Background: Posterior tibial slope (PTS) is an important factor in the knee joint biomechanics and one of the bone features, which leads to knee joint stability. Posterior tibial slope affects flexion gap, knee joint stability and posterior femoral rollback that are related to wide range of knee motion. During high tibial osteotomy and total knee arthroplasty (TKA) surgery, proper retaining the mechanical and anatomical axis is important. The aim of this study was to evaluate the value of posterior tibial slope in medial and lateral compartments of tibial plateau and to assess the relationship among the slope with age, gender and other variables of tibial plateau surface. Materials and methods: This descriptive study was conducted on 132 healthy knees (80 males and 52 females) with a mean age of 38.26±11.45 (20-60 years) at a medical center in Mashhad, Iran. All patients required to MRI admitted for knee pain with uncertain clinical history and physical examination that were reported healthy at knee
Bone is integral to the pathogenesis of osteoarthritis (OA). Whether the bone area of the tibial plateau changes over time in subjects with knee OA is unknown. We performed a cohort study to describe this and identify factors that might influence the change. One hundred and twenty-six subjects with knee OA underwent baseline knee radiography and magnetic resonance imaging on their symptomatic knee. They were followed up with a repeatmagnetic resonance image of the same knee approximately 2 years later. The bone area of the tibial plateau was measured at baseline and follow-up. Risk factors assessed at baseline were tested for their association with change in tibial plateau bone area over time. One hundred and seventeen subjects completed the study. The medial and lateral tibial plateau bone areas increased by 2.2 ± 6.9% and 1.5 ± 4.3% per year, respectively. Being male (P = 0.001), having a higher body mass index (P = 0.002), and having a higher baseline grade of medial joint-space narrowing (P = 0.01

N2 - Unicompartmental knee replacements have not performed as well in the lateral compartment as in the medial. This may be because the tibial components have flat or slightly concave surfaces which match the medial plateau but not the convex lateral plateau. The aim of this study was to find the optimal radius for a convex lateral tibial component.Twelve normal lateral tibial plateau were retrieved at knee replacement, and their surface contour in their mid sagittal plane was determined. The optimal circle was fitted and its radius measured. A series of different shaped tibial components were superimposed. From published information about the position of the femoral condyle relative to the tibia in different degrees of flexion, the flexion gap at these angles was determined. The average radius of the lateral tibial plateau was 40 ...

BACKGROUND: Accelerated rehabilitation protocols for medial opening wedge high tibial osteotomy (MOW HTO) using intraosseous implants have not previously been described. The present study provides early clinical and radiological outcomes of MOW HTO using a polyetheretherketone (PEEK) intraosseous system, in combination with an early weight-bearing protocol.. METHODS: Twenty consecutive knees (17 patients) underwent navigated MOW HTO using a PEEK implant with accelerated rehabilitation. Time to union and maintenance of correction were assessed radiographically for 12 months post-operative. Patient outcomes were monitored for a mean follow-up of 38 months (range 23-42) using standardised instruments (WOMAC, IKDC and Lysholm scores).. RESULTS: All knees were corrected to valgus. The mean time to unassisted weight-bearing was 55 days (SD 24, range 21-106). Bone union occurred in 95% of knees by 6 months, with correction maintained for 15 knees at 12 months post-operative. Knees for which correction ...

This study aimed to assess the mechanical static and fatigue strength provided by the FlexitSystem plate in medial opening wedge high tibial osteotomies (MOWHTO), and to compare it to six previously tested implants: the TomoFix small stature, the TomoFix standard, the ContourLock, the iBalance, the second generation PEEKPower and the size 2 Activmotion. Thus, this will provide surgeons with data that will help in the choice of the most appropriate implant for MOWHTO. Six fourth-generation tibial bone composites underwent a MOWHTO and each was fixed using six FlexitSystem plates, according to standard techniques. The same testing procedure that has already been previously defined, used and published, was used to investigate the static and dynamic strength of the prepared bone-implant constructs. The test consisted of static loading and cyclical loading for fatigue testing. During static testing, the group constituted by the FlexitSystem showed a fracture load higher than the physiological loading of slow

The tibia /ˈtɪbiə/ (plural tibiae /ˈtɪbii/ or tibias), also known as the shinbone or shankbone, is the larger and stronger of the two bones in the leg below the knee in vertebrates (the other being the fibula), and it connects the knee with the ankle bones. The tibia is found on the medial side of the leg next to the fibula and closer to the median plane or centre-line. The tibia is connected to the fibula by the interosseous membrane of the leg, forming a type of fibrous joint called a syndesmosis with very little movement. The tibia is named for the flute tibia. It is the second largest bone in the human body next to the femur. The leg bones are the strongest long bones as they support the rest of the body. In human anatomy, the tibia is the second largest bone next to the femur. As in other vertebrates the tibia is one of two bones in the lower leg, the other being the fibula, and is a component of the knee and ankle joints. The leg bones (femur, tibia and fibula) are the strongest long ...

N2 - Purpose: The anterior iliac crest, posterior iliac crest, and proximal tibia are common cancellous donor sites used for autogenous bone grafting. Donor site selection is partly dependent on the expected volume of available bone, but reports of cancellous bone volumes at each of these sites are variable. The goal of this study was to compare the volumes of cancellous bone harvested from donor sites within the same cadaver. Materials and Methods: Within each of 10 fresh frozen cadavers, cancellous bone was harvested from 3 donor sitesanterior iliac crest, posterior iliac crest, and proximal tibiausing established surgical techniques. Bone volumes were measured by fluid displacement. Mean compressed cancellous bone volumes from the 3 donor sites were compared ...
A system for bone segment navigation with a planning unit for planning a bone segment displacement, a marker arrangement to be connected to a bone segment, a position sensing unit which senses the position of the marker arrangement, and an indication and processing unit, which is connected to the position sensing unit and to the planning unit, in order to indicate the deviation of the present position of the bone segment from a planned final bone segment position or from a planned bone segment displacement path. The marker arrangement is connected in an unambiguously reproducible manner to the bone segment by a template allocated to the bone segment. After the template has been placed and fastened on the bone segment, a correlation is possible between the patient and the image data set or the planning data set, without probing and sensing individual bone points according to position by means of a pointer.

High tibial osteotomy (HTO) is intended to transfer the mechanical axis from medial to slightly lateral to the midline of the knee to decrease the load and subsequently delay osteoarthritis. Some studies showed that regenerative process began after realignment. HTO was considered as an option to treat an isolated medial compartment in varus knees. HTO became more popular in young active patients after improvement in surgical technique, fixation devices, and patient selection with fewer complications. As several new tibial osteotomy plates recently appeared on the market, the aim of the present study was to compare mechanical static and fatigue strength of three newly designed plates with gold standard plates for the treatment of medial knee joint osteoarthritis. Sixteen fourth-generation tibial bone composites underwent a medial open-wedge high tibial osteotomy (HTO) according to standard techniques, using five TomoFix standard plates, five PEEKPower plates and six iBalance implants. Static compression load to failure and load-controlled cyclic fatigue failure tests were performed. Forces, horizontal and vertical displacements were measured; rotational permanent plastic deformations, maximal displacement ranges in the hysteresis loops of the cyclic loading responses and dynamic stiffness were determined. Static compression load to failure tests revealed that all plates showed sufficient stability up to 2400 N without any signs of opposite cortex fracture, which occurred above this load in all

A new technique in oblique incomplete high tibial osteotomy that permits an increase of valgus correction while preventing fracture of the medial cortex was investigated. Closing wedge or opening wedge osteotomy was done on 23 tibias from cadavers before loading in an Instron testing machine. In seven specimens (Group 1), lateral oblique wedge osteotomy was done. In seven other specimens (Group 2), one medial oblique cut was made. In both groups, the osteotomy terminated 10 mm from the cortex and approximately 2 cm below the plateau. In nine specimens (Group 3), the osteotomy terminated in a 5-mm diameter hole, drilled in an anteroposterior direction, with its center positioned 10 mm from the medial cortex and 2 cm below the articular surface. The maximum angle of opening or closing before fracture of the cortex took place was recorded. In Groups 1 and 2, similar maximum correction angles were observed, 6.7 degrees versus 6.5 degrees, respectively. In Group 3, the stress relieving hole allowed ...

The goal of this study was to evaluate the effectiveness of a number of shin guards in protecting against tibia fracture in soccer players. A secondary purpose was to determine the relationship between the material and structural differences in shin guard design and the protection provided. Twenty-three commercially available shin guards were tested on a model leg containing a synthetic tibia that had been calibrated against human cadaver specimens. Each guard was categorized into one of four material types: plastic (N = 9), fiberglass (N = 6), compressed air (N = 4), and Kevlar (N = 4). The maximum combined force at the ends of the tibia, the principal strain on the posterior side of the tibia, and the contact time of the impact were measured using a drop track impact simulation. Shin guards provided significant protection from tibia fracture at all drop heights. The average guard reduced force by 11% to 17% and strain by 45% to 51% compared with the unguarded leg. At the higher drop heights, ...

Dr. Robert F. LaPrade operated on my right knee in May of 2010. I was hit by a car on my bicycle near Horsetooth Reservoir in CO. I was life flighted to MCR in Loveland, CO. My orthopedic injuries were severe, but totally missesd by the orthopedic team at Poudre. I could not bear weight on my right side though I tried repeatedly, but finally I went and got an MRI and one of the orthopedic surgeons that I worked with was shocked when he saw the MRI result. My right knee was totally destroyed; ACL, MCL, PCL all severely torn; the patella was the only thing intact in my right knee. Dr. La Prade had just moved to Vail and I was his 2nd patient @ The Steadman Clinic. I was told by one of the orthopedic surgeons that I worked with that I would never run again and would be lucky if I could ever hike again. This was devastating news after being a top triathlete (3rd in the world in my age group in 1989 & 1st nationally in my age group) and a big marathon runner. All I can say is Dr. La Prade did an ...
