*  Primary and Secondary Syphilis -- United States, 1997
Syphilis is an acute and chronic sexually transmitted disease (STD) caused by infection with Treponema pallidum. The disease is characterized by skin and mucous membrane lesions in the acute phase (primary and secondary {P&S} syphilis) and lesions of the bone, viscera, and cardiovascular and neurologic systems in the chronic phase. Because syphilis enhances transmission of human immunodeficiency virus (HIV), prevention of syphilis is important for controlling HIV (1). During 1986-1990, an epidemic of syphilis occurred throughout the United States (2). Syphilis rates began to decline in 1991 and have declined each year since that time. To determine whether this decline is reflected in changes in the epidemiology of syphilis, CDC analyzed notifiable disease surveillance data for 1997. This report summarizes the findings of the analysis, which indicate that reported P&S ...

We found the T pallidum PCR test to be both sensitive and specific in primary syphilis, thus facilitating rapid diagnosis and treatment. This evaluation has demonstrated that the sensitivity of T. pallidum was 89.1% for primary syphilis and 50.0% for secondary syphilis, including three infections in men that seroconverted to syphilis within five weeks. There was no difference in the sensitivity and specificity of the PCR test according to HIV status. The performance of T pallidum PCR testing in our evaluation was broadly comparable to previous studies using other PCR tests [14, 16-19].. Previous evaluations have generally relied on baseline serology alone [16-18]. Clinicians are aware that the diagnosis of syphilis is not so straightforward and recognise that the diagnosis and staging of syphilis relies on a combination of knowledge of the individual's past syphilis history and sexual ...

Syphilis is a sexually transmitted infection that causes genital sores and rash, but in some circumstances may result in more severe and unexpected symptoms. These severe symptoms could include eye infections, meningitis (infection of the membranes covering the brain and spinal cord), and liver infection. If not properly treated, syphilis can also lead to heart problems and dementia (a decline in reasoning, memory and other mental abilities) years down the road. There has been an increase in the number of reported cases of syphilis in North America, Europe, and Australia over the past decade. The number of new syphilis infections in Canada has increased roughly 10-fold over the past 10 years.. Since 1943, the antibiotic penicillin has been used to treat syphilis; however, very little information has been gathered to determine the proper dose of penicillin or appropriate duration of ...

This article was reported by Mynews4.com and KRNV.. Mynews4.com and KRNV reported that Washoe County, Nev., recorded a 32-case outbreak of primary syphilis in 2013, with highest incidence occurring among men who had sex with men (MSM). Jennifer Howell, sexual health program coordinator at the Washoe County Health Department, noted that more than 50 percent of Washoe County MSM diagnosed with syphilis also had HIV. She explained that people with syphilis skin lesions and sores (breaks in the skin) were three to five times more likely to acquire an HIV infection. Washoe County reported 20 new HIV infections in 2012 and 25 in 2013.. Howell described syphilis as a curable bacterial infection that had three stages. Primary syphilis could cause long-lasting, painless sores; secondary syphilis could produce "rashes and lesions" on the body and hands; tertiary-phase (latent) syphilis could cause ...

*  Association of the HLA-DRB1 locus with syphilis in a Chinese population. - Semantic Scholar
BACKGROUND Syphilis is a sexually transmitted infection (STI) caused by the Treponema pallidum subspecies pallidum spirochete bacterium. The association of human leukocyte antigen (HLA) with syphilis has been reported in several populations, but not in the Chinese population. Furthermore, serology methods have mostly been used in previous studies investigating the association between STIs and HLA alleles. The objective of this study was to analyze the association of the HLA-DRB1 alleles and susceptibility to syphilis in the Chinese population. METHODS A polymerase chain reaction with sequence-specific primers (PCR-SSP) method was used to genotype HLA-DRB1 alleles in 196 syphilis patients and 500 healthy controls. RESULTS The HLA-DRB1*14 allele was more prevalent in syphilis patients than in the healthy controls (p=0.013; corrected p|0.05). CONCLUSIONS The allele HLA-DRB1*14 was found to be associated with ...

Early detection of syphilis is critical to successful treatment before the disease progresses into debilitating or life-threatening stages. Syphilis testing is also a key to ensuring safe blood supplies, and the World Health Organization (WHO) recommends testing blood supplies for syphilis. CTK's syphilis diagnostic tests all utilize treponemal antigens for the direct detection of syphilis antibodies. ...

The methods of diagnosis (dark ground microscopy and serology), treatment, and follow up of 946 patients with primary and 854 with secondary syphilis who presented to a London STD clinic between 1965 and 1984 were reviewed retrospectively. On dark ground microscopy spirochaetes typical of Treponema pallidum were seen in 673 (78%) of 884 patients with primary syphilitic chancres. Of the patients with primary syphilis, 137 (14.5%) had negative serology results at presentation. Eight (0.9%) of the patients with secondary syphilis had negative results at presentation, but seven of these gave positive results one month later. Procaine penicillin was the treatment used most, and erythromycin the commonest alternative. The Jarisch-Herxheimer reaction occurred more often after treatment with penicillin than with erythromycin or tetracycline (p less than 0.005). The U.S. Preventive Services Task Force (USPSTF) has found convincing evidence that screening for syphilis infection in asymptomatic, nonpregnant persons at increased risk for infection provides substantial benefit. The report appears in the June 7 issue of JAMA.. This is an A recommendation, indicating that the USPSTF recommends the screening and that there is high certainty that the net benefit is substantial.. The number of cases of primary and secondary syphilis have been increasing since 2000. In 2014, approximately 20,000 cases of syphilis were reported in the United States. Left untreated, syphilis can progress to late-stage disease in about 15 percent of persons who are infected. Late-stage syphilis can lead to development of inflammatory lesions throughout the body, which can lead to cardiovascular or organ dysfunction. Syphilis infection also increases the risk for ...

Syphilis, caused by infection with Treponema pallidum subspecies pallidum, is a mucocutaneous sexually transmitted infection (STI) with high infectivity in the early infectious stages. It may also be passed transplacentally from the ninth week of gestation onwards. The primary stage, with an incubation period of 9-90 days, usually consists of a painless single ulcer at the site of inoculation that may be accompanied by regional lymphadenopathy. The secondary stage, with an incubation period of 6 weeks to 6 months, has many clinical manifestations including rash, mouth ulcers, condylomata lata, patchy alopecia, generalised lymphadenopathy, meningitis, and hepatitis. The later, non-infectious, tertiary stage manifestations of syphilis are now rarely seen in the United Kingdom and include gummatous syphilis, neurosyphilis, and cardiovascular syphilis. Treatment for syphilis is through prescription medication. Usually a single shot of antibiotics like penicillin can be enough to kill the syphilis. If a person has had the syphilis for less than one year, then a single shot is usually sufficient. No resistance has ever been documented for penicillin. For people allergic to penicillin, doxycycline is also an effective antibiotic for syphilis treatment. The objectives are to determine to what degree the intervention: (1) increases the detection rate of untreated syphilis; (2) increases the proportion of men who undergo syphilis testing at least annually (increased screening coverage); (3) reduces the interval between syphilis tests (increased screening frequency); and (4) reaches men at highest risk according to sexual behaviours. The main hypotheses are that the intervention will increase screening coverage to a minimum of 85% of men undergoing syphilis testing annually, increase screening frequency to a median of 3 tests per person per year, and increase the case detection rate by 75% or more.. Researchers said on Thursday that one child was born with syphilis every hour in China in 2008, as new money from the country's growing economy fuels the world's fastest-growing epidemic of the disease. According to an article in the New England Journal of Medicine, syphilis almost wiped out China 50 years ago, but it is now the most commonly reported sexually transmitted disease in Shanghai. The researchers said that female sex workers and homosexual men are driving the infection rate, and there is evidence that social pressures are discouraging people from seeking treatment at official clinics. "After China's economy became increasingly market-based in the 1980s, the growing numbers of Chinese businessmen with money and young women without money translated into expanded demand and supply for the country's commercial sex industry," the article said.. Syphilis is a bacterial infection that can be tackled with antibiotics if diagnosed early. However, if the ...

Our study also shows significantly higher serum RPR titers found in HIV-positive patients (range 1:64-1:16,348). Although it was previously suggested that the complex interaction of syphilis and HIV infection may cause non-treponemal tests such as VDRL or RPR serology to become falsely positive or negative, in general it has been suggested that non-treponemal test titers may be higher among HIV-positive patients than among HIV-negative patients [15-23]. Nevertheless, the high serum RPR titers in our HIV-positive patients are unlikely to be falsely positive because the diagnosis of ocular syphilis of all of our patients was supported by positive serum treponemal tests. In addition, the patients' high RPR titers decreased following IV penicillin treatment in those who were retested, and ocular inflammation and vision improved after treatment. While HIV is considered to be one of the important causes of a false-positive reaction to serum non-treponemal test, in HIV-positive ...

*  Syphilis - Biotec
Diagnosis of syphilis usually depends on the demonstration of antibodies in the blood. Two types of antibody are produced in response to infection, non-specific anti-lipid reagin antibody and specific anti-treponemal antibody. These appear soon after infection; the specific anti-treponemal antibodies may persist for many years, whereas the reagin antibodies may disappear after treatment.. Syphilis tests can be grouped into two categories. Non-treponemal tests detect non-specific treponemal antibody and include VDRL and RPR tests, these are used primarily as screening tests. Treponemal tests detect specific treponemal antibody and include TPHA, FTA-Abs, ELISA and Rapid Devices. These may also be used for screening and as confirmatory tests.. ...

*  Syphilis Returns to China … With a Vengeance : Sexually Transmitted Diseases
Syphilis was first recognized in China in the 1500s and was most likely brought to China by Portuguese traders.1 By the time of the "liberation" of China in 1949, syphilis was one of the most common causes for medical care, and prevalence surveys revealed a massive epidemic in sex workers and national minority populations.2. The Chinese Communist government made treatment and prevention of syphilis a priority3 and inspired a prolonged and successful but draconian "campaign" to accomplish this goal. By 1964, syphilis was very rare,4 and the infection was probably virtually eliminated until China opened its international borders to Western tourists and commerce in the early 1980s. The first resurgent cases of syphilis were recognized in China in 1979,5 and a national surveillance system has demonstrated a steady and disturbing spread of the disease throughout the country.6. In this issue of STD, 2 articles ...
