BACKGROUND: In pre-school aged children, the occurrence of emergence delirium (ED) is increased after sevoflurane anesthesia. The purpose of this study was to evaluate if intravenous inducting agents such as propofol, ketamine or thiopental sodium affected the development of ED. METHODS: A total of 62 children between 3 and 6 years of age scheduled for elective tonsillectomy and adenoidectomy were divided into 3 groups in a double-blinded manner. Anesthesia was induced using one of the three drugs intravenously: 5 mg/kg of sodium thiopental, 1 mg/kg of ketamine or 2 mg/kg of propofol. Anesthesia was then maintained with sevoflurane. The development of ED was assessed in the post-anesthetic care unit. In our case of cardiogenic shock associated with large-dose propofol infusion, the patient survived after institution of extracorporeal circulation and termination of the propofol infusion (1). Remote ischemic preconditioning (RIPC) was induced by 3 cycles of 5 min liver ischemia followed by 5 min of reperfusion. The hearts were excised within 5 min after the final cycle of preconditioning and perfused using Langendorffs system. The isolated perfused hearts were subjected to 30 min global ischemia followed by 90 min reperfusion. In states where death is determined by listening for a heartbeat, death could be pronounced in as little as 5 to 10 minutes after the injection, not 45 minutes. Thiopental is a rapid and effective drug for inducing unconsciousness, since it causes loss of consciousness upon a single circulation through the brain due to its high lipophilicity. Only a few other drugs, such as methohexital, etomidate, or propofol, have the capability to induce anesthesia so rapidly. (Narcotics such as fentanyl are inadequate as induction agents for anesthesia.) Supporters argue that since the thiopental is given at a much higher dose than for medically induced coma protocols, it is effectively impossible for the condemned to wake up.. Sedative-hypnotics are a relatively new class of anesthetics, beginning with the introduction of sodium thiopental in the early 1930s. Since then, several -sedative-hypnotics have been introduced (Table 6-1), with more in the drug development pipeline, such as remimazolam, fospropofol, and isomers of etomidate. Goals of these modified drugs include fast metabolism and breakdown as well as creating "soft" drugs with safer profiles. A major goal in developing methoxycarbonyl-etomidate is the removal of adrenocortical suppression by modifying the pyrrole ring in etomidate. Fospropofol is water-soluble as opposed to propofol, which is administered as an oil-water emulsion. Propofol decreases systemic blood pressure with corresponding changes in cardiac output and systemic vascular resistance. The blood pressure effects may be overt in hypovolemic patients, elderly patients and also patients with coronary artery disease compromising the left ventricle. Adequate hydration is often recommended to offset this effect of propofol. Unlike the effect of thiopental on blood pressure compansated by the increase in heart rate, propofol does not change heart rate. Furthermore, bradycardia and asystoli may also occur most probably because of the reduction in sympathetic outflow more than parasympathetic.
