The viridans streptococci are a large group of commensal streptococcal Gram-positive bacteria species that are either α-hemolytic, producing a green coloration on blood agar plates (hence the name "viridans", from Latin "vĭrĭdis", green), or nonhemolytic. The pseudo-taxonomic term "Streptococcus viridans" is often used to refer to this group of species, but writers that do not like to use the pseudotaxonomic term (which treats a group of species as if they were one species) prefer the terms viridans streptococci, viridans group streptococci (VGS), or viridans streptococcal species. Objectives: We studied the species distribution and antimicrobial susceptibility of viridans streptococci (VS) isolates causing nosocomial bloodstream infections (BSIs) in Finnish hospitals. Patients and methods: Patients with nosocomial BSIs due to VS were identified through a hospital-wide prospective laboratory-based surveillance in two university and two regional hospitals during September 1998-August 2001. Isolates of VS were sent to the reference laboratory for species confirmation and antimicrobial susceptibility testing.. Results: A total of 2038 nosocomial BSIs were identified; 108 (5%) of the BSIs were caused by VS. We thank Dr. DeSimone and colleagues for their interest in our paper. In our study, we focused on infective endocarditis (IE) microbiology to Staphylococcus, Streptococcus, gram-negative, and fungal organisms. Our study differs from previous papers from the Nationwide Inpatient Sample database (1,2) in 2 aspects. First, we did include infections from all streptococcal groups (A, B, C, D [enterococcus], G, and unspecified) and did not report viridans group Streptococcus (VGS) separately. Second, previous studies on IE trends in the United States had a very limited follow-up of only 2 years after the guideline publication. Longer follow-up studies are necessary to assess the impact of any "practice changing" guideline because it takes years to note the impact of such change. Patients with bacteremia after dental extraction, third-molar surgery, dental scaling, endodontic treatment, and bilateral tonsillectomy were studied by means of lysis-filtration of blood samples with subsequent aerobic and anaerobic incubation. Samples were obtained before, during, and 10 min after treatment. Bacteremia was observed in 100% of patients after dental extraction, 55% of patients after third-molar surgery, 70% of patients after dental scaling, 20% of patients after endodontic treatment, and 55% of patients after bilateral tonsillectomy. There are challenges in viridans group streptococci (VGS) identification especially for the mitis group. Few studies have investigated the performance of MALDI-TOF MS system in VGS identification. Using 16S rRNA gene and gyrB gene sequencing as a gold standard, the performance of two MALDI-TOF MS instruments in the identification of 181 VGS clinical isolates was studied. The Bruker Biotyper and Vitek MS IVD systems correctly identified 88.4% and 98.9% of the 181 isolates, respectively. The Vitek MS RUO system was the least reliable, only correctly identifying 38.7% of the isolates to species level with several misidentifications and invalid results. The Bruker Biotyper system was very unreliable in the identification of species within the mitis group. Among 22 non-pneumococci isolates (S. mitis/S. oralis/S. pseudopneumoniae), Biotyper misidentified 21 of them as S. pneumoniae leading to a low sensitivity and low positive predictive value in these species. How my infection was found is I was in extreme pain with pain traveling up my left arm and into the neck area a constant pain that wouldnt let up so after three days of it I went to my p.m. doc who thought I had a torn muscle in my left arm...so was put on steroids for two days along with a muscle relaxer, nothing seemed to help so my p.m. doc admitted me to his hospital to rule out other stuff by doing a few x-rays and some blood work and again nothing showed up and the pain was so bad the doc gave me a morphine drip and a butten to push when I needed it, which was alot! After three days he transferred me to a larger hospital where again a ton of procedures were done and after all the scans ,x-rays an MRI picked up a huge abcess was deep into the left side of my chest and it also showed that the left collar bone was infected as well as the sternum and a rib , I still have some of my sternum as it was scrapped but the doc did remove all the other bones that was infected. During a single year six patients with bacterial endocarditis due to Streptococcus viridans (two cases) or enterococcus (four cases) were successfully treated with vancomycin. There were no failures. Vancomycin has been effective for staphylococcal endocarditis, but we could find only one reported case of vancomycin cure of S. viridans endocarditis and none of enterococcus endocarditis. In our cases vancomycin was administered either because of a history of serious penicillin allergy or because penicillin or penicillin-streptomycin therapy could not be tolerated. To describe the rate of response to an antibiotic-lock technique (ALT) in the treatment of venous access port (VAP)-related bacteremia and to analyze the role of the reservoir in the persistence of infection, we reported the data from 12 human immunodeficiency virus-infected and 8 oncologic patients with VAP-related bacteremia. The ALT consisted of intracatheter delivery of antibiotics and was associated with a systemic antibiotic infusion. We monitored clinical manifestations and performed qualitative and quantitative blood cultures during and at the end of the treatment. Four patients had catheters removed before antibiotic treatment. Of the 16 patients who were treated with the ALT, 5 (31%) were cured, as determined by negative cultures of blood and of samples from the catheter; 2 (12.5%) were cured but had recurrent infection with another microorganism; and 9 (56%) had persistent positive cultures of blood and of samples from the tip, reservoir, or both of the VAP. Patients who have persistent positive symptoms can benefit from cognitive behavior therapy. Impairments in social competence can be addressed with social skills training. Supported employment programs may increase the ability of patients who have schizophrenia to obtain competitive employment. Family psychoeducational approaches, treatments for comorbid substance abuse, and cognitive rehabilitation approaches that enhance or bypass impairments in cognitive functioning are discussed.
