Public health education about poisons and infrastructure limiting access and exposure to the most highly toxic chemicals are practically absent in much of the developing world. Access to medical care is often limited for financial, cultural, and geographic reasons. Medical toxicology is frequently not a recognized specialty and patients are evaluated by general physicians with little training-although often with great experience. Diagnostic facilities are few, effective treatment options even more rare. Due to recent advances in data storage and sharing for further data processing in predictive toxicology, there is an increasing need for flexible data representations, secure and consistent data curation and automated data quality checking. Toxicity prediction involves multidisciplinary data. There are hundreds of collections of chemical, biological and toxicological data that are widely dispersed, mostly in the open literature, professional research bodies and commercial companies. In order to better manage and make full use of such large amount of toxicity data, there is a trend to develop functionalities aiming towards data governance in predictive toxicology to formalise a set of processes to guarantee high data quality and better data management. The American College of Medical Toxicology established the Toxicology Investigators Consortium (ToxIC) Case Registry in 2010. The Registry contains all medical toxicology consultations performed at participating sites. The Registry has continued to grow since its inception, and as of December 31, 2015, contains 43,099 cases. This is the sixth annual report of the ToxIC Registry, summarizing the additional 8115 cases entered in 2015. Cases were identified by a query of the Registry for all cases entered between January 1 and December 31, 2015. Specific data reviewed for analysis included demographics (age, race, gender), source of consultation, reason for consultation, agents and agent classes involved in exposures, signs, symptoms, clinical findings, fatalities, and treatment. By the end of 2015, there were 50 active sites, consisting of 101 separate health-care facilities; 51.2 % of cases involved females. Adults between the ages of 19 and 65 made up the majority (64.2 %) of Registry cases. Caucasian

He received his MD from the University of Wisconsin School of Medicine and Public Health in Madison, Wisconsin in 2004. Dr. Wiegand completed a categorical Internal Medicine residency at Hennepin County Medical Center, Minneapolis, Minnesota and completed fellowship training in Medical Toxicology and Clinical Pharmacology at the University of California, San Francisco in 2006 and 2007 respectively. Dr. Wiegand also became Board Certified in Addiction Medicine in 2015. He was the Medical Director of the Rochester Poison Center until 2010 and currently serves as the Director of Toxicology and of the Toxicology Consult Service at the University of Rochester Medical Center. An overdose of an SSRI alone produces serotonin toxicity of only moderately severity in 10% 20 % of cases (sufficient for admission and active medical treatment), but with no serious sequelae or fatalities (1).. If an MAOI or RIMA + SRI have been ingested then about 50% of such cases experience severe serotonin toxicity (2,3). It is these combinations that are most likely to require active medical intervention including intensive care admission, cooling, 5HT2A antagonists, intubation and neuromuscular paralysis.
