A 72-year-old man was admitted for recurrent episodes of chest pain 3 months after stent deployment in the left anterior descending (LAD) artery. The patient underwent a computerized tomographic (CT) examination of the thorax with a multislice spiral CT scanner (MSCT) (Somatom Volume Zoom, Siemens). A dynamic study was performed with acquisition of 20 scans at the level of the LAD distal to the stented segment during injection of 20 mL of nonionic contrast medium (370 mgI/mL); the resultant time-density curves were suggestive of high-grade stenosis (Figure 1A). BACKGROUND: Conventional coronary angiography (CA) is still recommended before valvular surgery. Preliminary studies suggest that multislice spiral computed tomography coronary angiography (MSCT-CA) can be used to rule out coronary artery disease (CAD). AIM: To assess prospectively the safety of ruling out CAD before surgery solely on the basis of normal MSCT-CA in patients with severe aortic valve disease. METHODS: We included all consecutive patients scheduled for aortic valve surgery. We first estimated the calcium score (Agatston score equivalent [ASE]). Patients underwent injected MSCT if the ASE was|1000. CA was cancelled when MSCT-CA quality was sufficient and showed no significant CAD. Our primary endpoint was the occurrence of perioperative myocardial infarction in patients who underwent surgery with no prior CA. While the program was more successful in increasing the rotation time for schools that started it with little available time for student rotations, for any school with a 2003 baseline rotation time of up to seven weeks, Pipeline schools received a positive boost over the 2007 rotation time reported by students at non-Pipeline schools. With the CT scanner used in this study, the mean scan time of a thinly collimated spiral CT scan of the brain was 30 seconds, approximately 2 times longer than the mean scan time of a thickly collimated sequential CT scan. A minor disadvantage of this relatively long scan time is the increase of the risk of motion of the patient during data acquisition. In this study, at least 1 patient had moved during acquisition of the spiral scan, which affected the image quality and caused the observers to have a preference for the sequential technique. The ability to visualise coronary arteries directly, assess cardiac function, and detect subclinical disease from a 20 second scan make MDCT more attractive than conventional stress testing. Whole-brain helical tomotherapy with integrated boost for brain metastases in patients with malignant melanoma – final results of the BRAIN-RT trial
OBJECTIVE The purpose of this study was to compare two time delays between injection of contrast material and helical CT scanning to determine relative conspicuity of hepatic metastases. SUBJECTS AND METHODS Twenty-five patients with hepatic metastases were examined with helical CT. The first imaging phase was initiated at 50 sec and the second 75 sec after the start of contrast material injection (3 ml/sec, 150 ml). Differences in lesion and liver attenuation were measured quantitatively. Four radiologists used a 5-point scale to assess lesion conspicuity subjectively. RESULTS Mean differences in enhancement between liver and lesion were 41 H during the first phase and 59 H for the second phase (p = .0001). Radiologists conspicuity score averaged 2.4 for lesions in the first phase versus 3.3 for lesions in the second phase (p = .0001). The apparatus and method comprise driving the transfer belt provided in an image forming apparatus by a predetermined distance when the transfer belt has been stopped for a predetermined time, and thus varying the contact portion with a plurality of rollers provided on the transfer belt. Image contamination due to deformation on the belt can be avoided. Additionally, because the rotation time of the driving source can be set to a predetermined time during the power-saving mode, power consumption can be minimized.
