The overall goals of the Thoracic Surgery Residency Program are: 1) to ensure that, upon completion of the full program, the resident will have the clinical educational experience needed to prepare for successful completion of the certification process with the American Board of Thoracic Surgery; and 2) to lay the foundation for a lifetime of self-education and improvement in a residents chosen field.. BACKGROUND: In this clinical randomized study, the effects of four anaesthesia techniques during one-lung ventilation [total intravenous anesthesia (TIVA) with or without thoracic epidural anaesthesia (TEA) (G-TIVA-TEA and G-TIVA), isoflurane anaesthesia with or without TEA (G-ISO-TEA and G-ISO)] on pulmonary venous admixture (Qs/Qt) and oxygenation (OLV) were investigated. METHODS: In 100 patients (four groups, 25 patients in each) undergoing thoracotomy, a thoracic epidural catheter was inserted pre-operatively. In G-TIVA-TEA and G-ISO-TEA, bupivacaine 0.1% + 0.1 mg/ml morphine was administered intra-operatively (10 ml of first bolus + 7 ml/h infusion). Propofol infusion or isoflurane concentration was adjusted to keep a bispectral index (BIS) of between 40 and 50 in all groups. FiO(2) was 0.8 during OLV and 0.5 before and after OLV. Therefore, its estimation before operation may enhance risk stratification. The objective of this study was to develop and validate a model to predict FEV1 on the first postoperative day after major lung resection. Methods: FEV1 was prospectively measured on the first postoperative day in 272 patients submitted for lobectomy or pneumonectomy at two centers. A random sample of 136 patients was used to develop a model estimating the first day FEV1 by using multiple regression analysis including several preoperative and operative factors. The model was then validated by bootstrap analysis and tested on the other sample of 136 patients. Lipoblastornatous tumors are quite rare neoplasms deriving from embryonic white fatty tissues, most often seen in infancy and childhood. Lipobtastoma is the focal and well-circumscribed type, whereas tipoblastomatosis is the more infiltrating type of lipoblastomatous tumors. We present a 14-month-otd mate infant presenting with a mediastinal mass showing paraparesis, who had previously undergone the removal of the cervical part of this mediastinal mass. We removed the mediastinal part of the mass with a posterotateral thoracotomy which proved to be a tipobtastomatosis. We emphasize that this very uncommon tumor located at the neck and mediastinum should be included in the differential diagnosis of infants having gait disturbance.
