Overall, 383 patients underwent EVAR between 2007 and 2016. Among them, 317 (82.8%) had EVAR performed within, and 66 (17.2%) had EVAR performed outside the IFU guidelines. Of the outside-IFU patients, 14 (21.2%) had short neck length, 27 (40.9%) had greater neck angulation, 9 (13.6%) did not meet neck diameter criteria, and 28 (42.4%) did not meet iliac diameter criteria.. Among all patients, 99 (25.8%) had T2ELs. Median follow-up duration was 39 months (interquartile range=18-69 months) in the T2EL group, whereas, it was 19 months (interquartile range=3-51 months, P,0.001) in the control group. Among 99 patients, 14 were excluded: 9 of them had T2ELs simultaneously with other types of endoleaks, 2 had T2ELs that occurred following other types of endoleaks, and 3 of them were lost to follow-up. I relate the case of a young man I employ who had a car accident where he was thrown out of the open top and the vehicle rolled on his left leg. Doctor at the scene told him it was just bruised but told him to go A&E once he was home. He did so with same result. A week later once his leg had swollen, turned blue and was very painfull he was urged to return and was nearly turned away again except for his father, my friend insisting a second opinion. A wordly wise elderly Consultant took one look, ordered an immediate X-ray and found no pulse at the ankle. He was rushed on blue lights to a vascular unit and the pulse was reinstated but he then had a further week of waiting that culminated, when he at last saw an orthpaedic Consultant, in an emergency operation to repair his fratured tibia and reattach his torn ligaments.
