The term Occult Pneumothorax (OPTX), describes a pneumothorax (PTX) that while not suspected on the basis of either clinical examination or plain radiograph, is ultimately detected with thoraco-abdominal computed tomograms (CT). This situation is increasingly common in contemporary trauma care with the increased use of CT. The incidence appears to approximately 5% in injured populations presenting to hospital, with CT revealing at least twice as many PTXs as suspected on plain radiographs. While PTXs are a common and treatable (through chest drainage) cause of mortality and morbidity, there is clinical equipoise and significant disagreement regarding the appropriate treatment of the OPTX. Based on level III evidence, some authors have recommended observation without chest drainage for all but the largest OPTXs, recommendations that contravene the standard dictum for ventilated patients as recommended by the Advanced Trauma Life Support Course of the American College of Surgeons. Professor Luk graduated from the University of Hong Kong in 1977 and received his orthopaedic training in Hong Kong and the United Kingdom. He obtained his FRCS in 1981, MCh Ortho from the University of Liverpool in 1984, FRACS in 1985 and is a founding fellow of the Hong Kong College of Orthopaedic Surgeons. At present he holds the Tam Sai Kit Chair in Spine Surgery, and is Professor and Chief of the Spine Division at the Department of Orthopaedics and Traumatology at the University of Hong Kong.. Professor Luk was the Head of the Department of Orthopaedics and Traumatology, the University of Hong Kong from 2003 until 2012, Honorary Hospital Chief Executive of the Duchess of Kent Childrens Hospital from 1992 until 2002. Femoral neck fractures remain a difficult clinical problem for orthopaedic surgeons. Attempting to conserve the femoral head often leads to healing complications, while the more predictable prosthetic replacements are associated with poorer function and significant complications [1]. The treatment of these fractures depends on the age of the patient, fracture displacement, bone quality, timing of surgery and activity level of the patient. Displaced fractures in healthy, active patients are best treated by reduction and internal fixation. A good anatomic reduction is mandatory. Femoral neck fractures reduced anatomically are best fixed with three pins or screws [2]. There are however, complications unique to femoral neck fractures which are almost impossible to predict. Non-union and avascular necrosis are amongst the most prominent.. The study set out to evaluate the efficacy of two antibiotic prophylaxis regimens in patients with facial fractures admitted to the Oral and Maxillofacial Surgery and Traumatology services of the Onofre Lopes University Hospital attached to the Federal University of Rio Grande do Norte.This clinical study was prospective, randomized and controlled. Patients underwent surgery for facial fracture reduction and fixation under general anesthetic. Prior to surgery, patients were randomly divided into two groups. Group 1 patients each received 2g of Cefazolin (Cefazolin Sodium - Ampoule- 1g - Genéricos Brasil), administered intravenously but none was administered in the post-operative period. Group II patients received the same dose prior to the operation but in the post-operative period they also received 4 additional 1 g doses of Cefazolin intravenously, completing a 24 period of antibiotic prophylaxis. The beneficial effect of epinephrine during resuscitation from out-of-hospital cardiac arrest (OHCA) has been inconclusive, and potential harm has been suggested, particularly in trauma victims. Although no significant improvement in neurological outcomes has been found among resuscitated patients using epinephrine, including trauma patients, the use of epinephrine is recommended in the Advanced Trauma Life Support protocol. Given that the use of vasopressors was reported to be associated with increased mortality in patients with massive bleeding, the undesirable effects of epinephrine during the resuscitation of traumatic OHCA should be elucidated. We hypothesised that resuscitation with epinephrine would increase mortality in patients with OHCA following trauma. This study is a post-hoc analysis of a prospective, multicentre, observational study on patients with OHCA between January 2012 and March 2013. Background: It has been common practice in trauma to place patients in cervical collars and on long backboards (LBBs) to achieve spinal immobilization. LBBs are used to help prevent spinal movement and facilitate extrication of patients. Cervical collars (C-Collars) are used to help prevent movement of the cervical spine and often are combined with lateral head blocks and straps. The theory behind this is that spine immobilization prevents secondary spinal cord injury during extrication, transport, and evaluation of trauma patients by minimizing movement. Most of this information has been passed on from historical teachings, like the Advanced Trauma Life Support (ATLS) courses, and not from scientific research. This improvement in outcome has been largely attributed to increased use of FFP10,30 instead of crystalloid intravenous fluids.
