The aim of this study was to evaluate the feasibility of sentinel node (SN) biopsy in occult breast lesions with different radiopharmaceuticals and to establish the optimal lymphoscintigraphic method to detect both occult lesions and SNs (SNOLL: sentinel node and occult lesion localisation). Two hundred and twenty-seven consecutive patients suspected to have clinically occult breast carcinoma were enrolled in the study. In addition to the radioguided occult lesion localisation (ROLL) procedure, using macroaggregates of technetium-99m labelled human serum albumin (MAA) injected directly into the lesion, lymphoscintigraphy was performed with nanocolloids (NC) injected in a peritumoral (group I) or a subdermal site (group II). Since a fluid is defined as a liquid or a gas, the airways of the lungs qualify as a fluid space. In developed countries, cancer of the endometrium (EC) is the most common cancer of the female reproductive organs, affecting mainly postmenopausal women. The highest incidence of EC is in Northern America and Europe, and the lowest incidence in Africa and Asia, but its incidence is growing around the world. Several factors influence the risk of developing EC, including: impaired hormone levels, obesity, physical activity, family history, and having been diagnosed with breast or ovarian cancer in the past. For EC, a standard screening test does not exist. It is often diagnosed at stage I/II due to frequent vaginal bleeding, while invasive EC (stage III/IV) is mainly diagnosed in its advanced stage. Intrapulmonary arteriovenous anastomoses (IPAVs) are large diameter connections that allow blood to bypass the lung capillaries and may provide a route for right-to-left embolus transmission. These anastomoses are recruited by exercise and catecholamines and hypoxia. Yet, whether IPAVs are recruited via direct, oxygen sensitive regulatory mechanisms or indirect effects secondary to redistribution pulmonary blood flow is unknown. We have developed a multi-modal imaging approach for SIRT, combining 99mTc-MAA SPECT/CT and/or 90Y PET, 18F-FDG PET/CT, and contrast-enhanced CBCT for voxel-based dosimetry, as a tool for treatment planning and verification. For radiation dose prediction calculations, a segmentation of the total liver volume and of the liver perfusion territories is required. In this paper, we proposed a procedure for multi-modal image analysis to assist SIRT treatment planning. The pre-treatment 18F-FDG PET/CT, 99mTc-MAA SPECT/CT, and contrast-enhanced CBCT images were registered to a common space using an initial rigid, followed by a deformable registration. The registration was scored by an expert using Likert scores. The total liver was segmented semi-automatically based on the PET/CT and SPECT/CT images, and the liver perfusion territories were determined based on the CBCT images. OBJECTIVE: The aim of the study was to evaluate the efficay of lymphatic mapping and sentinel node biopsy in nonpalpable breast cancer (NPBC) patients in comparison with palpable breast cancer patients (PBC). Qualitative analysis of lymphoscintigrams is subject to wide variability and may miss subtle differences in ilioinguinal uptake between normal and abnormal limbs. This study compared quantitative analysis to qualitative analysis of lower‑limb lymphoscintigraphy in diagnosing lymphedema. Fifty‑two lymphoscintigrams performed using standardized protocol, 99‑metastable technetium nanocolloid intradermal injection at the first interdigital space, were analyzed quantitatively. Fifty‑three normal and 51 abnormal limbs were analyzed. For each limb, a region of interest (ROI) was drawn around the injection site, and ilioinguinal nodes on the 1.5 h static images and the counts in these ROIs were recorded. Percentage ilioinguinal nodes uptake was then computed. Analysis of variance (ANOVA) was performed to determine the difference in ilioinguinal uptake between normal and abnormal limbs. After optimizing the reliability and safety of dynamic sentinel node biopsy in cNO penile cancer patients, reducing false negative and complication rate to 4.8% and 5.7% respectively [1], the Netherlands Cancer Institute Penile Cancer Group studied the anatomical mapping of the lymphatic drainage of penile cancer in 50 consecutive patients (August 2006-August 2007!) who underwent SPECT-CT imaging immediately after dynamic and static imaging, 30 min and 2 h after injection of technetium-99m nanocolloid [2]. The inguinal regions were divided according to the five Daseler zones [3] and the nodes of the pelvic regions were classified separately. Lymphatic drainage was visualized in 82 of the 86 category cNO-groins, and a total of 115 sentinel nodes were found on scintigraphy and SPECT-CT. A total of 122 sentinel nodes were removed and only 9 tumour positive sentinel nodes were found in 8 patients. During conventional cardiopulmonary bypass (CPB) there is no active perfusion of the pulmonary circulation and the mechanical ventilation is ceased leaving the lungs exposed to warm ischemia. Pulmonary dysfunction is seen in varying degrees after major surgery, but more severe in cardiac surgery patients probably due to the effects of CPB. The evidence for effect and safety are limited, but active pulmonary artery perfusion during CPB could be beneficial for the patients postoperative oxygenation. Our aim was in a randomised clinical trial to assess primarily the effect of pulmonary artery perfusion during CPB on postoperative oxygenation in patients diagnosed with chronic obstructive pulmonary disease (COPD), secondarily to assess other possible benefits and harms.
