Im going to quickly (and rudely) stereotype and categorize you. If youre reading this post, you fall into one of two categories: You are either a moderator or an abstainer.. A moderator is someone who can easily balance multiple things and doesnt live to extremes. Hence, moderators can have one glass of wine and be done.. An abstainer, on the other hand, is someone who is generally all or nothing. Hence, when an abstainer falls off the wagon, they crash and burn. Yet, when highly committed and passionate, abstainers can do things no one else can because of their intensely focused passion.. Which are you?. Chances are, if youre reading this blog, you are an abstainer. How do I know? Only an abstainer would be seduced by an article talking about 100 percent commitment.. Now that weve got that out of the way, lets talk about how horrible it is to constantly fail at your commitments. Abstainers tend to be hard on themselves because we are often extremists. Yet, were human and fail often. And ...
Acamprosate (Campral(®), Aotal(®), Regtect(®)) is one of a limited number of pharmacological treatment options approved as an adjunct to psychosocial interventions to facilitate the maintenance of abstinence in alcohol-dependent patients. It has been used in Europe, the USA and other countries for many years and was recently approved for this indication in Japan. In several randomized, double-blind, placebo-controlled trials (without active comparators), acamprosate in conjunction with psychosocial therapy for 3-12 months was generally significantly better than placebo plus psychosocial interventions in improving various key outcomes, including the proportion of patients who maintained complete abstinence from alcohol (complete abstinence rate), the mean cumulative abstinence duration, the percentage of alcohol-free days and the median time to first drink ...

The management of chronic, relapsing disorders can be viewed as a control problem in that multi-stage treatment decisions are made with the goal of optimizing mean response. For example, in the prevention of relapse by recovering alcoholics, the response might be percent days abstinent and the treatment decisions might be, which preventative treatment should be used initially, how long should we wait to declare the initial treatment ineffective and switch to a secondary treatment, which secondary treatment should be used, when should treatment be stopped, etc. These treatment decisions would be made on the basis of time varying covariates such as number of days heavy drinking, measures of craving, measures of stress, patient preference and results of urinalyses ...

Alcohol dependence is a heavy burden on patients, their families, and society. Epidemiological studies indicate that alcohol dependence will affect many individuals at some time in their lives, with men affected more frequently than women. Since alcohol-dependent patients often exhibit a lack of social skills and suffer from interpersonal problems, the aim of this study is to elucidate whether men and women experience the same interpersonal problems. Eighty-five alcohol-dependent patients (48 men; 37 women) after detoxification and 62 healthy controls (35 men; 27 women) were recruited. Interpersonal problems were measured with the Inventory of Interpersonal Problems (IIP-64). Additionally, alcohol-dependent patients were interviewed with the Alcohol Use Disorders Identification Test (AUDIT) and were subtyped according to Leschs Alcohol Typology (LAT). There were no significant gender differences in the AUDIT and LAT between alcohol-dependent men and women. Interpersonal problems of alcohol-dependent

Aims - To explore client characteristics that predict drinking outcomes using data from the UK Alcohol Treatment Trial (UKATT).. Methods - Multiple linear regression was used to determine if there were any characteristics, measured before the start of treatment, that could predict drinking outcomes at three and 12 months, as measured by percent day abstinent (PDA) and drinks per drinking day (DDD) over the preceding 90 days.. Results - Lower baseline DDD score and greater confidence to resist drinking predicted lower DDD at both three and twelve months following entry to treatment. In addition to baseline PDA and having greater confidence to resist heavy drinking, female gender, aiming for abstinence, more satisfaction with family life and a social network that included less support for drinking were predictors of percent days abstinent.. Conclusions - Overall the strongest and most consistent predictors of outcome were confidence to avoid heavy drinking and social support for drinking. More ...

Most long-term studies on drinking alcohol have found that light-drinkers are less likely to die prematurely than abstainers. This study re-assesses the effects of moderate alcohol consumption on health and mortality. The team hypothesised that misclassification of former drinkers and occasional drinkers had introduced consistent errors, affecting the results of previous studies.. This study re-examines 54 all-cause mortality and 35 Coronary Heart Disease mortality studies. Most of these studies committed the consistent and serious error of including as abstainers, people who had cut down or ceased drinking alcohol due to declining health, frailty, medication use or disabilitysometimes as the result of past heavy alcohol consumption. When combined in the same group as long-term abstainers or very light drinkers, deaths among these people may have increased the apparent risk of long-term abstinence for the group as a whole.. This study shows that it is possible to perform new analyses on studies ...

One of the great things about the 12 step sobriety program is that youll be able to get lots of support within almost any community. Because one of the steps in the program is to help other people when youve been helped, youll find people who have been through the program, who have been sober for a long time, but know the problems that you face. Theyll know how to help you with your life.
Many anti-smoking efforts can be characterized as attempts to cause peoples preferences to align with their second-order preferences. But this is done by lowering the net benefits of smoking until quitting is the preferred option. That is ethically justified if (and those of us who believe in individual liberty would argue, only if) those who are affected genuinely want an external actor to do that to them, and they understand that the action is not going to make being abstinent as good as smoking, but is going to make smoking as bad as being abstinent. So, a drug like Chantix that makes someone not want to smoke is fine (ignoring, in this analysis, the side effects), so long as someone understands that it probably will not make the experience during long-term abstinence as enjoyable/productive/etc. as life when they smoked, but is merely going to lower the benefits of smoking in the short run to help achieve a new habit of abstinence that is in line with the second-order preference that the ...
The research was restricted to current drinkers who had no history of heart disease. Non-drinkers were excluded to minimise the possibility of reverse causality - for example, former drinkers who had abstained because of poor health.. All the participants were followed up for at least 12 months, giving 5.4 million person years of follow-up. Around half of the total study sample reported consuming more than 100g of alcohol a week and 8.4% drank more than 350g a week. There were 40,317 deaths from all causes and 39,018 from cardiovascular disease events.. The researchers found a positive link between alcohol use and -premature death. The lowest risk of premature death was limited to people drinking 100g of alcohol a week or less.. Alcohol consumption was directly associated with a higher risk of stroke for each 100g per week higher consumption. It was the same for coronary artery disease (excluding myocardial infarction), heart failure, fatal high blood pressure , and fatal aortic aneurism. ...
But lets stick with Sellman here and have health as the only permissible goal. The very best evidence suggests that overall mortality risk is minimized at consumption of a bit over half a standard drink per day (average for men and women), and yes these numbers have never-drinkers separated from former drinkers as reference group. Relative to these very moderate drinkers, teetotalers have about a 16% increased risk of death. Should we be forcing non-drinkers to take their daily dose and whinge endlessly about the risks of under-drinking? If not, then the 16% increased risk isnt worth worrying about, right? If its only risks over 16% that are worth worrying about, mortality risk (relative to teetotalers) is only 16% higher for folks drinking just under 60 grams per day (eyeballing from the graph): the cutoff for high risk drinking for men. So why count men drinking more than 40 grams, Doug? Is it so you can get a bigger number? You should be careful: when the numbers get too big, folks start ...

Temperance not only is undervalued but also misunderstood today. It does mean moderation, but not in a quantitative, mathematical sense.
