Purpose:: To characterize the first in-human intraoperative imaging using a custom prototype spectral-domain microscope-integrated optical coherence tomography (MIOCT) device during vitreoretinal surgery with instruments in the eye. Methods:: Under institutional review board approval for a prospective intraoperative study, MIOCT images were obtained at surgical pauses with instruments held static in the vitreous cavity and then concurrently with surgical maneuvers. Postoperatively, MIOCT images obtained at surgical pauses were compared with images obtained with a high-resolution handheld spectral-domain OCT (HHOCT) system with objective endpoints, including acquisition of images acceptable for analysis and identification of predefined macular morphologic or pathologic features. We evaluated the potential utility of live volumetric (4D) intraoperative swept-source microscope-integrated OCT (SS-MIOCT) in DTRD surgery.. This study points to a trend towards subspecialisation and tertiary referral in vitreoretinal surgery. We perceive a change in professional attitude with respect to RRD repair in recent years. A decade ago, the treatment of RRD in the Mersey region was mainly the responsibility of the general ophthalmologist. Recent advances meanwhile have led to a wider use of expensive technology in vitreoretinal surgery, including indirect operating systems with wide angled optics, perfluorocarbon liquids, and retinectomy. Coupled with the growth of day case surgery and its implications on the provision of beds and theatre resources, surgeons working in general ophthalmic units may now be less well equipped in treating RRD. BACKGROUND
We evaluated the anesthetic efficacy and the postoperative analgesic effects of 0.75% levobupivacaine versus 0.75% ropivacaine for peribulbar anesthesia in patients undergoing primary vitreoretinal surgery.
METHODS
We investigated 120 patients subjected to vitreoretinal surgery under peribulbar anesthesia. They were randomized into two equal groups according to the local anesthetic (LA) used, namely, 0.75% levobupivacaine or 0.75% ropivacaine, both with the addition of hyaluronidase. Nerve block was carried out by injection of 5-7 mL of the LA using single injection percutaneous peribulbar anesthesia with a short needle.
RESULTS
When compared with 0.75% ropivacaine, 0.75% levobupivacaine provided more successful akinesia at 10 min after block (P=0.026), fewer supplementary injections (P=0.026), and less volume (mL) was used (P=0.031). A force sensor (FS) integrated with conventional surgical tools may significantly improve the surgery outcome by providing tactile feedback to the surgeon. We report on an implementation of coherence revival-based heterodyne swept source optical coherence tomography that is capable of simultaneously imaging the anterior and posterior eye. A polarization-encoded sample arm was used to efficiently focus orthogonal polarizations on the anterior segment and retina. Depth encoding was achieved using coherence revival, which allows for multiple depths within a sample to be simultaneously imaged and frequency encoded by carefully controlling the optical pathlength of each sample path. We present optical coherence tomography (OCT) as a method to image myofibers in excised intact heart preparations. Three-dimensional (3-D) image sets were gathered from the rabbit right ventricular free wall (RVFW) using a microscope-integrated OCT system. An automated algorithm for fiber orientation quantification in the plane parallel to the wall surface was developed. The algorithm was validated by comparison with manual measurements. Near-infrared (NIR) fluorescence-based imaging has the potential to detect tumors with high sensitivity and high specificity in real time. Current approaches employ fluorescent dyes and operate mostly in the first NIR window (NIR-I, 700 - 900 nm) for image-guided use. To image tissue, visible light and fluorescence images are either taken with the same detector or with multiple cameras in different spectral ranges simultaneously or sequentially. Systems that can detect both visible and fluorescent imaging simultaneously use silicon-based detectors that are sensitive to visible and NIR-I spectral bands. However, to properly separate these narrowly-spaced passbands, expensive spectral filters with narrow fields-of-view are required. Other systems which instead use multiple cameras to detect visible light, NIR-I, and/or the second NIR window (NIR-II, 1000 - 1700 nm), must fuse the images from all these detectors to create the combined image via post processing.
