Incapacitating vertebrobasilar insufficiency is generally associated with bilateral vertebral artery disease, whereas unilateral vertebral artery stenosis usually is clinically silent. Regional brain perfusion has not been part of the routine evaluation of patients with vertebrobasilar insufficiency. This report describes two patients who had isolated unilateral vertebral artery stenosis operatively corrected to eliminate their incapacitating vertebrobasilar insufficiency. Hindbrain hypoperfusion was identified preoperatively and evaluated postoperatively, then correlated with patient presentation and response to revascularization.. Two patients with incapacitating vertebrobasilar insufficiency presented with isolated unilateral vertebral artery stenosis with patent, nonstenotic internal carotid arteries. Hindbrain hypoperfusion was demonstrated by iodine-123-iodoamphetamine single-photon emission computed tomography preoperatively and demonstrated significant improvement following ...

Treatment of acute basilar artery occlusion remains clinically challenging. In this preliminary study of diffusion-perfusion MRI changes in patients treated with intra-arterial thrombolysis and studied serially with multimodal MRI, we found that diffusion-perfusion mismatch can be visualized clearly in the posterior circulation, including the brain stem, and can be seen in some patients well beyond 6 hours from symptom onset. Moreover, we were able to demonstrate improvement of this mismatch region after basilar artery recanalization with intra-arterial thrombolytic therapy. This improvement may represent therapeutic salvage of the mismatch region; however, a controlled study is required to confirm this.. Several prior reports have begun to assess the role of advanced MRI techniques in acute posterior circulation stroke.10,11 Du Mesnil de Rochement and colleagues10 studied 4 patients with acute basilar artery occlusions with DWI and MR angiography sequences. They found variable patterns of DWI ...
Approximately 700,000 strokes occur annually in the U.S. making it the third leading cause of death and the leading cause of permanent disability among adults. Over one third of strokes occur in the posterior circulation, the leading cause of which is vertebrobasilar occlusive disease secondary to atherosclerosis. Symptomatic vertebrobasilar disease (VBD) carries a high annual risk of stroke, averaging 10-15% per year despite medical therapy. This represents a potentially treatable high risk stroke etiology. Advances in endovascular angioplasty and stenting have created new treatment options, but these interventions carry significant risks, and the selection criteria for appropriate candidates remains uncertain. Determining predictors of stroke in this population is the first step toward identifying those high risk patients most suitable for consideration of intervention. Our preliminary studies suggest that the risk of stroke in VBD is strongly related to the extent to which intracranial blood ...

Background and Purpose-Detection of acute infarction in the posterior circulation is challenging. We aimed to determine the additional value of tomograpy (CT) perfusion to noncontrast CT and CT angiography source images for infarct detection and localization in patients suspected of acute ischemic posterior circulation stroke. Methods-Patients with suspected acute ischemic ... read more posterior circulation stroke were selected from the Dutch acute Stroke Trial (DUST) study. Patients underwent noncontrast CT, CT angiography, and CT perfusion within 9 hours after stroke onset and CT or MRI on follow-up. Images were evaluated for signs and location of ischemia. Discrimination of 3 hierarchical logistic regression models (noncontrast CT [A], added CT angiography source images [B], and CT perfusion [C]) was compared with C-statistics. Results-Of 88 patients, 76 (86%) had a clinical diagnosis of ischemic stroke on discharge and 42 patients (48%) showed a posterior circulation infarct on follow-up ...
BACKGROUND: Vertebrobasilar dolichoectasia (VBD) is a rare vasculopathy. The etiology of this disease is unknown. Transient ischemic attacks (TIAs) of vertebrobasilar system refer to a transient (, 24 hours) lowering of blood flow in the posterior circulation of the brain. We present a case of dolichoectasia in the vertebrobasilar artery that presented with TIAs. CASE REPORT: A hypertensive 54-year-old man with true vertigo, nausea, imbalance, dysarthria, dysmetria, horizontal nystagmus, and gait ataxia was referred to Alzahra Hospital, Isfahan, Iran ...
Stroke is one of the important causes of disability and death in the world, in which more than half were ischemic strokes. About 1/4 of the ischemic stroke occurred in the vertebral basilar artery system, especially when in the presence of extracranial proximal vertebral artery stenosis. Vertebral artery stenting is a minimally invasive method for the reconstruction of vertebral artery stenosis and the early clinical studies showed that it was feasible, safe and effective, but the high rate of restenosis has become a bottleneck restricting its development. Previous systematic review had suggested that the drug eluting stent might reduce the incidence of restenosis of vertebral artery. However, prospective randomized controlled trials comparing the efficacy of bare metal stents and drug eluting stents on the prevention of restenosis remains absent. ...

Fourteen patients (14.1%) received pre-procedure IV rt-PA. The mean time-from-symptom-onset (TSO) to arterial puncture was 9.23±12.45 h (median, 6.15) with 34.1% of patients treated ,8 h. All patients were treated with at least one pass of Merci (average, 2.3±1.6). Other treatments included: intra-arterial rt-PA/urokinase (53.5%), proximal angioplasty (22.1%, 19/86) or stenting (14.0%, 12/86), Penumbra (15.3%, 11/72), and other mechanical (29.1%, 25/86). Successful recanalization was achieved in 88.9% (88/99). The overall mean (median) NIHSS dropped from 20.5±9.4/19 on admission to 13.4±14.5/6 on discharge. The overall rate of good outcomes was 31.6% (31/98). 90-day mortality was 38.8% (38/98). The SICH rate was 8.9% (7/79). The rate of good outcome was numerically higher (34.5% vs 9.1%) and mortality was numerically ...
by Davide Strambo. In these exciting years of continuous innovations in stroke care, there is something never changing: stroke is a clinical-based diagnosis and we dont have to be too confident on MRI: DWI-NEGATIVE STROKE: A SYSTEMATIC REVIEW AND META-ANALYSIS OF FREQUENCY AND OUTCOMES (R. The incidence of ischemic arterial vascular events (stroke plus transient ischemic attacks) during pregnancy is approximately equal to the incidence of subarachnoid hemorrhage, but death is rare unless the stroke is symptomatic of systemic disease. Pregnancy increases the risk of ischemic stroke at least by a factor of 3 or 4.30 Most pregnancy-associated ischemic strokes occur in women younger than 30 years because most pregnancies occur before that age. The risk of cerebral ischemia increases with age for pregnant and nonpregnant women.. Approximately 30% of pregnancy-related arterial occlusions occur in each of the second and third trimesters.30,31 Fully one quarter of pregnancy-associated strokes occur in the first post-partum week. Thirty-five percent are occlusions of the middle cerebral artery, which, in the absence of demonstrable atheromatous disease, may be emboli from or through the heart. The internal carotid artery is occluded in 20%. Vertebrobasilar stroke is rare in both ...

N2 - Background and aims: Idiopathic sudden hearing loss (ISHL) is usually unilateral and can be anything within a range of a slight impairment of hearing to virtual deafness. One of the most common etiologies for ISHL is circulatory disturbance (most often vertebrobasilar ischemia). Vertebrobasilar ischemia (VBI) causes deafness because most of the auditory system including the inner ears is supplied from the vertebrobasilar system. ISHL of vascular cause is important for neurologists to recognize, because it sometimes appears as a warning sign of impending vertebrobasilar ischemic stroke. Previously, we produced cochlear ischemia by occluding both vertebral arteries in gerbils and established ...

Artery disease in the neck vessels (carotid/basilar) is associated with risk for stroke. Your doc, vascular provider can best assess all the associated factors and give you a risk assessment. Treatments vary from medical management, nicotine cessation, stenting, surgery and combinations of them all.
Benedikt syndrome, also called Benedikts syndrome or paramedian midbrain syndrome, is a rare type of posterior circulation stroke of the brain, with a range of neurological symptoms affecting the midbrain, cerebellum and other related structures. It is characterized by the presence of an oculomotor nerve (CN III) palsy and cerebellar ataxia including tremor and involuntary choreoathetotic movements. Neuroanatomical structures affected include CNIII nucleus, Red nucleus, corticospinal tracts, brachium conjunctivum, and the superior cerebellar peduncle decussation. It has a very similar cause, morphology and signs and symptoms to Webers syndrome; the main difference between the two being that Webers is more associated with hemiplegia (i.e. paralysis), and Benedikts with hemiataxia (i.e. disturbed coordination of movements). It is also similar to Claudes syndrome, but is distinguishable in that Benedikts has more predominant tremor and choreoathetotic movements while Claudes is more marked ...

BACKGROUND: The National Institutes of Health Stroke Scale (NIHSS) is the most adopted stroke patients evaluation tool in emergency settings to assess the severity of stroke and to determine the patients eligibility for specific treatments. Computed tomography perfusion (CTP) is crucial to identify salvageable tissue that can benefit from the reperfusion treatment. The aim of this study is to identify the relation between the NIHSS scores and the hypoperfused volumes evaluated by CTP in patients with hyperacute ischemic stroke. METHODS: This retrospective study was conducted on 105 patients with ischemic stroke who underwent NIHSS assessment and CTP in the hyperacute phase. Hypoperfused volume was evaluated by CTP maps processed with semi-automatic algorithm. An analysis was conducted to determine the degree of correlation between the NIHSS scores and the ischemic lesion volumes and to investigate the relation between the anterior and the posterior circulation strokes, as well as between the ...
A 55-year-old man presented in October 2004 with general unease, vomiting, and gait disturbance. Initially diagnosed with an inner ear infection, the patients symptoms did not improve and he was evaluated further. MRI revealed a cerebellar lesion, which led to the suspicion of a posterior circulation stroke, and he was started on antiplatelet drugs. However, an angiogram suggested no vascular pathology. In March 2011, he had another episode of ataxia and MRI showed a right brainstem lesion. Cryptogenic stroke was reconsidered. In March 2013, the patient developed paraparesis with urinary and bowel symptoms. MRI revealed myelitis of the midthoracic spinal cord. Oligoclonal bands were present in both serum and CSF and steroids were begun. In February 2014, he was admitted with expressive aphasia and right hemiparesis. MRI (figure 1) confirmed a new T2-hyperintense lesion in the left frontal lobe and he was given IV methylprednisolone and subsequently underwent plasma exchange. Aquaporin 4 (AQP4) ...
