem was also considered to be a failure. Total knee replacement is an effective option for treating chronic knee conditions that cause pain and functional impairment. Significant improvements in quality of life, as measured by both disease specific and generic quality of life measures have been well documented. Carefully monitored introduction of new implant designs through small, randomized trials using radiostereometric analysis (RSA) is gaining support. RSA is a radiographic technique that uses small tantalum balls implanted into the patient's bone to measure micromotion (,1 mm) at the bone - implant interface, something which is not possible to do reliably using regular x-ray techniques. The amount of micromotion that occurs within 2 years of surgery can be used to predict the long term survival of knee replacements. A new Roentgen Stereophotogrammetric Analysis (RSA) system is reported; it can measure penetration of the metallic femoral component of a Total Knee Replacement (TKR) prosthesis into the polyethylene bearing on the tibial component. This system was used to analyse a study group of six Anatomic Graduated Components (AGC) knee prostheses more than 6 years post-implantation, and to compare with a control group of six newly implanted AGC prostheses. The volumetric loss of polyethylene was estimated by imaging each prosthesis at a series of different knee flexion angles. The mean difference between the RSA measured polyethylene bearing thickness and the manufacturer's quoted values for the control group was -0.03 mm (SD 0.17). The estimated linear penetration at 6.4 years in this prosthesis was determined to be 0.1 mm/year. Volumetric wear was estimated to be 600-700 mm(3)/year at 6.4 years, equating to approximately 100 mm(3)/year. The main objective of the system presented in this paper is to provide surveyors and engineers with a new photogrammetry device that can be easily integrated with surveying total stations and a global navigation satellite system (GNSS) infrastructure at a construction site, taking advantage of their accuracy and overcoming limitations of aerial vehicles with respect to weight, autonomy and skilled operator requirements in aerial photogrammetry. The system moves between two mounting points, in a blondin ropeway configuration, at the construction site, taking pictures and recording the data of the position and the orientation along the cable path. This technique uses a computer program to process photographs of the structure and turns them in to a series of accurately positioned points. Images are taken of the structure from different places around it and at different elevations. The images are then put into the computer program and points on the structure are identified on each image, points that are the same are then identified on each image and some idea of scale is added. Using an adjustment technique similar to that used to adjust tape measurements the positions of the points are calculated in three dimensions.
