The literature on right ventricular systolic dysfunction (RVSD) in peripartum cardiomyopathy (PPCM) patients is scanty, and it appears that RV reverse remodelling in PPCM has not been previously described. This study thus aimed to assess RVSD and remodelling in a cohort of PPCM patients in Kano, Nigeria. A longitudinal study carried out in 3 referral hospitals in Kano, Nigeria. Consecutive PPCM patients who had satisfied the inclusion criteria were recruited and followed up for 12 months. RVSD was defined as the presence of either tricuspid annular plane systolic excursion (TAPSE) |16 mm or peak systolic wave (S') tissue Doppler velocity of RV free wall |10 cm/s. For the purpose of this study, recovery of RV systolic function was defined as an improvement of reduced TAPSE to ≥16 mm or S' to ≥10 cm/s, without falling to reduced levels again, during follow-up. A total of 45 patients were recruited over 6 months with a mean age of 26.6 ± 7.0 years. RV systolic function recovery occurred in a total of

GENEVA (9 March 2015) - The United Nations Special Rapporteur on the human rights situation in Eritrea, Sheila B. Keetharuth, today started an official visit to Belgium (9-11 March) to collect first-hand information from Eritrean refugees and migrants on the human rights situation in Eritrea.. Since 2014, Ms. Keetharuth has carried out similar information-gathering missions to Djibouti, Ethiopia, Germany, Italy, Malta and Tunisia. "Due to lack of access to Eritrea," she said, "I have been engaging with Eritrean refugees and migrants outside of their home country, and all others concerned by human rights in Eritrea, including those who consider themselves to be victims of alleged human rights violations, human rights defenders and other civil society actors." The human rights expert has made several official requests to visit Eritrea since her appointment in November 2012, which have so far not been granted. She has repeatedly urged the Eritrean authorities to cooperate with her mandate in order ...

SANTOS MARTINEZ, Luis Efrén et al. Massive pulmonary embolism, thrombus in transit, and right ventricular dysfunction. Arch. Cardiol. Méx. [online]. 2007, vol.77, n.1, pp.44-53. ISSN 1665-1731.. Massive pulmonary embolism is associated with an increased mortality. It is secondary to migration of a venous thrombus to the right atrium or ventricle (thrombus in transit) towards the pulmonary circulation. The hemodynamic performance depends on the baseline cardiopulmonary status of the patient and the extent of obstruction. Right ventricular dysfunction will appear as a direct consequence of a major obstruction and hemodynamic collapse. The treatment of choice is thrombolysis, either intravenous in a peripheral vein, or local administration associated with percutaneous thrombus fragmentation or surgical embolectomy. We present the clinic case of a woman with massive pulmonary embolism. The transthoracic echocardiogram showed the presence of three auricular thrombus, right ventricular dysfunction ...
Introduction: Arrhythmogenic right ventricular cardiomyopathy (ARVC) is characterized by functional abnormalities of the right ventricular (RV). RV ejection fraction (EF) determined by cardiac magnetic resonance (CMR) imaging is accepted as gold standard measure of RV systolic function.. Hypothesis: RV global longitudinal strain (GLS) by 2-dimensional speckle tracking echocardiography (2DSTE) is correlated with CMR-RVEF, and associated with cardiac events in patients with ARVC.. Methods: We studied 50 patients referred with suspected ARVC. Of them, 19 patients (Group A) were diagnosed as ARVC, while remaining 31 patients (Group B) were not diagnosed due to not meeting criteria. All patients underwent CMR for measurements of RVEF, and echocardiography with conventional assessment including tricuspid annular plane systolic excursion (TAPSE), RV end-diastolic and end-systolic area (RVEDA, RVESA) and RV fractional area change (RVFAC), and with 2DSTE analyses of GLS which was expressed as the ...
To identify the echocardiographic features that can be used to distinguish between hypoplastic left heart syndrome and other causes of right ventricular overload in the sick neonate cross sectional echocardiographic studies of 10 neonates with hypoplastic left heart syndrome were analysed and compared with those in 15 neonates with other causes of right ventricular overload and 15 normal controls. Left ventricular and right ventricular cavity dimensions and the shape and size of the mitral valve annulus and aortic root were recorded and presented both as absolute values (mm) and corrected for body surface area (mm/m2). Logistic regression was used to produce a classification rule to estimate the probability of a neonate having hypoplastic left heart syndrome. The diameter of the mitral valve annulus was the single most discriminative variable. There was, however, considerable overlap of all the calculated features between neonates with hypoplastic left heart syndrome and those with other causes ...
In June 2008, Honduras supported a Resolution on Human Rights, Sexual Orientation, and Gender Identity unanimously adopted by the General Assembly of the Organization of American States (OAS). Honduras, with the rest of the OAS, expressed its concern over violence faced by people because of their sexual orientation or gender identity and made a public commitment to end it.. In June 2009, Honduras is hosting the 39th General Assembly of the OAS in San Pedro Sula under the theme "Toward a Culture of Non-Violence." In the proposed draft resolution, the states declare their commitment "to promote, within a framework of the rule of law, a culture of peace and non-violence" and specifically note "the importance of adopting measures necessary to prevent, impede, and punish violence ... against women, and groups in vulnerable situations.". While Honduran authorities have been prompt in signing international agreements pledging to curb violence and protect vulnerable groups, attacks on transgender ...

Beirut) - Vague provisions of the Gulf Cooperation Council (GCC) joint security agreement raise concerns. Member countries could use the agreement to suppress free expression and undermine privacy rights of citizens and residents.. Five of the six GCC countries - Saudi Arabia, Bahrain, Qatar, the United Arab Emirates, and Oman - have ratified the November 2012 agreement. The Kuwaiti parliament is debating its ratification, though some members have expressed strong objections. Kuwait should not ratify the agreement in its current form, Human Rights Watch said.. "The security agreement gives gulf governments another legal pretext to stamp out dissent," said Joe Stork, deputy Middle East director at Human Rights Watch. "Citizens and residents of the gulf should note that their governments have agreed to share personal information at the whim of an interior minister.". The agreement's 20 provisions include a vaguely worded article that would suppress "interference in the domestic affairs" of other ...

Residual right ventricular outflow tract and pulmonary valve disease is common in adults with repaired tetralogy of Fallot. Chronic severe pulmonary regurgitation as a result of surgical repair can lead to myriad complications including right ventricular dysfunction, decreased exercise tolerance, right heart failure and symptomatic arrhythmias. The aim of restoring pulmonary valve integrity is to preserve right ventricular size and function with the intent of mitigating the development of symptoms and poor long-term outcomes. Right ventricular size thresholds by cardiac MRI have emerged beyond which reverse right ventricular remodelling after pulmonary valve replacement is less likely. Though pulmonary valve replacement has been shown to improve right ventricular dimensions and symptoms, no consistent improvement in right ventricular ejection fraction or objective measures of exercise capacity have been demonstrated. Furthermore, there are no long-term studies showing that normalisation of right ...
Introduction: The right ventricular outflow tract (RVOT) patch after Tetralogy of Fallot (ToF) surgery may contribute to ventricular dysfunction, but the specific effects on RV function are not clear. We aimed to investigate RV function in patients with ToF and RVOT patch by comparing to patients with valvar pulmonary stenosis (vPS) after balloon valvuloplasty (no RVOT patch).. Methods: We conducted a retrospective review of 33 CMR studies of ToF and vPS patients with similar pulmonary regurgitation (PR) Table 1. Feature tracking strain (FTS) analysis of balanced steady state free precession (SSFP) cines was performed on the RV in apical 4-chamber (4ch), RVOT sagittal & mid-RV short axis views (TomTec 2D Cardiac Performance Analysis, v1.0), Figure 1. Unpaired t-tests were applied for comparison of means between groups.. Results: Lagrangian longitudinal strain (LS) at the RVOT was significantly lower in ToF compared to vPS (mean -14.1% vs -18.2%, unpaired t-test p = 0.03), Table 1. Analysis of ...
Background: In this study, we aimed to investigate frequency, patterns, etiologies, and costs of unplanned readmissions after left ventricular assist device implantation.. Methods: Between April 2012 and September 2016, 99 unplanned readmissions of a total of 50 consecutive bridge-to-transplant patients (45 males, 5 females; mean age 46.9±10.3 years; range, 19 to 67 years) who were successfully discharged after left ventricular assist device implantation were retrospectively analyzed. Patient demographic data, hemodynamic measurements before implantation, and readmissions after discharge were recorded. Hospitalizations due to major problems which were unable to be managed in routine outpatient clinic were accepted as unplanned readmissions. Survival analysis was performed.. Results: The readmission rate was 1.7 per year after discharge. Survival of patients who were readmitted within the first 90 days was found to be significantly lower than those without early readmission. The most common ...
There are allegations that war crimes were committed by the Sri Lankan military and the rebel Liberation Tigers of Tamil Eelam (Tamil Tigers) during the Sri Lankan Civil War, particularly during the final months of the Eelam War IV phase in 2009. The alleged war crimes include attacks on civilians and civilian buildings by both sides; executions of combatants and prisoners by both sides; enforced disappearances by the Sri Lankan military and paramilitary groups backed by them; acute shortages of food, medicine, and clean water for civilians trapped in the war zone; and child recruitment by the Tamil Tigers. A panel of experts appointed by United Nations Secretary-General (UNSG) Ban Ki-moon to advise him on the issue of accountability with regard to any alleged violations of international human rights and humanitarian law during the final stages of the civil war found "credible allegations" which, if proven, indicated that war crimes and crimes against humanity were committed by the Sri Lankan ...

Background: Evidence suggests that childhood physical activity may play a role in the etiology and prevention of adult chronic diseases. Because researchers must often depend on self-recalled physical activity data many years after the exposure, it is important to understand factors which may influence adult recall of childhood physical activity. This study evaluated the influence of adult characteristics on reported childhood physical activity and the association between adult physical activity and self-recalled childhood physical activity. Methods: 48,066 post-menopausal women from the Women's Health Initiative Observational Study reported their physical activity level during ages 5-9, 10-14, and 15-19. Results: In this cohort, over 65% of the population reported the same category of physical activity over the three childhood age groups. While higher levels of childhood physical activity were significantly associated with higher adult physical activity, this association varied by race/ethnicity,
Twenty-five patients were enrolled; all presented cardiac index , 4 l/min/m2. The following parameters were evaluated: heart rate (HR), mean arterial pressure, central venous pressure (CVP), pulmonary capillary wedge pressure (Pw), CO, systolic pulmonary artery pressure (SPAP) and diastolic pulmonary artery pressure (DPAP), systemic vascular resistance index (SVRI), pulmonary vascular resistance index (PVRI), left ventricular stroke work index (LVSWI) and right ventricular stroke work index (RVSWI). The determinations were performed before disconnection from mechanical ventilation, 30 min after disconnection and 4-6 hours after disconnection. ...
Patients undergoing valvulectomy for isolated tricuspid valve endocarditis offer the unique opportunity to study the effects of acquired right ventricular volume overload on left ventricular filling in persons free of pulmonary hypertension and preexisting left heart disease. Eleven patients who had undergone total or partial removal of the tricuspid valve were compared with 11 age-matched control subjects; Doppler echocardiographic techniques were used to quantify changes in left ventricular filling and to relate them to changes in left ventricular and left atrial geometry caused by right ventricular and right atrial distension.. The late diastolic fractional transmitral flow velocity integral, a measure of the left atrial contribution to left ventricular filling, was significantly decreased in patients undergoing tricuspid valvulectomy compared with control subjects (0.22 ± 0.11 versus 0.32 ± 0.09; p , 0.04). Severe tricuspid regurgitation in these patients resulted in marked right atrial ...

Also known as arrhythmogenic right ventricular cardiomyopathy. ARVD stands for Arrhythmogenic Right Ventricular Dysplasia. Arrhythmogenic means causing an arrhythmia. The right ventricle is the chamber of the heart that is affected and dysplasia means there is an abnormality of the structure. The right ventricle is dilated and contracts poorly. As a result, the ability of the heart to pump blood is usually weakened. Patients with ARVD often have arrhythmias (abnormal heart rhythms), which can increase the risk of sudden cardiac arrest or death.. ARVD is a specific type of cardiomyopathy (a disorder of the cardiac muscle).. Simply put, ARVD is a genetic, progressive heart condition in which the muscle of the right ventricle is replaced by fat and fibrosis, which causes abnormal heart rhythms. ARVD is estimated to affect one in 5,000 people. The disease can affect both men and women. Although it is a relatively uncommon cause of sudden cardiac death, it accounts for up to one fifth of sudden ...

Chronic pulmonary thromboembolic disease is an important cause of severe pulmonary hypertension, and as such is associated with significant morbidity and mortality. The prognosis of this condition reflects the degree of associated right ventricular dysfunction, with predictable mortality related to the severity of the underlying pulmonary hypertension.1 In recent years the epidemiology of this condition has been revised considerably. Once considered a rare condition, chronic thromboembolic pulmonary hypertension (CTEPH) was recently documented to complicate 3.8% of acute pulmonary embolic events.2. CTEPH is the only cause of severe pulmonary hypertension which is potentially curable without the need to resort to lung transplantation. Pulmonary endarterectomy (PEA) is the surgical procedure which removes the obstructing thromboembolic material, resulting in significant improvements (and in many cases normalisation) in right ventricular haemodynamics and function. This procedure requires a high ...
A 46-year-old white woman presented to the emergency department with hemodynamically stable sustained ventricular tachycardia (VT). She was chemically cardioverted with lidocaine. Her electrocardiogram, showing sinus rhythm, was unremarkable, and serial cardiac enzyme tests excluded myocardial infarction. A signal-averaged electrocardiogram was abnormal, with a filtered QRS duration of 187 milliseconds. Echocardiography showed normal left and right ventricular systolic function but revealed diastolic dysfunction of the left ventricle. Electrophysiologic testing revealed easily inducible sustained VT of 4 distinct morphologies. A diagnosis of possible arrhythmogenic right ventricular dysplasia was made based on the signal-averaged electrocardiographic and electrophysiologic findings. Cardiac magnetic resonance imaging showed no evidence of arrhythmogenic right ventricular dysplasia, however. An endomyocardial biopsy revealed noncaseating granulomas consistent with sarcoidosis. This case ...

The definition provided by the CEDAW Committee was later codified, albeit in the form of a declaration, in more or less a similar formulation in article 1 of the Declaration on the Elimination of Violence Against Women (DEVAW). The Declaration goes further than prohibiting violence against women. It, under article 2, includes a list of prohibited acts regarded as violence against women. This includes '[p]hysical, sexual and psychological violence occurring in the family'. More importantly, it explicitly declares FGM as a form of violence against women.. As alluded to earlier, FGM 'is a traumatising and painful surgical procedure, performed on young girls, without the aid of anaesthesia to temporarily dull the pain, or proper hygiene to prevent potential infection'.59 This exposes women to severe pain and suffering, which has both physical and psychological effects. The practice inflicts physical, sexual and mental harm to women. Moreover, the violence does not end at the completion of the ...
Acute pulmonary embolism is the third most common cause of cardiovascular death. Pulmonary embolism increases right ventricular afterload, which causes right ventricular failure, circulatory collapse and death. Most treatments focus on removal of the mechanical obstruction caused by the embolism, but pulmonary vasoconstriction is a significant contributor to the increased right ventricular afterload and is often left untreated. Pulmonary thromboembolism causes mechanical obstruction of the pulmonary vasculature coupled with a complex interaction between humoral factors from the activated platelets, endothelial effects, reflexes and hypoxia to cause pulmonary vasoconstriction that worsens right ventricular afterload. Vasoconstrictors include serotonin, thromboxane, prostaglandins and endothelins, counterbalanced by vasodilators such as nitric oxide and prostacyclins. Exogenous administration of pulmonary vasodilators in acute pulmonary embolism seems attractive but all come with a risk of ...
There are still many unanswered questions about the damage that silicone gel implants may cause. When silicone implants break, silicone gel is released into the body. The long-term effects that this has on women's health have not been adequately researched.. Women with silicone breast implants have reported a variety of autoimmune diseases, including scleroderma, rheumatoid arthritis, and lupus. While some studies that concluded that breast implants do not cause autoimmune disease, most of that research has focused on women who had implants for a few months or a few years. Furthermore, these studies have tended to include small numbers of women. Since these diseases are rare and take many years to develop and to be diagnosed, it is not possible for studies of small groups of women who have implants for a short period of time to determine if implants increase the long-term risks of getting these diseases.[8]. Researchers have suggested that exposure to toxins in silicone gel may be the trigger ...
Exposure to traumatic events may lead to mental health problems, such as post-traumatic stress disorder (PTSD). PTSD is a type of anxiety disorder that is characterized by the presence of persistent frightening thoughts and memories of the traumatic event. Additionally, people with PTSD often feel emotionally numb, and they may experience sleep problems or be easily startled. Homeless people, in particular, are at risk for experiencing a variety of traumas. Homeless women tend to experience more traumatic events and develop PTSD at higher rates than housed women. Although effective treatments for PTSD are available, homeless women are unlikely to seek them out. There is a need for PTSD treatment approaches that are specifically designed for these women. This study will develop a cognitive-behavioral therapy treatment that will be adapted to best serve the needs of homeless women. The study will then determine the therapy's effectiveness in treating a group of homeless women with PTSD.. This ...
Authenticity. Resilience. Strength. These are the words you hear over and over when you put a group of nearly 60 women veterans together in a room, as a national support group called The Mission Continues did earlier this month at its Women Veterans Leadership Summit.. But so far, both military and veterans' communities are failing to recognize and fully utilize the unique leadership strengths of the women who serve our country.. Women achieved a number of milestones in the military in 2015, from the first female Army Rangers to a decision to open all combat jobs to women. The Navy even issued its iconic white Dixie cup hats to women recruits as part of broader effort to make uniforms and job titles more gender-neutral.. Gender-neutrality is an important first step for the U.S. military, where women account for only 15 percent of active-duty personnel.. But for women veterans working to reintegrate into civilian life, there's a growing awareness that it's also important to let women in the ...

Recently researchers have debated the relevance of stereotype threat to the workplace. Critics have argued that stereotype threat is not relevant in high stakes testing such as in personnel selection. We and others argue that stereotype threat is highly relevant in personnel selection, but our review focused on underexplored areas including effects of stereotype threat beyond test performance and the application of brief, low-cost interventions in the workplace. Relevant to the workplace, stereotype threat can reduce domain identification, job engagement, career aspirations, and receptivity to feedback. Stereotype threat has consequences in other relevant domains including leadership, entrepreneurship, negotiations, and competitiveness. Several institutional and individual level intervention strategies that have been field-tested and are easy to implement show promise for practitioners including: addressing environmental cues, valuing diversity, wise feedback, organizational mindsets, reattribution
Marilyn Yarbrough with Crystal Bennett. Excerpted from Marilyn Yarbrough with Crystal Bennett, Cassandra and the 'Sistahs': the Peculiar Treatment of African American Women in the Myth of Women as Liars Journal of Gender, Race and Justice 626-657, 634-655 (Spring 2000)(254 footnotes omitted). In colonial times, white men often viewed white women with suspicion and distrust. They associated white women with sexuality. However, as time passed, white women were no longer portrayed as sexual temptresses. They became celebrated as the 'nobler half of humanity' and depicted as goddesses rather than sinners. White women were thereafter represented as virtuous, pure and innocent. The historical and social experiences of African women during slavery resulted in numerous images that defined African American women as deviant.. For centuries, African American women have been contrasted with white women. While the Victorian concept of 'true womanhood' defined white women as possessing unquestionable moral ...

Breastfeeding initiation rates in some developed countries are high (98 % in Sweden and 96 % in Australia) whereas in others, they are not as favourable (46 % to 55 % in Ireland). Although the World Health Organization recommends exclusively breastfeeding for six months, 15 % of Australian women, 11 % of Swedish women and less than 7 % of Irish women achieve this goal. Awareness of what women in different countries perceive as essential breastfeeding support is a gap in our knowledge. Our aim was to explore Australian, Irish and Swedish women's perceptions of what assisted them to continue breastfeeding for six months. An exploratory design using critical incident techniques was used. Recruitment occurred through advertisements in local newspapers and on social networking platforms. Initial sampling was purposive, followed by snowball sampling. Telephone interviews were conducted with 64 Irish, 139 Swedish and 153 Australian women who responded to one question

BackgroundThere is growing recognition of the difficult reproductive decisions faced by HIV-positive women. Studies in both resource-constrained and developed countries have suggested that many HIV-positive women continue to desire children in spite of their understanding of the possible risks that HIV poses. This study investigates the factors associated with fertility desire among HIV-positive women in Tigray region, Ethiopia.MethodsA cross-sectional survey was conducted among 964 HIV-positive women receiving HIV care in 12 health centers of Tigray region. In each health center, the number of study participants was allocated proportionally to the load of HIV-positive women in the chronic care clinics. A descriptive summary of the data and a logistic regression model were used to identify factors associated with fertility desire using odds ratios with a 95% confidence interval and P-value of 0.05.ResultsFour hundred and thirty nine (45.5%) of the participants reported a desire to have children ...
