Cervical spine abnormalities associated with Down syndrome. (57/223)

Atlantoaxial instability (AAI) affects 10-20% of individuals with Down syndrome (DS). The condition is mostly asymptomatic and diagnosed on radiography by an enlarged anterior atlanto-odontoid distance. Symptomatic AAI, which affects 1-2% of individuals with DS, manifests with spinal cord compression. Cervical spondylosis, which is common in DS, also has the potential for cord damage but it has received less attention because paediatric populations were mostly studied. Forty-four Kuwaiti subjects with DS, whose ages were > or = 15 years, were evaluated clinically and radiographically. Lateral neck radiographs were taken in the neutral and flexion positions. Asymptomatic AAI was diagnosed in eight subjects (18%) and congenital anomalies of C1-2 were found in five (12%). Five patients had AAI in flexion only while three patients had it in both views. Three patients with AAI had odontoid anomalies contributing to the condition. When assessing AAI, the posterior atlanto-odontoid distance has to be considered because it indicates the space available for the cord. Cervical spondylosis was noted in 16 (36%) subjects. Degenerative changes increased with age, occurred earlier than in the normal population, and affected mostly the lower cervical levels. Half the patients with AAI had cervical spondylosis, a comorbidity that puts the cord at increased risk.  (+info)

Techniques in the treatment of craniovertebral instability. (59/223)

The techniques of craniovertebral region stabilization introduced and used by the senior author over the last 20 years are summarized. The lateral masses of atlas and axis are strong and largely cancellous in nature and can be used for direct implantation of screws. Opening up of the joint and placement of bone graft within the joint stabilizes the region and provides a large area for bone fusion. Distraction of the facets provides an opportunity to treat a range of congenital craniovertebral anomalies. The technique of exposure of the lateral mass of the atlas and axis and the atlantoaxial joint is technically relatively complex and needs precise understanding of anatomy of the vertebral artery and training with cadavers.  (+info)

Atlantoaxial subluxation in different intraoperative head positions in patients with rheumatoid arthritis. (60/223)

BACKGROUND: Disorders of the cervical spine are often observed in patients with rheumatoid arthritis (RA). However, the best head position for RA patients with atlantoaxial subluxation in the perioperative period is unknown. This study investigated head position during general anesthesia for the patients with RA and proven atlantoaxial subluxation. METHODS: During anesthesia of patients with RA and proven atlantoaxial subluxation, the authors used fluoroscopy to obtain a lateral view of the upper cervical spine in four different positions: the mask position, the intubation position, the flat pillow position, and the protrusion position. Copies of the still fluoroscopic images were used to determine the anterior atlantodental interval, the posterior atlantodental interval, and the angle of atlas and axis (C1-C2 angle). RESULTS: The anterior atlantodental interval was significantly smaller in the protrusion position (2.3 mm) than in the flat pillow position (5.1 mm) (P < 0.05). The posterior atlantodental interval was significantly greater in the protrusion position (18.9 mm) than in the flat pillow position (16.2 mm) (P < 0.05). The C1-C2 angle was, on average, 9.3 degrees greater in the protrusion position than in the flat pillow position (P < 0.05). CONCLUSION: This study showed that the protrusion position using a flat pillow and a donut-shaped pillow during general anesthesia reduced the anterior atlantodental interval and increased the posterior atlantodental interval in RA patients with atlantoaxial subluxation. This suggests that the protrusion position, which involves support of the upper cervical spine and extension at the craniocervical junction, might be advantageous for these patients.  (+info)

Preoperative evaluation of extension capacity of the occipitoatlantoaxial complex in patients with rheumatoid arthritis: comparison between the Bellhouse test and a new method, hyomental distance ratio. (61/223)

BACKGROUND: The authors devised a new method, the hyomental distance ratio (HMDR), for preoperatively identifying patients with a reduced occipitoatlantoaxial extension capacity, which was defined as the ratio of the hyomental distance in head extension position to that in the neutral position. They compared the accuracy of the HMDR with that of the Bellhouse test in 40 patients with rheumatoid arthritis. METHODS: Each patient wearing goggles on which a goniometer was mounted sat upright with the head in the neutral position and then extended the head maximally. The angle of the goggles and the hyomental distance were measured in the two head positions, and a lateral cervical radiograph was taken simultaneously. The Bellhouse angle was defined as a difference in the angles of the goggles between these positions. RESULTS: Median values of the radiologic occipitoatlantoaxial extension angle and the Bellhouse angle were 11.2 degrees and 24.9 degrees , respectively. In 21 of 40 patients, the radiologic occipitoatlantoaxial extension angle was less than 12 degrees (reduced occipitoatlantoaxial extension capacity). In these patients, extension of the median angle of 16.4 degrees occurred at the subaxial regions and was greater than that of 8.5 degrees in patients with a radiologic occipitoatlantoaxial extension angle of 12 degrees or more (P < 0.01). As a result, a strong relation between the Bellhouse angle and radiologic occipitoatlantoaxial extension angle was not established (P < 0.01, r = 0.48). In contrast, the HMDR correlated well with the radiologic occipitoatlantoaxial extension angle (P < 0.0001, r = 0.88). The areas under the receiver operating characteristic curve of the Bellhouse test and the HMDR were 0.72 and 0.95, respectively. CONCLUSIONS: The HMDR was a good predictor of a reduced occipitoatlantoaxial extension capacity in patients with rheumatoid arthritis, but the Bellhouse test was not a clinically reliable method.  (+info)

Anterior retropharyngeal fixation C1-2 for stabilization of atlantoaxial instabilities: study of feasibility, technical description and preliminary results. (62/223)

Posterior transarticular screw fixation C1-2 with the Magerl technique is a challenging procedure for stabilization of atlantoaxial instabilities. Although its high primary stability favoured it to sublaminar wire-based techniques, the close merging of the vertebral artery (VA) and its violation during screw passage inside the axis emphasizes its potential risk. Also, posterior approach to the upper cervical spine produces extensive, as well as traumatic soft-tissue stripping. In comparison, anterior transarticular screw fixation C1-2 is an atraumatic technique, but has been neglected in the literature, even though promising results are published and lectured to date. In 2004, anterior screw fixation C1-2 was introduced in our department for the treatment of atlantoaxial instabilities. As it showed convincing results, its general anatomic feasibility was worked up. The distance between mid-sagittal line of C2 and medial border of the VA groove resembles the most important anatomic landmark in anterior transarticular screw fixation C1-2. Therefore, CT based measurements on 42 healthy specimens without pathology of the cervical spine were performed. Our data are compiled in an extended collection of anatomic landmarks relevant for anterior transarticular screw fixation C1-2. Based on anatomic findings, the technique and its feasibility in daily clinical work is depicted and discussed on our preliminary results in seven patients.  (+info)

Surgical treatment of subaxial cervical myelopathy in rheumatoid arthritis. (63/223)

Between 1978 and 1988 a total of 27 operations were performed on 26 patients for cervical myelopathy due to rheumatoid disease in the subaxial spine. Three different causes were recognised: the first group had cord compression due to subluxation of the cervical spine itself (6 patients); the second had cord compression occurring from in front, with rheumatoid lesions of vertebral bodies or discs (6); the third had compression from behind the cord due to granulation tissue within the epidural space (14). Group I was treated by closed reduction of the subluxation followed by surgical fusion either from in front or behind. Group II was decompressed by subtotal resection of the involved vertebral bodies and discs, followed by interbody fusion. The patients in group III were decompressed by laminectomy and excision of fibrous granulation tissue from the epidural space. Good recovery of neurological function was observed after 18 of the operations, fair recovery after five, poor recovery followed three, and one was worse. Myelopathy recurred in four patients, all of whom had had anterior interbody fusion.  (+info)

Craniovertebral malformation complex in a child with Weismann-Netter-Stuhl syndrome. (64/223)

OBJECTIVE: Bowing of the legs is usually thrown into the basket of vitamin D deficiency rickets; therefore, a significant number of affected children can be misdiagnosed and improperly managed. This case illustrates how the careful clinical and radiological assessment of such a case can lead to the adequate understanding of its etiology. DESCRIPTION: We report a sporadic case of a 2-year-old male child who presented with radiological features that were compatible with Weismann-Netter-Stuhl syndrome. In addition, we observed craniovertebral malformation complex. He was of normal intelligence. To our knowledge, the combination of Weismann-Netter-Stuhl syndrome and presence of a hypoplastic occipitalized atlas and further C2-C3 fusion has not been reported before. The diagnosis of Weismann-Netter-Stuhl is discussed. Classically, Weismann-Netter-Stuhl syndrome is characterized by short stature, mental retardation (in some individuals), dural calcification, and anterior bowing of the tibiae. However, we believe that careful clinical and radiological examinations can reveal more striking data which might positively reflect on the whole process of management. COMMENTS: We postulate that the congenital limitations in neck movements in our patient developed because of the marked fusion of the hypoplastic and occipitalized atlas and simultaneous C2-C3 fusion. Therefore, if this form of malformation is disregarded, there may be involvement of the atlantoaxial structure, and this can possibly lead to serious neurological and even life-threatening complications. The use of CT scanning for the detection of such abnormalities can be remarkably important.  (+info)
