Exploring the information needs of patients with unexplained chest pain Ingrid Ølfarnes Røysland,1 Elin Dysvik,1 Bodil Furnes,1 Febe Friberg1,2 1Department of Health Studies, Faculty of Social Sciences, University of Stavanger, Stavanger, Norway; 2Institute of Health and Care Sciences, Sahlgrenska Academy at Göteborg University, Göteborg, Sweden Background: Unexplained chest pain is a common condition. Despite negative findings, a large number of these patients will continue to suffer from chest pain after being investigated at cardiac outpatient clinics. Unexplained chest pain covers many possible complaints, and diagnosing a single cause for a patients pain is often described as difficult, as there are a number of possible factors that can contribute to the condition. For health professionals to meet patients expectations, they must know more about the information needs of patients with unexplained chest pain. The aim of this study was to describe information needs among patients with

BACKGROUND: Coagulopathy after major hemorrhage has been found to be an independent risk factor for mortality after traumatic bleeding. It is unclear whether similar associations are present in other causes of major hemorrhage. We describe the prevalence, use of plasma, and outcomes of patients with coagulopathy after acute nonvariceal upper gastrointestinal bleeding (NVUGIB). STUDY DESIGN AND METHODS: This study was a multicenter UK national audit. Data were collected prospectively on consecutive admissions with upper gastrointestinal bleeding over a 2-month period to 212 UK hospitals. Coagulopathy was defined as an international normalized ratio (INR) of at least 1.5. Logistic regression was used to examine the relationship between coagulopathy and patient-related outcome measures of mortality, rebleeding, and need for surgery and/or radiologic intervention. RESULTS: A total of 4478 patients were included in the study. Coagulopathy was present in 16.4% (444/2709) of patients in whom an INR was
Various strategies can be used in the diagnosis of upper gastrointestinal tract bleeding. This study investigates the relevance of anamnestic and clinical findings for the diagnosis of the bleeding source. The authors introduced a computer-aided diagnostic system using Bayes theorem and compared it with clinicians predictions using anamnestic and clinical findings only. There was no difference in the overall accuracy rates, but a difference was observed in the diagnostic behaviors of the two systems. In addition, the discriminatory ability of the computer-aided system, the sharpness of the predictions obtained, and the reliability of the posterior probabilities were analyzed. It is concluded that the clinician and the computer-aided system are not able to discriminate well between the disease categories. Derived classification matrices and probability-based measures show the reasons for the inadequacy of diagnostic information obtainable from the clinical history and physical findings. ...

The thesis also looks at the development of symptoms and the prognosis for patients with unexplained chest pain over a period of time, compared with patients suffering from angina and patients who had suffered a heart attack. A register study revealed that from 1987 up until 2000, the number of patients with diagnosed unexplained chest pain increased, and then leveled out. The number of patients with angina increased up until 1994 and has since fallen, while the number of patients who have suffered heart attacks has fallen throughout the whole period examined.. ...

Gastroduodenal disease (GDD) was initially thought to be uncommon in Africa. Amongst others, lack of access to optimal health infrastructure and suspicion of conventional medicine resulted in the reported prevalence of GDD being significantly lower than that in other areas of the world. Following the increasing availability of flexible upper gastro-intestinal endoscopy, it has now become apparent that GDD, especially peptic ulcer disease (PUD), is prevalent across the continent of Africa. Recognised risk factors for gastric cancer (GCA) include Helicobater pylori (H. pylori), diet, Epstein-Barr virus infection and industrial chemical exposure, while those for PUD are H. pylori, non-steroidal anti-inflammatory drug (NSAID)-use, smoking and alcohol consumption. Of these, H. pylori is generally accepted to be causally related to the development of atrophic gastritis (AG), intestinal metaplasia (IM), PUD and distal GCA. Here, we perform a systematic review of the patterns of GDD across Africa ...
Upper endoscopy helps Dr. B C Shah evaluate symptoms of persistent upper abdominal pain, nausea, vomiting or difficulty swallowing. Its the best test for finding the cause of bleeding from the upper gastrointestinal tract. Its also more accurate than X-ray films for detecting inflammation, ulcers and tumors of the esophagus, stomach and duodenum.. Dr. B C Shah might use upper endoscopy to obtain a biopsy (small tissue samples). A biopsy helps Dr. B C Shah distinguish between benign and malignant (cancerous) tissues. Remember, biopsies are taken for many reasons, and Dr. B C Shah might order one even if he or she does not suspect cancer. For example, he might use a biopsy to test for Helicobacter pylori, the bacterium that causes ulcers.. Dr. B C Shah might also use upper endoscopy to perform a cytology test, where he or she will introduce a small brush to collect cells for analysis.. Upper endoscopy is also used to treat conditions of the upper gastrointestinal tract. Dr. B C Shah can pass ...
Background Omeprazole by intravenous infusion is occasionally warranted in children during episodes of acute upper gastro-intestinal bleeding. This is to ensure the continued suppression of stomach acid to reduce the of risk of re-bleeding.. Current practice in UK paediatric intensive care units varies, but the use of 160 mg/1.73 m3 over 24 hour has previously been reported.1 This is commonly prepared as a 0.8 mg/ml solution changed either at 12 or 24 hrs after preparation depending on reference source used.2 - 3 The current practice of using a 0.8 mg/ml (40 mg/50 ml) is off label and not endorsed by manufacturers who recommend a maximum concentration of 0.4 mg/ml3.. Following these recommendations would lead to a fluid load of 400 ml/1.73 m2 which is likely to be significant. Excess fluid intake has been shown to correlate with increased oxygenation index and increased PICU stay.4 The aim of this study is to provide stability data for using 0.8 mg/ml for intravenous infusion which is currently ...

Upper endoscopy, also known as esophagogastroduodenoscopy or EGD, is a diagnostic procedure used to visually examine and diagnose certain conditions of the upper gastrointestinal, or digestive tract. The upper gastrointestinal tract includes the esophagus, stomach and duodenum, or upper part of the small intestine. An upper endoscopy is performed using a flexible tube with an attached light and camera, called an endoscope. It is inserted through the mouth and guided along to thoroughly examine the upper gastrointestinal tract. | Robert Greenblatt

Endoscopic diagnosis of GVHD. Three hundred thirty eight endoscopies of the upper gastrointestinal tract were performed in 197 patients with suspect diagnosis of acute or chronic GVHD; macroscopic endoscopic data compatible with GVHD were present in only 83 patients with GVHD (34.2%). On the other hand, in patients without histological and clinical confirmation of GVHD, the 35.3% (n = 30) showed an endoscopy compatible with GVHD. Endoscopic findings to the diagnosis of GVHD have a sensitivity (S) of 34%, specificity levels (SP) of 65%, a positive predictive value (PPV) of 73% and a negative predictive value (NPV) of 48%.. Endoscopic findings in patients with GVHD according to the classification of endoscopic severity -Cruz-Correa (12) classification- were not related with clinical severity.. Histology. Biopsy was performed in 331 out of the 338 upper endoscopies (98%). The esophagus was biopsied in 19 explorations (7.8%), the stomach in 324 (92.2%) and the duodenum in 18 (7.4%). Prospective analyses have yet to uncover a consistent relationship between vitamin D status and incidence and mortality of rarer cancers including esophageal and upper gastrointestinal cancers. We searched PubMed for literature about the epidemiology of upper gastrointestinal cancers and vitamin D published over the last decade and then summarized and critiqued the results of these studies in this review. The search yielded nine relevant studies. Overall, no consistent relationship was reported between serum vitamin D levels or a surrogate and upper gastrointestinal cancers. Four studies reported negative correlations between vitamin D status and upper gastrointestinal cancer, three reported positive correlations, one reported no correlation, and one reported both positive and negative correlations. No relationship has been established on the basis of epidemiologic data, but studies examining sun exposure consistently report an inverse association with esophageal cancer. The current literature ...

In this study, we examined the role of t-DARPP in regulating β-catenin.Results: The pTopFlash construct that contains multiple TCF/LEF-binding sites was used as a measure of β-catenin/TCF transcription activity. Gastric (AGS, MKN28) and esophageal (FLO-1) adenocarcinoma cancer cell lines that lack t-DARPP expression were utilized to establish stable and transient in vitro expression models of t-DARPP. The expression of t-DARPP led to a significant induction of the pTOP reporter activity, indicative of activation of β-catenin/TCF nuclear signaling. Immunofluorescence assays supported this finding and showed ...
Investigation is unnecessary for the typical patient who has typical symptoms of gastroesophageal reflux disease, providing they do not have worrying alarm symptoms such as weight loss or dysphagia. If treatment with a proton pump inhibitor, as a therapeutic trial, effectively relieves symptoms then further treatment may be prescribed as needed.. If there are red flag (alarm) symptoms, and /or symptoms of GERD do not improve, then further investigation is needed. Upper gastro-intestinal endoscopy (gastroscopy) is the preferred investigation. This is because endoscopy is highly sensitive in identifying esophageal strictures, ulcers, erosions and cancer. Barretts esophagus can also be identified and confirmed by biopsy by endoscopy. During endoscopy it is possible to take biopsies of suspicious looking areas of mucosa.. Prolonged and severe exposure of the lower esophagus to reflux of acidic stomach contents leads to the development of Barretts esophagus. About 2-4 percent of patients with GERD ...
We use multi-microsecond simulations of the hexameric channel assembly to investigate the atomic detail mechanics that regulate pH-gating, solute selectivity, and flux limitation. In addition, we will study the export route and transport mechanisms of the urea hydrolysis products, CO2 and NH3, back into the periplasmic space. Inhibition of either urea or CO2 and NH3 transport would render H. pylori susceptible to acidic degradation, thus presenting a good opportunity for targeted drug development. A thorough understanding of the molecular mechanisms underlying channel function is vital for the design of suitable inhibitors and new therapies. Blocking of this channel would be expected to result in a specific and effective monotherapy for eradication of the organism. This would provide a preventive approach to serious upper gastro-intestinal diseases, particularly gastric cancer. Our studies require multi-microsecond simulations that are able to capture channel function of the native UreI ...
YOUNG adults presenting to general practice with unexplained chest pain (UCP) return earlier and more frequently than patients with diagnosed coronary artery disease (CAD), US researchers say.In a retrospective study, nearly 750,000 US veterans, median age 34, were followed from 2001 to 2010. The 20,521 with UCP had recurrent chest pain earlier and 1.5 times more frequently than the 5303 with known CAD.The authors hypothesised that patients with UCP felt their chest pain was less controllable and less understandable than those with CAD. Since first reported EIPD in 1960, there were around 200 cases published world-wide.1,2 Up to 90% of patients with EIPD have associated oesophageal stenosis of various levels most commonly in the upper oesophagus secondary to chronic stasis oesophagitis.3,4 EIPD is often regarded as benign oesophageal disease but there are several reports where EIPD was found in patients with oesophageal carcinoma. 5 The aetiology behind EIPD developing oesophageal cancer could be due to metaplastic changes of the squamous epithelium within the excretory ducts of oesophageal submucosal glands in EIPD. 6 The pre-malignant nature of metaplastic epithelium is widely recognised in the upper gastro-intestinal tract. The increased prevalence of EIPD in patients with oesophageal carcinoma may warrant periodic surveillance in this small population of patients. ...

Most people who complain to their doctors about first-time chest pain dont get additional diagnostic testing, but the symptom should be viewed as a possible warning sign, a new study suggests.
Medications that cause slow emptying should typically be stopped, and reversible conditions (example: hypothyroid) treated prior to testing. A history of early satiety, bloating, nausea, regurgitation or vomiting with meals would normally prompt an evaluation to determine the cause of symptoms. Inflammation, ulcer disease, or obstruction by a tumor can also cause these symptoms and diagnostic tests would be used to determine the cause. Radiographic tests, endoscopic procedures, and motility tests are used to exclude obstruction, to view the stomach lining and obtain biopsies, and to examine muscle contraction patterns. These tests are described below.. Upper Endoscopy is a test that is performed by inserting a thin flexible tube through the mouth into the stomach. The endoscope has camera capabilities and allows the upper gastrointestinal tract to be evaluated for ulcers, inflammation, infection, cancer, hernias or other abnormalities. These conditions can cause symptoms similar to ...
