TY - JOUR. T1 - Low body mass index is associated with increased waitlist mortality among children listed for heart transplant. AU - Davies, Ryan R. AU - Haldeman, Shylah. AU - McCulloch, Michael A.. AU - Gidding, Samuel S.. AU - Pizarro, Christian. PY - 2015/11/1. Y1 - 2015/11/1. N2 - Background In adults, low body mass index (BMI) and high BMI have been associated with increased mortality after heart transplantation. Studies of BMI in children with heart failure have had inconsistent results. Methods The United Network for Organ Sharing database has 4,035 listings for primary, isolated heart transplant in patients 3 to 18 years old (1995-2012). BMI percentile-for-age (BMI%) was calculated, and patients were stratified based on BMI% into 4 groups: underweight (BMI% ,5, n = 701 [17.4%]), normal weight (BMI% 5-84, n = 2,321 [57.5%]), overweight (BMI% 85-94, n = 440 [10.9%]), or obese (BMI% ≥95, n = 573 [14.2%]). Outcomes of patients on the waitlist and after transplantation were assessed. ...

TY - JOUR. T1 - Meld and Peld. T2 - Application of survival models to liver allocation. AU - Wiesner, Russell H.. AU - McDiarmid, Sue V.. AU - Kamath, Patrick S.. AU - Edwards, Eric B.. AU - Malinchoc, Michael. AU - Kremers, Walter K.. AU - Krom, Ruud A.F.. AU - Kim, W. Ray. PY - 2001/1/1. Y1 - 2001/1/1. N2 - Compared to the CTP score (Table 9), the MELD and PELD models provide the means to more accurately measure liver disease severity and to better predict which patients are at risk of dying on the waiting list. The relation between the MELD score and the risk of 3-month mortality is shown in Fig. 7. Most importantly, by de-emphasizing waiting time these two models will allow organ allocation based on medical urgency, as mandated in the DHHS final rule. However, while the MELD score is an extremely powerful predictor of the probability of death in patients with chronic liver disease, it does not address one of the guidelines in the final rule, that an organ allocation system should promote the ...
While the General Surgery Department at the All India Institute of Medical Sciences (AIIMS) has a minimum waiting period of three to five months, in Gastro-intestinal Surgery it is six months to one year and the longest waiting period is in the Neuro-surgery department: 18 months to three years. | The long wait for a surgery at govt hospitals
Wait time, defined as time spent in the waiting and exam rooms waiting to see a provider, is a key quality metric in a number of national patient experience surveys. However, the literature on wait time does not show a consistent correlation between long waits and worse overall patient care experiences. Herein, we examine contextual factors that can shape the manner in which patients may respond to different wait times. We also identify actions providers and clinics can take to promote positive wait experiences and mitigate negative ones. We conducted over 130 h of semi-structured interviews with patients new to two HIV primary care clinics in Houston, Texas. We interviewed patients before the first provider visit, again within two weeks of the first visit, and again at 6-12 months. We analyzed the interviews using directed and conventional content analysis. Our study showed that patients

Everyone has an opinion about health care.. If Ive learned anything from writing this column its that seniors are fiercely protective of our health care system. Many of the seniors that I talk to believe we have the best health care in the world. Are they right? Lets look at what some of the recent research says.. Earlier this year the Frontier Centre for Public Policy, a Canada-based think tank, released the Euro-Canada Health Index. The Index compared health care in Canada with that of 29 European countries. The rationale here is to look to other countries for ideas on how to make our system better. Take, for instance, the example of wait lists. We have very long wait lists in Canada. England and France have been able to reduce their wait times by tying money to patient care and requiring hospitals to meet wait time targets.. In 2007, BC introduced a pilot project that paid hospitals more money for cutting emergency wait times. At Lions Gate Hospital, 57 per cent of patients requiring ...
Ever since that day in May last year Ive put my Very Long Wait titles at the top of my queue because I know all too well that is the last stop before Saved, and with Netflix and its customers seemingly being over DVDs, and alternatives for DVD rentals of older movies and shows essentially nonexistent, I have to act now if I want to see that content. Ive been renting nothing but Very Long Wait titles in my disc plan for over a year. I figure I wanted to see these anyway even if they were in my queue for 10 years. A byproduct of this is if they do skip a bunch at the top to ship, say, the 10th or 20th title in the list, they often ship something closer to or at the top the next day or day after from a farther away shipping center, so I sometimes get more than I would on a typical week before the Very Long Wait saga began. On the other hand, and this may have more to do with the Post Office, the shipping time has become less predictable, with some titles shipped locally arriving a day or more ...
Audience. All DonorNet® match users. On July 24, 2019, we added a new report feature to the DonorNet match page that shows the distance in nautical miles from the donor hospital to each transplant hospital that appears on the match run. To access this feature, users can simply click on the orange link titled "Show distance to transplant center(s)" which appears at the top of the match run page.. This feature is currently visible for heart, lung and heart/lung matches because only those organ allocation policies classify candidates based on distance in nautical miles. We will expand the availability of this feature when we implement OPTN policy changes for other organs that use distance to classify candidates.. Contact us. The maximum wait time for callers in a call center (FIG. 10) is lowered by selecting, for an agent (25) who has just become available to handle a call, a highest-priority waiting call that would most likely wait the longest if it were not selected at this time. Anticipated wait times are computed for the calls at the heads of the non-empty highest-priority call queues that correspond to the agents skills or splits (202-210). The anticipated wait time of a call is computed as the calls present (elapsed) wait time plus the average rate of advance of calls in the calls queue (210). The call with the longest anticipated wait time is then selected first and is assigned to the available agent for handling (212-216). The process is repeated each time that any agent becomes available.

So it seems that I find myself in the two week wait, bored as can be, nothing to do, and wishing that I knew for sure when I ovulated. After todays temp, its no clearer than it was yesterday. Well see if I get yet another positive OPK. I had some spotting last night. Very light and mixed with CM. It was only there when I wiped. I think that must be ovulation spotting. Right? Well, I dont know how long it takes for ovulation spotting to work its way out, but it seems a little late if I truly am 4dpo. I guess it shouldnt matter. We covered all our bases when it came to the sex part of thing. ;-) I think Im more anxious about my ovulation date because thatll tell me when to expect aunt flo and when I can possibly test and expect a truthful response out of the pee stick. Its close already to testing time and I find Im more apprehensive than excited. I am, (how you say?) weirded out by the fact that this could actually work. Ive spent the last 3 years of life being the pissed off ...

It is quite a leap to go from mouse studies to humans. In most cases, it takes up to 10 years before a laboratory discovery is put into clinical practice. In our case, this leap occurred within months of our initial discovery when one of our 5-month-old patients developed severe PN liver injury that included bridging fibrosis. He was listed for a liver transplant. We were asked by our colleague to try our intravenous fish oil protocol on this patient since it worked so well on the mice. Originally, the goal was to help the child survive until organs for transplant became available. After discussing the nonstandard approach with the parents, treatment was started at 0.2 g/kg/day and increased over a 2 week period to a goal dose of 1 g/kg/day. Within 2 months of starting Omegaven the childs liver function improved and his jaundice resolved. He was removed from the transplant list shortly thereafter. He is still TPN dependent and on Omegaven two years later, but his bilirubin remains low and is he ...

My dad died this April, which, if you are reading this, you probably know me, so you probably already know. My mom has pretty advanced Alzheimers disease (at 69 years old) which you know as well but this is just my tiny update. She moved in with us for two months after the funeral because my dad had not made plans for her in his sick state. He had non-alcoholic liver cirrhosis so basically from the time he was diagnosed with it his mind was somewhat compromised from being toxic. We tried to "save him" through getting him on liver transplant lists and encouraging him to take his meds and eat right but it was so hard from afar. In the end none of our efforts ended up saving him. He progressed too fast for the doctors to get him a transplant and the rest of his organs started to shut down from the strain of the liver disesase. And mom got worse mentally as she suffered through watching her "person" die. We were with her pretty constantly for the last month of his life but some really crazy stuff ...

John Roberts, 56, from Irlam, has been on the transplant list for six months and fears he may not live to see his grandkids grow up News on NewsHub.org
Advances in surgical and diagnostic techniques have substantially increased the success of organ transplant operations. In 2000, a total of 22,827 organs were transplanted in the United States. However, in the preceding decade, the discrepancy between the number of available organs and the number of patients requiring a transplant operation has increased significantly. A British Medical Association (BMA) report has indicated that in the period between 1995 and 1999, 1,000 patients in the United Kingdom died whilst waiting for a heart, lung, or liver transplant. An average of 15 patients die every day in the US whilst awaiting an appropriate organ. The genuine figure will likely be inflated by the deaths of patients that are never waitlisted for a transplant. In addition, substantial numbers of patients die annually on account of the absence of both kidney donors and the lack of dialysis machines. The sale of human organs can be considered as a possible solution to the crippling shortage. The ...

Hi Marji, I think you better just take it easy and talk to your doctor. Like in the morning. I have been off of steroids since March. But I was in the hospital for a month while I was getting off of them. During that time they had a very hard time getting my blood pressure straightened around. It was consistantly 200/ 110 or something in that area. They had me on 3 different meds for my heart plus switched them around. Im not saying that the steroids is what was doing it as I was really sick already. I have to be off of them to get on the transplant list but sometimes I get very upset not having them. I get more tired. My inflamation is worse. Im sure that after being on them for so long my body is better off without them as far as the thinning bones and losing hair. They switched my meds to build up my bones but of course my antibiodies are going to start attacking me again. So they have me on strict ordered to get to the hospital at the first sign of a problem and they will give me a steroid ...

In my earlier post on the "engines of evolution" I promised a list of the "engines of variation" that provide the raw material for evolutionary change. Its taken me a while, but here it is. This list includes "random mutation, of course, but also 46 other sources of variation in either the genotypes or phenotypes of living organisms. Note that the list is not necessarily exhaustive, nor are any of the entries in the list necessarily limited to the level of structure or function under which they are listed. On the contrary, this is clearly a list of the minimum sources of variation between individuals in populations. A comprehensive list would almost certainly include hundreds (and possibly thousands) of more detailed processes. Also, the list includes processes that change either genotypes or phenotypes or both, but does not include processes that are combinations of other processes in the list, again implying that a comprehensive listing would be much longer and more detailed ...

No, they wont. There will be a new normal.. Wind the clock way back to 2019 (feels like years ago doesnt it). As we learned in Accentures study, businesses (as well as their consumers) were already reliant on marketing, customer service, and automation technologies to streamline buying journeys, reduce costs, and improve service. And some businesses were already using decision hubs to provide 24 / 7 next-best-actions, resulting in faster resolution times and better customer experience.. In the months ahead, digital experience bars will go even higher. More customers, shopping and looking for service online, will be even less tolerant of long wait times, poor service, and irrelevant recommendations. In short, this crisis just put customers digital expectations on steroids. Yet with any crisis comes conundrums and hard trade-offs. Consumers expect brands to provide flawless digital services and CX. Paradoxically, as companies watch labor, capital, and other resources dwindle, theyre realizing ...

As I thought about this, I began to wonder why I couldnt feel this way, and offer my patients a better experience, on a regular basis. I think there are internal and external factors. Internally, I may allow myself to become flustered. Thats a habit I can work on. Externally, it comes down to an access problem. Long wait lists translate into pressure to fit in as many patients as possible in a given clinic. That increases the chance of running late, and forces me to rush, leading to an unsatisfying experience for both me and my patients.
