I have been in practice for 7 years, 2 years in academics and 5 years in hospital-based practice. I am currently the medical director of a childrens hospital cleft lip and palate team. My practice consists of approximately 75% pediatric patients and 25% adult patients. I excise pigmented nevi and other soft tissue lesions over 100 times per year. I remove larger nevi requiring serial excision perhaps 5-6 times per year. The largest nevi that require tissue expanders to recruit skin prior to excision are rarer, and I do perhaps only 2-3 per year. In hopes of avoiding future disease, my young age, and because of the stage of my cancer, I elected to have both breasts surgically removed . The surgery aims to remove all breast tissue that potentially could develop breast cancer. An expander is used to prepare the breast area for reconstruction by slowly stretching the area and making room for an implant. I am currently at home recovering which is a WAY more daunting task than I expected. Besides the obvious physical discomfort and pain, I have been a lot more emo these past few weeks than I was during my chemo treatments for 6 months. The mouthpiece has two wings that join near the incisors and then become parallel and spaced apart around the molars. Near the molars, an arch expander joins to the mouth piece. The arch expander has two telescoping shells with a threaded jack and cooperating springs. Each shell joins to a corresponding wing of the mouthpiece. Turning the jack lengthens a rod that pushes the shells outwards against the mouthpiece thus widening the maxillary arch without brackets or bands upon the teeth.
