Critically, what is poorly understood is how signalling is fine tuned to avoid over- or underactivity, either of which could fatally compromise the generation of functional neuro-muscular units. Our contention is that Hpse2, an endogenous inhibitor of the enzyme activity of classical heparanase, and Lrig2, a member of a family of plasma membrane proteins that control growth factor signalling, are key components of such a regulatory network. With colleagues in the Centre for Genomic Medicine, we discovered that either HPSE2 or LRIG2 can be mutated in urofacial syndrome, a congenital motor neuropathy. With the widespread practice of routine prenatal ultrasound, the number of fetuses with detected HN has increased commensurately. Management of the many patients who have persistent HN after birth remains controversial and devoid of guidance based on high levels of evidence. Among contentious topics, the routine prescription of CAP has gained attention due to the perceived lack of clinically significant benefit in VUR trials, as well as concerns for bacterial resistance and long-term adverse effects. We had a lot of midnight visits to the vet ER. It took us a long time to get a proper diagnosis, (after going from vet to vet) but since then, weve managed to keep it fairly well controlled. His is very much stress-related, and weve learned there are many, many nuances to this condition. Hes even on an anti-anxiety med that helps, too. Poor baby, hes been through an awful lot with this. So glad to see the veterinary community addressing this more and more! Still, the wonderfulness of this guy far outweighs any difficulty weve had in dealing with his condition!
