*  Radiation proctitis - Wikipedia
Radiation proctitis (and the related radiation colitis) is inflammation and damage to the lower parts of the colon after exposure to x-rays or other ionizing radiation as a part of radiation therapy. Radiation proctitis most commonly occurs after treatment for cancers such as cervical cancer, prostate cancer, and colon cancer. Radiation proctitis involves the lower intestine, primarily the sigmoid colon and the rectum and is part of the conditions known as pelvic radiation disease and radiation enteropathy. Radiation proctitis can occur at two times after treatment: Acute radiation proctitis - symptoms occur in the first few weeks after therapy. These symptoms include diarrhea and the urgent need to defecate, often with pain while doing so (tenesmus). Acute radiation proctitis usually resolves without treatment after several months, but symptoms may improve with butyrate enemas. This acute phase is due to ...

*  Rectal Tacrolimus in the Treatment of Resistant Ulcerative Proctitis - Full Text View - ClinicalTrials.gov
Ulcerative Colitis (UC) is a life-long incurable disease with remissions and exacerbations. Inflammation confined to the rectum occurs in a quarter of patients and can be extremely hard to treat. Many medications have been tried in order to control the inflammation, but they do not always work. One of the newer medications is the immunosuppressing medication, tacrolimus that has been shown to be effective in UC when taken orally. Unfortunately, the oral use of this medication can have numerous serious side effects. In order to overcome these side effects, the use of topical rectal tacrolimus has been examined. Pilot studies in ulcerative proctitis (inflammation confined to the rectum) resistant to conventional therapies have demonstrated a clinical remission in 75% of patients and although the medication was well absorbed through the lining of the bowel, the levels in the blood were very low and no serious side effects were reported. The findings suggest that this preparation is indeed ...

Assessments of diseases are usually performed by a combination of clinical symptoms, signs and objective measurements. Biochemical markers of inflammation are important in evaluation of treatment of acute and chronic diseases. In this thesis we have focused on two inflammatory diseases: acute radiation proctitis and rheumatoid arthritis. We examined established and new biomarkers, with focus on two closely related proteins, calprotectin (S100A8/S100A9) and S100A12 (ENRAGE). These and other biomarkers were compared to clinical symptoms and signs with the aim of defining their usefulness in monitoring the course of the diseases. Characterizing the changes induced by radiation therapy to the pelvic region showed incongruent time profiles in symptoms, histological scoring of inflammation and biochemical measurements. Compared to the examination before start of radiation therapy, biopsies from rectal mucosa showed a maximal inflammation at the first examination during radiation therapy, while ...

For some people, this issue might be brought up after a first bout of diverticulitis. Surgery is usually offered because once diverticulitis happens, repeated episodes can be more severe. Once diverticulitis happens twice, the chance of it happening again gets higher with each episode. The question of whether to undergo surgery, as well as how many repeat episodes should pass before surgery is performed, needs to be decided on an individual basis. QXP 8/31/09 5:39 PM Page 43 PART THREE Medications I've heard there is a new type of prednisone with fewer side effects called budesonide (Entocort). Topical therapy applied rectally comes in four preparations (see Table 5): Steroid creams and ointments are used mostly for anal fissures and inflammation around the anus (perianal dermatitis). Ditto. My disease started in the rectum and the first doctor I saw just gave me a steriod enema and azulfadine (back in 1986). Three weeks later my sypmtoms were worse and I saw another doctor who then put me on Prednisone. Just over two months after original diagnosis my entire colon and rectum were affected (small intestine was clear). Then 25 years later dx with Crohns, so back then I guess it was Crohns Colitis and not UC but oh well. Definitely want to inform your doctor anytime your symptoms are getting worse ...

Results We returned 64 patient records over 67 visits, 39 pre and 25 post-guideline changes. 31% (20/64) were HIV positive. Commonest presentations were PR bleeding (49%), rectal discharge (44%) and diarrhoea (28%). 55/64 (88%) had rectal microscopy, with 42/55 (76%) having pus cells present; of these 3/42 (7%) had GC seen on microscopy. There were very low levels of urethral STI rates (just one case of each), but high rates of rectal GC and CT (24% and 13% respectively). LGV was positive in 5% (3/54) and rectal HSV was found in 25% (10/40). There were more HSV swabs sent before versus after guideline modification (19/40 versus 21/27, p=0.01). ...

Eleven consecutive patients with diarrhoea from whose stools campylobacter were isolated were investigated by sigmoidoscopy and rectal biopsy. Eight had definite proctitis, and in seven biopsy specimens were abnormal with histological changes ranging from non-specific colitis to gross colitis with goblet-cell depletion and crypt-abscess formation. Nine of the patients passed blood in their stools, and in all but one abdominal pain was a feature of the illness. Severe campylobacter colitis may be clinically, sigmoidoscopically, and histologically difficult to differentiate from ulcerative colitis and is a differential diagnosis in acute colitis. ...
