When medication and other non-surgical treatments are either unavailable or cannot relieve symptoms, surgery is the accepted treatment for a broad range of conditions that affect the male reproductive organs and the organs of the urinary tract. These conditions include, but are not limited to, prostate cancer, ureteropelvic junction (UPJ) obstruction, bladder and kidney cancer and vesicoureteral reflux.. Facing any kind of urologic surgery creates a great deal of anxiety for most men. Among your concerns is: Will my body function normally following surgery? Traditional open urologic surgery - in which large incisions are made to access the pelvic organs - has been the standard approach when surgery is warranted. Yet common drawbacks of this procedure include significant post-surgical pain, a lengthy recovery and an unpredictable, potentially long-term impact on continence and sexual function.. Fortunately, less invasive surgical options are available to many patients facing urologic surgery. ...

The Urological Society of Australia and New Zealand (USANZ) has issued guidelines to assist urological surgeons prioritise patient cases, including those requiring treatment for prostate and other urological cancers, following the suspension of all non-urgent elective surgery in Australia by the Federal Government. There is a need to re-define urgent urological surgery from those procedures that relatively safely could be deferred for several months. Ventilators and anaesthetists will be in high demand to look after COVID-19 patients and as a group of ethical professionals we must at all times ensure that scarce resources are directed appropriately. The guidelines were developed by the USANZ Board of Directors as a general guide only and shared decision making with patients is recommended where practicable.. Conditions which may require urgent surgical intervention include high risk prostate, kidney, testis, and bladder cancer as well as traumas and kidney stones.. The guidelines suggest the ...
Whats known on the subject? and What does the study add?. Very few studies have examined end-of-life urological studies in men with prostate cancer. These studies reported fewer procedures in men who received primary therapy for prostate cancer. However, these studies were typically single institution or had a short follow-up period.. The present study is the first population-based study examining end-of-life urological procedures and uses a geographic region encompassing 385 000 patients. Furthermore, this study incorporates both hospital- and office-based procedures. This approach has not been previously undertaken. ...

Testing for prostate cancer can involved various exams. Typically, screening for prostate cancer is done with a blood test and an prostate exam (rectal examination). Prostate specific antigen (PSA) is a blood test often used to screen men for prostate cancer. A higher level may indicate that prostate cancer is present, although there are other non-cancerous causes for a increased PSA level. While the timing of obtaining a PSA in men may vary, typically PSA screening is done beginning at age 45 to 55 based on risk factors. A higher PSA level or an abnormal prostate exam may require further evaluation with a prostate biopsy. While PSA screening is simple and can find prostate cancer early before it becomes incurable, it can also find cancer that will not spread or cause problems.. ...

What is Open Access?. Open Access is an initiative that aims to make scientific research freely available to all. To date our community has made over 100 million downloads. Its based on principles of collaboration, unobstructed discovery, and, most importantly, scientific progression. As PhD students, we found it difficult to access the research we needed, so we decided to create a new Open Access publisher that levels the playing field for scientists across the world. How? By making research easy to access, and puts the academic needs of the researchers before the business interests of publishers. ...
There are totally different laparoscopic procedures that may be completed for the aim of weight loss. One is gastric banding and it is a minutely invasive procedure where a silicon band is placed around the abdomen and it helps in making the stomach quite a bit smaller. Due to this fact, smaller doses of food will make the patient full already. One other one is sleeve gatrectomy and with this process, 2/3 of the stomach is stapled. The abdomen becomes the shape of a banana or a sleeve, thus the name. There isnt a reconnecting or rerouting involved in this procedure. One other one is the Roux en Y procedure which is likely one of the best procedures that can additionally management diabetes well. Stapling is finished to type an higher stomach pouch that can help in making the particular person feel full with little food. The calories and vitamins are absorbed a lot later. Last of the Laparoscopic Urology Course surgery options is Imbrication which is very gentle and less risky. The stomach is ...

In a nutshell, my story is I had 1 tumor in my right kidney it was almost 7cm. Had that removed. Turned out to be Stage 3 Grade 3 clear cell RCC.. Here are some of the things I wish Id been told at the beginning.. 1. Know your specialties and what they are. A urologist is a *surgeon that treats things by cutting them out. They specialize in urologic surgery (kidneys, bladder, etc.). A urologic oncologist is a *surgeon that treats things by cutting them out (and maybe using RF or heat to destroy them in place). The difference is the latter has extra special training in kidney masses and the different ways of treating them and probably more specialized training in robotic surgery/laparascopy. Neither of these doctors will treat you if your cancer ever spreads or has spread. They will pass you off to a medical oncologist. A medical oncologist is an *internist* that specializes in treating systemic cancer using chemotherapy and/or managing other tumors by passing you to surgeons, ...
Our fall Vietnam mission is complete. With the support of our mission partner IPSAC and the Provincial Hospital leadership and surgical, medical and nursing staff, we were able to successfully perform 24 urologic procedures on children in need. In addition to these surgeries, knowledge was transferred at all points in the continuum of care - from the bedside to the conference room. We collaborated to do screenings, provide pre- and post-op care and family education and, of course, hands-on training in the OR. On our last day, we did a workshop on optimizing surgical outcomes with a multidisciplinary approach. It ended with a good discussion illustrating the differences between our healthcare settings and the impact resources have on patients and teams. We were also supported by a very committed and helpful team of interpreters all along the way. ...
Occasionally, people with bacteremia (bacteria in the bloodstream) have the infecting bacteria lodge in the kidney; this is termed hematogenous spread. Similarly, people with infected areas that are connected to the urinary tract (for example, prostate, epididymis, or fistulas) are more likely to get a UTI. Additionally, patients who undergo urologic surgery also have and increased risk of UTIs. Pregnancy does not apparently increase the risk of UTIs according to some clinicians; others think there is an increased risk between weeks six through 26 of the pregnancy. However, most agree that if UTIs occur in pregnancy, the risk of the UTI progressing in seriousness to pyelonephritis is increased according to several investigators. In addition, their baby may be premature and have a low birth weight. Patients with chronic diseases such as diabetics or those who are immunosuppressed (HIV or cancer patients) also are at higher risk for UTIs ...

Urinary Stress Incontinence. Muscle education and resistive exercise with the Perineometer produce dramatic results in the treatment of true urinary stress incontinence. This type of incontinence must be distinguished from urge incontinence caused by various pathologic conditions involving the upper urinary tract, such as infections, strictures of the ureter, stones, diverticula, developmental anormalies, etc.; incontinence due to fistulae; and spastic incontinence due to spinal cord changes following injuries, poliomyelitis, multiple sclerosis, etc.. In simple urinary stress incontinence, control of the urinary outlet is partially lost with coughing, sneezing, laughing, or other sudden strains. In the past, women tolerated this annoying and embarrassing condition with all its undesirable psychological effects because it was felt that the conditions did not warrant surgical intervention.. With physiologic therapy, complete relief from simple urinary stress incontinence has been consistently ...

Perioperative care has been identified as an area of wide variability in quality, with conflicting models, and involving multiple specialties. In 2014, the Loma Linda University Departments of Anesthesiology and Urology implemented a perioperative hospitalist service (PHS), consisting of anesthesiology-trained physicians, to co-manage patients for the entirety of their perioperative period. We hypothesized that implementation of this PHS model would result in an improvement in patient recovery. As a quality improvement (QI) initiative, the PHS service was formed of selected anesthesiologists who received training on the core competencies for hospitalist medicine. The service was implemented following a co-management agreement to medically manage patients undergoing major urologic procedures (prostatectomy, cystectomy, and nephrectomy). Impact was assessed by comparisons to data from the year prior to PHS service implementation. Data was compared with and without propensity matching. Primary outcome

Port placement for robot-assisted laparoscopic urologic surgery can be critical to effective completion of the minimally invasive procedure. An ideal port template would allow unhampered access to all critical structures during surgery, easy access for the bedside assistant, and minimization of arm collision with the fewest ports necessary to minimize cosmetic impact. We present a comprehensive plan for the placement of ports across different procedures for a variety of upper tract (radical/partial nephrectomy, retroperitoneal radical/partial nephrectomy, and pyeloplasty), lower tract (prostatectomy, and cystectomy), combined upper/lower tract (nephroureterectomy, retroperitoneal lymph node dissection), and female pelvic surgeries ...

Despite being a minimally-invasive surgery, PCNL is not pain-free due to the procedure itself and the presence of postoperative diversion. Pediatric PCNL has evolved from adult size instruments to Micro-PCNL in time. It has been reported that the use of smaller access sheaths was associated with less complication and less post-operative pain (8,9,10,11). A study on patients with 1-2 cm stones, mini-PCNL and standard PCNL showed same stone-free rates where mini-PCNL was found to be advantageous in terms of analgesic requirement, hemoglobin drop and hospital stay (11). Also, tubeless and totally tubeless PCNL were developed in order to reduce post-operative pain. In a retrospective study on 1469 patients, who underwent PCNL, the subjects were divided into three groups. In the first group, a 14 F nephrostomy catheter was placed, the second group received ureteral catheter and the third group received double J stent. In the nephrostomy group the length of hospital stay, need for blood transfusion, ...

View Poster. INTRODUCTION. Opioid misuse is an accelerating public health crisis in the United States. Increasing attention has been directed towards surgical sources of opioid over-prescription. To better inform efforts to improve the safety of post-operative analgesia, we aimed to examine factors associated with opioid prescribing practices following urological surgery. METHODS. We used a centralized electronic health record (EHR) system platform at a multi-site academic medical center to identify patients who underwent short stay and ambulatory urologic surgeries from January to December 2016. Dispensed opioid doses were extracted from the Epic EHR. Using standard conversion factors, we calculated equivalents of 5mg of oxycodone, which were characterized as high (, 30 5mg oxycodone tablets) or low (≤ 30). We tabulated and compared clinical, demographic, surgical, and provider characteristics across prescription dose thresholds using descriptive statistics. Multivariable logistic regression ...
In the United States, fibrin sealants have been used to achieve hemostasis for nearly two decades. Although their clinical utility was first demonstrated in cardiac surgery, their effectiveness and safety have since been demonstrated to extend to a wide array of procedures. Fibrin sealants typically contain two components-fibrinogen and thrombin-that are combined and delivered simultaneously to a target bleeding site in order to achieve hemostasis.

Management of end-stage bladder failure is complicated; the low capacity and non-compliant bladder increases urinary back-pressure to the kidneys, resulting in renal failure, and causes intractable incontinence. Once conservative and medical treatments have failed, bladder reconstructive surgery is offered. The gold standard treatment is enterocystoplasty, which involves augmentation of the bladder with enteric tissue. Although increasing capacity and compliance of the bladder, the procedure carries with it a number of serious complications attributable to exposure of the bowel mucosa to urine, chief among which is the risk of malignant transformation. The purpose of this thesis was to compare the current treatment with composite cystoplasty, a novel tissue-engineering technique designed to create a neobladder lined by urothelium. Patients who had undergone conventional enterocystoplasty provided biopsies of native and augmented segments of bladder for histological and immunohistochemical ...

Pyeloplasty may be done using open surgery or laparoscopic surgery. A catheter will be placed to allow urine to drain. During open surgery, an incision will be made in your side. The renal pelvis will be reconstructed. The blocked section will also be removed. The remaining healthy sections of ureter will be re-attached. The incision in the skin will then be closed with stitches.. Laparoscopic surgery only requires a few small incisions. Special tools will be passed through these incisions to complete the surgery. The repair steps are the same as the open procedure.. In some surgeries, a temporary tube called a stent may be placed in the ureter. This will allow urine to pass while the area heals. ...
