Introduction: Perilunate and lunate dislocations are uncommon high energy injuries and have extensive soft tissue, cartilaginous and bony damage. The most common pattern is trans-scaphoid perilunate fracture dislocation which constitutes approximately 50% of these injuries. Unusual injury pattern like Scaphocapitate syndrome can occur with fracture through scaphoid waist and capitate that rotates the proximal capitate 180° so that its proximal articular surface points distally. With this case report, we would like to present a novel presentation of an uncommon wrist injury. Case report: We present a case of simultaneous lunate dislocation in one wrist and perilunate fracture dislocation (Scapho-capitate syndrome) in the contralateral wrist which occurred simultaneously in a young male following a two wheeler accident. The injury was missed initially at the primary treating centre. Upon diagnosis, the patient underwent open reduction and internal fixation along with ligament repair. The patient was

With a name like that, all physiotherapists will remember this one! But how much detail is remembered? Have a quick think about how many wrist patients you see, compared to knee or back patients. If you are in a private practice/outpatient clinic with a demographic anything like mine you will be seeing mostly backs, knees, shoulders, ankles - but not so many wrist presentations. Regardless of the number of wrist injuries you see in a typical week your patients will still be expecting the same high level of accuracy and efficiency you offer with other regions of the body.. With Brukner and Khans latest edition of the Clinical Sports Medicine (2011) listing TFCC tears amongst 11 other subacute/chronic conditions (with pain on the ulnar aspect of the wrist) and at least 9 acute conditions we really need to have our diagnostician hats firmly pulled on when dealing with wrist patients.. ...

OBJECTIVE:. To analyze the long-term results and the prognosis of patients who have undergone proximal row carpectomy (PRC) in our hospital.. MATERIAL AND METHOD:. This is a retrospective study of 14 patients undergoing PRC with a follow-up of 3 to 16 years. The ability to perform daily activities, mobility and functional capacity after the surgery were assessed using the Patient-Rated Wrist Evaluation questionnaire (PRWE).. RESULTS:. The range of movement was satisfactory. The mean score on the PRWE scale was 20.9±17.2 for the pain subscale and 39±35.5 for the functional capacity subscale. None of the patients required revision surgery or developed complications. Eighty percent of the patients were satisfied or very satisfied. Ninety percent of the patients would undergo surgery again.. DISCUSSION:. The wrists range of movement achieved with this technique is similar to that obtained with other mobility preservation techniques. Secondary arthritic changes in the radio-capitate joint do not ...
Wrist arthroscopy is used for diagnosis and treatment. It works best for showing ligament tears, damage to the triangular fibrocartilage complex (TFCC), or joint cartilage.
What is scaphoid non-union?. The scaphoid bone is one of the eight small bones that comprise the wrist joint. The two rows of small wrist bones act together to allow the wide variety of wrist positions and motions that we take for granted. The scaphoid bone spans or links these two rows together and, therefore has a special role in wrist stability and coordinating wrist motion (see Figure 1).. The scaphoid bone is vulnerable to fracture because of its position within the wrist and its role in wrist function. When the scaphoid bone is broken, it may not heal properly because it has a very fragile blood supply. Scaphoid fractures that do not heal are referred to as a scaphoid non-union. Ultimately, scaphoid non-unions can lead to loss of wrist motion and eventual wrist arthritis.. Diagnosis of scaphoid non-union. Patients with a scaphoid non-union usually present with a history of previous wrist injury, especially a fall onto an outstretched wrist. They will typically have pain along the thumb ...

Athletic injuries of the hand and wrist continue to increase as athletes get bigger, faster, and the weekend warrior continues to be active throughout his or her lifespan. The management of hand and wrist injuries in these athletes depends on the sport, position, timing during the season the injury occurs, and how many years the athlete has left to play in competition. In the past, these injuries were thought to be trivial; but if not ideally managed, complications can occur which may keep the skilled athlete or the weekend warrior out for a prolonged period of time. This article concentrates on common athletic injuries of the hand and wrist.

Hand and wrist injuries are reported to account for fifteen to twenty percent of emergency room injuries, and metacarpal fractures represent a significant number of those injuries. Hand injuries of this sort are most prevalent among fifteen- to thirty-five-year-old males, and the fifth metacarpal is the one most commonly affected.[10] Males are nearly fifty percent more likely to sustain fracture from a punch mechanism than females. Male intentional punch injuries are correlated predominantly with social deprivation, while female punch intentional injuries show more correlation with psychiatric disorders.[11] Approximately 3.7 male hand injuries, per 1000, per year, and 1.3 female hand injuries, per 1000, per year, have been reported. Common mechanisms of injury are gender specific. Although the fiscal cost is not available, it can be asserted that the cost is reasonably significant per individual, depending on the cost of emergency care, immobilization, surgery, follow up doctors visits, etc. ...

When it comes to alpine sports, snowboarding provides plenty of thrills as well as risks. Injuries are common, especially in beginners and those pushing the limits. While wrist injuries are most common, your knees are also susceptible to injury. Surprisingly, just getting on or off the chairlift puts your knees at risk. Overuse can also strain tendons while jumps, tricks and collisions can stretch or tear crucial ligaments in the joint, leading to pain. Ski and snowboard injuries happen if you fall incorrectly, have equipment issues, turn wrong, or overuse the muscles. Heres the good news, these injuries can be.... ...

Baton Rouge - The LSU gymnastics team is one of the best in the country and Tigers head coach DD Breaux feels she has one of the best support teams as well.. Team managers Nico Cusimano and Shonacee Oliva each took different paths towards joining the squad as team managers, but they both are vested in the Tigers success. Cusimanos family owns a gym in Saint Louis and he wants to run a gym himself one day. Hes been on Breauxs radar for a while: he was recruited to join the team as a manager when he was 13 years old.. I was at a tailgate and DD was there and she said, would you like to be a manager? Cusimano recalls. I said yes and she said okay, well when you graduate, I want you to come join my team.. Meanwhile Oliva was an LSU gymnast herself in 2013 and early 2014, but a wrist injury ended her career. Breaux offered her a medical scholarship to remain with the team and shes enjoyed the transition from athlete to manager.. Im able to talk with (the gymnasts) kind of like a coach ...

Background: Computed tomography can be an adjunct to radiographs when evaluating intra-articular fractures of the distal part of the radius. Acute-phase multidetector computed tomography has better temporal, spatial, and contrast resolution than a conventional scanner has. The aim of this study was to determine prospectively whether the addition of a multidetector computed tomography scan (with various reconstructions) results in changes in the evaluation of intra- articular distal radial fractures and thus changes in the plans for further management. Methods: Radiographs and multidetector computed tomography scans were compared prospectively in the evaluation of 117 patients (120 wrists) with acute intra-articular distal radial fractures. The parameters that were measured included the ability to detect intra-articular step and gap displacements, central articular depression, coronal plane fracture, the number of articular fragments, comminution, and associated injuries in the wrist region ...
Pinned wrist fracture. Coloured X-ray of a pinned fracture of the wrist-end of the radius, one of the two lower arm bones. The three pins have been surgically implanted to promote osteosynthesis (bone growth) in a patient with osteoporosis. Osteoporosis is a condition where bone mass and density is lost. This weakens the bones, and fractures like this can happen more easily. This is the right-hand wrist, with the thumb seen at upper left. The other arm bone is the ulna. - Stock Image M330/1359

Arthroscopy is a way to find problems and do surgery inside a joint without making a large cut (incision). Your doctor put a lighted tube with a tiny camera-called an arthroscope, or scope-and surgical tools through small incisions in your wrist.. You will feel tired for several days. Your wrist will be swollen. And you may notice that your skin is a different colour near the cuts (incisions). This is normal and will start to go away in a few days. Keeping your arm higher than your heart for the first few days after surgery will help with swelling and pain.. When you can go back to work or your usual routine depends on the kind of surgery you had. You will probably need about 6 weeks to recover. If you had a repair of damaged tissue, recovery will take longer. You may have to limit your activity until your wrist strength and movement are back to normal. You may also be in a rehabilitation (rehab) program. ...
Long durations of rotating nightshift work may contribute to risk of hip and wrist fractures, although the potential for unexplained confounding cannot be ruled out.
The purpose of this study is to compare early return to function in patients treated with closed reduction percutaneous pinning and open reduction internal fixation in displaced fractures of the distal radius.. Hypothesis: Wrist range of motion, grip strength and outcome at 2-3 months after injury are better in patients treated with open reduction, internal fixation (ORIF) than in patients treated with closed reduction percutaneous pinning techniques (CRPP). In addition patients treated with ORIF return to work at faster rates. ...
The fixator is an apparatus for repairing fractures of the distal radius and wrist. Distal, pivot, distraction, and radial members provide an anatomically contoured, radiolucent apparatus that permits the wrist to move through a substantially normal range of motion. A means for distraction of the bones by the fixator is also provided. The fixator may be affixed to the lower arm and hand by spaced-apart elongate distal mounting pins with lower ends adapted or mounting in the metacarpal bone and by spaced-apart elongate radial mounting pins with lower ends adapted for mounting in the radius.

Background. Broken wrists are fractures at the lower end of the radius, one of the two forearm bones. Typically in older people, these can occur when people put out a hand to break a fall. The broken bones are often pushed apart, resulting in a displaced fracture.. For most broken wrists, the bones can be moved back together (reduced) and held in place without an operation by a plaster cast for a few weeks. When fractures are unstable, the parts do not stay together. Another way of managing broken wrists is to hold the bones back in place with a surgical procedure, such as percutaneous pinning. This involves the insertion of pins or wires through the skin (percutaneous) to hold the bones in a proper position while they heal. In most pinning methods, pins or wires are used to fix the fragments together. In Kapandji pinning, the wires are placed to support the distal (lower end) fragment.. Methods This is an update of a Cochrane review first published in 2007. We looked at the evidence from ...

Thats why you shouldnt be frustrated that you dont have the late release of Sergio Garcia. His lag, which is golf lingo for a late release, might not be the best thing for you anyways. David Toms has an early release and has won 13 times on the PGA Tour, including a major championship. Had he spent all his time on the range trying to change his release, we might never have heard of him.. Lets start from the beginning so you can see what I mean.. At the top of the swing, the angle between the lead arm and the golf club is usually somewhere near 90 degrees. Every golfer begins to diminish this angle at some point in the downswing to eventually arrive at impact at 180 degrees. When golfers ...

View more ,Background: Giant cell tumors (GCTs) of the distal ulna are rare. Despite being benign, they can be locally aggressive and may recur following conventional treatment by curettage and bone grafting. Salvage reconstructive options after failed conventional treatment include Darrach resection, Suave-Kapandji procedure, hemi-arthroplasty, or total joint arthroplasty. Methods: We discuss reconstruction options for the distal radio-ulnar joint following tumor resection, and present the outcomes of a constrained distal radio-ulnar prosthesis in a 29-year-old male following resection of a distal ulna GCT. Results: Reconstruction of the distal radio-ulnar joint by a constrained prothesis yielded excellent functional outcomes following resection of a GCT of the distal ulna. Conclusions: This case demonstrates that successful oncologic and functional outcomes can be achieved by radical resection of a recurrent GCT of the distal ulna and reconstruction with a constrained total joint arthroplasty ...
We present the case of a patient with extensor carpi ulnaris tendon subluxation who was first treated for distal radioulnar joint sprain. A 25-year-old Caucasian man was seen at our policlinic one month after he had fallen on his outstretched hand. A diagnosis of extensor carpi ulnaris subluxation was made clinically but we also had the magnetic resonance imaging scan of the patients wrist which displayed an increased signal on T2-weighted images consistent with inflammation around the extensor carpi ulnaris tendon. The extensor carpi ulnaris tendon was found to be dislocating during supination and relocating during pronation. The sheath was reconstructed using extensor retinaculum due to attenuation of subsheath. There was no recurrent dislocation of the extensor carpi ulnaris tendon of the patient at his last follow up 12 months after the operation.
The hand and wrist consists of dozens of bones and joints, as well as ligaments, tendons, muscles, nerves, and cartilage that work together to perform routine, everyday tasks. The frequency in which we use our hands and wrists makes them prone to injuries, dislocations, and fractures. Repetitive motions while at work or participating in activities can lead to tendonitis, arthritis, or carpal tunnel syndrome.. At Advanced Center for Orthopedics and Plastic Surgery, our specialty-trained orthopedic hand and wrist surgeons provide highly specialized care and treatment for hand and wrist injuries and conditions. This care includes, but is not limited to, carpal tunnel release (for carpal tunnel syndrome), De Quervains release, trigger finger release, ganglion excision, arthroplasty of the wrist, tendon and nerve reconstruction, compression and congenital reconstruction, and fracture repair.. The treatment that you receive for a TFCC injury depends on your symptoms and the extent of your condition. Some injuries can be treated with protective supports, such as splints or casts. Your doctor may recommend modalities such as icing, medication, or cortisone injections to ease pain. Arthroscopic surgery is recommended if nonoperative treatments will not or do not provide symptom relief.. If arthroscopic wrist surgery is recommended, your surgeon will make a few small incisions near your wrist to insert the arthroscope and thin surgical instruments. The arthroscope is used to remove torn tissues or to repair the TFCC when possible. As arthroscopy uses small incisions and is less invasive than traditional surgeries, it has a shorter recovery time with less bleeding, swelling, and pain than open procedures.. If you have questions, or to make an appointment, call: 404-778-3350.. ...
Introduction. Distal radius fractures are common fractures of the upper extremity, estimated to account for one-sixth of all fractures seen and treated in emergency rooms.1-3 These fractures are seen in a bimodal age distribution and usually result from a low-energy trauma. Displaced fractures involving the articular surface of the distal radius have the worst prognosis because of the potential for incongruity and arthrosis of the radiocarpal and distal radio-ulnar joints.4 On plain radiographs, Robert Medoff5 introduced the teardrop angle (TDA), which is a radiographic parameter that gives information on the congruity of the lunate facet. This is measured as the angle formed between the central axis of the teardrop and the radial shaft. Although many surgeons are unaware of this parameter, it is important because an abnormal TDA can herald the presence of articular incongruity of the lunate facet.. The purpose of this study was to assess the reduction of the lunate facet in intra-articular ...

Project Abstract: As recreational sports such as in-line skating (the fastest growing recreational sport in America) become more popular, there has been an alarming increase in the incidence of skate-related injuries. The Consumer Product Safety Commission reported that there were 76,116 in-line skating injuries in 1994, which increased more than twice than in 1993. Wrist and distal forearm injuries accounted for 59% of the injuries in 1994, with fractures being the most common form of injury, leading to a significant disability from joint stiffness, arthritis, and loss of strength. In-line skaters have injuries on their wrists despite wearing wrist guards. Although wrist guards appear to protect against wrist fracture, fractures may be higher up the arm due to the guard configuration and therefore less easily reduced. Many earlier studies based on cadaveric testing, however, failed to identify the possible injury mechanisms nor to recommend further guideline of the guard design and efficient ...
There is a significant trend towards early operative fixation of distal radial fractures. This is particularly common in unstable fractures, occurring in osteoporotic patients. In the past distal radial fractures were manipulated and placed in a plaster of Paris, and held either by just the plaster of Paris or wire fixation. However, due to the poor quality of bone, the wire was often cut out, and patients were left with a recurrent deformity and significant problems. Over the last five to ten years there has been development of special surgical plates, which allow the fracture to be fixed in an extremely stable configuration, even in the presence of thin, poor quality bone. Having performed this operative fixation, with a scar usually through the front of the wrist, but occasionally on the back, patients are mobilised within the first week of surgery.. ...

The carpus, or wrist, represents a highly complex anatomic structure, comprising a core structure of 8 bones; more than 20 radiocarpal, intercarpal, and carpometacarpal (CMC) joints; 26 named intercarpal ligaments; and 6 or more parts of the triangular fibrocartilage complex (TFCC).2 While these structures can be differentiated anatomically, they are functionally interrelated with movement in one joint having an effect on the motion of neighboring joints. This relationship extends as far as the elbow. ...

My next experience with Yamamoto New Scalp Acupuncture was about four weeks ago. Id made two small cards which involved cutting and fine motor work with my hands. I have a stiff and thick right wrist from the wrist fracture which led to CRPS. I have limited movement in my wrist because of the severity of the fracture which resulted in permanent disability. Fine motor activity appears to cause irritation to the nerves of my right hand/wrist. This is what happened in this case. I had difficulty closing a fist, holding things, squeezing a peg etc. My nerves were firing and my perception of feeling changed in my fingers and hand. The pain traveled quickly to my elbow. Soon my shoulder became painful. Very quickly old pains returned.( You may remember Ive mentioned before that sometimes old pains suddenly return.) By the time I went to the doctor I was in a great deal of pain in my right wrist, elbow, shoulder, right leg, foot and left shoulder, back and my chest. My doctor said it was inflammation ...

Sixty-nine-year-old Mark Eisen says he waited seven years for a surgery he hopes will change his life. Last week, he had the first total wrist replacement with KinematX, a new implant co-designed by Scott Wolfe, MD, chief emeritus of the Hand and Upper Extremity Service at Hospital for Special Surgery (HSS). Dr. Wolfe performed the surgery on April 15.. Severe arthritis in his right wrist caused ongoing pain and prevented Mr. Eisen, who lives in Atlanta, from engaging in activities he enjoys. He is right-handed, which makes it even more difficult. Seven years ago, he started to investigate treatments and learned about total wrist replacement surgery. But unlike knee and hip replacements, which are common and have excellent outcomes, wrist replacement surgery has not enjoyed the same degree of success.. Mr. Eisens online research prompted him to take his first trip to New York for a consultation with Dr. Wolfe in 2014. Mr. Eisen had read about Dr. Wolfes extensive research into how the wrist ...
