Introduction: Urinary incontinence ranges from 4 to 31% after prostatic surgery. The aim of our study was to assess efficacy and quality of life outcomes of 37 patients who underwent artificial urinary sphincter AMS (American Medical System) 800 placement in a single academic urologic clinic. Materials and methods: Pre and post operative stress urinary incontinence (SUI) was evaluated using the daily pad use (pad per day used PPD) and the Italian validated International Consultation on Incontinence Questionnaire - short form (ICIQ-SF), whereas health related quality of life (HRQoL) and subjective satisfaction of the patients was evaluated with the Italian validated Patient Global Impression of Improvement (PGI-I) questionnaire. Results: Median preoperative PPD used was 4; after a median follow up of 4 years, median PPD used was 1. With regard to ICIQ-SF questionnaire, 4 patients (12.5%) responded that they never lose urine and 22 (68.76%) only during exercise and / or sneezing. Moreover, median PGI-1

The artificial urinary sphincter is an implanted device that controls urinary continence in people who are otherwise incontinent of urine. Its use is reserved for severe incontinence, which has failed to respond to other treatments, or in those who are not suitable for insertion of a sling to control continence Atoms Sling or Advance Sling.. A typical indication in men is severe incontinence after prostate procedures such as radical prostatectomy or TURP. The sphincter can also be used for incontinence associated with certain neurological conditions, but the way it is implanted is different, and this particular indication is not discussed here. An AUS can be an excellent treatment for incontinence, for the right man. Urinary incontinence can be one of the most crippling conditions suffered by men after surgery for prostate cancer - an AUS can vastly improve and sometimes completely eradicate incontinence.. ...

I am new here as you can see from my posting but I have been reading alot of your post. I had robotic surgery in 2009 for prostate cancer. It had escaped the capsule and so I had radiation in 2010. Have been incontinate from the start after surgery, 4 - 5 pads a day. In 2012 had the male sling installed, 3 months to the day they removed the male sling, Rubbing the bladder causing an infection. Now I am totaly incontenate, wear tenna underware, and tryed pads with that, changing every 2 hours. I am a pipefitter at work and this was not working out being wet all he time and welding really dont go together, I am sure you get the picture. I now use an extrenal cathadure and collection bag to make it thru the day. This has been working well for me but still not free. I want to be dry, panty liner dry would be great. I have been back to my dr. and I plan on have the AUS insalled on june 12 20013. I just had scar tissue removed and was dialated three weeks ago. All looks good to go now, But would like ...

Artificial urinary sphincter (AUS) is designed to supplement the function of the natural urinary sphincter that restricts urine flow out of the bladder.
Based on current therapy algorithms, the perineal single-cuff AUS is the gold standard in the treatment of moderate and severe male SUI [2,4]. Adjustable male slings are currently of emerging interest in this patient group, but evidence regarding their use is still weak. In the current multi-institutional study, we compare perioperative safety as well as explantation rates of 2 adjustable sling systems (Argus classic and Argus T) and the perineal single-cuff AMS 800 in a large, multi-institutional contemporary patient cohort.. To address differences in postoperative complication and device explantation rates in an adequately generalizable way, we focused on patients that have undergone surgery in a sufficiently experienced incontinence center.. We found a significantly increased intraoperative complication rate during adjustable male sling implantation compared to AUS implantation. When performing a subgroup analysis of the adjustable male sling patient cohort, we found that the vast majority of ...
Keywords: Patient reported outcome measurements, Artificial urinary sphincter, IRF5 Male sling, Stress incontinence The quality of surgical care is commonly assessed by objective indicators of operative success such as perioperative morbidity and mortality, intraoperative complications, length of hospital stay and readmission rates. While these are fundamentally important and useful markers of surgical performance, the need for better qualitative, subjective assessment of health and care delivery from the patients own perspective has led to increased interest in patient reported outcome measures (PROMs).1,2 Indeed, PROMs are deemed useful and important to healthcare policy makers in prioritisation decisions, to benchmark quality and compare outcomes between institutions.3 Moreover, there is often variability between surgeon reported outcomes and patient reported outcomes. For example, a meta-analysis of studies investigating surgeon measured urinary continence recovery following robotic radical ...
Male stress urinary incontinence (SUI) is a secondary intrinsic sphincter deficiency and a prevalent condition after radical prostatectomy [1-5]. Individuals with SUI may limit activities of daily living to reduce the chance of urine leakage; hence, SUI can have a major impact on the quality of life (QOL) of patients. The standard therapy of male SUI is pelvic floor muscle training, and clenbuterol hydrochloride, which is solely approved for SUI indication in Japan; however, the efficacy of these treatments is still unsatisfactory. Another potential therapy is periurethral injection of collagen, but its efficacy has only been shown in short-term trials [6, 7], and this treatment is not currently approved for SUI in Japan. Another potential alternative involves the implantation of an artificial urinary sphincter, which is recommended for patients with severe incontinence [8-10]. Unfortunately, according to a Japanese report from the Office of Pharmaceutical Industry Research (OPIR News No.45), ...
OBJECTIVES: To present a modified technique in artificial urinary sphincter (AUS) placement that is associated with low rates of erosion and infection in a high-risk population. PATIENTS AND METHODS: After Institutional Review Board approval, we identified patients who underwent primary AUS placement using the modified technique between January 2007 and November 2015. Our modification consists of preserving the dorsolateral fibromuscular tissue surrounding the bulbar urethra and horizontally transecting the ventral bulbospongiosus muscle during urethral cuff placement. Preoperative variables such as radiotherapy (RT) and bladder neck contractures were recorded. Effectiveness and complications including infections, erosions, and re-operations were recorded at follow-up. RESULTS: The new technique was used on 208 patients: 40% had a history of RT, including 15% who had had a salvage radical prostatectomy; 26% had had previous bladder neck contractures. No patients developed infection. Overall, erosion

The main profile of our clinic are the laparoscopic procedures which are continiously increasing in number. In 2014, we were the first in Hungary to perform a radical cystectomy and urinary deviation laparoscopically. The procedures we perform are aimed by the latest technology, a 3D laparoscopic tower. There are more and more kidney-sparing interventions in our department. These operations are mainly laparoscopic as well. We perform nerve-sparing radical prostatectomies on a daily basis as routine surgeries.. In the last few years, microsurgical andrologist procedures appeared in our department as a new profile. For patients with severe incontinence, we can offer an artificial urinary sphincter placement. If a patient has drug.resistent erectile dysfunction, a penile implant can be the solution for him. The urogynaecal procedures which were introduced recently are performed on a regular basis. Reconstructive urology is also among our new profiles, which consist of the plastic surgery of ...

Urinary incontinence (UI), defined as the involuntary loss of urine during the filling phase of the bladder (Abrams et al. 2002), is a commonly seen problem in veterinary practice. Urinary sphincter mechanism incompetence (USMI) after spaying is the most common micturition disorder, and its medical treatment is normally successful, even though the underlying pathophysiological mechanism is not fully understood. Hormonal changes inducing structural and functional alterations in the bladder, as well as in the urethra composition, are discussed. To manage incontinent patients successfully, possible underlying abnormalities besides USMI should be ruled out. In the majority of cases, history, physical examination and simple tests including urinalysis and urine bacterial culture lead to a presumed aetiology. If USMI is the most likely cause, then the advantage of further diagnostic tests should be discussed with the owner before starting a trial therapy with alpha-adrenergic drugs. Potential side ...

Answer: This is a frustrating problem and there are several potential solutions. First, it would be useful to know the rest of your history, but I am guessing you experience significant urinary frequency, urgency, hesitancy and urge incontinence during the day as well. The first thing you should do, if not done already, is see a urologist, preferably one with expertise in neurogenic disorders of the bladder and MS. Your constellation of symptoms and lack of response to therapy suggests a low capacity bladder, possibly under high pressure, with overactivity of urinary sphincter that helps control the release of urine. This type of problem often does not respond adequately to standard treatment with high doses of oxybutynin (called anti-muscarinic treatment) or even self catheterization. If this is the problem, effective treatment often involves botox injections into the bladder neck or even neuromodulation with the InterStim device made by Medtronics. As a temporizing measure, your physician may ...
There are some studies suggesting a negative impact from pre-pubertal neutering in large breed dogs. These studies implicate certain bone developmental abnormalities, urinary sphincter incontinence, cancers and undesirable behaviours, in some neutered large and giant dog breeds, particularly when neutered before full maturity (9-12). More research is required to explore these issues, as current data are conflicting, few breeds have been investigated, small sample sizes are studied, and most are retrospective studies. Longitudinal and case control studies are lacking ...

Implantable medical grafts fabricated of metallic or pseudometallic films of biocompatible materials having a plurality of microperforations passing through the film in a pattern that imparts fabric-like qualities to the graft or permits the geometric deformation of the graft. The implantable graft is preferably fabricated by vacuum deposition of metallic and/or pseudometallic materials into either single or multi-layered structures with the plurality of microperforations either being formed during deposition or after deposition by selective removal of sections of the deposited film. The implantable medical grafts are suitable for use as endoluminal or surgical grafts and may be used as vascular grafts, stent-grafts, skin grafts, shunts, bone grafts, surgical patches, non-vascular conduits, valvular leaflets, filters, occlusion membranes, artificial sphincters, tendons and ligaments.
The invention provides a prosthetic implant for supporting the urethra, the implant comprising a flexible elongate element and being provided: with a central portion for being positioned beneath the urethra when the elongate element is in its functional position in the body; and two ends situated on either side of the central portion; wherein said ends are positioned relative to the central portion in such a manner that when the elongate element is in the functional position, one of the ends of the elongate element is disposed between the right internal obturator muscle and the right external obturator muscle, while the other end is disposed between the left internal obturator muscle and the left external obturator muscle. The prosthetic implant is suitable for correcting urinary incontinence.
How it Works. This innovative, safe, and effective outpatient procedure involves placing a small sling of synthetic mesh inside the body through small incisions. The sling then supports the urethra, restoring normal bladder control. Most patients are continent immediately following the procedure and can resume normal, non-strenuous activities shortly thereafter.. Benefits ...

Artificial muscles hold promise for safe and powerful actuation for myriad common machines and robots. However, the design, fabrication, and implementation of artificial muscles are often limited by their material costs, operating principle, scalability, and single-degree-of-freedom contractile actuation motions. Here we propose an architecture for fluid-driven origami-inspired artificial muscles. This concept requires only a compressible skeleton, a flexible skin, and a fluid medium. A mechanical model is developed to explain the interaction of the three components. A fabrication method is introduced to rapidly manufacture low-cost artificial muscles using various materials and at multiple scales. The artificial muscles can be programed to achieve multiaxial motions including contraction, bending, and torsion. These motions can be aggregated into systems with multiple degrees of freedom, which are able to produce controllable motions at different rates. Our artificial muscles can be driven by ...
A poly vinyl chloride (PVC) gel actuator shows great potential for use as an artificial muscle because of such positive characteristics as movement in the air, large deformation, and being light in weight. A bending type actuator using PVC gel was studied previously. In order to construct an artificial muscle it is necessary to compose an actuator which has the characteristics of contraction type deformation. In the present paper we propose an electrode arrangement for a contraction type actuator using PVC gel. Also, we investigate the characteristics of the proposed actuator experimentally and show the effectiveness of the actuator as an artificial muscle. The experimental results showed that the contraction strain of the actuator was about 14%, the response rate was 7Hz, and the output stress was 4kPa. In order to apply the artificial muscle as a control element, we build a linear mathematical model of the electronic and mechanical characteristics. Based on the mathematical model, we design a ...
INTRODUCTION: Awareness of prostate cancer is growing in the Western population, and an increasing number of patients are being referred to prostate surgery. This is a significant contributor to male stress urinary incontinence (SUI). Implantation of an artificial sphincter (AUS) is considered the gold standard treatment of SUI. This study investigates the role of minimally invasive treatment with the Argus sling in a heterogenic hard-to-treat patient group.. METHOD: The study was a retrospective follow-up study with patients as their own controls. Forty-one patients were enrolled and treated with the Argus sling. Patients enrolled had persisting SUI after prostate surgery for more than 12 months, despite conservative treatment. The pre-operative daily usage of pads and the 24 hours urinary leakage were compared to the post-operative findings. The primary goal was to achieve complete continence or a reduction of more than 50% in pad usage or urinary leakage.. RESULTS: In total, 71% of the 41 ...

Artificial muscle is a generic term used for materials or devices that can reversibly contract, expand, or rotate within one component due to an external stimulus (such as voltage, current, pressure or temperature). The three basic actuation responses - contraction, expansion, and rotation - can be combined together within a single component to produce other types of motions (e.g. bending, by contracting one side of the material while expanding the other side). Conventional motors and pneumatic linear or rotary actuators do not qualify as artificial muscles, because there is more than one component involved in the actuation. Due to their high flexibility, versatility and power-to-weight ratio compared with traditional rigid actuators, artificial muscles have the potential to be a highly disruptive emerging technology. Though currently in limited use, the technology may have wide future applications in industry, medicine, robotics and many other fields. While there is no general theory that ...

Want to witness the power behind the next generation of artificial muscles for prosthetics and exoskeletons? Look in your fishing tackle box.. According to a recent announcement from researchers at the University of Texas - Dallas, a process that twists and coils fishing line results in an artificial muscle that can lift more than 100 times the weight of a similar-sized human muscle and can be controlled by relatively simple temperature changes. The research has been reported in Discover magazine, Health24, and WebMD, among other outlets. The twisted polymer filament process was developed at the NanoTech Institute at the university. Researchers believe that the technology has immediate applications for exoskeletons but could also be commercialized to create clothing that adapts to changes in temperature. While far less efficient than human muscle, the fibers can generate more than 5 times the energy of an automobile engine.. ...

SRIs electroactive polymer artificial muscle (EPAM) technology has the potential to fundamentally shift the way many types of industrial, medical, consumer, automotive, and aerospace products are powered and operated. It offers significant advantages over typical electromagnetic-based technologies: it is much lighter, smaller, quieter, and cheaper. EPAM also offers more controllable and flexible configurations.. The patented, rubbery polymers powerfully contract and expand when subjected to electricity. EPAM enables robots to mimic the dexterity and mobility of humans, and offers performance characteristics similar to those of natural muscle such as high strain, high peak power, and high compliance. In addition to acting as a muscle-like actuator, EPAM can also operate in reverse and generate power from being stretched and contracted.. The technology was featured on the cover of Scientific American in October 2003. SRI spun off Artificial Muscle, Inc. (AMI) in 2004 to further develop the ...

A new conducting fibre made of plastic and nanotubes could one-day be used to make super-muscles and smart clothes that can power your laptop, say Australian researchers.. Professor Gordon Wallace of the University of Wollongong will present the research at next weeks Connect 2005 conference of the Royal Australian Chemical Institute in Sydney.. Wallace and teams innovation is based on modifying conducting polymers, a type of plastic that conducts and stores electricity, and expands or contracts in response to charge.. If you configure it properly you can actually make artificial muscles, says Wallace of the ARC Centre for Nanostructured Electromaterials at the university.. His team has so far used such polymers to create a prototype glove of artificial muscles to help rehabilitate people after hand surgery or injury.. But Wallace says the challenge is making artificial muscles strong enough and getting the charge in and out of the polymers quickly so they can contract as fast.. So the ...
