Reports on varicose veins, proximal venous outflow obstruction as in May Thurner Syndrome, and gonadal vein reflux resulting in varicocele or pelvic congestion syndrome have described these venous conditions primarily as separate and unrelated. This case suggests that in at least some selected patients, there may be a possible interrelationship between these three venous conditions. The varicocele, a male equivalent of the female pelvic congestion syndrome, is due to reflux in the left testicular vein or gonadal vein. Several causes of gonadal vein reflux have been reported including a venous outflow obstruction in the left renal vein as seen in the Nutcracker syndrome and a quantitative and/or functional abnormality in the venous valves within the gonadal vein. OBJECTIVE: To evaluate prenatal management and to define the criteria of gravity for accurate assessment of the renal and overall prognosis of fetuses presenting malformations of the urinary tract. METHODS: We carried out a retrospective study of 127 cases of urinary tract malformation. We carried out descriptive statistical and univariate analyses as a function of severity criteria and the outcome of pregnancy. RESULTS: One-third of fetuses presented associated extrarenal malformations and 10% of the karyotypes were abnormal. There were more abortions in case of decrease in amniotic fluid volume (p | 0.001), extent of renal damage (p | 0.05), presence of associated extrarenal malformations (p | 0.05), early diagnosis of the malformation (p | 0.001) and presence of chromosomal syndrome (p = 0.01). In our study, there was an excellent correlation between prenatal data and pathological findings for the fetus following abortions for medical reasons or obtained during the surveillance of live-born children.

A 47-year-old Hispanic woman developed a chronically obstructed left kidney, due to a long-segment ureteric stricture deemed not amenable to reimplantation, following left ovarian cyst excision in 2004. Therefore, a ureteral stent requiring exchange every 3 months was necessary, due to hydronephrosis, recurrent urosepsis, chronic pain, and a poor quality of life. Her medical history was complicated by hypertension, poorly controlled diabetes mellitus, and microalbuminuria, suggesting early diabetic nephropathy. A left nephrectomy was recommended. This was deferred, due to concern for progressive kidney failure associated with her comorbidities. A radionuclide Tc-99m MAG3 renal scan revealed differential perfusion as follows: 44% left kidney and 56% right kidney, with symmetrical uptake on the renogram phase and delayed excretion on the left, which were correctted following furosemide administration.
