Psychosocial adjustment of pediatric patients after allogeneic stem cell transplantation. (17/4235)

The purpose of this study was to assess the psychosocial adjustment of patients who had been treated with allogeneic stem cell transplantation (SCT) in our clinic. Selection criteria for patients were to be aged 14-30 years at the time of the follow-up, to be at least 2 years post-SCT and to have a very good knowledge of German. Among 31 eligible patients, 26 participated (84% response rate). The patients were between 15 and 27 years old and were on average 7 years (range 2-13) post-SCT. Research instruments consisted of a demographic questionnaire and various subscales of established psychological measures for which data from a sample of bone cancer survivors and population norms were available. About 35% of patients showed high levels of anxiety, 62% appeared to be extremely sensitive and vulnerable, and 35% showed strong, unfulfilled needs in their love lives. In the other domains tested (self-esteem, family and peer relationships, school/vocational performance, etc), no noticeable differences were found between the subjects and comparable populations. There was no significant association between psychosocial outcome and demographic features or clinical data. Our results suggest that patients who underwent SCT in their childhood or adolescence are at risk of developing long-term emotional or social problems. Due to the retrospective design of our study and the small sample size, no predictive factors for psychosocial distress could be identified.  (+info)

The extent of drug therapy for attention deficit-hyperactivity disorder among children in public schools. (18/4235)

OBJECTIVES: The purpose of this study was to determine the extent of medication use for attention deficit-hyperactivity disorder (ADHD) in southeastern Virginia. METHODS: Students enrolled in grades 2 through 5 in school districts in city A (n = 5767 students) and city B (n = 23,967 students) were included. Nurses recorded students who received ADHD medication in school. RESULTS: The proportion of students receiving ADHD medication was similar in both cities (8% and 10%) and was 2 to 3 times as high as the expected rate of ADHD. Receipt of drug therapy was associated with social and educational characteristics. Medication was used by 3 times as many boys as girls and by twice as many Whites as Blacks. Medication use increased with years in school, and by fifth grade 18% to 20% of White boys were receiving ADHD medication. Being young for one's grade was positively associated with medication use (P < .01). The prevalence of ADHD was 12% in district A, 63% in district B. CONCLUSIONS: These findings suggest that criteria for diagnosis of ADHD vary substantially across US populations, with potential overdiagnosis and overtreatment of ADHD in some groups of children.  (+info)

Estimating the prevalence of disordered gambling behavior in the United States and Canada: a research synthesis. (19/4235)

OBJECTIVES: This study developed prevalence estimates of gambling-related disorders in the United States and Canada, identified differences in prevalence among population segments, and identified changes in prevalence over the past 20 years. METHODS: A meta-analytic strategy was employed to synthesize estimates from 119 prevalence studies. This method produced more reliable prevalence rates than were available from any single study. RESULTS: Prevalence estimates among samples of adolescents were significantly higher than estimates among samples of adults for both clinical (level 3) and subclinical (level 2) measures of disordered gambling within both lifetime and past-year time frames (e.g., 3.9% vs 1.6% for lifetime estimates of level 3 gambling). Among adults, prevalence estimates of disordered gambling have increased significantly during the past 20 years. CONCLUSIONS: Membership in youth, treatment, or prison population segments is significantly associated with experiencing gambling-related disorders. Understanding subclinical gamblers provides a meaningful opportunity to lower the public health burden associated with gambling disorders. Further research is necessary to determine whether the prevalence of disordered gambling will continue to increase among the general adult population and how prevalence among adolescents will change as this cohort ages.  (+info)

Twelve-year tracking of blood pressure in Korean school children: the Kangwha Study. (20/4235)

Longitudinal data from different populations have shown different degrees of tracking of blood pressure (BP). To examine BP tracking in Korean school children, 219 of 430 children (100 males, 119 females) who were 6 years old in 1986 in Kangwha County, Korea have been examined annually up to 1997 in the Kangwha Study. BP was measured twice with standard mercury sphygmomanometers and the average of the two measurements was used for the level of BP. Diastolic BP were measured at the fourth Korotkoff sound. Tracking was examined using a time-lag correlation analysis and McMahan's tracking index tau, which indicates the proportion of variation attributable to tracking apart from the natural growth component. As well the jackknife method was used to obtain the confidence interval of tau. Correlation coefficients between systolic BP from age 6 to 17 ranged from 0.39-0.54 for males and 0.44-0.57 for females. Taus for systolic BP were 0.875 (95% CI: 0.803-0.947) and 0.900 (95% CI: 0.809-0.991) in males and females, respectively. Correlation coefficients between diastolic BP from age 6 to 17 ranged from 0.28-0.47 for males and 0.14-0.47 for females. Taus for diastolic BP were 0.983 (95% CI: 0.897-1.000) and 0.800 (95% CI: 0.717-0.883) in males and females, respectively. These findings showed strong evidence for BP tracking in Korean school children from childhood to late adolescence.  (+info)

Medical informatics education: the University of Utah experience. (21/4235)

The University of Utah has been educating health professionals in medical informatics since 1964. Over the 35 years since the program's inception, 272 graduate students have studied in the department. Most students have been male (80 percent) and have come from the United States (75 percent). Students entering the program have had diverse educational backgrounds, most commonly in medicine, engineering, computer science, or biology (59 percent of all informatics students). A total of 209 graduate degrees have been awarded, with an overall graduation rate of 87 percent since the program's start. Alumni are located in the United States (91 percent) and abroad (9 percent); half (51 percent) have remained in Utah. Former students are employed in a wide variety of jobs, primarily concerned with the application of medical informatics in sizable health care delivery organizations. Trends toward increasing managerial responsibility for medical informatics graduates and the emergence of the chief information officer role are noted.  (+info)

Environmental factors and asthma and allergy in schoolchildren from Western Australia. (22/4235)

The association of environmental factors with atopic disease in children remains poorly understood. The aim of this study was to investigate the relationship between socio-environmental factors and symptoms of asthma and atopy in 6-7-yr-old children assessed as an adjunct to Phase I of the International Study of Asthma and Allergy in Childhood in Perth, Western Australia. Parental questionnaire responses were obtained for 2,193 children (73.6%) in 34 randomly selected primary schools in the Perth metropolitan area. Children born in Australia had a significantly increased risk of current asthma (odds ratio (OR) 237, p = 0.001). Having a mother born in Australia was the only factor independently associated with an increased risk of current hay fever (OR 1.56, p = 0.005). Increasing numbers of people living in the home were significantly associated with a multiplicative decrease in risk of current asthma (OR 0.88, p = 0.03) and eczema (OR 0.82, p = 0.01). Houses made of fibrocement (OR 2.40, p = 0.02) and the presence of mats on less than half of the floor area in the "main bedroom" (relative to wall-to-wall carpet) (OR 3.50, p = 0.003) were associated with an increased risk of current eczema. All reported associations were independent of socioeconomic status (categorized by school), age and sex. This study suggests that household and country-specific environmental factors are associated with asthma, hay fever and eczema risk in 6-7-yr-old schoolchildren, and may have substantially contributed to the increased prevalence of these diseases in Australia.  (+info)

Predictors of success in undergraduate human physiology. (23/4235)

This study tested the hypothesis that measurable attributes in students' backgrounds are related to their successful completion of an undergraduate human physiology course. Demographic, general academic performance, and science achievement data were obtained from student records for students enrolled during the 1995-1996 academic year, and additional demographic data were obtained from students enrolled during the 1996-1998 academic years. A hierarchical logistic regression analysis explored the relationship fo these variables to the percentage of students passing the human physiology course. Predicted passing versus failing showed a sensitivity of 85.5% and specificity of 69.7%. Two independent validations of the logistical regression equation correctly predicted the performance of subsequent groups of students 75.9% and 77.6% of the time.  (+info)

Undergraduate students' misconceptions about respiratory physiology. (24/4235)

Approximately 700 undergraduates studying physiology at community colleges, a liberal arts college, and universities were surveyed to determine the prevalence of our misconceptions about respiratory phenomena. A misconception about the changes in breathing frequency and tidal volume (physiological variables whose changes can be directly sensed) that result in increased minute ventilation was found to be present in this population with comparable prevalence (approximately 60%) to that seen in a previous study. Three other misconceptions involving phenomena that cannot be experienced directly and therefore were most likely learned in some educational setting were found to be of varying prevalence. Nearly 90% of the students exhibited a misconception about the relationship between arterial oxygen partial pressure and hemoglobin saturation. Sixty-six percent of the students believed that increasing alveolar oxygen partial pressure leads to a decrease in alveolar carbon dioxide partial pressure. Nearly 33% of the population misunderstood the relationship between metabolism and ventilation. The possible origins of these respiratory misconceptions are discussed and suggestions for how to prevent and/or remediate them are proposed.  (+info)
