Sacral nerve stimulation, or neuromodulation, has been shown to restore voiding in women with a specific type of urinary retention that is attributed to urethral sphincter overactivity. The therapy has gained popularity in this and other voiding dysfunctions, but its mechanism of action remains unexplained. This thesis explores the effects of neuromodulation on women with urinary retention. It incorporates a urodynamic study of the effect of neuromodulation on bladder and urethral (peripheral) function, a functional brain imaging PET (Positron Emission Tomography) study of cerebral (central) effects, and a review of the long-term efficacy of the technique. Investigations of micromotion characteristics of bladder wall strips and pressure wave phenomena in total bladders in vitro and in vivo indicate that micromotion phenomena occur in the bladder wall. Local contractions can occur without an increase in tension or pressure, because other parts are in antiphase. Local contractions stretch surrounding tissues, which can stimulate fast stretch receptors. Synchronisation of these micromotion phenomena appears to be possible. OBJECTIVE: To determine the congruence between self-reported and objective data on incontinence, voided volume (VV) and voiding frequency (VF), in a prospective study of treatment of functional urinary incontinence (UI) due to urge syndrome or dysfunctional voiding in children. PATIENTS AND METHODS: In all, 202 children, enrolled in the European Bladder Dysfunction Study (EBDS), provided self-reported data on UI, VV and VF, before and after treatment, with validated questionnaires and 72-h voiding diaries. Objective data were obtained with uroflowmetry and a 12-h pad test, also before and after treatment. Questionnaires and diaries were checked and scored by a urotherapist, at scheduled office visits that were combined with uroflowmetry. A total of 30 women with stress incontinence underwent periurethral injection of autologous fat under spinal anesthesia, The fat was harvested from the abdominal wall by liposuction, Preoperative evaluation consisted of history, physical examination, and urodynamic evaluation, For study purposes, some patients also underwent bladder and urethral ultrasonography and magnetic resonance imaging studies, The first 13 patients received a single periurethral lipoinjection, and the following 17 patients received sequential injections when needed at 3-month intervals, Results were assessed by subjective questionnaire performed at 3 and 12 months. All patients had intrinsic sphincteric deficiency, Of the first group, there were only four patients (31%) cured after 1 year of follow-up, On the other hand, in the group that received repeated injections, there were 11 patients (64%) cured with a mean of two injections at 1-year follow-up.
