Identifying predictors of high quality care in English general practice: observational study | The BMJ
Four variables stood out as predictors of quality of care. The largest effect was the relation between the booking interval for routine consultations and the quality of management of chronic disease. Other authors have emphasised the importance of adequate time for consultations.23 The effect was greater for asthma than for diabetes and angina, possibly because the last two conditions are more likely to be treated in separate clinics than in routine surgeries. These data provide strong support for the view that general practice should be structured to allow time for the increasing complexity of the work required of general practitioners.. Secondly, we found significant associations between size of practice and quality of care, as has been seen in other studies, 19 24 although the relation was not simple. Smaller practices scored better than larger ones for access to care, but for diabetes care larger practices had higher scores than smaller ones. This emphasises that no single type of practice ...

The environmental epidemiology of atrial arrhythmogenesis | Journal of Epidemiology & Community Health
Atrial fibrillation (AF) is the most common cardiac arrhythmia seen in clinical practice, and makes an important contribution to cardiovascular disease (CVD) and all-cause mortality. The focus of AF research has recently shifted, from concentrating on treatments and complications, to the evaluation of putative risk factors including ambient air pollution. Although the present study pertains specifically to AF, much of its content is drawn from, and therefore is applicable to, the study of other arrhythmias, the conduct of which is confronted by many of the same challenges. Meeting these challenges involves recognising the collective importance of 1. large, ethnically and geographically diverse, clinically well-characterised populations; 2. methods for reducing uncertainty in outcome ascertainment, distinguishing effects of pervasive environmental exposures and improving their estimation; 3. approaches to evaluation of susceptibility; and 4. strategies for informing regulatory policies designed ...

Clinical risk management in obstetrics: eclampsia drills | BMJ Quality & Safety
Clinical risk management is recognised as an important component of obstetric clinical governance. In a report by the Department of Health, maternity care has been identified as an area for improvement.1 By 2005 the department would like the number of cases of negligent harm in obstetrics and gynaecology that result in litigation to be reduced by a quarter. The use of "fire drills" was advocated in the 1999 Confidential Enquiry into Maternal Deaths and Towards Safer Childbirth in anticipation of obstetric emergencies.2,3 Implementation of these drills is necessary for level 2 accreditation by the Clinical Negligence Scheme for Trusts, which conveys a 20% discount in liability premiums for UK trusts.4. Simulation is useful for training both doctors and midwives to manage obstetric crises.5,6 Training with high fidelity simulation has been shown to improve the speed with which anaesthetists respond to emergencies and the quality of their care.7 Simulation can also be used to rate technical skills ...
