Three hundred-ninety presentations were evaluated, of which 215 (55%) were ultimately published in a peer-reviewed journal within five years of the presentation date. The mean time from presentation to publication was 16 months (SD 25 months). There was no significant difference in the distribution of the level of evidence between studies that were ultimately published, versus those that were not published (n.s.). The level of evidence of the published study was not a significant predictor of the impact factor of the journal in which it was published (n.s.). Presentations were most commonly published in Knee Surgery, Sports Traumatology, Arthroscopy (24%) and The American Journal of Sports Medicine (22%).. CONCLUSIONS ...

The term Occult Pneumothorax (OPTX), describes a pneumothorax (PTX) that while not suspected on the basis of either clinical examination or plain radiograph, is ultimately detected with thoraco-abdominal computed tomograms (CT). This situation is increasingly common in contemporary trauma care with the increased use of CT. The incidence appears to approximately 5% in injured populations presenting to hospital, with CT revealing at least twice as many PTXs as suspected on plain radiographs. While PTXs are a common and treatable (through chest drainage) cause of mortality and morbidity, there is clinical equipoise and significant disagreement regarding the appropriate treatment of the OPTX. Based on level III evidence, some authors have recommended observation without chest drainage for all but the largest OPTXs, recommendations that contravene the standard dictum for ventilated patients as recommended by the Advanced Trauma Life Support Course of the American College of Surgeons. The ...

Advanced trauma life support (commonly abbreviated ATLS) is a training program for medical providers in the management of acute trauma cases, developed by the American College of Surgeons. Similar programs exist for immediate care providers such as paramedics. The program has been adopted worldwide in over 60 countries, sometimes under the name of Early Management of Severe Trauma, especially outside North America. Its goal is to teach a simplified and standardized approach to trauma patients. Originally designed for emergency situations where only one doctor and one nurse are present, ATLS is now widely accepted as the standard of care for initial assessment and treatment in trauma centers. The premise of the ATLS program is to treat the greatest threat to life first. It also advocates that the lack of a definitive diagnosis and a detailed history should not slow the application of indicated treatment for life-threatening injury, with the most time-critical interventions performed early. ...

Femoral neck fractures remain a difficult clinical problem for orthopaedic surgeons. Attempting to conserve the femoral head often leads to healing complications, while the more predictable prosthetic replacements are associated with poorer function and significant complications [1]. The treatment of these fractures depends on the age of the patient, fracture displacement, bone quality, timing of surgery and activity level of the patient. Displaced fractures in healthy, active patients are best treated by reduction and internal fixation. A good anatomic reduction is mandatory. Femoral neck fractures reduced anatomically are best fixed with three pins or screws [2]. There are however, complications unique to femoral neck fractures which are almost impossible to predict. Non-union and avascular necrosis are amongst the most prominent.. Non-union usually can definitely diagnosed within a year of fracture fixation with the same being achieved within 3 months at times [3]. After non-union has been ...

The Trauma care focus on care delivered at fixed facilities like clinics and hospitals in which unlike most emergency room patients, the trauma patients require highly specialized care, including surgery and blood transfusions. Prehospital emergency care systems addresses prehospital trauma care at the scene and in route to fixed facilities. There are different levels of trauma care that refer to the kinds of resources available in a trauma center. These are the categories defining national standards for trauma care hospitals. A Level I Trauma Center is capable of providing total care for every injury aspect from prevention through physical medicine and rehabilitation. Level II Trauma Centre initiates definitive care for all injured patients. A Level III Trauma Center provides prompt assessment, resuscitation, intensive care, surgery, and stabilization of injured patients and emergency operations. A Level IV Trauma Center has demonstrated to provide advanced trauma life support and A Level V ...
Orthopedic trauma may be a severe injury within the part of musculoskeletal system, it usually the results of an abrupt accidents requiring immediate medical attention. And not all orthopedic traumas is life-threatening, it is life altering. Physicians of orthopedic trauma are unique and they were specializing in complex injuries tobones soft joints (like muscles, tendons, and ligaments) and joints throughout the entire body. Mostly orthopedic specialists specialize in just one body part. Others they may provide more general care but they wont treat more acute fractures, which are more difficult to fix. Orthopedic trauma physicians receive the training in the field of orthopedic surgery with a special focus on the treatment of joint realignment to promote the safety recovery and return of functionality to the injured body parts so, they often treat patients with compound fractures ,multiple broken bones and fractures near a joint (like a hip or knee).. The beneficial effect of epinephrine during resuscitation from out-of-hospital cardiac arrest (OHCA) has been inconclusive, and potential harm has been suggested, particularly in trauma victims. Although no significant improvement in neurological outcomes has been found among resuscitated patients using epinephrine, including trauma patients, the use of epinephrine is recommended in the Advanced Trauma Life Support protocol. Given that the use of vasopressors was reported to be associated with increased mortality in patients with massive bleeding, the undesirable effects of epinephrine during the resuscitation of traumatic OHCA should be elucidated. We hypothesised that resuscitation with epinephrine would increase mortality in patients with OHCA following trauma. This study is a post-hoc analysis of a prospective, multicentre, observational study on patients with OHCA between January 2012 and March 2013. We included adult patients with traumatic OHCA who were aged ≥15 years and excluded those with
Background: It has been common practice in trauma to place patients in cervical collars and on long backboards (LBBs) to achieve spinal immobilization. LBBs are used to help prevent spinal movement and facilitate extrication of patients. Cervical collars (C-Collars) are used to help prevent movement of the cervical spine and often are combined with lateral head blocks and straps. The theory behind this is that spine immobilization prevents secondary spinal cord injury during extrication, transport, and evaluation of trauma patients by minimizing movement. Most of this information has been passed on from historical teachings, like the Advanced Trauma Life Support (ATLS) courses, and not from scientific research. To date there has been no high-quality evidence that use of spinal immobilization improves patient outcomes. In this post, we will review the evidence associated with spinal immobilization in trauma patients....Read More ...
BLUNT TRAUMA ABDOMEN Blunt abdominal trauma initially is evaluated by FAST examination in most major trauma centers, and this has largely supplanted DPL .FAST is not 100% sensitive, however, so diagnostic peritoneal aspiration is still advocated in hemodynamically unstable patients without a defined source of blood loss to rule out abdominal hemorrhage FAST is used…
Discussion Use of standardised protocols for massive transfusion has been shown to lead directly to a significant reduction in patient mortality. This improvement in outcome has been largely attributed to increased use of FFP10,30 instead of crystalloid intravenous fluids. The standardised approach to resuscitation as taught by the Advanced Trauma Life Support (ATLS) course for adults with significant bleeding, advocates use of 1-2 litres of isotonic crystalloids for initial volume resuscitation.24 Some authors feel that this approach is only appropriate in trauma patients not requiring a massive blood transfusion.22 Infusion of large of amounts of crystalloids not only precipitates the dilutional coagulopathy, but also has pro-inflammatory effects34 as well as increasing the risk of subsequent infection.35,36 Conversely, use of smaller amounts of crystalloids and larger amounts of fresh frozen plasma in the initial resuscitation period is associated with improved 24hour and 30-day ...
