CDC has also focused education efforts on travelers and provides a comprehensive website devoted to travelers health (www.cdc.gov/travel). Nevertheless, many travelers do not access health resources prior to departure.6,7 In this study, we surveyed 1,254 international travelers departing from a major US airport, to identify barriers to the pursuit of health information and to understand which, if any, sources of health information were being utilized by travelers to high-risk countries.. We investigated the effect of international travel on the gut resistome of 122 healthy travelers from the Netherlands by using a targeted metagenomic approach. Our results confirm high acquisition rates of the extended-spectrum β-lactamase encoding gene blaCTX-M, documenting a rise in prevalence from 9.0% before travel to 33.6% after travel (p<0.001). The prevalence of quinolone resistance encoding genes qnrB and qnrS increased from 6.6% and 8.2% before travel to 36.9% and 55.7% after travel, respectively (both p<0.001). Travel to Southeast Asia and the Indian subcontinent was associated with the highest acquisition rates of qnrS and both blaCTX-M and qnrS, respectively. Investigation of the associations between the acquisitions of the blaCTX-M and qnr genes showed that acquisition of a blaCTX-M gene was not associated with that of a qnrB (p = 0.305) or qnrS (p = 0.080) gene. These findings support the increasing evidence that travelers contribute to the spread of antimicrobial drug
To understand geographic variation in travel-related illness acquired in distinct African regions, we used the GeoSentinel Surveillance Network database to analyze records for 16,893 ill travelers returning from Africa over a 14-year period. Travelers to northern Africa most commonly reported gastrointestinal illnesses and dog bites. Febrile illnesses were more common in travelers returning from sub-Saharan countries. Eleven travelers died, 9 of malaria; these deaths occurred mainly among male business travelers to sub-Saharan Africa. The profile of illness varied substantially by region: malaria predominated in travelers returning from Central and Western Africa; schistosomiasis, strongyloidiasis, and dengue from Eastern and Western Africa; and loaisis from Central Africa. There were few reports of vaccine-preventable infections, HIV infection, and tuberculosis. Geographic profiling of illness acquired during travel to Africa guides targeted pretravel advice, expedites diagnosis in ill returning

Global travelers should make pretravel consultation appointments several weeks to months before traveling to allow time for adequate immunizations.1 The pretravel consultation should be performed in a structured and standardized manner (Table e124-1).1,3,4,9 There should be an assessment of the travelers health and pertinent medical history, including a thorough medical history, including travel history, medications, vaccinations, and allergies. Next there should be an assessment of the travelers risk, including discussion about the destinations, itinerary, accommodations, and planned activities. The consultant should have access to up-to-date travel health references on the travel destination. The consultant should provide preventative advice. Principal discussion points (depending on destination) may include vaccine-preventable illness, avoidance of insects, malaria prophylaxis (if applicable), prevention and self-treatment of travelers diarrhea (including food and drink safety), ...

A recent measles outbreak in New Hampshire demonstrates that this recommendation is not adequate.. The authors present the current state of knowledge about the long term sequelae of the common travel-related infections associated with immunological dysregulation. Travellers diarrhoea, chikungunya and dengue are common in travellers and protracted manifestations such as post-infectious irritable bowel syndrome (travellers diarrhoea), arthritis (chikungunya) or post-dengue syndrome may be more prevalent than previously thought. Attention should be paid to active prevention of the infections during travel, especially when an effective treatment against long-term sequelae has not been established. Further studies should be designed in order to determine the interactions between the host, the environment and the pathogens, as well as to identify the prognostic factors for developing long-term manifestations after infections in a large group of travellers. ...

International travel can be fun and adventurous. But these days, its important to check travel health advisories before you travel.
Travelers diarrhea is the most common health impairment in persons visiting developing countries, affecting 20% to µ50% of tourists. Although it is usually benign, travelers diarrhea represents a considerable socioeconomic burden for both the traveler and the host country. The most common enteropathogens are enterotoxigenic and enteroaggregative Escherichia coli. Travelers compliance with dietary precautionary measures is poor. Despite the excellent protection rates provided by antibiotics, routine administration of prophylaxis is currently not recommended because of potential adverse reactions. Of the various antibiotics that have been tested, quinolones are considered to be the first choice worldwide; however, quinolone-resistant pathogens are increasingly being isolated. Because it is frequently administered and provides only moderate protection, bismuth subsalicylate is not considered a recommendable option for prophylaxis in Europe, where it is rarely available anyhow. To date, no ...
Dr. Amy Wesolowski, an assistant professor in the department of epidemiology at Johns Hopkins Bloomberg School of Public Health, received a $1.5 million, five-year grant from the National Institutes of Health to study how human travel patterns directly relate to the spread of infectious disease.. Human population dynamics underlie infectious disease transmission. The movement of infected individuals can introduce pathogens into susceptible populations driving disease outbreaks. However, there is no systematic model for human movement dynamics and existing models are based on the understanding of travel in high-income countries. The proposed research uses innovative sources of big data from mobile phone call records to quantify mobility patterns of millions of individuals.. This $1.5 million grant will allow Dr. Wesolowski to focus on developing a data-driven framework to model human mobility validated across multiple countries. Human travel fuels the size and spread of disease outbreaks, ...

BACKGROUND: Travellers diarrhoea is among the most frequently reported problems for travellers. Prevention remains a challenge in travellers and the appropriate management of travellers diarrhoea remains paramount.. OBJECTIVE: This article provides an overview of the general approach to diarrhoea in the returned traveller, including identification of common causes and management.. DISCUSSION: The most common causes of travellers diarrhoea are bacterial contamination of food and water, particularly with enterotoxigenic Escherichia coli. Most cases of travellers diarrhoea are mild, are of short duration and do not require antibiotic treatment. Where the diarrhoea is severe, bloody and/or prolonged, then laboratory investigation is necessary. Where the patient is severely ill and possibly septicaemic, blood culture is mandatory. Presently, one of the most useful groups of the antibiotics routinely available for treatment is the fluoroquinolones. ...
