To explore and summarize the reasons why urethral calculi cause a urethral fistula. We retrospectively studied 1 patient in Xiangya hospital and all relevant literature published in English between 1989 and 2015. The patients (including those reported in the literature) were characterized by age, origin, location of calculus, size of calculus, fistulous track, and etiological factors. Most of urethral calculi associated with a urethral fistula were native generated. Urethral calculi can be formed in various locations of the urethra, and the size of the calculus ranged from small (multiple) calculi to giant stones. The fistula external orifice located at the root of the penis was relatively common, and there were various etiological factors, such as urethral strictures, urethral trauma induced by long-term catheterization, lumbar fractures, and congenital anomaly factors. They were managed by the excision of the fistulous tract, retrieval of the urethral stones, and/or debridement and pus drainage
Two female infants diagnosed with type IV and type III sacrococcygeal teratomas underwent preoperative evaluation in the postnatal period. The first patient was an 18-month-old female patient who presented with a metastatic type IV teratoma resected after neoadjuvant therapy. The second patient was a 6-day-old female infant with a prenatal diagnosis of cystic type III teratoma. Using laparoscopy in both patients, the presacral space was reached by opening the peritoneal reflection with blunt dissection and the middle sacral artery was identified. It was then carefully isolated and divided with a 5mm LigaSure™ vessel-sealing device. The pelvic components of the tumors were partially dissected using laparoscopy. The first patients tumor resection was completed using a posterior sagittal approach and the second patient required a standard Chevron incision ...
The type of clinical signs in these patients depends on whether the rectal and anal canals are closed or stenosed and whether the fistula allows decompression of the colon and rectum. Typically, patients with rectovaginal fistula and atresia ani show few clinical signs while they are on a liquid diet.139 The mother cleans the common opening; thus, local irritation is initially minimal. If the fistula is wide enough to permit passage of solid feces from the vulva after weaning, the condition may remain undiagnosed for months. In patients with a urethrorectal fistula, the classic sign is leakage of urine from the rectum or urination from the urethra and anus simultaneously.41 Eventually, clinical signs in patients with either rectovaginal or urethrorectal fistula can include vulvar or perianal irritation, cystitis, dysuria, hematuria, pollakiuria, chronic or recurrent urinary tract infections, tenesmus, diarrhea, and megacolon.41,138,217 Struvite urolithiasis has also been diagnosed in conjunction ...

The current research aims to evaluate the risk factors of urethrocutaneous fistula after hypospadias surgery among the youth in China. One hundred twenty hypospadias patients were enrolled in our study. All of them were defined as Tanner 4 or 5. The information collected from the participants include age, urethral operation history, urinary comorbidities before operation, urine test before operation, body temperature before and after operation, type of surgical repair, chordee degree, urethral defect length and whether received vesicostomy after surgery or not. Independent t test, chi-square test and multivariate logistic regression were performed to evaluate the risk factor of urethrocutaneous fistula. Among the enrolled patients, 39 patients (32.5%) developed urethrocutaneous fistula after hypospadias repair. Our result showed significant association between the group with urethrocutaneous fistula and the group without urethrocutaneous fistula with respect to age, pyuria before operation, urethral

FOR DOCTORS - PART IV Grade II or Distal Hypospadias: The Slit-like adjusted Mathieu (SLAM) Technique The meatal-based flap technique of Mathieu is the most popular technique for distal hypospadias repair and has withstood the test of time. However, the major drawback of the original Mathieu technique is the final appearance of the meatus (a smiling meatus that is not very terminal). The Slit-like adjusted Mathieu (SLAM) helps to employ the Mathieu operation in all forms of distal hypospadias and gives a terminal, slit like meatus. This will include about 70 to 80 % of patients with hypospadias. The only contraindication is the presence of severe chordee distal to the hypospadiac meatus (very rare in distal hypospadias). Operative steps: The boundaries of the urethral plate are outlined. A U-shaped incision is outlined. The two parallel incisions at the glans region start along the true mucosal urethral plate to have large, wide glanular wings. At the distal end, the two incision converge as ...
Early complications include risk of abscesses within the abdominal walls. This is more likely in old, obese and diabetic patients.. Bleeding is another common complication immediately after surgery. Bleeding in uerine or hematuria may also be seen in some patients.. A major abdominal surgery weakens the abdominal muscles and may lead to a risk of abdominal hernias over the site of the incision called incisional hernia. Obese, diabetics, those with rejection are more at risk.. Some patients may develop infectious tracts or canals called urinary fistulae.. There is a risk of post-operative formation of blood clots or arterial thrombosis. These may get dislodged from the operative site and travel up to the lungs or brain leading to life threatening complications.. ...

In all, 214 consecutive children with distal hypospadias were identified who had had a GRAP repair, of whom 205 were suitable for further study. The position of the urethral meatus in these children is summarized in Table 1. While some children were admitted before surgery for social reasons, most operations, i.e. in 183 patients (89%), were undertaken as day cases.. Problems immediately after surgery were rare; seven children (3.4%) were admitted overnight with medical problems. Of these boys, two had difficulty in voiding, one of whom required catheter drainage of the bladder. Two patients with uncontrolled pain required analgesia, two had significant nausea and vomiting, and one had a penile haematoma. Fifteen children (7.3%) stayed for one night after repair, with no clearly documented clinical indication.. A fistula occurred in 15 patients (7%); of these, one is known to have healed spontaneously, eight have been corrected surgically on a day-case basis and one fistula was not initially ...

PURPOSE: This research was conducted to compare the management and the outcome of patients with colovesical fistulae of different aetiologies. METHODS: Retrospective data were collected from 2002 to 2012 and analyzed with SPSS ver. 17. Age, gender, aetiology, management, hospital stay, postoperative complications, and mortality were studied and compared among colovesical fistulae of different aetiologies. RESULTS: A total of 55 patients, 46 males (84%) and 9 females (16%), with a median age of 65 years (interquartile range [IQR], 48-75 years) were studied. Diverticular disease was the most common benign cause and recto-sigmoid cancer the most common malignancy. Anterior resection and bladder repair were the most frequent operations in benign cases, as was total pelvic exenteration in the malignant group. Multiple intestinal loop involvement and subsequent resection were significantly higher in those with Crohn disease than it was in patients of colovesical fistula due to all other causes ...

Hypospadias is a defect in the opening of the penis, the urethra ends on the underside of the penis or bottom of the penis, or in the scrotum. Hypospadias is present at birth. Hypospadias repair is done most often between 6 months and 2 years old. The extra tissue of the foreskin may be used to repair the hypospadias during surgery. So the children with hypospadias should not be circumcised at birth. After surgery the penis will look almost or completely normal and function well. However if the hypospadias defect is complicated type, it may be need more operations to repair.. ...
Parents usually ask the Urologist that their child urinates finely and has bowel movements just fine. So why are we having the surgery done if it doesnt hurt him and he urinates just fine? Simple answer to these parents is Surgery is essential because Hypospadias can cause problems as he gets older. Problems are listed below: 1. Irregular urine stream .He would have to pee sitting down his whole life. 2. His penis would look different than other boys. 3. It could potentially create fertility issues. The opening of the penis is not close to the cervix as a result of which the sperm is not in the right place to cause pregnancy. 4. If the penis has chordee (downward curvature of the penis) it becomes very painful, difficult or even impossible to have intercourse as an adult.. If the hypospadias needs to be repaired, this is done by surgery. The aim is to repair and reconstruct the urethra. Depending on the degree of the hypospadias, the surgery can range from relatively simple to very challenging. ...

For thousands of years, women simply tolerated the distressing symptoms generated by rectovaginal fistulas (RVFs). Such tolerance is no longer necessary, because most RVFs can be surgically corrected via a number of approaches.
PURPOSE: To analyze agreement on postoperative complications after hypospadias surgery according to medical records and parents reports. MATERIALS & METHODS: In this retrospective cohort study, data were collected from 409 children who received an initial one-stage hypospadias correction in the Radboudumc, The Netherlands. Postoperative complications according to medical records were compared with parent-reported complications in an online questionnaire. Main complications studied were wound-related complications, urinary tract infections, fistulas, stenosis, and prepuce-related complications. Agreement was determined by Cohens kappa coefficient. RESULTS: Slightly less complications were mentioned in medical records (37%) compared to parents reports (42%). Overall agreement was moderate (kappa=0.50, 95% confidence interval (CI):0.41-0.59), but poor for some specific complications. Agreement was higher for complications that needed reoperation compared to when no reoperation was performed ...

Vesicouterine fistulas are generally triggered by: obstetrical causes, surgery, radiotherapy, intravaginal or intravesical foreign bodies. We report the clinical case of a patient who, subsequent to Caesarean section, developed vesicouterine fistula and endometriosis of the vesicouterine ligament. Vesicouterine fistulas are rare pathologies but rarest are genito-urinary fistulas, which are generally complications of Caesarean section. Patients with vesicouterine fistulas present urinary incontinence in the postoperative period but even years later may present cyclic hematuria, amenorrhea, vaginal losses of urine, infections of the urinary ways and secondary infertility. Fistulas may determine urological damage with impairment of vesical function; damage to the reproductive organs with impaired patient fertility; the onset of endometriosis in the vesicouterine ligament affecting the relational sphere and sexual relations.. ...
Introduction: The presence of a recurrent proximal penile hypospadius represents a surgical challenge to the urologist due to the presence of excessive scarring and fibrosis of the tissues. This problem is more pronounced in circumcised patients, in whom there is no enough skin for one stage procedures. Buccal mucosal grafts represent a good surgical option. The aim of this study was to evaluate the results of two stages buccal mucosal urethroplasty in pediatric & adolescent patients, presenting with recurrent proximal penile hypospadias who are circumcised. Methods: Thirty seven pediatric & adolescent patients underwent two stages buccal mucosal urethroplasty for recurrent proximal penile hypospadius. In all cases the buccal graft was placed dorsally followed by the second stage closure after 6 months. Results: The mean age was 17.7 (14-20) years. With a mean follow-up of 28.3 months, 33 patients (89.2%) had a final successful outcome. Of the 4 cases that were considered as failure, 3 patients (8.1%)

Carl Friedrich Philipp von Martius (1794-1868) was one of the most famous naturalists of the nineteenth century. From 1817 to 1821 he explored Brazil, with the zoologist Spix. During that period he collected more than 10,000 herbarium specimens that are now housed in the Botanische Staatssammlung, Muenchen, Germany. Importantly, von Martius trip was the first to inspire academic interest in Brazils rich flora. Martius quickly became an expert on palms and published three volumes of the Historia Naturalis Palmarum between 1823 and 1850. Additionally, he co-founded, with Endlicher, the magnificent Flora Brasiliensis, a monographical flora series. During his life time 46 fascicles were published, the remaining were completed later by Eichler and Urban in 1906 making a total of 130 volumes. Von Martius private botanical collection grew, by purchase and exchange, to become one of the most important private herbaria of the nineteenth century. When he died, it contained ca. 300,000 specimens ...

Another month down and I am still in awe at home fast these little people learn....you quickly forget from baby 1 to baby 2. One big thing that happened since last month was he had surgery. We didnt post much about it because it involves what some would consider sensitive subject matter but as you know, we like to share our experience in hope of helping others going through the same thing. He had Hypospadias surgery which involved redirecting his urethra, which we discovered had a 90 degree curve and pointed downward instead of up. We traveled to St. Louis to have the surgery with specialist Dr. Palagiri at Mercy Childrens. The scariest part for me was the Caudal Block anesthesia, which is similar to an epidural. Well, we are a week and a half out from surgery and he is healing great! The first few days were rough and it involved dressing changes and soaks but by day 5, the dressing came off and he was practically back to his old self. We have to avoid any type of straddle toys until after our ...
