Case report: Iridium 192-health effects during 20 years after irradiation. (1/170)
This Case Report has presented health effects of high level of irradiation with gamma rays from Ir 192m on the patient M. L. during 21 years after the accident. The main purpose of this investigation was to analyze long time consequences of partial high-level irradiation on human health. Locally, short-term irradiation, with high deadly dose, caused acute radiation syndrome with reversible disorder function of the individual, most exposed, organs. Frequency of chromosomal aberrations (dicentric), characteristic for direct irradiation, was increased. However, dicentric analyzes did not give expected result for an acute radioactivity illness. Radio dermatitis had begun already after the incident, while changes on the heart began not earlier than six months and gradually have been increased up to two years after the incident. Irradiated parts of the patient skin have been replaced with auto-transplanted skin and appropriate therapy had been taken. More than two decades after the irradiation, system (leukemia) nor solitary tumor on near organs (liver, heart, lungs, bones) have not been appeared. Radiation illness did not happen in spite of very large dose, because impacts had been local and body was unlikely irradiated. The in vitro antioxidant and antimicrobial activities of two Achillea millefolium (Adanson) Koch s.l species essential oils (A. collina Becker ex Heimerl s.l. and A. pannonica Scheele, Asteraceae) originating from the Golija and Radan mountains (Serbia) were investigated. The chemical profiles of the essential oils were evaluated by GC-MS. Antioxidant activity was assessed as free radical scavenging capacity (RSC) towards 2,2-diphenyl-1-picrylhydrazil (DPPH) radicals, together with effects on lipid peroxidation (LP). Antibacterial activity was examined on 21 bacterial strains. Based on the chemical composition of the essential oil, A. collina s.l. from Mount Golija was classified as a chamazulene chemotype (tetraploid). The high percentage of oxygenated monoterpenes and absence of azulene in the essential oil obtained from A. pannonica from Radan pointing that this population is octaploid. Essential oil of A. pannonica expressed stronger antimicrobial activity on almost all tested bacteria. Furthermore, this essential oil expressed higher scavenging effects on DPPH radical (IC(50) = 0.52 comparing to 0.62 mug/mL). Only in the LP evaluation, essential oil of A. collina s.l. from Golija exhibited stronger antioxidant activity (IC(50) = 0.75 comparing to 2.12 mug/mL). OBJECTIVES: The aim of this study was to establish the morbidity structure among testators whose wills were challenged as well as to reveal if there is a specific relationship between certain diagnostic categories in the testators' health status and forensic psychiatry expert opinion on testamentary incapacity. SUBJECTS AND METHODS: The authors analyzed 156 consecutive forensic psychiatry reports on retrospective, determination of testamentary capacity made in the Forensic Psychiatry Centre, Institute of Psychiatry, Belgrade in the period 1965 - 2005. The wills covered by this study were mostly made by male, 65 years old or older testators, with primary education, who executed a holograph will and survived it for less than a year. RESULTS: Testamentary incapacity for medical reasons was established in 47% of the testators, while a strong, statistically significant relationship between a diagnostic category and testamentary incapacity was established among the testators suffering from an organic mental disorder (Chi-square=133.256, p=0.000) or a substance induced mental disorder (Chi-square=6.971, p=0.008). CONCLUSION: Testamentary capacity is a specific focus of medical assessment given that the evidence for overturning a will is generally dependent upon medical assessment. In that respect, much litigation and expenses could be avoided if medical experts were given a chance to correctly assess the testamentary capacity of a person at the time of making a will. The fact remains that our findings may not be a representative cross-section of the general population, and this important issue certainly deserves to be addressed by future research based on a larger sample.
