As noted by the Food and Drug Administrations (FDA) Office of Minority Health, genetic differences can make a cancer treatment more toxic in one ethnic group than it would be in another. These variations can also make drugs like antidepressants and blood-pressure medications less effective in one group than another. For this reason, it is vitally important that the process used to test the effectiveness of new medicines accurately reflects the patient population that will eventually take them.. Clinical trials are also an important piece of the scientific framework of evidence-based medicine, which inform clinical guidelines and quality measures that direct how we manage most medical disorders. Unfortunately, the underrepresentation of African Americans, Hispanic Americans, Native American and Asian Americans in clinical trials can mean that those guidelines and quality measures are frequently silent or must rely on lower levels of evidence to determine their benefit for minority ...
Background: The underrepresentation of women in academic medicine at senior level and in leadership positions is well documented. In the United Kingdom, the National Institute for Health Research (NIHR) announced that eligibility for funding for Biomedical Research Centres (BRCs) required at least Silver award status of the Athena SWAN Charter. However, the evidence base for monitoring gender equity (GE) in BRCs is underdeveloped. Methods: An exploratory online survey distributed to an entire population of NIHR Oxford BRC affiliates (N=683) who ranked the importance of 13 markers of GE on a five point Likert scale. Data were summarised using frequencies and descriptive statistics. Interrelationships between the markers and underlying latent dimensions (factors) were determined by exploratory and confirmatory factor analyses. Thematic analysis was used to analyse open-ended comments. Results: The response rate was 36% (243 respondents). Respondents were more frequently female (55%, n=133), aged 41-50

Background. Assessment of the age-dependent cancer risk conferred by germline predicted pathogenic variants (PPV) in cancer susceptibility genes is often hampered by the way the data are collected. Cohort-based data frequently contain an overrepresentation of patients carrying a gene variant of interest and an underrepresentation of cancer-free gene variant carriers. In order to overcome this problem, penetrance estimates can be grounded on family-based study designs, through the evaluation of index patients (IP) and their family relatives. The purpose of the TUMOSPEC study is to estimate the penetrance of PPV in genes whose literature data are currently inaccurate or limited and to determine their associated tumour spectrum. This will lead to an appropriate assessment of the clinical utility of testing these genes. Methods. IP are enrolled consecutively among patients who are being offered a germline genetic test in a hereditary breast and ovary cancer (HBOC) context in a participating cancer genetics
Heres the problem. When you do linear regression on a small number of datapoints (which is all a person has, given that they dont observe that many interactions) youre going to have huge uncertainty in the inferred values. To illustrate this, I ran a simulation using the model above with two groups, call them A and B, each half the population. I set the parameters so there was a strong discrimination effect against B. Specifically, even though A and B are equal along other dimensions, the average person in A will be ranked higher than about two thirds of people in B, due solely to discrimination; if you look at people in the top 5%, less than a third will be B. So this is enough discrimination to produce substantial underrepresentation. But when we try to infer the value of the discrimination coefficient, we cant be sure theres discrimination. In the plot below, the horizontal axis is how many interactions we observe; the blue area shows the 95% confidence interval for the discrimination ...

The results of this study illustrate the remarkable regenerative capacity of the skeletal muscle of young mice to ischemic injury. The muscles most affected by FAL were the soleus and gastrocnemius of the lower leg. There was also evidence of ischemic injury in the thigh, but to a lesser extent. During the first 3 days following surgery, there was microscopic evidence of muscle damage and a pronounced loss of function. By day 7, the histological evidence showed a substantial resolution of the injury, and contractile force had been essentially normalized. By 28 days the muscles microscopic appearance was largely restored to baseline. Consistent with these findings is the fact that of the 1,289 known transcripts we identified as differentially expressed between ligated and sham surgery controls, none were induced or repressed by the day 28 time point.. Because the gene expression results are too extensive to be discussed in a single paper, we present selective results from the data set that we ...
Discussions on the first day of the sixth Pacific regional meeting on the elimination of violence against women and girls in Nadi today focussed on the availability of shelters and ethical standards that must be adhered to.. One of the major issues surrounding womens refuges in the Pacific is that of confidentiality, according to Fiji Womens Crisis Centre deputy coordinator Edwina Kotoisuva. She said institutions which deal with women survivors of violence need to put in place codes of ethics that comply with international standards of care that are rights based.. While it may be difficult to keep the actual location of a safe-house secret, Ms Kotoisuva said, those who work there must be trained never to reveal any sort of information that could put at risk the lives of those in the refuge.. Women are most at risk of further violence when they decide to leave the violent situation, says Ms Kotoisuva. If we dont have a clear understanding of the situation of women and violence against ...

Perhaps there arent enough women bloggers because they all have jobs in the notoriously female-dominated MSM? (Just kidding, of course: The MSM is sissy but its very patriarchal, especially at upper levels. When Ellen Goodman isnt whining about female underrepresentation in the blogosphere, Im guessing shes whining about underrepresentation in all the senior positions at the Boston Globlosphere, where she works.). Ed Driscoll notes this is hardy perennial in the MSM; Captain Ed notes political blogging is a dorkwad hobby, similar to Dungeons & Dragons, so why should one even care about a skew in interest in the thing at all?. Dr. Ranga visited the Einstein AITRP twice, once in 2003 and the other time in 2004 to initiate studies on HIV-associated dementia and its correlation to HIV-1 viral subtype differences. In collaboration with Dr. Vinayaka Prasad at Einstein AITRP, Dr. Ranga and his students examined the effect of a natural single amino acid variation in the Tat protein of clade C of HIV-1 on the transactivation and chemokine properties. This collaboration resulted in the identification of the defective chemokine nature of clade C Tat protein and led to the proposition that this variation might contribute to the underrepresentation of HIV-associated dementia in India. This hypothesis garnered much experimental support subsequently by various research groups including that of Dr. Prasads. Dr. Pankaj Serhs laboratory at National Brain Research Centre, New Delhi, India took cue from the hypothesis and investigated into the cytotoxic properties of Tat proteins of viral clades on primary human neurons. Dr. Seths ...
Purpose: : In animal models for bacterial corneal ulcers, high IL-10 levels were associated with a better clinical outcome. We investigated whether IL-10 promotor polymorphisms, known to influence IL-10 expression in vitro, were associated with susceptibility to and/or clinical outcome of bacterial corneal ulcers. Methods: : IL-10 promotor polymorphisms C-819T, G-1082A, A-2763C, and A-2849G were determined in 83 patients with bacterial corneal ulcers and 115 healthy controls by restriction fragment length PCR analysis. For 63 patients and all healthy controls IL-10 haplotypes could be inferred using the program SNPHAP. Results: : A significant underrepresentation of the A-2849A genotype was observed in the patient group compared to healthy controls. Both the -2763A allele and the IL-10.1 haplotype were associated with a poor clinical outcome. While the IL-10.2 haplotype was associated with a better clinical course. Conclusions: : IL-10 promotor polymorphisms that are associated with low IL-10 ...

The makeup of the current Supreme Court can be seen, in one way, as a big success story for certain minorities. It is a triumph, in fact, for two groups which have historically had to put up with a lot of discrimination and lack of political representation in America. These two groups are not defined by gender or race, but rather by religion. Broken down on religious lines, todays Supreme Court has members from just two religions, both of which had been historically underrepresented on the highest court: Roman Catholics and Jews. There are six Roman Catholics currently serving on the court (Samuel Alito, Anthony Kennedy, John Roberts, Antonin Scalia, Sonia Sotomayor, and Clarence Thomas) and three Jews (Stephen Breyer, Ruth Bader Ginsburg, and Elena Kagen). This is undoubtedly a story of rising up from underrepresentation. But, bearing in mind that America is a country with almost too many religions to count, have we actually moved into a problem of overrepresentation or lack of diversity? The ...

Chest pain concerning for a coronary syndrome leads to millions of hospital admissions each year that is often evaluated with diagnostic testing. As a noninvasive diagnostic study, coronary computed tomography angiography (CCTA) has been shown to have excellent diagnostic capabilities. However, concerns over CCTA remain, including radiation exposure, false-positives, and trials of the diagnostic modality that were mostly limited to low-risk patients with underrepresentation of minorities and women. In this single-center randomized, controlled comparative effectiveness trial, 400 intermediate-risk patients (63% women, 54% Hispanic, 37% African-American) admitted to a telemetry floor were randomized to either CCTA or myocardial perfusion imaging (MPI) and followed for 40 months. Patients with baseline renal dysfunction were excluded. At 1 year, there were no significant differences in the primary outcome of coronary angiography that did not lead to revascularization (hazard ratio [HR], 0.77; 95% ...
Quite striking in many of the initial large, randomized, controlled trials of interferon-based therapy of hepatitis C has been the underrepresentation of African Americans. In a combined analysis of studies of standard interferon monotherapy and combination therapy, less than 5% of patients enrolled were black, despite the fact that African Americans have a higher rate of hepatitis C than non-Hispanic white Americans and probably account for more than 20% of cases of chronic hepatitis C in the United States (Gastroenterology 2000;119:1385-1396). Also striking has been a lower response rate to interferon-based therapies among African Americans compared with whites. In several studies, the sustained response rate among African Americans was one third to one half of that in whites. In the recent multinational studies of peginterferon and ribavirin, blacks represented less than 5% of patients enrolled and had response rates that were less than the average, even after controlling for genotype. The ...

Sherman, Scientific Director of the Institute of Nutrition, Metabolism and Diabetes at the Canadian Institutes of Health Research. PS: But you really have to question human superiority What justifies the things we do to animals? What justifies keeping a person in a vegetative coma alive?
