Background: Breaking bad news to cancer patients is one of the important responsibilities in the oncology setting. The purpose of this study is develop and validate a new theoretically based tool for measurement of attitude and practice of physicians toward breaking bad news.. Methods: The psychometric properties of the scale were established by following the guidelines of Clark and Watson. In the first phase, a literature review was performed to create items; then items were assessed for content validity through individual interview (n = 12) and construct validity was assessed by using factor analysis. Reliability was evaluated by Cronbachs alpha. Research data was gathered from physicians working in breast cancer setting. Results: A total of 12 expert reviews concluded that a large amount of items of attitude and practice questionnaires were important and essential (Content Validity Ratio , 0.73). The exploratory and confirmatory factor analyses for a sample of physicians (n = 200) indicated ...
We want our services to support an open, timely and consistent approach to communicating with service users and their families when things go wrong in healthcare. This is called open disclosure.. An open disclosure programme was piloted at two hospitals, the Mater Misericordiae University Hospital, Dublin and Cork University Hospital from October 2010 until October 2012.. Utilising the learning from the pilot programme the HSE has developed, in conjunction with the State Claims Agency, a national policy and national guidelines on open disclosure with supporting documents which include a patient information leaflet, a staff support booklet and a staff briefing guide.. The HSE and State Claims Agency launched these documents on 12th November, 2013.. Work is now ongoing across all health and social care services in relation to the implementation of the national open disclosure policy and guidelines. Although Anti Retroviral Therapy (ART) is the single most important management of HIV infection, success in decreasing viral load depends on adherence to a strict regimen that requires taking the prescribed drugs daily at the specified times. Failure to daily intake of Anti Retrovirals (ARV) not only prevents treatment failure but may also lead to viral development of resistance to the drugs. The fact that HIV is mainly sexually transmitted means that it is associated with socially unaccepted practices such as premarital and extramarital as well as multiple sexual partners. Fear of such stigma related to these practices may thus prevent HIV status disclosure and deny the patient social support that may be important ...
I met a traveller from an antique land who said: `two vast and trunkless legs of stone stand in the desert. near them, on the sand, half sunk, a shattered visage lies, whose frown, and wrinkled lip, and sneer of cold command, tell that its sculptor well those passions read which yet survive, stamped on these lifeless things, the hand that mocked them and the heart that fed. and on the pedestal these words appear. My name is Ozymandias, king of kings: look on my works, ye mighty, and despair! nothing beside remains. round the decay of that colossal wreck, boundless and bare the lone and level sands stretch far away ...

People with learning disabilities are now enjoying a longer life expectancy than ever before as a result of enhanced medical and social interventions and improved quality of life. Some, particularly individuals with Downs syndrome, are susceptible to dementia at a significantly younger age than the average age of onset in the rest of the population. Currently, there is limited guidance on how to talk to people with learning disabilities about dementia and, until such information is shared, individuals cannot be positioned as an authority on their own condition. The new model presented here suggests a way of supporting staff and families to have enabling conversations about dementia that centre on the persons current situation, level of understanding and capacity.Read More:http://journals.rcni.com/doi/abs/10.7748/ldp.18.7.16.e1672. If the television shows I watched this week really reflected reality, many of the characters would be talking about the Breaking Bad finale, with some catching up on Netflix and the AMC marathon. Breaking Bad has done what few shows can do in increasing interest, and ratings, as it approached its finale. Breaking Bad has succeeded against its rivals the same way that Walters blue meth has-being superior to the competition. The show has also become as addictive as blue meth. This is primarily due to basics such as high quality acting and writing, but Breaking Bad has also been assisted by modern technology. The ability of many people to catch up at no additional cost on streaming services such as Netflix has helped increase the audience over past years. High definition big screen televisions are necessary to fully appreciate the camera work.. Things appeared to be as bad as possible for Walt and Jesse at the end of Ozymandias. This article reports on what is at work during individual supervision of medical students in the context of teaching breaking bad news (BBN). Surprisingly, there is a relative lack of research and report on the topic of supervision, even though it is regularly used in medical training. Building on our research and teaching experience on BBN at the undergraduate level, as well as interviews of supervisors, the following key elements have been identified: learning objectives (e.g., raising student awareness of structural elements of the interview, emotion (patients and students) handling), pedagogical approach (being centered on students needs and supportive to promote already existing competences), essentials (e ...
The trial judge went on to say he found neither party to be credible. However, on the basis of a note of D.C.s doctor made seven years earlier referring to a broken condom, the judge concluded that D.C. had lied to her doctor, and inferred that no condom had been worn. On those speculative grounds he convicted D.C. The Court of Appeal went on to concluded that the trial judges reasoning as to whether a condom was used was a reasonable inference, but set aside the convictions on the ground that even without condom use, the requirement of a significant risk of serious bodily harm was not met. It is only at the SCC level, that the underlying premise that D.C. was recognized as speculative and insufficient to prove guilt beyond a reasonable doubt, leaving important questions about whether stigma and preconceptions about HIV positive people, not just biology, is impacting when non-disclosure of HIV positive status results in a criminal conviction.. ...

New Year… a clean slate, a fresh start etcetera. Or not, if my first working day of 2012 is anything to go by.. Because it brings me the following three scenarios:. 1. An elderly bloke whose opening gambit is to complain he still hasnt heard anything about his cataracts, and that the salbutamol aerosol I had the audacity to prescribe in place of his previous shiny, sleek, turbo-charged version is: Cheap rubbish - its all about money, isnt it? Well, yes, it is. Those inhalers are equally effective, I explain, But your original version is four times as expensive. Making this switch saves the NHS cash.. Enough cash, say, to get your cataracts sorted out. And who knows, with better vision, you might be able to see what a cantankerous old git youre being.. 2. A woman whos returned for the results of her blood glucose test. I adopt my Doctor breaks bad news, but may be able to save you, serious face to break the news.. The book is organized around a flowchart comprising 16 key questions concerning truth, ranging from Is truth a property? to Is truth epistemically constrained?. It expounds and engages with the ideas of many thinkers, from Aristotle and the Stoics, to Continental analytic philosophers like Bolzano, Brentano, Frege, and Kotarbinski, to such leading figures in contemporary debates as Dummett, Putnam, Wright, and Horwich. In the course of this discussion, many important distinctions (between varieties of correspondence, for example, between different readings of making true, between various kinds of eternalism and temporalism) are emphasized that have so far been neglected in the literature.According to our workaday concept of truth, what we think is true if and only if things are as we think they are. A modest account of truth‐apt thinkables and of truth, in terms of higher‐order quantification over propositions, can spell out this platitude without invoking notions like correspondence, fact,

More loose ends are tied in episode two of season five (say it with me!) part one, the delightfully Mike-heavy Madrigal. We find out right fucking quick that no, that flash forward from last week will not be a running thing a la season twos more vague series of flash forward cold opens. Instead of a possibly fifty-two-turning Walt doing shit with a fucking machine gun we get some German dude named Schuler tasting new sauces with all the joy of that shithead food critic from Ratatouille. (The Franch sounded good to me but Im a dumb American.) Turns out hes the guy that connects the major corporation Madrigal to Gus Frings chicken chain. Guy gets word the cops have arrived and kills himself in a science-y way that Walt probably would have admired.. Hank hasnt decided to call it quits at Gus Fring, flying in a bunch of Madrigal execs from Germany in for questioning. Whether those Germans will become a major aspect to the story from here on out seems doubtful because, you know, how many of ...

In general, most, but not necessarily all, patients want truthfulness about their health. Available evidence indicates that truth-telling practices and preferences are, to an extent, a cultural artefact. It is the case that practices among nurses and doctors have moved towards more honest and truthful disclosure to their patients. It is interesting that arguments both for and against truth-telling are established in terms of autonomy and physical and psychological harm. In the literature reviewed here, there is also the view that truth-telling is essential because it is an intrinsic good, while it is argued against on the grounds of the uncertainty principle. Based on this review, it is recommended that practitioners ought to ask patients and patients families what informational requirements are preferred, and research should continue into truth-telling in clinical practice, particularly to discover its very nature as a cultural artefact, and the other conditions and contexts in which truth-telling may
Duterte continues to have high popularity ratings nationwide, especially in Mindanao and here in the city he ruled for 22 years as mayor.. Gaspar said several political and social scientists have come up with all kinds of theories to explain the Duterte phenomenon but my hunch is that one reason is to appropriate the metaphor of us Filipinos as a collective battered wife.. We know that a battered wife - once she internalizes the oppression - will be the first to justify the violence of her abusive husband. Just because he makes her laugh with his corny jokes, she will not mind the battering. Just because she is provided financial and other kinds of security, she will endure the beating even if she ends up in the hospital. Because her husband is now idolized by tens of thousands, she is willing to forgive and forget for she believes that the spotlight also shines on her, he said.. But another hunch tells us that perhaps one of the other reason is the problem of truth-telling in our day and ...
When making disclosures, arbitrators should consider all aspects of their professional and personal lives and disclose all ties between the arbitrator, the parties, and the matter in dispute, no matter how remote they may seem. Full disclosure also requires arbitrators to include events and circumstances that occurred while using a former name.. If you need to think about whether a disclosure is appropriate, then it is: Make the Disclosure. Failure to disclose may result in vacated awards which undermine the efficiency and finality of our process. Failure to disclose may also result in removal from the roster.. Neutrality starts with complete and accurate disclosures in FINRAs arbitrator application. FINRA uses the application as the foundation for the Arbitrator Disclosure Report (Disclosure Report)-a summary of an arbitrators background-which is provided to parties to help them make informed decisions during the arbitrator selection process. Contest guidelines did not specify that a rationale for disclosure was required; however, most videos provided one, suggesting that providing rationales to individuals with HSV infection may be important as they decide to disclose to partners. Possibly because of the stigma associated with an STI, manageability and prevalence of the disease emerged as frequent motivations for disclosure. Healthcare providers (HCPs) could focus on these during counselling around disclosure to diminish the difficulties of disclosing genital infections. This may encourage disclosure and the notion that having HSV does not need to be a secret.8 ,9. While many resources promote direct in-person disclosure of HSV status,3 these videos contained a wide range of creative methods. Technology (phones, text messages) may be helpful if disclosure feels difficult to do in person. For patients who prefer this method, HCPs should review the potential risks of using technology, for example, the potential for viral spread. ...
A University of Toronto report based on two neural imaging studies that monitored brain activity has found individuals are more satisfied to get a reward from telling the truth rather than getting the same reward through deceit.. Our findings together show that people typically find truth-telling to be more rewarding than lying in different types of deceptive situations, says Prof. Kang Lee from the University of Toronto, whose research is partially funded Social Sciences and Humanities Research Council (SSHRC).. The findings are based on two studies of Chinese participants using a new neuroimaging method called near-infrared spectroscopy. The studies are among the first to address the question of whether lying makes people feel better or worse than truth-telling.. The studies explored two different types of deception. In first order deception, the recipient does not know the deceiver is lying. In second order deception, the deceivers are fully aware that the recipient knows their intention, ...
I applaud Dr. Bernard on her powerful article disclosing her battles with depression. I agree that attention to mental health with disclosure to trusted colleagues, medical care, and saying no to the often-overwhelming opportunities in medicine are key. I also agree with Dr. Bernard that disclosure is challenging, and that reprisals from regulators stigmatizes mental health, to the detriment of physicians.. For example, the College of Physicians and Surgeons of Alberta requires during the initial application and annual renewal of a license disclosure of any physical, cognitive, mental and/or emotional condition that is negatively impacting your work, or is reasonably likely to negatively impact your work in the future, both at present, and in the past 10 years. Firstly, this is so incredibly broad that an argument could be made for any and all conditions to require disclosure. Secondly, there is no information provided as to what the implications of disclosure could be. Just for ...
Inconsistent with concerns of uninformative boilerplate or copy and paste disclosure, I find that managers time their identification of new risk factors and removal of previously identified ones to align with the expected occurrence of future adverse outcomes. By using individual risk factors as the unit of disclosure, I am able to provide novel evidence that managers remove stale disclosures on a timely basis. After controlling for firm-specific heterogeneity, I find that the count of individual risk factors disclosed, rather than an aggregate word count, explains time-series variation in managerial disclosure decisions, consistent with the regulatory intent. To shed light on what shapes the disclosure equilibrium, I study the managerial response to demand shocks from public and private enforcement actions. The results show that firms respond to investor demand in a manner consistent with the litigation shield hypothesis, and that this effect persists for multiple years. Consistent with the ...

June 27, 2010Doug Wojcieszak, Founder & Spokesperson Contact phone/e-mail address: 618-559-8168; [email protected] STUDY: BEHIND-THE-SCENES DOCS & DISCLOSURE There was a study released recently on anesthesiologists and disclosure - the abstract is below. I tagged this newsletter behind-the-scenes docs and disclosure because what is true for anesthesiologists and disclosure also rings true for radiologists, the lab, some nurses, and other clinicians and staff who dont have as much face time with the patient or family as, say, the surgeon or other clinicians who have built a relationship. This is critical, because the hot-shot surgeon who has the trust of the grieving widow can choose not to disclose a team error and get everyone sued, or can pass the blame for his own mistake to the faceless anesthesiologist, who is now in the trick bag.. This is an important and serious issue, and we are glad this study is out there contributing to the conversation. And I think it ultimately argues for the ...
Abstract Many countries apply lower fines to tax evading individuals when they voluntarily disclose the tax evasion they committed. I model such voluntary disclosure mechanisms theoretically and show that while such mechanisms increase the incentive to evade taxes, they nevertheless increase tax revenues net of administrative costs. I confirm the importance of administrative costs in a survey of German competent local tax authorities. I then test the effects of voluntary disclosure on the tax evasion decision, using the introduction of the 2009 offshore voluntary disclosure program in the U.S. for identification. The analysis confirms that the introduction of voluntary disclosure increases tax evasion. ...
Some areas seemed too simplistic-for example, telephone communication whereas other areas new to me and very useful-for example, breaking bad news, gathering information for history taking, presenting info. Excellent resources and management of sessions throughout course. Have picked up lots of useful information. Much better with actors-not comfortable with role plays with colleagues, even though I interact so well with them (very unrealistic). This I found difficult, felt uncomfortable and as though I was acting in a way I wouldnt normally. This didnt happen with the actors scenarios. (1). Early sessions I felt did not always recognise your previous experience in communication; however, this improved as we progressed through the modules. Teaching methods I found good and very thorough leaving us with valuable resources to refer to in our future practice. Role plays not so good among ourselves but excellent with actors/actresses. Learnt a lot about my communication skills. Always better ...
My son Teddy was prenatally diagnosed with Down Syndrome at the Fetal Diagnostic Center at University of Michigan Von Voigtlander Womens Hospital. At the time, I felt like I was really supported - people were really helpful and I was well taken care of. I felt like at the Fetal Diagnostic Center I was given the space to make a decision on my own about what decision was right for my family. But as I got more involved in the Down Syndrome community, I found that not everyone was getting that level of support when they are given this diagnosis. So I became interested in helping shape how this diagnosis is given so its not just presented as a list of potential problems. I wanted to help shape that conversation a little bit more.. What are some ways youve been able to do that?. Mostly I tell my story - Teddys story, our story of what happened for us. For the past 3 years I have been speaking to the medical students entering their 2nd year of med school in a session called Breaking bad news. ...

The security of information technology and computer networks is effected by a wide variety of actors and processes which together make up a security ecosystem; here we examine this ecosystem, consolidating many as- pects of security that have hitherto been discussed only separately. First, we analyze the roles of the major actors within this ecosystem and the processes they participate in, and the the paths vulnerability data take through the ecosystem and the impact of each of these on security risk. Then, based on a quantitative examination of 27,000 vulnerabilities disclosed over the past decade and taken from publicly available data sources, we quantify the systematic gap between exploit and patch availability. We provide the first examination of the impact and the risks associated with this gap on the ecosystem as a whole. Our analysis provides a metric for the success of the responsible disclosure process. We measure the prevalence of the commercial markets for vulnerability information ...

Six volunteers were told to shoot a toy gun and then lie, saying that they hadnt done so, while three more volunteers told the truth about firing the gun. One person dropped out of the study.. The researchers used functional magnetic resonance imaging (fMRI) to study real time images of the subjects brain activity while they made their testimony. The scans showed a total of seven areas of activation in the deception (group), Dr Scott Faro, director of the Functional Brain Imaging Center at Temple University School of Medicine in Philadelphia said. His team found four areas of activity in the truth-telling arm.. The study found that lying takes more brain effort than telling the truth, Reuters reports.. Lying causes activity in the frontal part of the brain, including the medial inferior and pre-central areas, as well as the hippocampus and middle ...

I am reading a book right now and one story in the book from another woman with Fibro was her sharing her story and how she was learning to live well.So after several doctor visits for a year or so she finally got the diagnosis of Fibro ( and we certainly know how that is a nightmare). But her doctor said The good news is its not terminal, you wont die from it but the bad news is you will feel like crap the rest of your life!!!!! Isnt that the truth! So I thought of a few other funny or inspiring good and bad news to add to that. ...
Mandatory disclosure reporting is new and it is important that companies report at the appropriate level of granularity, and that governments make these disclosures easily accessible and usable for the public. The best practice for presenting mandatory disclosure data is to provide machine-readable files as is found on the UK site. This allows stakeholders to skip the steps of scraping and cleaning the data as was necessary with the Total report, which was filed under Frances transposition of the EU Directives ...
Full disclosure - the practice of making the details of security vulnerabilities public - is a damned good idea. Public scrutiny is the only reliable way to improve security, while secrecy only makes us less secure.. Unfortunately, secrecy sounds like a good idea. Keeping software vulnerabilities secret, the argument goes, keeps them out of the hands of the hackers (See The Vulnerability Disclosure Game: Are We More Secure?). The problem, according to this position, is less the vulnerability itself and more the information about the vulnerability.. But that assumes that hackers cant discover vulnerabilities on their own, and that software companies will spend time and money fixing secret vulnerabilities. Both of those assumptions are false. Hackers have proven to be quite adept at discovering secret vulnerabilities, and full disclosure is the only reason vendors routinely patch their systems.. To understand why the second assumption isnt true, you need to understand the underlying economics. ...
Insurance customers are legally required to provide truthful and complete personal disclosures, so that insurance companies can calculate fair premiums. Any redacted disclosures or misrepresented disclosures not only detract from the usefulness of this data for risk classification but also expose the customer to serious legal remedies, even the cancellation of the insurance contract when…
Downloadable! We propose a sequential auction mechanism for a single object in which the seller jointly determines the allocation and the disclosure policy. A sequential disclosure rule is shown to implement an ascending price auction in which each losing bidder learns his true valuation, but the winning bidders information is truncated from below. As the auction ends, the winning bidder only has limited information, namely that his valuation is sufficiently high to win the auction. The sequential mechanism implements the allocation of the handicap auction of Esö and Szentes [10] but strengthens the participation constraints of the bidders from interim to posterior constraints. Due to the limited disclosure of information, the participation constraints (and incentive constraints) of all the bidders are satisfied with respect to all information revealed by the mechanism. In the special case in which the bidders have no private information initially, the seller can extract the entire surplus.(This

Gosling, in no uncertain terms, overshadows and owns this film completely. He lives and breathes as The Driver, embracing the identity with an incredibly convincing performance that is guaranteed to command your attention and keep you glued to the screen. There are numerous scenes where he conveys the turmoil of the inner workings of the characters through his eye movements alone. And the gorgeous Carey Mulligan is also stellar in this film. Known to Doctor Who fans as Sally Sparrow from the epic Steven Moffat-penned Blink, Mulligan again continues to surprise and entrance the audience, with another absorbing performance in which even simple gestures tell volumes of stories about her character. Mulligan is always a treat to view in film, and her presence in Drive is reason alone to see this movie in my mind. Bryan Cranston puts in an admirable role as well, in a character which is kind of like what if Walter White from Breaking Bad was actually Breaking Bad Luck? His demeanor also provides a ...
I thought the episode as a whole was absolutely brilliant, and I appreciate the symbolism of the final scene. However, I do find myself having a small problem with the implications. Obviously, the point, as was stated by you Alan, and Gillians interview, is to show how Walts actions have led to a quite literal catastrophe. My problem is this: even though Walt is to blame for Janes death, the fact is that as an addict, she very well could have passed away even without Walts intervention. And if her father was too consumed with grief, he had a responsibility to take time off from work, especially in a line of employment in which peoples lives are at stake. Moreover, his supervisors had a responsibility to advice him to take some time off. I just dont think its fair to place all the blame on Walt, nor is it the intention of the writers, of course, but I really feel like this wasnt an inevitable outcome set in motion by Walts actions. Jane was in danger of dying regardless of whether or not ...
A happy Walt climbs into bed with Skyler, who doesnt want anything to do with him. He kisses her and acts romantic, explaining they do what they do for good reasons, and theres no better reason for family. Its clear Walt doesnt believe that anymore, but that doesnt mean it isnt true for others. Marie did as much as she could to help Hank recover; Jesse was great to Brock, despite him not being family. Walt is so blind that he thinks hes still acting in the familys best interests, when really, he cant do any more than serve them breakfast ...
Rubicon received generally favorable reviews. On Rotten Tomatoes, it received a 69% approval rating with an average score of 7.8 out of 10 based on 26 reviews and a critical consensus of, Rubicon creates an intellectual puzzle for the mind that is pieced together with skillful writing, acting, and carefully-crafted storytelling.[25] On Metacritic, it scored 69 out of 100 based on 28 critic reviews.[26] Most of the critics praised the shows cast and atmosphere, but many have criticized the lack of action and the slow development of the central mystery.[27][28][29] The show has often been compared to AMCs other shows, Mad Men and Breaking Bad, because of their success and originality, Entertainment Weeklys Ken Tucker ended his review with such a comparison saying Rubicon doesnt have the glossy panache of Mad Men or the in-your-face confrontations of Breaking Bad, but I think thats a good thing. It establishes Rubicon as its own distinct creation from AMC.[30] About the lack of action on ...

Something that has bugged the Rude Pundit lately about the TV show no one can shut the fuck up about (including the Rude Pundit), Breaking Bad, is not really about Breaking Bad itself but about the way in which people react to the show and, especially, the protagonist, Walter White, the chemistry teacher turned meth-making kingpin turned retired drug kingpin turned on-the-lam outlaw. Too many analyses of the show argue over whether or not Walt is an antihero (a protagonist who we root for even though he has awful qualities) or a villain. Indeed, for many, they just accept that hes an antihero and justify his actions. And even when someone is asserting that hes not one, that hes become the villain, its done by telling you about all the terrible events weve seen on the show: the murder of Gale, the manipulation of Jesse, the lying to his family, the inaction while Jane died, the poisoning of Brock ...

Popular culture makes gratuitous use of powerful lie-repelling agents known as Truth Serums. They are usually depicted as injected drugs which strongly inhibit a subjects ability to lie, causing him or her to mechanically recite the truth to an interviewer upon questioning.. Such drugs have been utilized by some of the three-letter government agencies in the not-so-distant past (CIA, FBI, DOD, KGB, etc.), particularly during the rampant paranoia of the Cold War. And in the aftermath of 9/11, there was some discussion on the idea of bringing them back into use for interrogation. But are these truth serums effective? Do they produce any useful results? The short answer is, no. The long answer is Noooooooooooo!, preferably while running in slow-motion.. Many barbiturates fall under the truth serum category, including scopolamine, sodium amytal, and Sodium Pentothal. Scopolamine was tested in the 1950s as a truth serum in project MKULTRA, and is now infamous as a date-rape drug due to its ...
Every man who arrives to truth does so by a different path. How you have come to understand the real nature of the world is different from how I did, but there are certain behaviors and actions which have a greater tendency to truth discovery. Here are five:. 1. Being a player who sleeps with a large quantity of women. The actual sex act of thrusting inside a woman does little in showing you truth, since it is nothing more than an animalistic act that takes little higher thought, but being able to fornicate with different women means you are required to know the true nature of women. Understanding the true nature of women therefore allows you to understand the true nature of men. You now possess truth of both sexes and, effectively, human nature, including those facts that are being concealed by progressive ideologies. Every notch you accumulate has the effect of splashing cold truth onto your face, however unpleasant it may be.. As an added benefit, being summarily rejected so many times ...
