We studied 39 patients with thromboangiitis obliterans to determine their cellular and humoral immune responses to native human collagen Type I and Type III, which are constituents of blood vessels. Cell-mediated sensitivity to these collagens was measured by an antigen-sensitive thymidine-incorporation assay. The mean stimulation index--the ratio of thymidine incorporation in the presence of antigen to that in its absence--with both Type I and Type III collagens used as antigens was significantly higher in patients with thromboangiitis obliterans than in patients with arteriosclerosis obliterans or in healthy male controls. Lymphocytes from 77 per cent of the patients with thromboangiitis obliterans exhibited cellular sensitivity to human Type I or Type III collagens (or both). Furthermore, in 17 of 39 serum samples from the patients with thromboangiitis obliterans a low but significant level of anticollagen antibody activity was detected, whereas there was no antibody activity in serum samples ...
Thromboangiitis obliterans (TAO), an inflammatory vasculopathy also known as Buerger disease, is characterized by an inflammatory endarteritis that causes a prothrombotic state and subsequent vaso-occlusive phenomena. The inflammatory process is initiated within the tunica intima.
Gangrene of the lower extremities in cases of diabetes is due primarily to impairment of circulation and secondarily to the effects of the diabetes on the resistance of the skin to infection. In cases of long standing, without adequate treatment, arteriosclerosis is common. It develops early in life and to a more advanced degree than in nondiabetic patients. In some cases, however, the occlusive process is of a different type. The present report, with the exception of a case reported by Adams in 1930, includes all cases of thromboangiitis obliterans complicated with diabetes observed at The Mayo Clinic. The small ...
Thromboangiitis Obliterans is characterized by an initial infiltration of the vascular wall with neutrophils. Over time, the inflammatory infiltrate is replaced with one predominated by mononuclear cells and the occasional granuloma may appear. During the inflammatory period thrombosis of the vascular lumen often occurs, resulting it its occlusion. Over time, vascular fibrosis ensues followed by recanalization of the obstructed vessel ...
Autoamputation is the spontaneous detachment (amputation) of an appendage from the body. This is not to be confused with self-amputation. It is usually due to destruction of the blood vessels feeding an extremity such as the finger tips. Once the vessels are destroyed, the tissue is starved of oxygen and dies, which is often followed by gangrene. Autoamputation is a feature of ainhum, cryoglobulinemia and thromboangiitis obliterans. In 1881, Thornton made the case of autoamputation. Autoamputation could be the result of severe cases of certain chronic wounds. These chronic wounds might be due to some vascular and pathogenic conditions like Buerger disease other called thromboangiitis obliterans, Raynauds disease and Reynauds phenomenon. Also, an uncontrolled diabetes can predispose one to autoamputation. However, autoamputation has been described as spontaneous. Autoamputation has often been associated with fingers and toes but other parts of the body can suffer this condition as well. There ...

A 46‐year‐old man with a history of heavy marijuana use for 30 years presented to the emergency room with 1 month of progressively worsening distal extremity pain and numbness that had developed into necrosis at the tips of his fingers (Figure 1A, arrows). The patient did not have other systemic symptoms. Transthoracic echocardiogram was normal. An evaluation for hypercoagulable disorders, blood cultures, C‐ANCA, cryoglobulins, Hepatitis C serologies, and human immunodeficiency virus (HIV) serologies were all negative. Erythrocyte sedimentation rate was within normal limits. An arteriogram of the hands showed distal subsegmental obliteration of digital arteries (Figure 1B, arrows), which was consistent with thromboangiitis obliterans (Buergers Disease). While the patient rarely consumed tobacco, he reported smoking up to 10 marijuana cigarettes daily and a diagnosis of cannabis arteritis was made. The patient was encouraged to discontinue smoking and at follow‐up he had marked ...
BACKGROUND: Treatment of critical limb ischemia (CLI) by bypass operation or percutaneous vascular intervention is occasionally difficult. The safety and efficacy of multiple intramuscular adipose tissue-derived mesenchymal stem cells (ATMSC) injections in CLI patients was determined in the study.. METHODS AND RESULTS: The study included 15 male CLI patients with ischemic resting pain in 1 limb with/without non-healing ulcers and necrotic foot. ATMSC were isolated from adipose tissue of thromboangiitis obliterans (TAO) patients (B-ATMSC), diabetes patients (D-ATMSC), and healthy donors (control ATMSC). In a colony-forming unit assay, the stromal vascular fraction of TAO and diabetic patients yielded lesser colonies than that of healthy donors. D-ATMSC showed lower proliferation abilitythan B-ATMSC and control ATMSC, but they showed similar angiogenic factor expression with control ATMSC and B-ATMSC. Multiple intramuscular ATMSC injections cause no complications during the follow-up period (mean ...
BACKGROUND: Thromboangiitis obliterans (TAO) is a nonatherosclerotic thrombotic-occlusive vasculitis that affects the vessels of the small and medium sized extremities. No explicit etiology or pathogenesis of TAO has been proven, and more effective treatments are needed. Objective: To summarize and present an overview of recent advances regarding the risk factors, mechanisms and treatments of TAO and to organize the related information into figures to provide a comparatively complete reference. Methods: We searched PubMed for English-language literature about TAO without article type limits, including articles about the risk factors, pathological mechanisms and treatments of TAO in the last 10 years with essential supplements (references over ranges and English abstracts of Russian literature). Results: After screening content of literatures, 99 references were evaluated. We found that risk factors of TAO include smoking, gene factors and periodontal diseases. The underlying mechanism of TAO ...

Buergers disease, also called thromboangitis obliterans, is a rare form of vasculitis characterized by acute inflammation and clotting of the arteries and veins of the hands and feet. The inflammation can lead to blockages of the arteries of the lower portions of the arms and legs. These blockages can cause critical limb ischemia and/or claudication, rest pain and non-healing sores or ulcers, in the hands and/or feet.. Buergers disease, which occurs almost exclusively in heavy smokers, is different from peripheral arterial disease (PAD) in that it is not caused by atherosclerosis (plaque buildup in the arteries). The disease can cause pain in the hands and feet in response to exercise due to reduced blood-flow.. ...

A series of 29 well-documented and properly analysed patients with thrombangiitis obliterans (Buergers disease) is presented. The diagnosis of Buergers disease was based on following criteria: smoking history, onset before the age of 50 years, infrapopliteal arterial occlusive disease, either upper limb involvement or phlebitis migrans, absence of atherosclerotic risk factors other than smoking. In the last 10 years (1986-1996), we identified 29 patients who met these rigid criteria. There were 24 men and 5 women, aged 32.4 years at the moment of the disease first clinical symptom. The cumulative tobacco use averaged 16 pack-years for each patient. The initial symptom was limited gangrene of a toe (n = 9) or a finger (n = 2), ...
Buergers disease (BD) is characterised by segmental inflammation of medium and small size arteries, affects mainly young adult men, and is more prevalent in the southeast Mediterranean and the Far East. Its pathogenesis remains unclear, but cigarette smoking is strongly implicated. Autoimmune diseases, hypercoagulable states, diabetes, and emboli need exclusion before a diagnosis is made. The tibial and leg digital arteries are usually affected, and arteriography shows typically corkscrew peripheral obstructions. The best treatment is stopping smoking. Prostacyclin infusion is currently the most effective treatment for pain control and healing of ischaemic lesions.1 In selected cases, regional guanethidine block may have good results.2 Sympathectomy does not always provide longstanding benefit,3 and arterial bypass is difficult due to poor run-off in the diseased arteries.3,4 Limited amputations are often necessary to remove necrotic tissue.. New methods are being tested for pain relief and ...

Background. Emerging evidence suggests that stem cells and progenitor cells derived from bone marrow can be used to improve cardiac function in patients after acute myocardial infarction. In this randomised trial, we aimed to assess whether intracoronary transfer of autologous bone-marrow cells could improve global left-ventricular ejection fraction (LVEF) at 6 months follow-up.. Methods. After successful percutaneous coronary intervention (PCI) for acute ST-segment elevation myocardial infarction, 60 patients were randomly assigned to either a control group (n=30) that received optimum postinfarction medical treatment, or a bone-marrow-cell group (n=30) that received optimum medical treatment and intracoronary transfer of autologous bone-marrow cells 4·8 days (SD 1·3) after PCI. Primary endpoint was global left-ventricular ejection fraction (LVEF) change from baseline to 6 months follow-up, as determined by cardiac MRI. Image analyses were done by two investigators blinded for treatment ...

It is probable that the process characteristic of thrombo-angiitis obliterans involves vessels in areas other than the extremities more commonly than is believed. Such involvement may only be recognized by careful clinical and pathologic investigation. We have not had an opportunity to examine cases of thrombo-angiitis obliterans after death. Necropsy was performed in seven cases recorded in the literature. Evidence of disease of the coronary arteries was found in four. A summary of these four cases follows:. Case reported by Buerger.-A man aged twenty-four years with thrombo-angiitis obliterans died suddenly following a Gritti-Stokes amputation of the right thigh. He had ...
Peripheral vascular disease (PVD) affects 13% of the population aged over 50 in the western world and can be divided into two main groups: organic and functional. Other less common causes to include in the differential diagnosis in younger patients are Buergers disease and anatomical variants such as duplication of the superficial femoral artery and persistent sciatic arteries (PSAs). PVD is a major cause of morbidity and mortality globally. Symptoms vary from cold extremities, intermittent claudication, to acute and critical ischaemia. All of these have a significant financial burden on critical healthcare resources. This case report evaluates the role of ultrasound in the diagnosis of bilateral PSAs. A 16-year-old female was referred by her GP to the vascular clinic presenting with bilateral intermittent claudication. On examination, missing dorsals pedis pulses were noted bilaterally. No previous imaging was available and the patient was referred for a lower limb Doppler ultrasound. Initial ...

Background: Many clinical trials and meta-analyses presented moderate but significant improvement of the left ventricular ejection fraction (LVEF) after intracoronary autologous bone-marrow (BM) or peripheral blood origin stem cells transfer. However, it remains controversial, whether this beneficial effects is comparable with the intramyocardial delivery of the stem cells, or could be maintained during moderate and long term follow-up. The BOOST trial suggested that cardiac stem cell therapy did not improve LVEF at 5-year follow-up. By contrast, BALANCE study showed a long sustained benefit of BM-stem cells treatment. Due to these divergent outcomes of the presented trials, the aim of the present meta-analysis is to compare the safety and effectiveness of the cardiac stem cell therapy in different patient population, delivery mode and cell type, to find out, which patients with which therapy mode can have the greatest benefit from cardiac stem cell therapy ...
