A team of researchers at Wayne State Universitys Parent Health Lab in the School of Medicine have received a three-year grant to develop a computer-delivered intervention for pregnant women at risk for alcohol use, which can lead to lifelong negative effects on the fetus. Prenatal exposure to alcohol affects attentional, cognitive, social and behavioral functioning and is a major cause of mental retardation. Infants born to African American women are at increased risk of adverse effects.
Infants born to African-American and/or low SES women appear to be at increased risk of adverse effects due to prenatal exposure to alcohol. Computer-delivered SBIRT approaches may provide consistent screening and evidence-based brief interventions, at low cost, without requiring substantial investments of time or energy from medical staff. However, several Stage I steps are necessary before moving to a Stage II clinical trial. This R34 application will therefore lay the groundwork for a fully powered clinical trial of a computer-delivered SBIRT for alcohol use during pregnancy. It will do so through the conduct of five key preliminary studies, including: (1) evaluation of the utility of handheld mobile devices and an anonymous self-interview format in screening for at-risk drinking among patients attending a prenatal clinic; (2) modification of an existing computer-delivered motivational intervention for alcohol use during pregnancy, to previously set standards of acceptability (to experts as ...

Internet-based interventions have been developed to improve access to audiovestibular health care. This review aimed to identify outcomes of Internet interventions for adults with hearing loss, tinnitus, and vestibular disorders. Electronic databases and manual searches were performed to identify studies meeting eligibility for inclusion. Fifteen studies (1,811 participants) met the inclusion criteria, with nine studies targeting tinnitus distress, five considering hearing loss, and one for vestibular difficulties. Only the tinnitus and hearing loss Internet intervention studies were eligible for data synthesis. Internet-based interventions for hearing loss were diverse. Overall, they showed no significant effects, although a statistically significant moderate effect (d = 0.59) was found after removing the study with the highest risk of bias (as a result of high attrition). Most Internet-based interventions for tinnitus provided cognitive behavioural therapy. They yielded statistically ...
OBJECTIVE: To evaluate impact of a computer-based intervention on arterial blood gas (ABG) usage in an intensive care setting. DESIGN: Retrospectively examined, via mixed group analysis, the effects of the intervention on ABG usage in the intensive care unit (ICU). ... by Srini Pillay, MD, Harvard Health Blog February 02, 2017 For some people, a self-help program can be a useful addition to professional services, or even be enough by itself. Despite the good intentions of the creators of such programs, the degree and quality of research backing up their claims vary. What s more, people also vary in how well they use such programs. Given the increasing number of books, tapes, podcasts, programs, and apps that

cCBT is an important therapeutic resource for patients with anxiety or depression.8 Access to this form of therapy may be particularly useful for patients requiring low-intensity therapy and for those who find it difficult to seek help from a therapist due to the waiting time or fear stigmatisation.11 ,12 The effectiveness of cCBT for treating anxiety and depression has been acknowledged in a number of randomised control trials.24 ,25 It has since been recommended as a standard first-step treatment for depression and anxiety by metareviews as well as by NICE (2004, 2006, 2009, 2011).5-8 The Department of Health in England has also recommended cCBT as part of the IAPT initiative as a low-intensity alternative to reduce the costs, resources and waiting time for patients seeking therapy.16 Without access to this form of therapy within the NHS buildings, many patients without internet access,19 inadequate internet speed21 or those finding it difficult to maintain privacy in the home22 may feel ...

Background. Research has shown that computerized cognitive behavior therapy (cCBT), including internet-delivered CBT), can be effective. However, less is known about clinicians attitudes towards this mode of treatment delivery. The aim of this study was to explore the attitudes of clinicians within child- and adolescent psychiatry towards cCBT and to explore if attitudes differed depending on rurality or theoretical orientation.. Methods. A random sample of Swedish child- and adolescent mental health services was selected for a survey study (N = 15). A total number of 156 surveys were collected.. Results. Results showed a generally low knowledge of cCBT. A majority of clinicians were positive to cCBT as a prevention program (73%), and as treatment for mild to moderate problems (75%). More caution was reported with regard to more severe mental health problems. Treatment orientation, but not rurality, had a significant effect on several ratings. Thematic analyses showed concerns regarding for ...
A recent pilot study found that internet-based cognitive behavior therapy (CBT) may facilitate a working alliance that is comparable in quality to the alliance formed in face-to-face therapy settings. The working, or therapeutic, alliance is a key tenet of CBT, and research suggests that it is crucial to positive therapeutic outcomes. The current study compares the working alliance between client and therapist in an internet-based intervention and in face-to-face treatment for depression.. Participants were randomized into two groups; one group (n= 25) received the internet-based treatment and the second group (n=28) received face-to-face treatment. Both groups received 8 weeks of manualized CBT treatment for depression, which was based on a standard, evidence-based CBT manual for depression. Treatment was delivered by therapists trained in CBT specifically for this study, and involved an introduction to CBT, behavioral analysis, activity scheduling, cognitive restructuring, promotion of social ...
This review examines the application of online interventions to the treatment of obesity. The unprecedented prevalence rates of overweight and obesity worldwide are indicative of the need to develop efficacious treatment alternatives. While research suggests that diet, physical activity and behaviour therapy are core components of effective obesity treatment programs, their ability to penetrate large portions of the population is limited. The empirically verified effectiveness of online interventions for disorders such as depression and social phobia, coupled with unparalleled rates of internet access, suggest that online interventions for overweight and obesity are a feasible and logical treatment modality. A limited number of randomised controlled trials reviewed in this article have evaluated the efficacy of this treatment alternative. Results are promising and encourage further use, development and empirical research in this area ...

Cognitive Behavioural Therapy (CBT) helps people work towards understanding difficulties in terms of the relationship between thoughts, feelings, body responses
In guided self-help patients take home a standardized psychological treatment and work through it more or less independently. The treatment can be written down in a book or be available via a web site, television, video or audio and some support is given by a professional therapist or coach. P. Cuijpers from the VU University in Amsterdam reviewed 21 studies, including 810 participants, comparing face-to-face therapy with guided self-help. They concluded that guided self-help was just as effective as face-to-face therapy, that its effects lasted for a year and that people were just as likely to carry on with their treatment ...

Cognitive behavior therapy (CBT) is an effective treatment for social anxiety disorder (SAD), but many patients do not respond sufficiently and a substantial proportion relapse after treatment has ended. Predicting an individuals long-term clinical response therefore remains an important challenge. This study aimed at assessing neural predictors of long-term treatment outcome in participants with SAD 1 year after completion of Internet-delivered CBT (iCBT). Twenty-six participants diagnosed with SAD underwent iCBT including attention bias modification for a total of 13 weeks. Support vector machines (SVMs), a supervised pattern recognition method allowing predictions at the individual level, were trained to separate long-term treatment responders from nonresponders based on blood oxygen level-dependent (BOLD) responses to self-referential criticism. The Clinical Global Impression-Improvement scale was the main instrument to determine treatment response at the 1-year follow-up. Results showed ...

Product #2: The self-healing computer-delivered with an Indian-in-a-box, who said (in a thick Indian accent) that his name was Brandon even though he hailed from New Delhi. "Brandon?" "There are more of us named Brandon all the time!" (Get it? Get it? Ha ha. Outsourcing is so silly!) Complete with red-dotted forehead, he was quite the computer-fixer-upper, who fixed Jays computer by slapping it. At the end, he asked Jay if he could do anything else for him. "Yeah," the comedy maestro replied, "get me a burger and fries." Somehow the retelling of this skit doesnt quite manage to evoke how shockingly offensive it actually was. Neither Mr. Shakes nor I find race-based humor inherently offensive (hello, Dave Chappelle); this, however, was not humor-it was just flat-out racism, and left us looking at each other in slack-jawed disbelief. It was just…unbelievable. Racist; xenophobic ...
Although there is a robust finding documenting the efficacy of Cognitive Behavioural Therapy (CBT) in treating anxiety, a significant proportion of clients fail to respond optimally to treatment. A major focus of psychotherapy research involves client-treatment matching, which examines client characteristics as potential predictors of treatment response. Client ambivalence has been identified as a key marker in psychotherapy with wide-ranging implications for engagement in therapy. Motivational Interviewing (MI) has strong empirical support for increasing client commitment for change through the resolution of ambivalence. Though it may be speculated that integrating MI into CBT may be more efficacious for clients high in ambivalence than CBT alone, the investigation of these critical client-treatment matching research questions has been hampered by inadequate measures of ambivalence. This study sought to examine this question in the context of CBT alone versus MI-CBT for 85 clients with severe ...
