Being invited into a womans space to hear her story is an honor and a privilege not to be taken lightly.. Two specific groups of women who are often vulnerable, isolated, and feel very misunderstood are those who have experienced abuse, either physical or sexual. Women who have been abused, whether past or present, have the inclination to struggle with shame and contempt, which can cause them to feel condemnation more readily.. While most of us desire to show loving-kindness toward these women, our words may not always flow well. Its normal when we come face to face with atrocities such as abuse that we shudder not knowing what to do. We may begin to frantically search our mind for the right thing to say-or worse yet- inadvertently say the wrong thing. Let me encourage you with a few practical steps you can take.. Seven Ways to Minister to a Vulnerable Woman. Simply be present.. Dont run away-literally or mentally.. Dont condemn.. Dont minimize.. Do acknowledge their pain (Gal. ...

CalEnviroScreen was designed by the California Environmental Protection Agency (CalEPA) to serve as a tool for identifying Californias most disadvantaged communities when prioritizing time, resources, and programs in those portions of the state that are most in need of assistance. However, the tool has long been criticized by disparate groups with widespread interests for its flaws in methodology. And while an update (CalEnviroScreen 3.0) was released late last year, the issue at hand remains unaddressed - and perhaps, worse. The latest version of the CalEnviroScreen tool continues to ignore Californias most disadvantaged communities, both rural and urban.. In total, 30 of Californias 58 counties do not have a single census tract identified in the top 25 percent scores of CalEnviroScreen - the methodology now used to consider applicants for Cap-and-Trade funding for disadvantaged communities. The use of CalEnviroScreen as a screening tool for grant solicitations and awards completely excludes ...
Almost 90 percent of residents living in long-term care facilities have some type of mobility limitation, which worsens quickly once they move into a long-term care facility. This means that they cannot easily walk or move around in their day-to-day lives and this can have serious consequences for their general health and well-being. Researchers have shown that exercise can help the mobility of even the oldest of adults but elderly residents in long-term care facilities still commonly sit or lie in bed for long periods of time, in many cases for most of their waking hours. The purpose of this research is to study the effect of a simple physical activity on the mobility and quality of life of residents in long-term care facilities. Researchers will introduce a sit-to-stand activity to health care aides which will become part of their daily routine with residents. The basic sit-to-stand movement is already a part of the daily activities that health care aides do with residents. Specifically, ...

When he was 7 years old, Guillermo Padilla landed in the hospital with appendicitis. While a frightening experience for any child, it was even more so for the young boy who spoke little English. His parents - Mexican immigrants who were determined that their son learn Spanish first - had to translate for him. That made an indelible impression on the youngster.
Its clear from seeing this that New York State cannot be relied on to police itself, said Assemblyman Brindisi, who is also running for Congress. When you have thousands of cases (of abuse and neglect) happening across the state-this being one of the most egregious-we must give some reassurance to families that their loved ones are being taken care of, he added, according to the AP. The call for an investigation followed a just-published AP story that indicated that it is often easier to find health and safety information for local restaurants than it is to learn about conditions at facilities serving approximately one million of the states most vulnerable residents.. In this particular case, a man, 41, was found to have maggot crawling around his breathing tube on two occasions. He was residing in a Rome, New York state-owned and -operated small group home after suffering severe brain trauma due to a 1991 car crash that left him unable to last walk, talk, or feed himself, according to ...

Improve Public Payer Reimbursement and Rationalize Payment for all Health Care Services. Levels of public reimbursement are a contributing factor to higher-than-necessary private insurance premiums. Medicaid, Medicare, and the Health Safety Net Trust Fund reimburse hospitals - on average -- at significantly below the cost of providing care. These payment inequities are particularly acute for services to some of our most vulnerable patients, such as for inpatient mental health care. Because of this, we are forced to identify alternative revenue sources to cover our operating costs. One such source is our reimbursement from private insurers. The overall reimbursement we receive from private health insurers ultimately subsidizes our losses from public payer contracts that fail to cover our costs. State budget reductions for academic medical centers over the last two fiscal years have had a sustained, significant negative impact on our fiscal health. But these two years have been different only in ...
The critics of the RCP report are responding with, show me the money. They are saying that the preceding can only be accomplished with a lot more money being put into the NHS. Maybe, the NHS should take a page out of the United States approach to health care reform: better care; reduced cost and improved population health. I am not familiar with how much the cost of health care is in Great Britain, but whenever you seek to improve the delivery and quality of care, there is usually a savings associated with such improvements. Such savings can be generated from Lean approaches, Six Sigma initiatives, labor savings, moving care outside the hospital, standardization of practices and the list goes on. It appears that NHS is really trying to address their care crisis and to protect their most vulnerable patients, the very sick and the elderly ...
To preserve our freedoms, to promote equality, to fight patriarchy and misogyny and to protect the most vulnerable women in society a raft of anti-gender legislation is urgently needed, and the UK government should proudly pioneer it.. This anti-gender legislation must:. 1: Enshrine in law the definition of a woman as a person of the female biological sex, the only exception being those individuals with defined, demonstrable objective chromosomal or physiological abnormalities who do not clearly fit into either sex (as determined by a panel of impartial medical practitioners).. 2: Make it illegal to provide a service, run an organisation, organise a sporting event or run a facility for both sexes where it is advertised as being for women or girls.. 3: Make it illegal to masquerade as a member of the opposite sex for the purposes of entering a single-sex environment or participating in a single-sex activity.. 4: Reinforce the inalienable right to refer to another person using scientifically ...

High HIV prevalence among hard to reach gay men in Jamaica is associated with high social vulnerability. Background: Due to strong social stigma associated with men who have sex with men (MSM) and HIV, and difficulty reaching MSM, there has been no HIV or behaviour survey among MSM in Jamaica since 1995. HIV prevalence among adults is 1.5%. The objective of this study was to estimate HIV prevalence and identify risk factors among MSM in order to improve prevention approaches.. Methods: Following discussion with MSM stakeholders, an experienced research nurse was introduced to the community and attended their activities. With the help of influential MSM and others she invited MSM in four parishes to participate in a cross-sectional survey between March 2007 and January 2008. MSM were interviewed, blood taken for HIV and syphilis, and urine taken for gonorrhea, Chlamydia and trichomonas (Gen Probe). Sensitive behavioural data was interviewer administered and self-completed. Confidential results ...

Fifty women were brought in from the streets to hear Marion, a former prostitute, share how she was able to find hope and a new life in Jesus.. Covering health issues as a freelancer based in India, Ankita Rao encountered plenty of misery and injustice, but one incident stuck with her. In less than six hours on a November day in 2014, a doctor sterilized 83 women using the same instrument, merely dunking it in disinfectant as he waited for the next patient to be placed in front of him. At least 13 of those women-most of them in their 20s and 30s-died.. In a story Rao wrote for Quartz India in 2015, she went beyond that shocking incident to uncover the societal forces that made it possible. She learned that thousands of poor women in the state of Chhattisgarh in central India were undergoing tubectomies, often after showing up at the hospital for a different reason, such as an infection. Community health workers get paid a fee for each woman they bring in for sterilization, and risk losing their jobs if they dont fulfill their quotas. The procedures are easy and lucrative for doctors, Rao found, and the women involved are mostly lower ...

New guidance say doctors and nurses must provide water to the dying. The National Institute of Health and Care Excellence (Nice) has drafted new guidance demanding that patients have access to fluids. The Liverpool Care Pathway which was withdrawn last year saw elderly and other vulnerable patients denied food and fluids, often without their knowledge or consent, or that of their relatives. However such practices may still be going on, albeit under different names. Roger Goss from Patient Concern commented saying: It is shocking that this has to be spelled out to doctors and nurses. [Telegraph, 29 July] Antonia Tully of the Patients First Network said: We are still seeing a lack of confidence among relatives about the way in which vulnerable patients are cared for. We are continuing to advise family and friends to maintain constant vigilance to make sure that those they love receive basic care, including food and water ...

Computer-assisted cognitive-behavior therapy (CCBT) for depression in primary care will be evaluated in a trial with 320 patients randomly assigned to CCBT or treatment as usual (TAU). The study will disseminate a therapy method found to be effective in psychiatric settings into primary care - a setting where there have been significant problems in delivery of adequate, evidence-based treatment for depression. The study will include a high percentage of disadvantaged patients - a population that has been largely ignored in previous research in CCBT. There have been no previous studies of CCBT for depression in primary care that have enrolled large numbers of disadvantaged patients. The form of CCBT used in this study is designed to increase access to effective therapy, provide a cost-effective method, and be a sustainable model for wide-spread use in primary care.. In order to deliver therapy in a practical manner that can be replicated in other primary care practices, patients with significant ...

We worked at Reach-Out Orphans and Vulnerable Children for two weeks as part of our global health externship. So far we have assisted in two free health clinics to the Reach-Out community and its surrounding areas, one in West Kochieng and the other in East Kochieng. Both of the clinics were very successful and had an exceptional turnout of 350 and 400 respectively. We were very thankful to have nursing students from Moi University assist us for both of the health clinics. Collectively we were able to recognize and treat a variety of illnesses such as malaria, respiratory tract infections and eye infections. We also provided wound care and voluntary HIV testing accompanied by counseling.. Pam McQuide and Hezekiah were a big influence in another major project we did of distributing sanitary pads to the female students. Background Crucial connections between sexual network structure and the distribution of HIV remain inadequately understood, especially in regard to the role of concurrency and age disparity in relationships, and how these network characteristics correlate with each other and other risk factors. Social desirability bias and inaccurate recall are obstacles to obtaining valid, detailed information about sexual behaviour and relationship histories. Therefore, this study aims to use novel research methods in order to determine whether HIV status is associated with age-disparity and sexual connectedness as well as establish the primary behavioural and socio-demographic predictors of the egocentric and community sexual network structures. Method/Design We will conduct a cross-sectional survey that uses a questionnaire exploring one-year sexual histories, with a focus on timing and age disparity of relationships, as well as other risk factors such as unprotected intercourse and the use of alcohol and ...

The Health Disparities Core is dedicated to fostering the ongoing evaluation of social differences in the outcomes of translation, the development of practical methodologies for monitoring changes in health status across population subgroups within practice settings, and the provision of expertise, guidance and mentorship to junior investigators interested in this special subtype of translation research. This work will take place in both fully insured populations (managed care settings included in this application) as well as in the vulnerable patients cared for by safety net providers (represented by the California Diabetes Program). A comprehensive evaluation of the translation process and its potential impact on disparities would be incomplete without understanding both settings.
The report cited several disturbing cases including one case where a male patient was secluded 90% of the time with no quality of life and no other service willing to take him. In another case, vulnerable women were housed with predatory men but were unwilling to take forward allegations of physical and sexual abuse because they were afraid of what would happen.. In some wards, detained patients were found sleeping on mattresses on floors in doctors offices and in rooms due to high levels of occupancy, commissioners found in the six months leading up to November 2007.. Other cases included a detained young man with a learning disability who had sustained a double fracture of his arm as a result of being restrained, and a deaf man who had been in a special hospital for 60 years.. In the first published research project worldwide on this new approach to testing medicines in children, the findings were announced in leading US medical journal Pediatrics.. The study, which involves the use of blood spots obtained from a simple heel-prick, took place in the Belfast Hospital for Sick Children and the School of Pharmacy at Queens.. The research was carried out by a team from the Universitys School of Pharmacy in partnership with the Regional Neonatal Unit in the Royal Maternity Hospital. It was funded by the Health and Social Care Research and Development Office (HSC R&D) and Action Medical Research.. Principal Investigator, Queens Professor of Pharmacy Practice James McElnay said: This type of testing will obviously reduce the discomfort of medicine testing in these vulnerable patients. What is even more important, however, is that it will ensure maximum accuracy in calculating the most appropriate dose of a medicine for a sick child.. Some 80 per cent of infants in ...

Mean systolic and diastolic BP both declined in the years before death in patients with diabetes. This annual rate of decline was significantly more marked (−3.2 mmHg systolic and −1.3 mmHg diastolic) than in the patients who remained alive. Moreover, the decline was evident after adjustment for major organ failure-heart, kidney, and liver-as well as for common causes of death such as malignancies and stroke. These findings may signal an underlying population of vulnerable patients, although it is unclear whether the BP decline is merely a symptom of this vulnerability or is one of several physiologic contributors to the dying process.. The rate of change in BP has been of lesser significance than threshold values that serve as guides to initiation or adherence to antihypertensive therapy (12). Studies of outcomes in those who experience significant changes (either increases or decreases) in BP would be appropriate. Although hypertension in middle age is a risk factor for death, it is not ...
