What is velopharyngeal dysfunction ? During normal speech, the palate moves to seal off the back of the nose so that the voice and sound energy are directed through the mouth for speech production. Velopharyngeal Dysfunction (VPD) refers to the lack of ability to seal off the back of the nose during speech. This results in the voice and sound energy being lost in the nose

Magnetic resonance imaging (MRI) is a promising method for noninvasive, three-dimensional (3D) assessment of velopharyngeal (VP) anatomy. The purpose of this study was to assess VP structure in patients with 22q11.2 deletion syndrome by using 3D MRI analysis. Design: This was a retrospective analysis of magnetic resonance images obtained in patients with VPD associated with a 22q11.2 deletion compared with a normal control group. Setting: This study was conducted ...
Purpose: Recent applications of the magnetic resonance imaging (MRI) technique introduced accurate 3-dimensional measurements of the velopharyngeal mechanism. Further standardization of the data acquisition and analysis protocol was successfully applied to imaging adults at rest and during phonation. This study was designed to test and modify a noninvasive protocol for evaluating young children without using general anesthesia. In addition, the velopharyngeal structures and their maximal motion were compared between adults and children. Method: MRI data were acquired in 12 young adults and 9 children at rest and during speech production. Multiple measurements were made on the velopharyngeal and craniofacial structures as well as on the levator veli palatini muscle. Results: Most of the ratio measurements of structural shape and maximal motion in the velopharyngeal and craniofacial regions were not significantly different between the adults and the children, despite the fact that the children had ...

Childrens Healthcare of Atlanta. CLINICAL FOCUS. While I provide comprehensive and compassionate care to all of my patients, I have a particular interest in treating development and neoplastic concerns of the head and neck region. Such craniofacial development abnormalities as cleft lip and palate, Pierre Robin Sequence, and velopharyngeal insufficiency are also a strong focus of my practice. Multidisciplinary collaboration is an important mechanism in my treatment approach, an example being my participation in teams of diverse specialists that evaluate and treat patients with vascular and neonplastic tumors of the head and neck region.. RESEARCH FOCUS. Cleft and craniofacial disorders are my primary clinical and basic research interests. Even though the surgical repair of cleft lip and palate is highly effective, patients will continue to be faced with ongoing medical, dental, and surgical care. Surgical outcomes can be variable, and the patients facial growth and development is primarily the ...

N2 - Despite the numerous progresses in the palatal surgery, one of the critical aspect of snoring and OSA surgery is the postoperative pain. Over the last decades several surgical palatal procedures have been proposed. Our aim was to evaluate the tolerability of the coblation-assisted barbed anterior pharyngoplasty (CABAPh) in terms of postoperative pain and wound healing, compared with bipolar assisted barbed anterior pharyngoplasty (BAPh). An observational study on 20 patients with simple snoring was conducted. The outcomes measured to assessing pain were a 10 cm visual analog scale (VAS) and the dose of paracetamol + codeine administrated postoperatively. The wound ...
The Royal Childrens Hospital Speech Pathology Department (RCH SPD) provides services to children with Cleft Palate (CP) and velopharyngeal dysfunction (VPD) in a geographical region that is more than twice the size of Texas, with 30% of the children residing in regional areas. The geographical distribution of the population means that many families are unable to access local speech therapy. To address this problem, the RCH SPD and Music Therapy departments (MTD) collaborated to create a clinical resource for regional children and families. The package is intended to facilitate an increase in childrens consonant inventory, frequency of vocalizations, vocabulary, and communicative opportunity and increase oral airflow during speech. It is also intended to facilitate family-centered care and increase the parent and childs motivation to participate in speech therapy activities within the home environment. The clinical resource has now been distributed to 70 children with cleft palate in ...
Purpose The objective was to determine velopharyngeal (VP) status of stop consonants and vowels produced by young children with repaired cleft palate (CP) and typically developing (TD) children from 12 to 18 months of age. Method Nasal ram pressure (NRP) was monitored in 9 children (5 boys, 4 girls) with repaired CP with or without cleft lip and 9 TD children (5 boys, 4 girls) at 12, 14, and 18 months of age. VP status was categorized as open or closed for oral stops and vowels in three contexts-consonant-vowel syllables, vowel-consonant-vowel syllables, and isolated vowels-on the basis of the presence or absence of positive nasal ram pressure. Results At 12 months of age, TD children produced 98% of stops and vowels in syllables with VP closure throughout the entire segment compared with 81% of stops and vowels for children with CP (p , .0001). There were no significant group differences at 14 or 18 months of age. Conclusions TD children exhibit consistent VP closure for stop consonants and ...

Vocal symptoms of fatigue, unsteady tone, hypophonia, abnormal prosody, tremor, and hyper-nasality may occur as isolated symptoms and represent the first sign of an underlying neurologic disorder (Hanson, Garratt, & Ward, 1984; Hanson, 1991; Feijo et al., 2004). Additionally, while clinical presentation of dysarthria is a hallmark for a neurologic etiology, dysphonia frequently presents as a primary feature of dysarthria and may make differentiation difficult (Kent, Vorperian, Kent, & Duffy, 2003). As well, laryngeal and nasopharyngeal findings of bowed vocal folds, abnormal movement patterns, and velopharyngeal incompetence may also herald the onset of neurologic disease. Furthermore, it is important to recognize that most neurologic disorders associated with laryngeal dysfunction do not occur in isolation and frequently affect other parts of the body (Hanson, 1991). Thus, the appropriate management of these individuals relies heavily on recognizing vocal signs and symptoms of specific ...

Since about June Ive not been very motivated for cycling. Not sure what the problem has been - Im still totally interested in cycle touring and spend much of my spare time looking at maps, watching cycle touring vids on YouTube, and perusing the touring journals at Crazy Guy On A Bike. Its just been that I physically dont feel up to going riding. A few times Ive gotten my bike packed up on a Friday night to go out riding, but I get up on Saturday morning and just physically dont feel into it, so I cancel. Ive even stopped going on my regular night training rides - I just felt out of fitness and out of breath. Then about 7 weeks ago I got a cold or something and lost my voice, had to have a few days off work. I never seemed to really recover from that, with a nagging cough and breathlessness that kind of came to a head earlier this week. Over the weekend Id been coughing so much that my ribs and shoulders started to hurt - then on my way to work on Monday I suddenly had a huge coughing ...

The Synapse was the only bike in our test with externally routed shift cables. Im not sure why the industry seems to be heading towards internal cables, but I prefer the easy install and serviceability of external cables.. The SAVE seat post was a real pain to adjust. It uses a U.S.E. clamp and making small adjustments was nearly impossible. Because you cant substitute a different post on the Synapse, a buyer is stuck with it. Once it was set up though, it never slipped even over some seriously rough roads in Belgium. It is also offered in both five and twenty millimeters of setback. (A seatpost change is in the works, though, as we saw in Europe under some Liquigas-Cannondale riders.). Value: 17 of 20 Points ...

Background Although increased nasality can originate from basal ganglia dysfunction, data regarding hypernasality in Parkinsons disease (PD) and Huntingtons disease (HD) are very sparse. The aim of the current study was to analyze acoustic and perceptual correlates of velopharyngeal seal closure in 37 PD and 37 HD participants in comparison to 37 healthy control speakers. Methods Acoustical analysis was based on sustained phonation of the vowel /i/ and perceptual analysis was based on monologue. Perceptual analysis was performed by 10 raters using The Great Ormond Street Speech Assessment 98. Acoustic parameters related to changes in a 1/3-octave band centered on 1 kHz were proposed to reflect nasality level and behavior through utterance. Results Perceptual analysis showed the occurrence of mild to moderate hypernasality in 65% of PD, 89% of HD and 22% of control speakers. Based on acoustic analyses, 27% of PD, 54% of HD and 19% of control speakers showed an increased occurrence of hypernasality. In
PMID: 19918590 Purpose: The restoration of speech after an extensive resection of the soft palate has been a challenge faced by both prosthodontists and surgeons. Few comparisons between prosthetic rehabilitations and surgical reconstructions of large soft palate defects exist in equally matched groups of patients. The purpose of this study was to evaluate speech outcomes in patients with soft palate defects that were rehabilitated with either a pharyngeal obturator or surgical reconstruction. Materials and Methods: Nine patients who were treated via prosthetic obturation were compared to nine patients who underwent surgical reconstruction of the oropharynx with a radial forearm free flap and a soft palate insufficiency repair modification. Speech intelligibility data, perceptual ratings of resonance, and aeromechanical measurements of velopharyngeal function were collected. Results: There were no differences in any of the speech outcome measures between the two groups of patients. Conclusion: ...
as in perro, for example is an alveolar trill. Trills are very different from flaps. Whereas with a flap (or tap), a specific gesture is used to strike the active articulator against the passive one, in the case of a trill the articulator is held in place, where the airstream causes it to vibrate. Usually a trill vibrates for 2-3 periods, but may be up to 5, or even more if geminate. However, trills may also be produced with only a single period. Although this might seem like a flap, the articulation is different; trills will vary in the number of periods, but flaps do not. Trill consonants included in the International Phonetic Alphabet: [r] - alveolar trill [ʙ] - bilabial trill [ʀ] - uvular trill [ʜ] - voiceless epiglottal trill [ʢ] - voiced epiglottal trill In addition, [ʩ] - velopharyngeal fricative found in disordered speech sometimes involves trilling of the velopharyngeal port, producing a snort. The bilabial trill is uncommon. The coronal trill is most frequently alveolar [r͇], ...
