TY - JOUR. T1 - Revision tympanoplasty utilizing fossa triangularis cartilage. AU - Moore, Gary Floyd. PY - 2002/9. Y1 - 2002/9. N2 - Objectives/Hypothesis: A small percentage of fascia graft tympanoplasties fail. Cartilage tympanoplasty has a reputation for excellent graft healing but potentially sacrifices maximum hearing improvement and creates difficulty in postoperative follow-up resulting from opacity and immobility. We sought to use a tissue thicker than fascia but thinner than tragal cartilage to repair tympanic membranes that had failed previous fascia grafting. Our hypothesis was that use of the thinner cartilage would maintain the excellent healing rate and resistance to chronic negative pressure while improving hearing and mobility. Study Design: The study is a retrospective review of all patients who received a total cartilage graft tympanoplasty after experiencing a failed standard fascia graft tympanoplasty. No previous operative series on impedance testing following cartilage ...

Tympanoplasty (eardrum repair) is a surgical procedure to rebuild a perforated eardrum (tympanic membrane) or the middle ears tiny bones. The problem of the tympanic membrane may be the outcome of severe infection or from any trauma to the eardrum. There are three layers of the tympanic membrane. Both inner and outer layers have epithelium cells. The condition of perforations can be due to defects in the middle ear that includes elastic collagen fibers. The small perforations can be easily healed but in the case of larger defect, the process of healing is hindered. The reasons for perforated eardrums could be an outcome of trauma like a slap on the ear, an explosion or any object in the ear. The aim of tympanoplasty surgery is to restore the perforated eardrum and also the middle ear bones consisting of stapes, incus and malleus.. ...

Debris, dirty water and bacteria may enter the middle ear through the perforation. This may result in an infection commonly associated with a discharge out of the ear. Most perforations are associated with hearing loss. The degree of hearing loss depends in part on the size of the perforation, the location of the perforation on the eardrum, and whether there is other pathology present in the ear. In some cases, skin can grow around the margins of the perforation, forming a skin cyst (cholesteatoma), in the middle ear and mastoid. However, in many patients with a perforation of the eardrum, the ear remains free of infection and can be safely left without surgical closure of the perforation. ...

OBJECTIVES: To describe a unique case of bilateral dehiscence of the malleus and incus heads into the middle fossa making contact with the temporal lobes, along with its clinical implications. METHODS: An analysis of a patient case and review of pertinent literature were performed. RESULTS: A patient with a history of right-sided mastoidectomy for cholesteatoma was evaluated for persistent conductive hearing loss. On computed tomography (CT) and magnetic resonance imaging (MRI), the patient had a complete dehiscence of the tegmen tympani on the right, with ossicular heads being located above the floor of the middle cranial fossa ...
Our research was made on 23 patients who were able to obtain Tissucol from foreign countries and surgery in the same technique was applied to another group of 23 patients who approximately and the same pathology. Pure tonal audiometric tests were made to compare the gain in their hearig. It was observed that Tisscola made possible teh montage to be stable and elastic especially in type II and III tympanoplasties which needed the ossicules to be mounted. It was also used in myringoplasties in applying the graft without sliding. Tissucol was useful in stable fixation of autograft bone cartilage which was used in reconstruction of canalis acousticus externus in EPM+Tympanoplasties in order to prevent the depression of posterior superior wall. In type II tympanoplasties where Tissucol was applied there was a gain of 15 dB in hearing when compared to the controlle group. This research led to the conclusion that Tussucol was hard to be used in routine because it cantt be found in our country, it is ...

When the doctor examined my ear, he said it is not healing at all, not even a little bit. Its no worse, but its also no better. For some reason, he wants to wait until the weather gets a little bit warmer, so Im to see him again in March. I can live with a hole in my eardrum for the rest of my life as long as I keep water out of my ears, but if I want it repaired, they will likely have to do a fat tympanoplasty, which will take some fat cells from behind my ear and use that to patch the hole and see if my body will take over the repair from that. He says it has about an 80% success rate. There are other options if that doesnt work, but well cross that bridge when we come to it ...
One tries to compensate with "active listening," as opposed to passive hearing, but learns to enjoy the garble, the minds futile efforts to make sense of the inadequately perceived, to fill in the blanks. This morning, an otolaryngologist will rebuild my left eardrum - the procedure is called tympanoplasty - and implant a prosthetic bone of titanium. I dont expect full stereo, just no more mono. In "In the Park" (Not Waving but Drowning, 1957), Stevie Smith overhears "two silvered gentlemen" talking. One says: "Pray for the Mute." His hard-of-hearing friend, after rhapsodizing the amphibian, says, "`Pray for the Mute? / I thought you said the newt." When Muriel Spark reviewed Smiths book in 1957 she observed: "The main theme of this collection is that life is fairly deplorable and yet must be praised." ...

TY - JOUR. T1 - Artificial intelligence to detect tympanic membrane perforations. AU - Habib, A.-R.. AU - Wong, E.. AU - Sacks, R.. AU - Singh, N.. PY - 2020/4/1. Y1 - 2020/4/1. N2 - Objective. To explore the feasibility of constructing a proof-of-concept artificial intelligence algorithm to detect tympanic membrane perforations, for future application in under-resourced rural settings. Methods A retrospective review was conducted of otoscopic images analysed using transfer learning with Googles Inception-V3 convolutional neural network architecture. The gold standard ground truth was defined by otolaryngologists. Perforation size was categorised as less than one-third (small), one-third to two-thirds (medium), or more than two-thirds (large) of the total tympanic membrane diameter. Results A total of 233 tympanic membrane images were used (183 for training, 50 for testing). The algorithm correctly identified intact and perforated tympanic membranes (overall accuracy = 76.0 per cent, 95 per ...
Introduction. Tympanoplasty is one of the surgeries most frequently performed by otorhinolaryngologists for the correction of tympanic membrane perforations and the reconstruction of the mechanism of sound transmission in the middle ear. The graft used, such as a temporalis muscle fascia, functions as a support on which epithelialization currents slide in order to repair a perforation of the tympanic membrane1. Preliminary retrospective studies have indicated greater vascularization of tympanic membrane grafts when a natural latex membrane is used as a transitory implant in tympanoplasty2. A natural biomembrane prepared with latex extracted from the rubber tree Hevea brasiliensis and polymerized with 0.1% polylysine was developed by the Department of Biochemistry of the Faculty of Medicine, of Ribeirao Preto, University of Sao Paulo. The membrane was used experimentally as a cicatrization-inducing material on the injured esophageal walls of dogs and was shown to lead to a marked increase in ...

Introduction: Tympanoplasty for the surgical correction of chronic suppurative otitis media is one of the most comman ENT surgical procedures. This surgery can be done both under general anaesthesia and local anaesthesia with sedation. Dexmedetomidine is said to have both analgesic and sedative properties. The aim of this study is to compare the results of monitored local anaesthesia with dexmedetomidine with general anaesthesia in patients undergoing tympanoplasty to find out the better efficacious anaesthesia for this surgery. Methods: 100 patients were planned for tympanoplasty were taken up for this study. 50 patients were operated under general anaesthesia and remaining 50 patients were operated under monitored local anaesthesia with dexmedetomidine. The results were evaluated intraoperatively and postoperatively for duration of surgery, hemodynamic stability and complications. Patient s and surgeon s satisfaction was also evaluated. Results: The results showed that there was lesser ...

A drain for the eardrum is formed of a high polymer with the properties of absorbency and swelling. An apparatus for inserting the drain for the eardrum includes a needle with a sharp tip, a tube sheathing the needle and a trigger to project the sharp end of the needle from the end of the tube. When the trigger is drawn, the sharp tip of the needle is extended from the end of the tube and is then inserted into the drain, and used to bore a hole through the eardrum and to insert the drain into the hole.
