This is a phase II randomized, double-blind, placebo controlled multi-center clinical trial involving a device, Transcutaneous Electrical Nerve Stimulation (TENS). TENS is a non-pharmacological agent which delivers electrical stimulation by a battery operated device via electrodes placed on the skin. TENS is considered to be a safe, inexpensive and non-invasive modality used to treat a variety of acute and chronic pain conditions. The initial phase of the study will randomly allocate subjects to receive active TENS, placebo TENS or standard care (No TENS). After participating in the 1 month random assignment, all subjects will receive active TENS for 1 month. The subjects will make 4 visits to the clinic approximately 2 to 3 1/2 hours each visit. The pain reducing effect of vibratory stimulation at 100 Hz, transcutaneous electrical nerve stimulation (TENS) at 100 Hz, electroacupuncture at 2 Hz as compared to placebo was studied in 36 patients suffering from myalgia. In 29 patients these modes of stimulation produced pain reduction. Out of these patients 18 rated vibratory stimulation, TENS or electroacupuncture as more effective than placebo while 6 patients rated placebo as more effective. Five patients rated the pain suppressive effect of vibratory stimulation, TENS and electroacupuncture as being equal to that of placebo. Four of the patients who experienced pain reduction with peripheral stimulation reported an effect only for one particular type of peripheral stimulation. Objectives. Newly developed bidirectional modulated sine waves (BMW) might provide some derived benefit to patients with low back pain. Pain relief by transcutaneous electric nerve stimulation (TENS) with BMWs was tested. Materials and Methods. Analgesic effects of BMWs and conventional bidirectional pulsed waves on chronic back pain in 28 patients were compared, and effects of repeated TENS using BMWs on chronic back pain were investigated in 21 patients by means of a randomized double-blind, sham-controlled, parallel-group method. Pain intensity was assessed using numerical rating scale (NRS). Results. There was significant immediate reduction in NRS in patients receiving BMWs, and 60 min after treatment compared to sham TENS. Weekly repeated treatments using massage and TENS with BMWs for 5 weeks resulted in a decrease of NRS, but there were no significant differences between the TENS plus massage and sham TENS plus massage groups.
