To investigate the abnormality of cerebral perfusion in narcoleptics with cataplexy, cerebral perfusion was measured by brain single photon emission computed tomography(SPECT). The difference of cerebral perfusion between narcoleptics and normal controls was tested by statistical parametric mapping(SPM) analysis. Twenty-five narcoleptics with cataplexy and 25 normal controls performed 99mTc-ethylcysteinate dimer brain SPECT study. A night polysomnography and multiple sleep latency test (MSLT) in next day were performed in all patients. Brain SPECT was carried out in all patients and normal controls during waking state. The clinical symptoms and the results of MSLT were concordant to the International Classification of Sleep Disorders criteria for narcolepsy in all patients. The possibility of coagulopathy in Babesia canis rossi infections in the canine patient has been suggested in the literature, but minimal work has been done to evaluate the clinicopathological nature of it in further detail. Pulmonary thromboembolism (PTE) has not yet been implicated in canine babesiosis (CB), but may also be one of the causes of the sudden dyspnoea and tachypnoea that are frequently seen in complicated CB patients. The objective of this study was to prospectively evaluate the scintigraphic pulmonary perfusion pattern in hospitalised dogs with babesiosis in an attempt to ascertain whether a scintigraphic pattern consistent with clinically relevant PTE does indeed occur in these patients. The subject was a 65-year-old woman with chest pain. An electrocardiogram revealed T-wave-inversion in leads III, aVF, V1-V5. 99mTc-tetrofosmin myocardial SPECT showed mildly reduced uptake in the anteroseptal wall and the apex. These findings suggested acute myocardial ischemia. Coronary angiography did not show any stenotic lesions, but diffuse coronary ectasia was noted in three vessels. Coronary flow velocity was remarkably reduced on coronary angiography. Epicardial coronary spasm was not provoked by ergonovine loading test. Left ventriculography showed diffuse hypokinesis. 123I-BMIPP myocardial SPECT showed mildly reduced uptake in the anteroseptal wall and the apex on the early images. But 4-hour delayed images showed an increase of 8% in myocardial 123I-BMIPP uptake. We treated this patient with ticlopidine and nicorandil. METHODS Eighty-two patients (56 male and 26 female; mean age, 62 y) assessed for known or suspected infectious processes underwent 88 SPECT/CT studies. Forty-seven patients underwent GS SPECT/CT (13 with fever of unknown origin, 21 with suspected osteomyelitis, and 13 with suspected soft-tissue infection), and 35 patients underwent WBC SPECT/CT (24 with suspected vascular graft infection, and 11 with suspected osteomyelitis). Which meant plenty of kick-around time, and so I visited a few tool stores, but strangely felt agitated in each so that I didn't buy anything. Then to a used CD & book store to peruse the stock, and then to another tool store, one of my long term faves. This time, I was allowed to purchase a few items, having a list on hand. Then to an outdoor store and peruse the goods there, buy a few trinkets and then got held up at the cash register as there was an item I was looking for and the stockist went to look for. While waiting they switched the cashier, from female to male, and he did the friendly nattering thing while waiting for the stockist to report back on a certain item's availability. It wasn't to be had as it turned out. I suppose this is a more elaborate exercise in checkout obstruction/delay; wait for a determination from the stockist AND switch cashiers.
