BACKGROUND: Reference values for umbilical artery Doppler indices are used clinically to assess fetal wellbeing. However, many studies that have produced reference charts have important methodological limitations, and these result in significant heterogeneity of reported reference ranges. OBJECTIVES: To produce international gestational age-specific centiles for umbilical artery Doppler indices based on longitudinal data and the same rigorous methodology used in the original Fetal Growth Longitudinal Study (FGLS) of the INTERGROWTH-21st Project. STUDY DESIGN: In Phase II of the INTERGROWTH-21st Project (the INTERBIO-21st Study), we prospectively continued enrolling pregnant women according to the same protocol from three of the original populations in Pelotas (Brazil), Nairobi (Kenya) and Oxford (UK) that had participated in FGLS. Women with a singleton pregnancy were recruited at |14 weeks gestation, confirmed by ultrasound measurement of crown-rump length, and then underwent standardized ultrasound

Results We included 28 patients in clinical remission (23 females and 5 males with a mean age (SD) of 51.9 (11.9) years and a disease duration of 108.2 (92.6) months). 71.9% of patients were positive for rheumatoid factor and anti-citrulline antibodies. 75% of patients were on at least one DMARD and 39.3% were on biological treatment. 60.7% of patients had power Doppler signal at baseline and 46.4% met criteria for our definition of active synovitis (synovial hypertrophy ≥2 + Power Doppler signal, Ramirez J et al, Arthritis Research and Therapy 2014). After one year of follow-up, 12 patients (42.9%) did not meet criteria for clinical remission. Of these, 9 had Power Doppler signal in the first ultrasound assessment (p=0.034) and 7 met criteria for defined subclinical synovitis by ultrasound (p=0.291). I am 36 years old. I am 5 feet 10 inches tall and I weigh 77 kg. I am suffering from hypertension. Two months ago my blood pressure (BP) was 120/176. I am on Losartan 50 mg (1 tab daily), Epitril 0.5 mg (half tablet), only if I feel uneasy and Pantocid (daily morning/evening). My problem is that I feel tired when I am stressed and I get a slight chest pain, which initially used to remain overnight but now remains only for an hour or so. OBJECTIVE: Doppler ultrasound provides a non-invasive method for the study of the uteroplacental circulation. In normal pregnancy, impedance to flow in the uterine arteries decreases with gestation, which may be the consequence of trophoblastic invasion of the spiral arteries and their conversion into low-resistance vessels. Pre-eclampsia and fetal growth restriction are associated with failure of trophoblastic invasion of spiral arteries, and Doppler studies, in these conditions, have shown that impedance to flow in the uterine arteries is increased. A series of screening studies involving assessment of impedance to flow in the uterine arteries have examined the potential value of Doppler in identifying pregnancies at risk of the complications of impaired placentation. The present study evaluated the predictive value of renal resistive index (RI) for renal function and blood pressure (BP) outcome in hypertensive patients with unilateral atherosclerotic renal artery stenosis submitted to successful revascularization. In 158 hypertensive patients with atherosclerotic renal artery stenosis RI was acquired. To evaluate the accuracy of a pulsed multigated Doppler system, 128 carotid arteries were examined. The spectral broadening index was calculated from the power spectrum of a small sample volume located in the center of the stream according to the flow profile and was related to the degree of stenosis as determined by contrast angiography. Blood flow measurement using an improved surface integration of velocity vectors (SIVV) technique was tested in in vitro phantoms. SIVV was compared with true flow (12-116 mL/s) in a steady-state model using two angles of insonation (45 degrees and 60 degrees ) and two vessel sizes (internal diameter = 11 and 19 mm). Repeatability of the method was tested at various flow rates for each angle of insonation and vessel. In a univentricular pulsatile model, SIVV flow measured at the mitral inlet was compared to true flow (29-61 mL/s). Correlation was excellent for the 19-mm vessel (r(2)= 0.99). There was a systematic bias but close limits of agreement (mean +/- 2 SD = -24.1% +/- 7.6% at 45 degrees; +16.4% +/- 11.0% at 60 degrees ).
