The role of curriculum in influencing students to select generalist training: a 21-year longitudinal study. (1/77)

To determine if specific curricula or backgrounds influence selection of generalist careers, the curricular choices of graduates of Mount Sinai School of Medicine between 1970 and 1990 were reviewed based on admission category. Students were divided into three groups: Group 1, those who started their first year of training at the School of Medicine; Group 2, those accepted with advanced standing into their third year of training from the Sophie Davis School of Biomedical Education, a five-year program developed to select and produce students likely to enter primary care fields; and Group 3, those accepted with advanced standing into the third year who spent the first two years at a foreign medical school. All three groups took the identical last two years of clinical training at the School of Medicine. These were no significant differences with respect to initial choice of generalist training programs among all three groups, with 46% of the total cohort selecting generalist training. Of those students who chose generalist programs, 58% in Group 1, 51% in Group 2, and 41% in Group 3 remained in these fields rather than progressing to fellowship training. This difference was significant only with respect to Group 3. However, when an analysis was performed among those students providing only primary care as compared to only specialty care, there were no significant differences. Analysis by gender revealed women to be more likely to select generalist fields and remain in these fields without taking specialty training (P < .0001). Differentiating characteristics with respect to choosing generalist fields were not related to either Part I or Part II scores on National Board Examinations or selection to AOA. However, with respect to those specific specialties considered quite competitive (general surgery, obstetrics and gynecology, and ophthalmology), total test scores on Part I and Part II were significantly higher than those of all other students. The analysis indicated that, despite the diverse characteristics of students entering the third year at the School of Medicine, no one group produced a statistically greater proportion of generalists positions than any other, and academic performance while in medical school did not have a significant influence on whether a student entered a generalist field.  (+info)

The International Health Program: the fifteen-year experience with Yale University's Internal Medicine Residency Program. (2/77)

The purpose of this study was to assess the impact of international health electives on physicians-in-training. A retrospective study was conducted using an anonymous, self-administered mailed survey to internal medicine residents who trained at Yale from 1982 to 1996 based on their experience with our International Health Program (IHP). The response rate was 61%, with 96 completed surveys in the participant group and 96 completed surveys in the nonparticipant group. Participants were more likely than nonparticipants to care for patients on public assistance (77 versus 49; P < 0.001) and immigrant patients (41 versus 23; P = 0.006). Among residents who changed their career plans, participants (22) were more likely than nonparticipants (14) to switch from subspecialty medicine to general medicine (P = 0.02). Participants were significantly more likely to have a positive view of health care delivery in developing countries. Compared with nonparticipants (64), IHP participants (74) believed that the physical examination is under-used by physicians from the United States as a diagnostic skill (P = 0.03). International health experiences appeared to have an important impact on the decisions and attitudes of residents.  (+info)

Medical students exchange program organized by Croatian Medical Students International Committee, 1996-2000. (4/77)

AIM: To assess the interest of Croatian medical students in going to foreign countries and foreign medical students' interest in coming to Croatia for one-month professional and research clerkship in the period from 1996 to 2000. METHOD: Data were obtained from contracts between Croatian Medical Students' International Committee and other National Member Organizations of the International Federation of Medical Students' Associations; from student Application forms; reports of the Local Committees of Croatian Medical Students' International Committee summed up in a Statistical form and used for statistical analysis in the International Federation of Medical Students' Associations; and from Cards of acceptance and Cards of confirmation. RESULTS: The number of available bilateral exchange places increased from 28 in 1996 to 66 in 1999. Croatian students going for clerkship abroad used 65.8% of available places and foreign students realized 59.3% of all places available in Croatia. Most of the exchange students chose clinical, whereas only a small number chose preclinical clerkship. The most visited were the departments of medicine and general surgery. All the students preferred doing their clerkships in August. Most foreign students came from Italy, Spain, Slovakia, and Hungary. Croatian students preferred to go to Italy, Spain, Egypt, and Finland. CONCLUSION: Political problems strongly affected students' interest in doing clerkship in a certain country. Croatia needs to attract more students from different countries and to increase the interest of Croatian students in doing clerkship in Eastern European countries.  (+info)

The EXCEL Program: strengthening diversity. (5/77)

The Boston University Henry M. Goldman School of Dental Medicine (BUSDM) initiated a program in the summer of 1993 to strengthen diversity in the entering class of first-year students. The Experiential Center for Excellence in Learning (EXCEL) Program is a voluntary, one-month-long prematriculation experience that combines didactic, laboratory, study skills, and social activities to prepare participants to transition into the rigorous first-year curriculum. From 1996 to 2000, ninety students participated in EXCEL. The two primary reasons cited for participating were to become familiar with the school, faculty, and classmates and to strengthen basic science background. Participants' ages ranged from twenty to over forty. Fifty-nine percent of participants had been out of college for more than one year; 10 percent had been out of school for three years or more. Thirty percent listed nontraditional predental school majors. Fifty-six percent listed a country other than the United States as country of birth. Of those completing an exit survey, 96 percent reported that EXCEL strengthened their decision to study dentistry, and 97 percent would recommend that future entering BUSDM students participate in EXCEL. The EXCEL Program may serve as a model for increasing diversity in U.S. dental school enrollment.  (+info)

The health of the Russian people has deteriorated dramatically since the fall of communism, due particularly to cardiovascular disease. The Eurasian Medical Education Program was developed in response to provide continuing medical education for Russian physicians. Programs are directed mainly toward primary care physicians and focus on outpatient management of diseases that cause high rates of mortality and morbidity. This experience provides an opportunity to assess the structure and functioning of the Russian health care system and emphasizes the importance of general internal medicine training in detection, management, and prevention of disease complications.  (+info)

Temptation of academic medicine: second alma mater and "shared employment' concepts as possible way out? (7/77)

Apparently, in developing and in well-developed societies we are confronted with a crisis of academic medicine in all aspects: health care, teaching, and research. Health care providers in teaching hospitals are under pressure to generate revenues, academic research is pressed to keep pace with institutions devoted solely to research, and teaching is often understood not as privilege and honor but as burden and nuisance. The key problem and the principal cause of the crisis are low interest of the best young graduates to follow an academic career in a world where the benefits and values of the private sector are prevailing. Confronted with these circumstances and the continuous perils of permanent brain-drain, we developed an innovative concept of "shared employment' where two academic institutions (one in a developed and one in a developing country) will collaborate in development and support of fresh talents, building elite academic staff. Most academic exchange programs developed so far have proved to be ineffective and of poor vitality, in spite of loud exclamations, high expectations, and a huge amount of good will involved. In contrast, the suggested cooperation will be based exclusively on mutual interest and clearly defined benefits for all involved parties.  (+info)

International pediatric cardiac assistance in Croatia: results of the 10 year program. (8/77)

AIM: In an effort to help alleviate the lack of an adequate pediatric cardiac surgical service that existed in Croatia following 1991 the International Childrens Heart Foundation (ICHF) was asked to provide the necessary surgery. Initially, this project was undertaken as a humanitarian program to provide pediatric cardiac operations. After 5 years, the Republic of Croatia financially sponsored the program. The intended purpose of the Government Sponsored Program was to provide staff education, clinical services, and the development of an organized pediatric cardiac service team in country. The surgical results of the humanitarian program and the educational and surgical results of the Government sponsored program are reported. METHODS: Review of 3 separate databases maintained in country and the database of the ICHF in Memphis, Tennessee, was undertaken. Risk classification of the operations performed was performed using the Risk Adjustment in Congenital Heart Surgery method. RESULTS: A total of 32 trips (11 during the Humanitarian Program, and 21 during the Government Sponsored Program) were made between April 1993 and July 2003. A total of 601 primary operations (151 - Humanitarian Program, 450 - Government Sponsored Program) were preformed. Overall mortality in the cardiac surgical department for the Government Sponsored Program was 11%, non-risk adjusted. There were 4 senior surgeons, 3 surgical residents, 6 cardiac anesthesiologists, 2 pediatric intensivists, and 3 pediatric cardiologists involved in the educational program. CONCLUSIONS: The combination of humanitarian and government sponsored pediatric cardiac surgical missions provided 601 Croatian children with operations. Substantial progress was made in the areas of anesthesia, perfusion, and postoperative care in the cardiac surgical intensive care unit. Despite these improvements a number of issues still exist that prevent the development of an independently functioning full service pediatric cardiac program.  (+info)
