OBJECTIVES: Arterial access closure after endovascular aneurysm repair (EVAR) can be achieved using three different approaches: percutaneous closure devices, surgical exposure and direct suture (cutdown), and the less invasive fascial closure technique. The aim of this study was to report on the intra-operative, in hospital, and three month outcome of fascial closure and cutdown, and to determine risk factors for failure.. METHODS: The primary outcome was assessed in 439 groins in 225 elective EVAR patients recruited consecutively and prospectively from February 1, 2011 to August 31, 2014. During the study period, fascial closure and cutdown were first and second line closing techniques. Compared with fascial closure, procedures completed with cutdown had lower BMI, thinner subcutaneous tissue of the groin and more complex femoral anatomy. Computed tomographic angiography (CTA) and duplex ultrasound (DUS) of the groin were performed pre-operatively and three months after EVAR. Retrospective ...
Objective: Prior studies suggest that percutaneous access for endovascular abdominal aortic aneurysm repair (pEVAR) offers significant operative and postoperative benefits compared with femoral cutdown (cEVAR). National data on this topic, however, are limited. We compared patient selection and outcomes for elective pEVAR and cEVAR. Methods: We identified all patients undergoing ... read more either pEVAR (bilateral percutaneous access, whether successful or not) or cEVAR (at least one planned groin cutdown) for abdominal aortic aneurysms from January 2011 to December 2013 in the Targeted Vascular data set from the American College of Surgeons National Surgical Quality Improvement Program database. Emergent cases, ruptures, cases with an iliac conduit, and cases with a preoperative wound infection were excluded. Groups were compared by c2 test or t-test or the Mann-Whitney test where appropriate. Results: We identified 4112 patients undergoing elective EVAR, 3004 cEVAR patients (73%) and 1108 ...

Although it is nowhere near as safe and non-invasive as most of the previous procedures, venous cutdown is another procedure that vascular surgeons sometimes have to perform. It is often done as an emergency response to severe trauma as well as hypovolemic shock. The way that it works is based on completely exposing the vein into which a doctor subsequently inserts a cannula. For those unfamiliar, cannulas are tubes that allow medical experts to provide and take essential fluids and nutrients from the body. Luckily, however, venous cutdown is nowhere near as prominent as it used to be. It is because it has been replaced by safer alternatives such as ultrasound-guided central venous catheters as well as the Seldinger technique.. Of course, the list goes on to include procedures like atherectomy, arteriotomy, various laser treatments, carotid stenting, revascularization, and many more. Since going through every single, one of them would not be feasible. However, the previously mentioned few are a ...
November 2, 2017-The Society for Vascular Surgery (SVS) announced that findings from a comparison of percutaneous access versus femoral cutdown during endovascular repair of ruptured abdominal aortic aneurysm (AAA) were published by Samuel L. Chen, MD, et al in the Journal of Vascular Surgery (2017;66:1364-1370).. The study addresses the question of whether the fully percutaneous technique offers any benefit over open femoral cutdown in the emergency setting. Investigators reviewed a large national database and found that using an entirely percutaneous approach is as safe as incision-based approaches.. According to SVS, led by Roy M. Fujitani, MD, researchers retrospectively studied 502 patients who underwent ruptured endovascular aortic aneurysm repair and were entered into the American College of Surgeons National Surgical Quality Improvement Program between 2011 and 2014. The percentages of different endovascular repair access rates were 24% for bilateral percutaneous, 64% for bilateral ...

Abstract:. BACKGROUND/PURPOSE: To evaluate the technical success and complications of image-guided central venous port (CVP) placement with subclavian vein (SCV) access in pediatric oncology population.. MATERIALS AND METHODS: Ninety-two children (52 boys, 40 girls; mean age, 8.5years) underwent CVP implantation under local anesthesia with conscious sedation. SCV access was firstly attempted under ultrasonographic guidance and CVP implantation was performed under fluoroscopic guidance. Technical success, peri-procedural (,24h) complication, and post-procedural (,24h) complication were assessed.. RESULTS: In total, 102 CVPs were implanted in 92 children with a mean catheter time of 364days (total, 38,224days; range, 14-1911days). In three small children, conversion of SCV access to internal jugular vein access yielded a primary technical success rate of 97.1% and overall technical success rate of 100%. Three minor peri-procedural complications were observed (2.9%) and seven post-procedural ...
Diabetes and smoking increase the risk of developing CLI. Diabetes is the cause of most nontraumatic lower-extremity amputations in the United States.16 Diabetes increases the risk of amputation nearly fourfold, even with similar levels of blood flow limitation as in nondiabetic patients.17 Cigarette smoking also increases the risk that PAD will progress to CLI. In a study of 343 consecutive patients with intermittent claudication, 16% of those who continued to smoke developed CLI, compared to none in those who were able to stop smoking.18 In 190 patients undergoing lower-extremity revascularization followed for 3 years, those who smoked more than 15 cigarettes a day had a 10-fold higher risk (21%) of amputation compared to those who smoked fewer than 15 cigarettes a day (2%).19 Critical limb ischemia occurs as a consequence of tissue ischemia at rest, and it manifests as foot pain, nonhealing ulcers, or tissue gangrene. The pain is often severe and unremitting and localized to the acral portion ...
On Memorial Day weekend Jim Yount of the American Cryonics Society (ACS) and Naomi Reynolds of BioPreservation, Inc. (BPI) drove down from the Bay Area for a weekend of intensive training. Our facility was deliberately emptied of everyone but myself, Jim, and Naomi. The objectives were tough ones: Jim and Naomi would perform four jugular cutdowns on a dog, and two femoral cutdowns. They would pass small and large bore cannulae on the jugular cutdowns to simulate getting vascular access for administration of transport medications during human cryopreservations. They would then (using the femoral-femoral approach) go on cardiopulmonary bypass, hemodilute the dog to a hematocrit of less than 20 (diluting its blood by about half), and finally come off bypass. They would do all of this without any significant physical assistance from me. (I didnt even scrub in; I just stayed in my street clothes and put on a mask and cap.) We started at noon on Saturday. By 4:30 PM they had two jugular cutdowns ...

MUMBAI: As the country enters the final week of the third phase of the COVID-19-induced lockdown, public sentiment is getting into negative territories with over 50 per cent of those polled having a negative outlook on income. What is more, younger and those in the middle-income consumers are more worried about their income post-COVID, loan repayments and spending, says an international survey. According to a survey by international consultancy Boston Consulting Group, as much as 85 per cent of those in the poll are worried about servicing loans post-moratorium, while more than 40 per cent are planning to cut down spending in the next six months as they fear about their jobs and income. The survey, carried out during the third phase of lockdown (May 4-17) among 1,300 consumers in metros and Tier 1 & 2 cities, is part of a global COVID-19 consumer sentiment research. Despite lockdown relaxations, concerns across health, economic situation and daily lifestyle continue to remain high as consumers ...

I really do love the simplicity of Unraid for my home server uses. My question and request is probably far from what has been posted here. But i run an ESX Server in the Data Centre for my critical cloud apps. I have been unsucessful trying to get Unraid working on ESX using a USB thumbdrive booting in an VM. Are there any plans to release a cutdown Unraid Version for VM. I am happy to pay a license for it. I simply want a stripped down version of Unraid for Docker and VM Management. Every other Linux OS has a weird and frankly annoying way to manage Dockers, i really love Unraids approach to this. In my research of trying to get this working there was a fair amount of interest in a VM version of Unraid. I would love to Beta test Unraid in a VM if youre ever looking for someone to do that. ...

VMwares entry in the hyperconverged space is both a big event and a non-event. Its big news because it will increase adoption of hyperconverged infrastructure, particularly in the SMB space, through increased awareness and because EVO:RAIL is backed by large vendors. Its a non-event in that EVO:RAIL doesnt offer anything new other than form factor - its standard VMware technologies and you could already get similar (some would say superior) products from the likes of Nutanix and Simplivity and others. Ill be posting my (generally positive) thoughts on EVO:RAIL soon (now posted).. NSX is here to stay. A cutdown version, NSX Lite, looks set to become a core part of vSphere at some point in the future, probably towards the end of 2015 (my guess). It may not have mass market adoption yet but theres a lot of interest and actual customer deployments. Its already baked into vCloud Air and will be part of the EVO:RACK stack when its released. VMware are clearly betting the business on NSX ...

Eating too much sugar is bad for your health. Youve heard it many times before. Excessive consumption can increase your risk for obesity, heart disease and a host of other health complications. The World Health Organization recommends the average adult consume no more than 25 grams of sugar a day, but exceeding this is all too easy.. A single can of Coca-Cola, for instance, packs 39 grams of the stuff. And added sugar sneaks into unsuspecting edibles, like hamburgers and healthy Greek yogurts. Cutting back on your sugar intake is a smart choice, but its tough to know where to start. If youre looking to slow down, start with a few of the tweaks below. Introduce them to your everyday routine, and eventually theyll turn into a habit.. Make over your morning coffee. The two sugars you routinely put into your cup of joe can add up. Try reducing the amount of sugar you use little by little, and rely on full-fat dairy to provide satisfaction. See if your taste buds respond well to cinnamon; the ...

A hot issue in District 7 is the Natural Areas Program that aims to cut down a few thousand healthy, non-native trees and replace them with native plants. It is a priority for me because it symbolizes how City Hall lacks common sense by letting ideology lead public policy. It also symbolizes how City Hall spends our money on pet projects before focusing on the basic functions of a city. For example, street repair. We can ban toys in Happy Meals and cut down non-native trees, but we cant fill potholes. And the homeowners in District 7 paid twice to fill those potholes: first with taxes and then again with a bond. It doesnt make sense that Rec & Park needs a $195 million bond for legitimate infrastructure needs, but still has the millions for its Natural Areas Program. Instead of cutting down trees, we should be planting more. There is nothing native in Golden Gate Park, by the way, except for the sand that used to be there. We cant forget that we live in an urban city of 800,000 people. We ...

Some people get derailed by sweet cravings but for some others like Owolabi, its the salty junk foods like crunchy chips, bread, cheese and big slices of pizza with pepperoni and mushroom that throw them off the Paleo train. There are a lot of processed food across the world that are very high in salt (even though they do not taste salty) but because we get mostly attracted to the appearance of a product or food per se, we subconsciously ignore our thoughts thereby not trying to find out what the content or nutrients of the product is all about and its benefits to our health.. Owolabi never considered the effect of excess salty food to his health until he became so ill that he visited the doctor. During his visitation, the doctor asked Owolabi while taking his vitals if he regularly engaged in physical activities and some other healthy initiatives. He said no and gave several reasons why he could not keep up with living a healthy life. Sadly, the doctor spelt out his result, summarizing that ...

There is an ongoing controversy among doctors and dietitians about exactly who needs to restrict sodium. But this is not because anyone thinks that more salt (sodium chloride) is a good thing for most Americans. Rather, its because cutting out - or at least cutting back on - chips and salty snacks, canned soup and fast food meals, and putting a limit on the salt shaker is thought to be too difficult for most people. To which we say: Really? Its that hard to live without those foods?
Reducing the intake of saturated fats and trans fats is vital for your livers health. Saturated fats are found in deep fried foods, red meats and dairy products. Trans fats are found in processed foods such as cookies and cakes. The liver stores excess dietary fat, and the buildup of fat can eventually lead to fatty liver disease.

Abstract:. Central venous port catheters that are too long are typically removed or revised. The subcutaneous position of port reservoirs precludes standard over-the-wire exchange techniques, and a method of percutaneous revision using an intravascular loop snare technique has been previously described. A retrospective review was conducted of 38 procedures that were performed at a single academic institution between 2005 and 2015. Technical success was 100%, without immediate or delayed complications with follow-up until port removal or death in 94% of patients. Percutaneous revision is an effective method for shortening too-long port catheters, allowing uninterrupted use of the port.. ...

TY - JOUR. T1 - Cut down by some cowardly miscreants: Plant maiming, or the Malicious Cutting of Plants as an Act of Protest in Eighteenth and Nineteenth Century Rural England. AU - Griffin, Carl. PY - 2008/4. Y1 - 2008/4. N2 - Since the publication of Hobsbawm and Rudés Captain Swing our understanding of the role(s) of covert protests in Hanoverian rural England has advanced considerably. Whilst we now know much about the dramatic practices of incendiarism and animal maiming and the voices of resistance in seemingly straightforward acquisitive acts, one major gap remains. Despite the fact that almost thirty years have passed since E. P. Thompson brought to our attention that under the notorious Black Act the malicious cutting of trees was a capital offence, no subsequent research has been published. This paper seeks to address this major lacuna by systematically analysing the practices and patterns of malicious attacks on plants (plant maiming) in the context of late eighteenth- and ...

U.S. Rep. Matt Gaetz, R-Fla., a member of the Climate Solutions Caucus, is championing a proposal to cut down on greenhouse gases. At the end of last week, Gaetz, a key ally of President Donald Trump on Capitol Hill, teamed up with U.S. Rep. Scott Peters, D-Calif., to showcase the Super Pollutants Act which, they insist, will slow climate change by regulating black carbon, hydrofluorocarbons, and methane -- some of the most potent greenhouse gases and noted, these short-lived climate pollutants, also called super pollutants, are significantly more potent than carbon dioxide.. Gaetzs office offered some of the details on the proposal. The Super Pollutants Act also seeks to preserve current standards on methane pollution from oil and gas production. The bill strengthens standards for global methane emissions, codifies methane emission standards for new sources of methane, and regulates emissions at existing oil and gas operations if voluntary industry efforts do not decrease methane ...

What the heck, Im just going to turn our remaining time on the Summer of 6 into a Historic US 80 travelogue since well see a lot of the old highway as we head home to Southern California (first as I-20, then I-10, and eventually I-8 when we cross into Casa Grande). A little bit of history about US 80: one of the original US transcontinental highways, running between San Diego, CA and Tybee Island, GA for a total of 2,747 miles (extended 1929 from Savannah, GA shortly after its commissioning in 1926), it represented a major southern thoroughfare and the first all-weather coast-to-coast highway until Californias wholesale US highway cutdown in 1964. Once it was decommissioned in the Golden State, US 80 was truncated to Yuma, AZ at the Arizona-California stateline in place of future Interstate 8 (and Interstate 8 routed over the old highway until the freeway was done, which survives as Old Highway 80 in San Diego county and ImpCo S80 in Imperial county). Chain Saws Ratings and Reviews Chain saws are handy tools for the homeowner and professional. They are a great way to prune large limbs, cut down dead or unwanted trees, and cut firewood. Many home owners find electric chain saws to be just the ticket for what they need. These saws are generally less powerful with shorter cutting bars than their gasoline powered counterparts. However, these handy little saws are lightweight and certainly powerful enough to prune limbs, cut up fallen limbs and even cut down smaller trees in the back yard. The limitation of an electric saws is its power cord resulting in limited mobility. Tree Trimming Is Much Easier With A Chain Saw Tree trimming with a chainsaw is a quick and efficient way to clean up dead or dying limbs, prune trees or top trees that are growing near utility lines. These saws are also useful in cutting the upper limbs out of ornamental trees, causing them to sprout may smaller limbs that provide more shade or help the tree take on a more desirable

Certainly, the study has limitations, says Horn. We dont fully understand the clinical relevance of ramping up daily activity to 20 or 30 minutes a day with these teens. But we do know that even modest improvements in exercise may have health benefits. Our study supports the idea that encouraging one healthy behavior can serve to promote another, and it shows that teens, often viewed as resistant to behavior change, can tackle two health behaviors at once. Additional research must confirm the key findings and prove that they apply to all teen smokers and not just those in West Virginia, Horn says. And researchers still do not know the mechanism that might explain the findings. However, she says that physical activity is known to spur the release of the bodys feel-good chemicals called endorphins. One possible explanation is that those substances might help teen smokers better deal with the cravings or weather the withdrawal symptoms that often lead to relapse, she said.. The study, ...

I bought the original formula and the smell is fantastic. It really does what it says it should do. At first, I was disappointed in the amount of suds, but Ive remedied this by first washing my beard with a moisturizing soap, then putting the finishing touches on with Bluebeards. My beard has never been softer and smell great too! Its also cut down a lot of the amount of dry skin under my beard. I did knock a star of on price. If youre going to use this daily and have a decently sized beard, you will go through it quickly, but my above tip on the soap has slightly cut down the amount I use. Definitely a great product!. ...

For years weve been hearing that the best way to cut down body fat was to cut down on the fats we eat. On the face of it, that seems totally reasonable; but only if you dont fully understand your bodys metabolism-how your body turns what you eat into fuel.. In recent years, research has indicated that we may have been doing it all wrong. To verify that, just ask yourself: If low-fat diets work, why are we so fat? Sure, you could blame some of it on people with weak willpower, or metabolic problems, or whatever. But when nearly two-thirds of the nation is overweight, those excuses just dont hold water.. There is now an enormous body of research that proves that the conventional wisdom about the role fats play in the human diet is, quite simply, wrong, and in fact, its a high-carb, high-fat diet that results in weight problems and all the diseases that go along with being overweight. Culprits include the USDA Food Triangle, which guides school lunch programs across the country-its top-heavy ...

Pruning should be done in winter or early spring when plants are dormant. Since gooseberries produce the most of its fruit on 2 or 3-year-old branches, you will need to cut down any branches older than this.. In the first year after planting, you will need to cut new shoots in half. Be sure to cut the stems to an outwardly facing bud. The following winter cut the leading branches by the half. From the third year, you will need to continually prune gooseberry bushes to maintain a well-shaped bush and stimulate new growth. First, remove any dead, weak, tangled or diseased branches. Next, cut any branches that grow close to the ground and dig up any root suckers.. The last years growth should be cut up to half. Remove any side stems by a couple of buds to maintain a goblet-shaped bush.. ...

This week, Amy in Durham emailed us about purchasing shelving to match her hardwood kitchen worktops. She has ordered 2m-long oak worktops - which she has had cut to size with our wood worktop cutting service - and would like to order floating shelves to match.. QUESTION:. I recently ordered solid oak countertops at your Gloucestershire showroom. My kitchen is fairly small and I used your bespoke worktop service to have one worktop cut down to 1800mm. My husband and I have decided to install open shelving (instead of wall cabinets) above this work surface to make the space feel more open. Ideally Id like shelves that are exactly the same length as the worktops; can I order floating shelves that are cut to size, too?. ANSWER:. Hello Amy,. Thanks for getting in touch. Oak kitchen worktops are a fantastic choice; with a gorgeous golden colouring and immensely hard-wearing surface, oak is the most popular timber in our extensive collection.. A great addition to any style of kitchen, floating ...
I cannot believe that it is already September! What is more, the weather is absolutely fabulous today! I just cant wait to get out from work and head to my favorite forest where my family and I like to spend so much time together. It is going to be so much fun to hang out there again! I am going to take my newborn in his pram with me, and we are all going to have a nice walk surrounded by trees.. Last time I went to that particular forest, we encountered a few forest workers cutting down some of the trees that were on the way. Something tells me that it takes time and practice to learn to cut down trees, but with some help and proper equipment everything becomes so much easier. If you re interested in doing it yourself, check out arborist sites like www.sherrilltree.com. Who knows, maybe one day I am going to learn how to cut some trees myself. It looks hard and complicated, but maybe it does not have to be like that. I will try to give the activity a try and see how it goes. I am positive that ...

Mediated dispute over Small Fry pretending to cut down a tree in the yard with a big stick (*gasp* Stick? Thats rebar!) and smacking Medium Fry in the head. I took them outside to have them show me what on earth happened. Small was taking the rebar to the tree and smacking the heck out of it to cut it down. More rebar was on the ground, covered by the mower shield, and various other things the kids had kiped for playthings...all of which were NOT playthings...when they have plenty of toys...and all but one or two had been taken out of the shed. Which Hubby explicitly told ALL of them, just the other day, that they were NOT to take things out of the shed and play with them ...

I had a discussion with my ex-gf over a year ago about kids (she wanted to have family at some point) and she asked me directly whether Crohns is inherited... which to some extent it is. I know 10% isnt much at all and most people probably dont really consider it when thinking about kids, but I was still wondering what people think ...

I asked SMP what it was like and he said it was completely relaxing and he was fully aware of everything Elliott was saying and fully in control of all his faculties throughout the entire session.. SMP went along for his first session on Friday, its now Monday and since seeing Elliott hes smoked one cigarette when he was out on Friday night and thats it. He hasnt bought any, he hasnt taken them with him when hes left the house, and he just hasnt felt like one at all. He said hes got so much more confidence that hes got the ability to stop and he feels amazing. I was gobsmacked, I was genuinely thinking it would take a while, like maybe hed cut down a bit but already hes pretty much cut them out already. He said going to work this week will be the true test but hes pretty sure he wont be bothering with the bifters. We went out yesterday for a few drinks and we got a waft of someone smoking and I asked him whether it bothered him at all and he said No, not in the slightest. Miracles ...
I use Super ATV universal kit on all my pilot. It forsure is not a slap it on type of Mod, it is a lot easier now that I have done 7 or 8. I am sure it could be installed onto a 350 doing it along the same way a pilot is done, I cant imagine driving with out it now that all my pilots have it.. The ps kits were 750.00 then dropped to 650 then to 519 last I checked, they come with funky 17mm splines that couplers do NOT exist for that size, I have been buying 11/16 x 3/4 couplers and modifying them by cutting a slot in them then welding cut pieces of 3/4 x 1/2 tubing on each side of the slot so I can put a bolt in and squeez it onto the splines. The kit comes with 2 U-joints to fit it but there is no way to use them in the tight confines of the pilot. I have modded the splines in the U joint to work on 1 of my pilots but the other way is easier. It comes with a plate to mount it that has to be cut down to fit. You also need some more plate to make it install, I use 1/4 4130 plate myselt and it ...
