The two most common causes of lower back pain in Malaysia are slipped disc and facet joint injury. Most of the time, these two conditions can co-exist and can cause severe pain in the back, buttocks or lower limbs. It is challenging to differentiate clinically between a slipped disc and facet joint injuries. There are a few clinical examinations which can be served as a guide to distinguish between a disc damage and facet joint injury. However, these examinations must be correlated with other clinical signs and symptoms to make a definite diagnosis ...

As a part of the three-column structure of vertebrae, facet joints play a key role in maintaining the stability of spinal motion, particularly in the lumbar area [24]. Therefore, most doctors are concerned with the biomechanical and mechanical factors affecting lumbar facet joints. However, the pathogenesis of FJOA has not been fully elucidated. Accordingly, in this study, we evaluated the expression of adiponectin in lumbar facet joints. Our results showed that adiponectin expression was significantly downregulated with increasing degeneration of the lumbar facet joint.. Adiponectin is an adipocyte factor specifically secreted from fat cells. Adiponectin is composed of 244 amino acids; the N-terminus contains one secretory signal sequence, and the C-terminus contains a spherical egg white function threshold. Previous studies have shown that adiponectin is involved in glucose homeostasis, insulin sensitivity, and vascular inflammatory diseases [25, 26]. Adiponectin in human peripheral blood ...

When these facet joints are injured patients can experience extreme discomfort. This condition is called cervical facet joint syndrome.
Background The most definitive diagnosis of neck pain caused by facet joints can be obtained through cervical medial branch blocks (CMBBs). However, intravascular injections need to be carefully monitored, as they can increase the risk of false-negative blocks when diagnosing cervical facet joint syndrome. In addition, intravascular injections can cause neurologic deficits such as spinal infarction or cerebral infarction. Digital subtraction angiography (DSA) is a radiological technique that can be used to clearly visualize the blood vessels from surrounding bones or dense soft tissues. The purpose of this study was to compare the rate of detection of intravascular injections during CMBBs using DSA and static images obtained through conventional fluoroscopy. Methods Seventy-two patients were included, and a total of 178 CMBBs were performed. The respective incidences of intravascular injections during CMBBs using DSA and static images from conventional fluoroscopy
There are several advantages of ultrasound imaging for performing cervical zygapophysial joint nerve blocks compared with the conventional fluoroscopic technique. Ultrasound guidance avoids radiation exposure and can be performed outside lead shielded facilities. Although radiation exposure of a single fluoroscopic-guided nerve block may be low, usually several blocks on different occasions are required to identify the symptomatic joint or to rule out zygapophysial joint-mediated pain, which may lead to considerable radiation exposure of patients and staff.11 An additional advantage is that the actual target (i.e. , the zygapophysial joint supplying nerve, may be identified in most people using ultrasound imaging,4 which obviously is not the case with fluoroscopy. Although our study was performed primarily in young and nonobese volunteers, our results indicate that ultrasound-guided blocks are accurate when the bony landmark of the fluoroscopic image is considered as the gold standard. Facet joint injections are done with the help of fluoroscopic guidance. The fluoroscope is an x-ray machine that allows the doctor to actually see an x-ray image while doing the procedure.
Facet joint block injections can confirm whether the facet joints are indeed the source of pain and can help relieve the pain and inflammation.
Most importantly, manipulation and exercise are indicated as the treatments of choice based on this research. Bogduk and co-researchers discovered by using a double anesthetic block technique that the posterior zygapophyseal joints were primary pain generators in chronic neck pain patients who had suffered a whiplash.1 The double block procedure rules out a placebo response, by using a short acting and long acting anesthetic in successive treatments. The patients must report both short acting relief and long acting relief that is consistent with the anesthetic they received. This occurred in 60 percent of the patients, thus proving the existence of posterior zygapophyseal joint pain in 60 percent of individuals with chronic neck pain after whiplash. Watson showed that decreased isometric strength and endurance of the deep neck flexors (longus collis and capitus) is correlated with chronic headaches.2 Measurement was with a simple strain guage.2 Treleaven and Jull showed that neck flexor ...

Facet joint arthrosis is an intervertebral disc disorder. The facet joints or zygapophyseal joints are synovial cartilage covered joints that limit the movement of the spine and preserve segmental stability. In the event of hypertrophy of the vertebrae painful arthrosis can occur. The lumbar facet arthrosis syndrome was described in a 1987 article by S. M. Eisenstein and C. R. In the absence of a reference standard, MBBs actually serve more of a prognostic than diagnostic role, enabling the selection of patients who might respond to radiofrequency denervation treatment the standard treatment for facet joint pain. Using double blocks reduces the false-positive rate of MBBs, but will invariably reduce the overall treatment success rate. Lumbar radiofrequency neurotomy uses heat to create a lesion (damaged area) on the medial nerve. The lesion impairs the medial nerves ability to transmit signals about facet joint pain. Because the nerve is turned off, pain is not felt.. A lumbar radiofrequency neurotomy is an outpatient procedure. You will wear a gown for the procedure and be positioned lying face down on a table. You will receive relaxation medicine before your procedure begins. Your lower back area will be sterilized and numbed with an anesthetic medication.. Your doctor will use a live X-ray image (fluoroscopy) to carefully insert and guide a needle-like tube (cannula) to the affected medial nerve. A small needle-like electrode (radiofrequency electrode) is inserted through the cannula. To ensure the cannula is in the correct position, a very mild electrical current is delivered through the electrode to the nerve. The nerve will briefly conduct pain signals and cause a muscle twitch, confirming that the correct nerve is ...
The facet joints, (or zygapophysial joints, zygapophyseal, apophyseal, or Z-joints) are a set of synovial, plane joints between the articular processes of two adjacent vertebrae. There are two facet joints in each spinal motion segment and each facet joint is innervated by the recurrent meningeal nerves. The biomechanical function of each pair of facet joints is to guide and limit movement of the spinal motion segment. In the lumbar spine, for example, the facet joints function to protect the motion segment from anterior shear forces, excessive rotation and flexion. Facet joints appear to have little influence on the range of side bending (lateral flexion). These functions can be disrupted by degeneration, dislocation, fracture, injury, instability from trauma, osteoarthritis, and surgery. In the thoracic spine the facet joints function to restrain the amount of flexion and anterior translation of the corresponding vertebral segment and function to facilitate rotation. Cavitation of the synovial ...
A facet joint injection usually takes less than 30 minutes and is performed using local anaesthetic. A small drip (called a cannula) may be inserted in the back of your hand so that we can give you drugs immediately should you feel unwell. The needle is very fine so you may not feel a thing when it is inserted. A little opening on the cannula allows fluids to be administered from an IV bag.. We will help you get into position for the injection. An antiseptic will be used to clean the area and a local anaesthetic will be injected. Once we confirm the area is numb, your consultant will carefully insert the needle for your injection. They may inject a dye (colourless contrast fluid) and take an x-ray to confirm the needle is in the right position. Sometimes an ultrasound scanner is used to help guide the needle. Please let us know if you feel any discomfort during the procedure. You may feel some pressure in the injection area or along a nerve. This is usually temporary. We will monitor you ...

After emerging from the spinal nerve, the C3 dorsal ramus courses posteriorly, medial to the posterior intertransverse muscles, and then crosses over the lateral aspect of the C2/C3 facet joint. The TON gives off the only sensory branches to the C2/C3 facet joint. The nerve courses further dorso-medially and cephalad to pierce and supply both the semispinalis and splenius capitis muscles at the C2 (axis) level. It continues to travel cephalad and emerges superficial by piercing the trapezius muscle at the C1 (atlas) level. At its final destination, the occiput level, the TON provides sensory innervation to a small area of skin just below the superior nuchal line ...

Study design: This is a retrospective study. Objective: The objective of this study was to evaluate lumbar spine synovial cyst recurrence rates of decompression-alone versus decompression/fusion procedures. Background: Improvements in imaging modalities allow for increased diagnosis and surgical treatment of symptomatic spinal juxtafacet synovial cysts. Conservative management may be used as a first-line management strategy, however rarely provides durable, effective relief of symptoms. Surgical treatment of spinal synovial cysts ranges from decompression and cyst excision to decompression with fusion procedures. Decompression procedures alone have a higher risk of recurrence of spinal synovial cysts. Methods: We retrospectively reviewed 87 patients undergoing surgical treatment of lumbar spinal juxtafacet synovial cysts as a single institution over 20 years. Surgical treatment consisted of either decompression versus decompression/fusion procedures. Preoperative symptoms included back pain,

Radiofrequency lesioning is a procedure in which sensory nerves are purposefully altered by a heated probe. This technique is usually performed for focal back or neck pain caused by degenerative facet joints. Facet joints are the true synovial joints that connect your posterior spinal column. They are subject to degeneration and inflammation and become a very common cause of pain, especially in the elderly. Each facet joint is innervated by two sensory nerves, and their location is predictable along the bony spine. A diagnostic block of each nerve with local anesthetic will determine which of the presumed facet joints is the cause of pain. If you feel significant pain relief after the diagnostic block, then you will return for the radiofrequency lesioning procedure.. The aim of radiofrequency lesioning is reducing your pain and improving your functioning. The procedure is not permanent; however, recent studies have determined that the average pain relief lasts nine months. As with any procedure, ...
Lets first look at the anatomy of the neck in order to better understand the structures of the cervical spine that can generate pain. Starting at the back of the spine, the facet joints allow us to move our neck and head in all directions, and each facet joint is surrounded by a joint capsule that is rich with nerve endings and when swollen, can generate pain both locally and radiating. One study of volunteers with existing neck pain looked at the various pathways that pain travels when investigators injected each individual facet joint capsule with normal saline solution. Interestingly, the subjects felt pain in parts of the body other than just the neck. For example, injecting the C6 facet joint capsule consistently reproduced radiating pain down the arm into the thumb side of the forearm and hand, similar to when a disk herniates and a nerve root is pinched. The primary difference was that a deep aching pain occurred in this area, as opposed to a more geographically well-defined pathway when ...

Whiplash is a type of neck strain that occurs when the head is forced backward and then forward. This is also known as cervical acceleration-deceleration injury. It arises when the body is in motion and then suddenly stops with the head whipping back and forth at the neck due to this instantaneous change in speed. This causes stretching and even tearing of the ligaments, muscles and tendons.. There can also be intervertebral disc and facet joint injury. In severe cases the bones may also be involved and whiplash can lead to fractures. In addition there is usually some degree of injury to the nerves in the area, such as compression, stretching and so on. Along with the neck injury, whiplash is often accompanied by a concussion. This is a result of the brain striking the inside of the skull with the sudden and forceful head movements.. Read more on whiplash and concussion.. ...
Basic Information. What is radiofrequency ablation?. Radiofrequency ablation is the use of radio waves to heat the fluid around certain structures. When it is used for spinal pain, it effectively destroys the tiny nerves transmitting pain signals to your brain from your facet joints (also called zygaphopyseal joints, see diagram below). Pain from facet joints is common, particularly as the general wear-and-tear on these joints over time causes degeneration and arthritis. Most people with facet joint pain have axial pain (along the spine), usually in the lower lumbar region. Your pain may be worse with lumbar extension, twisting backwards, and prolonged standing. What nerves are targeted?. Radiofrequency ablation is used to target pain from the facet joints, which are the articulating joints between the vertebrae of your spine. The actual nerves being destroyed are called medial branch nerves, and each facet joint receives its innervation from the medial branch at (ascending branch) and one ...

Facet arthropathy can affect the cervical, thoracic, and/or lumbar spine. These small joints, also known as z-joints or zygapophysial joints, can be affected acutely or damaged from chronic wear and tear. In the low back, these joints can be loaded or stressed by extension and lateral rotation (bending back at the waist and to one side). If your usual pain is reproduced when making this motion without any leg pain, this is likely pain from your facet joints. Pain can sometimes radiate to the buttocks and/or thigh, but rarely below the knee. In the neck, facet pain is reproduced when laterally flexing the neck (trying to touch your ear to your shoulder), as well as extending the neck (looking upwards).. Each individual joint has two nerves that supply it called medial branches. These nerves are important in the treatment of facet-mediated pain. This treatment involves medial branch nerve blocks and radiofrequency nerve ablation.. X-rays and MRIs can sometimes be helpful in diagnosing facet pain, ...
The broad goal of Beths research is to understand the mechanisms of injury that produce whiplash, sports-related, and other painful injuries. By combining biomechanical and immunological techniques, her lab can define the relationships between injury to the cervical spine/neck and physiological cascades of persistent pain. Particular emphasis is placed on understanding injury to individual structures in the neck, such as the facet joints, nerve roots and spinal cord and how mechanical loading to these structures elicits pain. Through this work she can begin to develop thresholds for mechanical injury that produce persistent pain; and work towards a definition of the necks tolerance for painful injury. Additional research efforts are aimed at understanding the role of biomechanics in the neuroimmunologic changes of the central nervous system that contribute to persistent pain. Applications of her current work are in the areas of automotive and whiplash-related injury and sports injuries and ...

GLISSADE Study The GLISSADE Study was a prospective, 40-patient trial that studied the Glyder® Facet Restoration System in the treatment of chronic facetogenic pain due to facet degeneration or mechanical facet joint disease.. Background: The zygapophysial (facet) joint is the primary pain generator in one third of chronic low back pain cases. Current treatment options include temporarily palliative nonsurgical approaches, facet injections, radiofrequency denervation, and, rarely, lumbar arthrodesis. The purpose of this study was to assess the safety and effectiveness of a minimally invasive implant intended to restore facet joint function in patients with chronic lumbar facetogenic pain.. Methods: This prospective, multi-center feasibility study enrolled patients with confirmed lumbar facetogenic joint pain at 1 or 2 levels who underwent at least 6 months of unsuccessful nonoperative care. Main outcomes included back pain severity using a visual analogue scale, back-specific disability using ...
Radiofrequency denervation is a treatment for some kinds of lower back and neck pain. It uses an electrical current created by radio waves.
Paradoxically, there are also patients who feel their worst when they first rise in the morning and at the end of the day. These people often feel best when they are moving their neck, and they often prefer warm, sunny days to cool, rainy or overcast days.. Such symptoms parallel those experienced by patients with osteoarthritis of the weight-bearing joints (e.g., hips and knees). It is presumed that arthritic changes in the facet joints play a role in these people.. Degeneration in the cartilage of the facet joints can produce pain and tends to occur in older adults (over 60 years old).. The facet joints are designed to move against smooth surfaces, but as the cartilage degenerates it develops a lot of friction and there is accompanied loss of motion. Often the chronic pain is worst first thing in the morning.. Range of motion exercises, physical therapy, traction, and manipulations can all help preserve motion and lessen chronic pain.. In addition to the above conditions, there are a number of ...

The patient is placed in the prone (lying face down) position with a pillow underneath the abdomen. The skin of the low back is sterilely prepared. Using fluoroscopic (X-ray) guidance, the bony landmarks are viewed. The skin is anesthetized. Under fluoroscopic guidance, thin needles are advanced to the appropriate area for medial branch blockade. A small volume of contrast solution is injected to prove that the medication will stay in the appropriate area. Then, a small volume of long-acting anesthetic is injected. The patient is cared for in the recovery area for 20 minutes.. Preparing for Medial Branch Blocks. ...
This is an injection of numbing medicine that bathes the medial branch nerves. These nerves are attached to the facet joints of the spine. Disease or injury of these joints can cause pain in the medial branch nerves.
Not much time today to blog, so I leave you with this awesome video of what a facet joint injection looks like. I had three of them today (same as about two weeks ago - worked well, but wore off a few days ago. Vicodin has been helpful in offsetting some of the resulting pain, but I really hate taking it.). ...
Lumbar spinal stenosis (LSS) is characterised by low back and leg pain, walking disturbances and in some cases impaired balance and numbness of the lower limbs. This condition is caused by degenerative changes in the lumbar spine, including bulging discs, osteophytes from the arthritic facet joints and thickened flavum ligament (FL), which concertedly cause a narrowing of the spinal canal and compression of lumbar nerve roots.1-3 LSS mainly affects populations aged ,65 years and is unusual in people ,50 years.4 LSS is attracting increasing interest given that the worlds population is ageing rapidly and increasing demands for physical activity. Today, LSS is the most common indication for spinal surgery among a geriatric population.5-7 The goal of surgical treatment in LSS is to decompress the nerve structures in the stenotic segments through a posterior approach. The hypertrophic ligaments and parts of the facet joints are removed (ie, decompression). Adding spinal fusion for stabilisation of ...
If youre suffering from back pain, you should know more about what our doctors call one of the most under-diagnosed causes of pain: Facet Joints. Often patients spend years trying to relieve the pain and find that nothing works. That may because the facet joints are to blame but the root of the problem has been misdiagnosed. A rhizotomy is a treatment for facet joint-related pain that can make a difference ...

A prosthesis replaces all or a portion of a natural facet joint on a vertebral body. The prosthesis has a prosthesis body accommodating fixation to the vertebral body at or near a pedicle and without support by a lamina. The prosthesis body has a fastening element installed within the vertebral body at or near a pedicle. The prosthesis further has an artificial facet joint structure carried by the prosthesis body at a location spaced from the fastening element. The artificial facet joint structure is adapted and configured to replace all or a portion of a natural facet joint.

Purpose: Epidural needle insertions and facet joint injections play an important role in spine anaesthesia. The main challenge of safe needle insertion is the deep location of the target, resulting in a narrow and small insertion channel close to sensitive anatomy. Recent approaches utilizing ultrasound (US) as a low-cost and widely available guiding modality are promising but have yet to become routinely used in clinical practice due to the difficulty in interpreting US images, their limited view of the internal anatomy of the spine, and/or inclusion of cost-intensive tracking hardware which impacts the clinical workflow. Methods: We propose a novel guidance system for spine anaesthesia. An efficient implementation allows us to continuously align and overlay a statistical model of the lumbar spine on the live 3D US stream without making use of additional tracking hardware. The system is evaluated in vivo on 12 volunteers. Results: The in vivo study showed that the anatomical features of the ...
Degenerative right L5-S1 facet joint synovial cyst which contacts the exiting right L5 nerve root and also compresses the traversing right S1 nerve root.
At another site that I used to work in, they did a lot of pain procedures such as epidurals, facet joint injections, etc. The way to make money in a pain center is to drive through huge volumes of patients. The pain doc, really an overpaid egotistical anesthesiologist, didnt want his patients to feel any pain upon injection and didnt want to wait for a routine anesthetic to kick in. So he demanded all his patients receive Versed 10 mg and Fentanyl 100 mcg IV boluses right before he injected. Of course that cocktail put the patients out like a light switch. Since pain procedures usually take about ten minutes to perform, the patient was usually still sound asleep when he was finished. Now you cant clutter up a small recovery room at a pain center with a bunch of sleeping patients. So the routine there was to give a bolus of Flumazenil and Narcan when the procedure was completed. That usually woke up the patient right away though many still felt groggy. They were discharged pretty quick, ...
