Commentary: Emerging and other communicable diseases. (1/64)

There is an increasing need for integrated, sustainable; and cost-effective approaches to the management of infectious diseases. For example, an emerging disease in one country may already be endemic in another country but nearing elimination in a third. A coordinated approach by WHO towards infectious diseases is therefore needed that will facilitate more effective support of on-going efforts for the prevention and control of endemic diseases, intensify efforts against those diseases targeted for eradication and elimination, and result in better preparedness and response to new and re-emerging diseases. In order to meet these challenges, WHO has created a new Programme on Communicable Diseases (CDS), which will replace the former Division of Emerging and other Communicable Diseases (EMC). The new Programme will better integrate surveillance, prevention, control, and research over the whole spectrum of communicable diseases. CDS will function as focal point for global data and information exchange on infectious diseases, and inter alia, will reinforce laboratory-based surveillance of bacterial, viral, and zoonotic diseases to ensure early detection of threats to international public health. Changes in susceptibility to infectious disease, increased opportunities for infection, and the ability of microbes to adapt rapidly will continue to challenge WHO to improve prevention and control of infectious diseases in the future by establishing strong partnerships with both the private and public sectors.  (+info)

An approach to the problems of diagnosing and treating adult smear-negative pulmonary tuberculosis in high-HIV-prevalence settings in sub-Saharan Africa. (2/64)

The overlap between the populations in sub-Saharan Africa infected with human immunodeficiency virus (HIV) and Mycobacterium tuberculosis has led to an upsurge in tuberculosis cases over the last 10 years. The relative increase in the proportion of notified sputum-smear-negative pulmonary tuberculosis (PTB) cases is greater than that of sputum-smear-positive PTB cases. This is a consequence of the following: the association between decreased host immunity and reduced sputum smear positivity; the difficulty in excluding other HIV-related diseases when making the diagnosis of smear-negative PTB; and an increase in false-negative sputum smears because of overstretched resources. This article examines problems in the diagnosis and treatment of smear-negative PTB in high-HIV-prevalence areas in sub-Saharan Africa. The main issues in diagnosis include: the criteria used to diagnose smear-negative PTB; the degree to which clinicians actually follow these criteria in practice; and the problem of how to exclude other respiratory diseases that can resemble, and be misdiagnosed as, smear-negative PTB. The most important aspect of the treatment of smear-negative PTB patients is abandoning 12-month "standard" treatment regimens in favour of short-course chemotherapy. Operational research is necessary to determine the most cost-effective approaches to the diagnosis and treatment of smear-negative PTB. Nevertheless, substantial improvement could be obtained by implementing the effective measures already available, such as improved adherence to diagnostic and treatment guidelines.  (+info)

Mathematical modelling of patients flow through an accident and emergency department. (3/64)

OBJECTIVES: The objectives of this project; (1) to evaluate the method, (2) to assess the information required for a more detailed model, and (3) to determine if it was worthwhile to undertake the data collection needed for a more detailed model. METHODS: A mathematical model was constructed using the operational research method of discreet event simulation. The effect of different SHO shift patterns on waiting time was assessed with the model. RESULTS: The model constructed was not an accurate representation of patient flow because of the large number of assumptions that had to be made in this preliminary model. However, the model predicted that an SHO shift pattern that more closely matched the patient arrival pattern would produce shorter waiting times. CONCLUSIONS: This method can be applied to an accident and emergency department. Extension of this approach with the collection of additional data and the development of more sophisticated models seems worthwhile.  (+info)

Calculation of relative weights for nuclear medicine procedures. (4/64)

Relative weights for nuclear medicine procedures have been assigned previously on the basis of subjective estimates of resources expended. Thus, ultimate values frequently have not reflected time and cost of personnel and equipment accurately. Based on computerized data collected on over 8,000 patients, we have attempted to calculate relative weights more objectively by analyzing the following parameters: (A) physician time; (B) technologist time; (C) nurse (or aide) time; (D) equipment time and depreciation; and (E) radiopharmaceutical cost, or cost equivalent. Most major organ scans were found to have quite similar values whereas procedures with low ratings on previous value schedules were found to have high relative values when analyzed objectively.  (+info)

Research that influences policy and practice - characteristics of operational research to improve malaria control in Mpumalanga Province, South Africa. (5/64)

BACKGROUND: Much communicable disease control research has had little impact on local control programme policy and practice for want of an operational component. The operational research model - the systematic search for knowledge on interventions, tools or strategies that enhance programme effectiveness - is gaining recognition as an appropriate method for addressing perplexing questions within public health programmes. METHODS: A series of operational research studies were conducted to refine malaria diagnosis in Mpumalanga Province, South Africa between 1995 and 1999. The grounded theory approach was used with groups of experienced Masters of Public Health students in South Africa and Australia to analyse a compilation of these studies for determining positive and negative attributes of operational research that affect its ability to influence communicable disease control policy and practice. RESULTS: The principal positive attributes of the operational research studies were high local relevance, greater ability to convince local decision-makers, relatively short lag-time before implementation of findings, and the cost-effective nature of this form of research. Potential negative features elicited included opportunities forfeited by using scarce resources to conduct research and the need to adequately train local health staff in research methodology to ensure valid results and accurate interpretation of findings. CONCLUSIONS: Operational research effectively influenced disease control policy and practice in rural South Africa, by providing relevant answers to local questions and engaging policy-makers. This resulted in accelerated inclusion of appropriate measures into a local communicable disease control programme.  (+info)

Adherence to tuberculosis treatment: lessons from the urban setting of Delhi, India. (6/64)

The Revised National Tuberculosis Control Programme (RNTCP), which incorporated the WHO DOTS strategy was introduced in India in the mid-1990s. An operational research project was conducted between 1996 and 1998 to assess the needs and perspectives of patients and providers in two chest clinics in Delhi, Moti Nagar and Nehru Nagar, during the introduction of the new strategy. This paper reports on the findings of the project, concentrating on information collected from 40 in-depth interviews with patient defaulters and from non-participant observations in clinics and directly observed treatment centres. In Moti Nagar chest clinic, 117 of 1786 (6.5%) patients and 195 of 1890 (10%) patients in Nehru Nagar left care before their treatment was complete. It was argued that the reasons for default stem from a poor correlation between patient and programme needs and priorities, and from particular characteristics of the disease and its treatment. Patient needs that were not met by the health system included convenient clinic timings, arrangements for the provision for treatment in the event of a family emergency and provision for complicated cases like alcoholics. The problems facing the provider were poor interpersonal communication with the health staff, lack of attention and support at the clinic, difficulty for patients to re-enter the system if they missed treatment and, in certain areas, long distances to the clinic. Problems related to diseases were inability of the staff to deal with drug side-effects, and patients' conception of equating well-being with cure. Simple, practical measures could improve the provision of tuberculosis (TB) treatment: more flexible hours, allowances for poor patients to reach the clinics and training health care staff for respectful communication and monitoring drug side-effects. The findings indicate a need to rethink the label of 'defaulter' often given to the patients. The important areas for future operational research is also highlighted.  (+info)

Introducing operations research into management and policy practices of a non-governmental organization (NGO): a partnership between an Indian leprosy NGO and an international academic institution. (7/64)

This paper reports on a partnership between LEPRA, a non-governmental organization (NGO), and the London School of Hygiene and Tropical Medicine (LSHTM) to explore the feasibility and appropriateness of incorporating operations research into the management and decision-making of a leprosy NGO. A pilot study in Orissa was used to determine the advantages and disadvantages of introducing operations research to assist in decision-making and programme implementation within the organization. The results highlight the difficulty and complexity of the process, but point to several important themes: partnership, changing perspectives, use of time and priority-setting, identification of gaps in systems, and building institutional and personal capabilities. The results of the study provide support to encourage NGOs to become actively involved in research. Because of their work and service to local communities, NGOs have the opportunity to collect information about the perceptions, resources and constraints of individuals, families and the communities themselves in accessing appropriate care. Their proximity to communities gives them a feeling of responsibility for ensuring that this information is translated to the district, national and ultimately international level. This will help to ensure the creation of appropriate infectious disease control policies that support the needs of patients. 'Outside' academic institutions can help NGOs to facilitate this up-stream flow of information from the local to the national and international level, to help to ensure that international disease control policies are appropriately serving local communities.  (+info)

Artifacts and collaborative work in healthcare: methodological, theoretical, and technological implications of the tangible. (8/64)

Although modeled as knowledge work with emphasis on data flow and decision making, healthcare is delivered in the context of a highly structured physical environment, with much effort and emphasis placed on physical and spatial arrangement and re-arrangement of workers, patients, and materials. The tangible aspects of highly collaborative healthcare work have profound implications for research and development of information and communication technology (ICT) despite the tendency to model work as flow of abstract data items. This article reviews field studies in healthcare and other domains on the role of artifacts in collaborative work and draws implications in three areas: methodological, theoretical, and technological. In regard to methodologies, assessment of new ICT and development of user requirements should take into account how artifacts are used and exploited to facilitate collaborative work. In regard to theories, the framework of distributed cognition provides a starting point for modeling the contribution and exploitation of physical artifacts in supporting collaborative work. In regard to technology, design and deployment of new technology should support the functions provided by physical artifacts replaced or disrupted by new technology, and profitable ways for new technology to support collaborative work by embedding ICT into existing infrastructure of physical artifacts.  (+info)
