Trimetaphan is a sulfonium compound and therefore carries a positive charge. Being charged, it cannot cross lipid cell membranes, such as those that comprise the blood-brain barrier. Due to this, trimethaphan does not have any effect on the central nervous system. The ciliary muscle of the eye functions to round the lens for accommodation and is controlled mainly by parasympathetic system input. With administration of a ganglion-blocking drug, the ciliary muscle cannot contract (cycloplegia) and the patient loses the ability to focus their eyes. Trimetaphan has a strong effect on the cardiovascular system. The size of blood vessels is primarily controlled by the sympathetic nervous system. Loss of sympathetic system input to the blood vessels causes them to get larger (vasodilation) which has the effect of lowering blood pressure. Postural hypotension is a common side effect of such drugs. Trimethaphan causes a histamine release which further lowers blood pressure. Effects on the heart include a ...

Autoregulation of cerebral blood flow (CBF) was studied by the arteriovenous oxygen difference method in 13 patients with untreated or ineffectively treated severe hypertension, nine patients with effectively treated, formerly severe hypertension, and ten normotensive controls. Resting mean blood pressure in these three groups was 145 +/- 17 (1 SD) mm Hg, 116 +/- 18 mm Hg, and 98 +/- 10 mm Hg, respectively. Blood pressure was decreased by trimethaphan infusion combined with head-up tilt. The lower limit of CBF autoregulation in the three groups was 113 +/- 17 mm Hg, 96 +/- 17 mm Hg, and 73 +/- 9 mm Hg, and the lowest tolerated blood pressure where mild symptoms of brain hypoperfusion were encountered was 65 +/- 10 mm Hg, 53 +/- 18 mm Hg, and 43 +/- 8 mm Hg. These pressures were all significantly higher (P less than 0.01) in the group of untreated or ineffectively treated hypertensive patients than in the normotensive group demonstrating a shift of CBF autoregulation in the former. The ...
We investigated the distinct ability of various antihypertensive drugs to modulate the extent (%) of rapid (15 minutes) resetting of the baroreceptors of normotensive rats to hypotensive levels. In one protocol, hemorrhage produced a complete resetting to hypotension in rats chronically treated (6 days) with captopril. Also, hemorrhage produced only partial resetting in rats acutely treated (10-15 minutes before baroreceptor recording) with captopril and in control (untreated) rats (73 +/- 7% and 49 +/- 5%, respectively). In another protocol, all vasodilators produced hypotension in normotensive rats. Nifedipine produced complete (93 +/- 4%) resetting to hypotension, whereas prazosin produced near-maximal (83 +/- 3%) resetting. The remaining drugs studied (phenoxybenzamine, trimethaphan, and MgSO4) induced a partial resetting (63 +/- 7%, 63 +/- 9%, and 50 +/- 5%, respectively) that did not differ significantly from the extent observed with hemorrhage in control (untreated) rats. These results ...

In Specific Aim 1, the investigators will use state-of-the-art measurements of sympathetic activity (autonomic function tests, response to trimethaphan, direct nerve sympathetic traffic recordings with microneurography, plasma norepinephrine, and intraneuronal metabolites), inflammatory mediators (C-reactive protein, inflammatory cytokines), and oxidative stress (isoprostanes) in patients with CFS-P. It is important that appropriate control groups be included, and we will also study patients with CFS without orthostatic tachycardia, patients with POTS without CFS, and normal controls.. The investigators have documented abnormalities in volume regulation in POTS patients. Hypovolemia can contribute to sympathetic activation and, vice versa, sympathetic activation can contribute to hypovolemia. Interrupting this vicious circle with acute saline infusion is the most effective treatment to improve symptoms in POTS patients. Not surprisingly, many POTS patients followed by the investigators, and CFS ...
Lanopepden, also known as GSK-1322322 or GSK-322, is a potent and selective peptide deformylase inhibitor with good in vitro activity against bacteria associated with community-acquired pneumonia and skin infections. GSK1322322 had bactericidal activity against S. pneumoniae, H. influenzae, S. pyogenes, and S. aureus, demonstrating a ≥ 3-log(10) decrease in the number of CFU/ml at 4× MIC within 24 h in 29 of the 33 strains tested. GSK1322322 represents a valuable alternative therapy for the treatment of infectious diseases caused by drug-resistant pathogens.
Amlodipine is used in the treatment of hypertension and myocardial ischemia caused by angina. It is marketed in the form of a compound with various salts such as amlodipine besylate, amlodipine maleate, amlodipine camsylate, and amlodipine orotate. There is also a form of the enantiomeric S-isomer of amlodipine called S-amlodipine. Among these, amlodipine besylate can be used for the prevention of angina pectoris or coronary revascularization in patients with no heart failure or less than 40% cardiac output among patients whose cardiovascular disease was confirmed by angiography. In addition, amlodipine is used not only as a single drug, but also as a combination drug with other blood pressure drugs to effectively control blood pressure while minimizing side effects, and it is also used with hyperlipidemia treatments to treat hyperlipidemia, which is likely to occur in patients with heart disease.. ...

In the present study, we sought to identify if a vascular amplifier was contributing to the heightened depressor responses to ganglionic blockade and the increased LF oscillations in SBP that we have previously reported in the hypertensive LPK model of CKD (Phillips et al. 2007; Harrison et al. 2010; Hildreth et al. 2013). Our findings indicate that sympathetic overactivity prevails over any presence of a vascular amplifier in this model.. In the LPK rats, the normalized depressor response to ganglionic blockade was greater than that seen in the Lewis controls for either of the direct acting vasodilators: SNP or adenosine, indicating that in this model of CKD, the enhanced depressor response is reflective of increased sympathetic vasomotor tone. This was evident when responses were assessed for both MAP and SBP. Our data, however, do not rule out a role of the vasculature in contributing to the maintained hypertension in this model, highlighted in particular by the finding that following ...

Mecamylamine (3-methylaminoisocamphane hydrochloride) is a nicotinic parasympathetic ganglionic blocker, originally utilized like a therapeutic agent to take care of hypertension. than those utilized to take care of hypertension, which diminishes the likelihood of peripheral side-effects. This review targets the pharmacological properties of mecamylamine, the differential ramifications of its stereoisomers, S(+)- and R(?)-mecamylamine, as well as the potential for performance in treating CNS disorders, including nicotine and alcoholic beverages addiction, feeling disorders, cognitive impairment and interest deficit hyperactivity disorder. oocytes expressing either rat or human being nAChR subtypes, possess shown that mecamylamine inhibits both neuromuscular and neuronal nAChRs (Chavez-Noriega et al., GNF-5 supplier 1997; Francis and Papke, 1996; Luetje and Patrick, 1991; Papke et al., 2008), offering additional proof its non-selective nAChR antagonist actions. Senecioylcholine (β,β-dimethylacrylylcholine, SCh) was synthesized from choline bromide and senecioyl chloride.. Senecioylcholine was found to be a ganglion stimulant. A number of its actions are described: 1. SCh produced a pressor response in the absence of atropine. This effect was partially blocked by adrenalectomy and almost completely abolished by ganglionic blockade. 2. The pressor response to SCh was reversed by adrenergic blockade. 3. SCh produced a contraction of the nictitating membrane. This action was antagonized by ganglionic blockade. 4. SCh produced contractions of the isolated frog rectus abdominis.. The results are discussed in terms of known structure-activity relationships.. ...

Noninvasive stellate ganglion block application using light irradiation (SG-LI) can be used as an alternative to conventional injection blockades. Based on the variety of application protocols among previous studies, it was needed to further identify the clinical efficacy of SG-LI in managing neuropathic pain or other disorders associated with sympathetic hyperactivity. DESIGN: A comprehensive search of online databases was performed to identify experimental or observational studies reporting the efficacy of SG-LI in treating patients with disorders requiring sympatholytic management. The included ...

A ganglionic blocker (or ganglioplegic) is a type of medication that inhibits transmission between preganglionic and postganglionic neurons in the Autonomic Nervous System, often by acting as a nicotinic receptor antagonist. Nicotinic acetylcholine receptors are found on skeletal muscle, but also within the route of transmission for the parasympathetic and sympathetic nervous system (which together comprise the autonomic nervous system). More specifically, nicotinic receptors are found within the ganglia of the autonomic nervous system, allowing outgoing signals to be transmitted from the presynaptic to the postsynaptic cells. Thus, for example, blocking nicotinic acetylcholine receptors blocks both sympathetic (excitatory) and parasympathetic (calming) stimulation of the heart. The nicotinic antagonist hexamethonium, for example, does this by blocking the transmission of outgoing signals across the autonomic ganglia at the postsynaptic nicotinic acetylcholine receptor. Because ganglionic ...
In the mammalian brain, nitric oxide (NO) is responsible for a vasodilatory tonus as well as the elevation of cerebral blood flow (CBF) induced by hypercapnia. There have been few comparative studies of cerebral vasoregulation in lower vertebrates. Using epi-illumination microscopy in vivo to observe CBF velocity on the brain surface (cerebral cortex), we show that turtles (Trachemys scripta) exposed to hypercapnia (inspired PCO2 = 4.9 kPa) displayed a 62% increase in CBF velocity, while systemic blood pressure remains constant. Exposing turtles to a PCO2 of 14.9 kPa caused an additional increase in CBF velocity, to 104% above control values, as well as a 30% increase in systemic blood pressure. The elevated CBF velocity during hypercapnia could not be blocked by a systemic injection of the NO synthase (NOS) inhibitor NG-nitro-L-arginine (L-NA). However, L-NA injection caused a temporary stop in CBF as well as a persistent increase in systemic blood pressure, suggesting that there is a NO tonus ...
