To our knowledge, this is the first multi-institutional study in children with tracheotomy reporting differences in clinical outcomes and hospital resource use based on the childs comorbid clinical conditions. Children with upper airway anomaly experienced lower in-hospital mortality rates at tracheotomy placement. Children with NI experienced both higher in-hospital mortality rates and greater hospital resource use 5 years after tracheotomy. These findings remained significant after controlling for important cofactors including ventilation support, use of postacute care facilities after hospital discharge, age, and gender.. This is the also the first study to document the substantial hospital use of children 5-years after tracheotomy. With an average length of stay exceeding 8 days, admissions for children with tracheotomy rank among the longest hospitalizations experienced by all children, including those with severe sepsis, Kawasaki disease, and hydrocephalus.32, 35-42 The impact of these ...
A tracheotomy scar revision is performed to remove an unsightly scar.A tracheotomy scar is formed after a tracheotomy tube is removed. Some scars heal very well and are unable to be seen. Other scars may be large and tether the skin to the deeper throat muscles making swallowing and speaking difficult.A tracheotomy scar revision is performed under local anesthesia. The old scar is removed and the remaining skin is sutured together. This is done with a plastics closure using fine instruments and small needles.
BACKGROUND: Whether early percutaneous tracheotomy in patients who require prolonged mechanical ventilation can shorten mechanical ventilation duration and lower mortality remains controversial.. OBJECTIVE: To compare the outcomes of severely ill patients who require prolonged mechanical ventilation randomly assigned to early percutaneous tracheotomy or prolonged intubation.. DESIGN: Prospective, randomized, controlled, single-center trial (ClinicalTrials.gov registration number: NCT00347321).. SETTING: Academic center.. PATIENTS: 216 adults requiring mechanical ventilation 4 or more days after cardiac surgery.. INTERVENTION: Immediate early percutaneous tracheotomy or prolonged intubation with tracheotomy 15 days after randomization.. MEASUREMENTS: The primary end point was the number of ventilator-free days during the first 60 days after randomization. Secondary outcomes included 28-, 60-, or 90-day mortality rates; durations of mechanical ventilation, intensive care unit stay, and ...
Pope John Paul II underwent a tracheotomy Thursday night to ease breathing problems, a serious turn for the worse in the health of the already fragile, 84-year-old spiritual leader of the worlds 1 billion Roman Catholics. [...] on Thursday evening, the Vatican announced that doctors, in response to an acute breathing insufficiency, had performed a tracheotomy -- that is, cut a hole at the base of the neck directly into the windpipe. The Holy Father was informed and gave his consent, the popes chief spokesman, Joaquin Navarro-Valls, himself a physician, said in a statement. Despite the operations apparent success, both the procedure and the underlying conditions that made it necessary seem likely to intensify questions about the popes ability to continue to lead the church. Because the cut is made below the voice box, medical experts say, people with tracheotomies cannot speak. People with Parkinsons disease are very sensitive to breathing problems, and any spasm in his throat, even

Tracheotomy and tracheostomy is a surgery on the neck to open a direct path by cutting a hole in the trachea (windpipe). They are performed by paramedics, veterinarians, emergency physicians, and surgeons. Both surgical and percutaneous techniques are now widely used. Tracheostomy was written about in ancient Egyptian tablets,[1] but, in the 12th century, Ibn Zuhr described the first correct tracheotomy operation for people who were suffocating (could not breathe).[2] The current tracheostomy was described in 1909 by Chevalier Jackson, an American doctor. ...

While non-invasive ventilation (NIV) is a recommended technique for weaning intubated patients after acute hypercapnic respiratory failure, little evidence exists for NIV as a technique for weaning tracheotomized patients undergoing prolonged invasive ventilation (IV). Methods: we prospectively studied a group of tracheotomized and chronically ventilated patients admitted to a weaning centre who could be suitable to decannulation and conversion to NIV because of absence of airway stenosis, normal swallowing function, preserved cough mechanism but unable to sustain a spontaneous breathing for more than 16 hours without increasing PaCO2. Data collected for follow-up were: demographic, functional, severity score (SAPSII), need to re-tracheotomy, survival, hospital admissions/year, maintenance of adequate gas exchange. The Fisher exact test and the log-rank test have been employed for statistical analysis. Results: 176 patients with tracheotomy and prolonged IV were evaluated; 26 patients (14 men) ...

This is a comprehensive, well-illustrated reference on classical tracheotomy in adults and children. It provides professionals with the essential tools needed to support safe, effective intervention in inpatient and outpatient settings and addresses special populations and their unique needs.
The term tracheotomy refers to a surgical procedure of an incision in the trachea to provide an alternative method of breathing, which was first recorded in 1649. The practice had been in use long before this, however, previously known as pharyngotomy, laryngotomy or bronchotomy.

Endoscopic debridement of obstructing laryngotracheal carcinoma with the microdebrider is an alternative to CO2 laser excision or emergency tracheotomy. This is a retrospective study of 27 patients with advanced laryngotracheal carcinoma treated with the microdebrider for airway obstruction from 1998 to 2002. In 26 patients, the carcinoma was debrided successfully and tracheotomy was safely avoided. Postoperative bleeding was encountered in 1 patient with a coagulopathy. Two of the patients later required tracheotomy during radiotherapy secondary to radiation-induced laryngeal edema. Both were safely managed without acute airway compromise. Laryngeal debridement provided up to 3 months of stable airway until definitive oncological treatment was performed. Endoscopic microdebridement of obstructing carcinoma is a relatively simple and highly accurate approach to restoring a stable airway. It provides extra time for preoperative oncological, psychological, and nutritional planning ...
TY - JOUR. T1 - Comparison of postoperative cardiopulmonary examinations and chest radiographs to detect pulmonary complications after adult tracheotomy. AU - Park, S. Y.. AU - Smith, R. V.. PY - 1999/1/1. Y1 - 1999/1/1. N2 - OBJECTIVE: To prospectively evaluate the effectiveness of clinical cardiopulmonary examination compared with that of chest radiography in detecting postoperative pulmonary complications after tracheotomy in adults. METHODS: One hundred consecutive patients undergoing tracheotomy were evaluated. Pulmonary complications were recorded, and the effectiveness of the physical examination was compared with that of the chest x-ray in detecting these complications. Patient age, sex, diagnosis, urgency of the procedure, and anesthetic technique were evaluated to determine any relationship to postoperative complications. RESULTS: Of the 100 patients, 87 patients underwent postoperative chest radiography, and all patients had postoperative cardiopulmonary examinations. One patient (1%) ...
TY - CHAP. T1 - Failure analysis in sleep apnea surgery. AU - McMains, K. Christopher. AU - Terris, David J.. PY - 2005/1/1. Y1 - 2005/1/1. N2 - Guilleminault et al. (1) coined the term obstructive sleep apnea (OSA) to describe patients with disrupted nocturnal breathing. In 1969, Kuhlo et al. (2) performed the first tracheotomy to bypass upper airway obstruction, which represented the first definitive surgical intervention in OSA. In 1979, Fujita et al. (3) introduced uvulopalatopharyngoplasty (UPPP) for treatment of OSA. In 1981, Sullivan et al. (4) published the first study of continuous positive airway pressure (CPAP) for nonsurgical treatment of OSA. As with tracheotomy, CPAP eliminates excessive daytime sleepiness (EDS) and cardiopulmonary sequelae of OSA (5) including normalization of blood pressure (6). Only complete compliance with CPAP was shown to be sufficient to derive treatment benefits from this therapy (7). Problematically, incomplete compliance with CPAP was prevalent (8-10). ...

A 62-year-old man came to our attention after an operation in a small dental outpatient clinic where only a single dentist was working. The man was showing complications after insertion of a dental implant in the anterior segments of the mandible. Bleeding led to a slow swelling of the neck with airway obstruction. Only an immediate intervention by a mobile emergency unit and prompt tracheal intubation avoided death by asphyxia. The patient was then transferred to our hospital. We inspected the patient and we performed a CT scan that showed complete airway obstruction. First, we performed a tracheotomy in order to ensure the airway patency and then we identified the source of bleeding: the mylohyoid artery placed anomalously close to the mandible. After clamping and tying the artery, the bleeding resolved. One day after the procedure, the tracheotomy was closed; the patient was discharged after 3 days.. ...
Way back in 2000 I was very ill with Pneumonia resulting as a complication from Pancreatitis. I was in intensive care and high dependency wards for months and for most of the time had a tracheotomy. A tracheotomy, for those unfamiliar with the procedure, is where someone, preferably a doctor, makes a hole in your neck and feeds a tube into your lungs for air to be drawn through, or, as in my case, for someone to stick a suction tube in and vacuum your lungs for gunk. Having a nervous F2 wielding a scalpel at your throat while you are passing out from lack of Oxygen rates pretty low on my list of things to do again. The advantage of the cough machine is that it is totally non-invasive. It removes secretions without the need for someone sticking a plastic straw through an unnatural orifice in your neck. Im not sure how much such machines cost but they must be cheaper than spending days, weeks or months in hospital ...
RESULTS: Conventional tracheotomy was performed in 7 patients. We performed tracheotomy during NPPV with local anesthesia in 13 patients. All but 3 patients had risk factors for difficult intubation. Hospital stay was 23.3 ± 10.3 d in the conventional group and 25.3 ± 12.9 d in the NPPV group (p = 0.87). The number of pneumonias was higher in the conventional-tracheotomy group (4 vs 1, p = 0.03). ...

1891: Concerns of Intubation: By the late 19th century many of the same concerns physicians have today about intubation were considered. One such concern being the ulceration of tissue due to pressure of the tube set upon it for a long period of time. Tubes were generally taken out after six days with success, although in some cases were left in 12 days or longer. Dr. Rank, a German physician, ultimately recommended removal of the tube after 10 days, and if necessary, the physician should consider tracheotomy. Some physicians recommended extubation after the 5th day, which would be in line with modern protocols. Feeding the patient was also a concern, and was either done with soft foods or liquids, or by nasalgastric tube. It was recommended that if the tube was accidentally spit up that the nurse take advantage of the moment to try feeding the patient prior to re-introducing the tube (if the tube was still needed). My father has been battling throat cancer since January. He had surgery in February to remove the tumor, and there was one still on his voicebox. He went through chemotherapy and radiation. In August, he was rushed in for an emergency tracheotomy because his throat was swelling shut. We were told it was from the radiation. When they did the thracheotomy, they found a tumor on the side and back of his throat. We had to wait almost a month for surgery, and last Tuesday, they opened him up to do a laryngectomy and reconstruct his esophogus, saw the cancer had gone onto his spine, and closed him back up. They told us we just need to make him comfortable now for the next months to a year that he has left ...

In arthroscopy, an endoscope is inserted into a joint using a small incision. It is performed to examine and treat various orthopedic disorders. It is used for conditions such as torn cartilage, ACL reconstruction, torn surface cartilage, and trimming of damaged cartilage. In bronchoscopy a practitioner will use endoscopy for viewing the inside of airways for therapeutic or diagnostic purposes. A bronchoscope is used in this procedure, which is inserted into the airways either through the nose or mouth, or sometimes through a tracheotomy. Colonoscopy lets a practitioner view inside the large intestine. Due to this procedure the doctor can detect any abnormal growths, ulcers, or inflamed tissue. Apart from these procedures there are other endoscopy procedures such as cystoscopy, laparoscopy, thoracotomy, mediastinoscopy, etc ...

The Final Days of Pope John Paul II. Pope John Paul II emerged from a week-long Lenten retreat on 20 February 2005. He was still suffering the effects of his bout with the flu, for which he had been hospitalized February 1-10 at the Gemelli Polyclinic. Nonetheless, his Angelus message showed that he remained intent on completing his Petrine mission in service to the unity of the Church. The Pope resumed his usual audiences with visiting bishops, but his convalescent state and bad weather prompted him to appear via television to pilgrims, gathered for his General Audience on February 23. He was unable to attend a consistory for the announcement of upcoming canonizations due to a difficulty breathing, and the next day, February 24, he returned to the hospital to undergo a tracheotomy. Three days after the operation, though unable to speak, he made a surprise appearance at his hospital window for the Sunday Angelus, to silently bless well-wishers gathered outside. As Cardinal Ratzinger observed, ...

Luis Jaramillos The Doctors Wife is a collection of short stories that reads saga captured in tableaux. This is a family narrative in 91 arresting and varied compositions halved into two parts, in which our narrator is the grandson of the Doctors Wife and the majority of the book is comprised of stories from his mothers childhood. Its a thrill to watch Jaramillo flirt with the range of breviloquence possible in prose. Some stories are as short as a conversation in three exchanges:. I was only depressed for, like, 40 years, Petrea says to me.. Because of John?. Because of John.. Other stories last for eight pages of sensuous prose and crystal dialogue. On a father-son fishing trip, the Doctor, our father archetype, instructs his thirteen year-old son how to perform an emergency tracheotomy, after an accident occurs:. If I pass out, I need you to help me, the Doctor says to his son. He takes a knife out of the tackle box, and a ballpoint pen from his shirt pocket, removing the ink ...
